2.Effect of bilateral subotal thyroidectomy for juvenile hyperthyroidism

Objective To evaluate the effect of medicial and surgical treatment for juvenile hyperthyroidism.Methods Ortapazole was administ rated separately in drug therapy group for 1.5-2 years.Bilateral subtotal thyroidectomy was done in surgical therapy group.Results In drug therapy group,effective rate was 60 percent in 6 months and 70 percent in one year.Recurrence rate was 40 percent after drug withdrawal in 2 years curative rate was 60 percent.In surgical therapy group,the average stay in hospital was 16 days.There was no nerve injury,parathyroidal hypofunction,thyroid crisis or hypothyroidism complications,with 100 percent curative rate after 2 years′ followup.Postoperative growth and development were normal.Conclusions Surgical treatment may be suitable for those who have no response to drug therapy,with recurrence after drug withdrawal,whose compression symptom was obvious,with moderate and severe hyperthyroidism or those who could not take medicine persistently.Bilateral subtotal thyroidectomy applied in juvenile hyperthyroidism could achieve quick and better recovery,and has no influence on the juvenile growth and development.
3.Intravascular ultrasound assessment of chitosan/heparin layer-by-layer self assembly coating stent on late stent malappoisition and vessel remodeling in porcine model

Objective To investigate late stent malapposition or vessel remodeling post chitosan/heparin layer-by-layer self assembly coating stent (LBL) implantation in porcine.Methods A total of 32 stents [ bare metal stent ( BMS,n =9),sirolimus-eluting stent ( SES,n =11 ) and LBL ( n =12 ) ] were implanted into coronary arteries of 16 porcine.Intravascular ultrasound (IVUS) was performed immediately after stenting and at 1 month after stenting to measure vessel area (VA),stent area (SA) and lumen area (LA).Neointima area (NA) was measured at 1 month post stenting by IVUS to detect signs of stent malapposition and to determine remodeling index (RI). Histopathology was performed at 1 month post stenting to observe vessel wall structure and stent malapposition status.Results No sign of stent malapposition was detected,VA and SA/LA were similar among groups immediately after stent implantation.At 1 month follow-up,none of three groups showed stent malapposition.VA,SA,NA and LA were ( 7.30 ±0.77),(6.83 ±0.76),(1.40 ±0.96) and (5.43 ±0.88)mm2 in LBL group,(7.13 ±0.69),(6.63 ±0.71 ),(0.28 ±0.35) and (6.34 ±0.89)mm2 in SES group,(7.48 ±0.70),(7.00 ±0.52),(2.69 ±1.58) and (4.31 ± 1.28)mm2 in BMS group.VA and SA were similar among groups ( all P >0.05 ).LA in LBL group was smaller than SES group ( P < 0.01 ) and significantly larger than in BMS group ( P <0.05 ).NA in LBL group was larger than SES group (P <0.01 ) and significantly smaller than in BMS group (P < 0.05 ).RI in LBL,SES and BMS groups was 0.95 ± 0.07,1.02 ± 0.04 and 0.98 ± 0.04 ( P >0.05).Conclusions There is no late stent malapposition or abnormal remodeling post LBL,SES and BMS implantation up to 1 month in this porcine model.LA in LBL group is smaller than SES group and larger than BMS group at 1 month after implantation in this porcine model.

Umbilical metastases from intraperitoneal malignancies are universally referred to Sister Mary Joseph's nodule (SMJN).The most frequent primary sites include the stomach and ovaries. SMJN caused by colon cancer is very uncommon. Likewise, carcinoma of the right side colon metastasizing to inguinal lymph nodes is considered almost impossible. To the best of our knowledge, no report of right side colon cancer synchronously involving both the umbilicus and inguinal lymph nodes is available. We present a case of right side colon cancer(RSCC) metastasizing to the umbilicus and inguinal lymph nodes, which was confirmed by pathology and immuohistochemistry.
5.Study on the expression of fibronectin after cerebral contusion in rats for timing of injuries.

An experimental model of reproducible focal cerebral contusions in rats was made by a free-drop impacting right hemisphere. The expression of fibronectin and its mRNA after cerebral contusion were detected respectively by immunohistochemical staining and in situ hybridization. Results indicated that the expression of fibronectin and its mRNA increased after injury, and there existed a relationship between increased fibronectin and its mRNA and different intervals after brain injury. It is inferred that the expression of fibronectin and its mRNA can be used for timing of brain injuries and distinguishing antemortem and postmortem brain contusions.

This study was purposed to investigate the vWF gene A1381T polymorphism in patients with coronary heart disease (CHD). A case-control study was designed, including 104 continuously hospitalized patients with CHD, aging from 40 to 75 years (average 59) and 96 persons underwent physical examination in outpatient department as controls, aging from 39 to 70 years (average 56). The plasma vWF: Ag level of CHD patients and control persons was detected by ILISA. vWF gene A1381T polymorphism was analyzed by the polymerase chain reaction-restriction fragment length polymorphism and sequencing when it is necessary. The data were grouped by gender, blood group and/or genotype in CHD group and control groups. The difference of plasma vWF level between male and female was analyzed by independent sample t test; one way ANOVA was used to analyze the difference of vWF level between different blood group genotypes, while the factorial design ANOVA was used to test the difference of vWF level in plasma between A1381T genotype and/or ABO blood groups. χ(2) Crosstabs were used to test the CHD susceptibility. The results showed that the frequencies of GG genotype (wild type) of vWF gene A1381T polymorphism were 62.5% in CHD group and 67.7% in control group, and the frequencies of AG genotype (heterozygous variant) were 37.5% in CHD group and 32.3% in control group. χ(2) Crosstabs showed no significant correlation between vWF gene A1381T polymorphism (AG) and CHD (OR = 1.258, 95% CI = 0.702 - 2.255, χ(2) = 0.595, p = 0.440). The plasma vWF level in CHD group was statistically very higher than that in control group (p < 0.001), even though the relationship of vWF A1381T polymorphism (rs216311) and susceptibility of CHD in CHD group was not found. The plasma vWF level of AG or GG genotype was higher in CHD group than in control group (p < 0.001). The plasma vWF level of AG genotype was higher than that of GG in CHD group (p < 0.05), but not in control group. The plasma vWF of O blood group was lower than that of A, B and AB blood groups (p < 0.05), while among A, B, AB blood groups, the vWF level was not different (p > 0.05). Among O, A, B, AB blood groups in CHD group, vWF level was not different (p > 0.05). Although the two-way analysis of variance ANOVA showed no interaction of A1381T genotype and ABO blood groups on plasma vWF level, the plasma vWF level in AG mutant of vWF A1381T gene polymorphism with O blood group was higher than that of GG mutant (p = 0.023) in CHD group, not different in other blood groups. It is concluded that there is no association between vWF gene A1381T polymorphism and CHD susceptibility. The plasma vWF level in CHD group interrelated with ABO blood group and A1381T polymorphism, in which the plasma vWF level in AG genotype increase mostly. Plasma vWF level in vWF gene A1381T polymorphism with AG mutant was significantly much higher than GG mutant in CHD. This change may be beneficial to further study the effect of A1381T polymorphism on vWF gene expression and activity.

8.Investigation on a case of Amanita neoovoidea poisoning.

This paper reported a case of poisoning caused by ingestion of Amanita neoovoidea. The patient experienced nausea, vomiting, oliguria, acute renal function injury, and was discharged after symptomatic support treatment and blood purification treatment. Given the different toxicity of different mushrooms, species identification of poisonous mushrooms can help clinicians in diagnosis and treatment.

9.Association of CD133 and endothelin-converting enzyme expressions with prognosis of non-small cell lung carcinoma.

OBJECTIVETo investigate the expression of tumor stem cell marker CD133 and endothelin-converting enzymes (ECE) in non-small cell lung carcinoma (NSCLC) and their association with NSCLC lymphoid metastasis.
METHODSCD133 and ECE expressions was detected immunohistochemically in the specimens from 77 patients with NSCLC, and the association of CD133 and ECE expressions with the tumor size, histological type, differentiation, lymphoid metastasis, and prognosis of NSCLC was analyzed.
RESULTSThe positive expression rate of CD133 and ECE was 51.9% (40/77) and 45.5% (35/77) in these specimens, respectively. Both CD133 and ECE expressions were associated positively with lymphoid metastasis (r=0.246 and 0.339, P<0.05), and inversely with the survival time of the patients (P<0.05). CD133 and ECE expressions were not related to tumor size, histological type, and differentiation of the tumor (P>0.05). CD133 expression was associated positively with ECE expression in NSCLC (r=0.249, P<0.05).
CONCLUSIONCD133 and ECE expressions are associated with lymphoid metastasis and prognosis of NSCLC, and their overexpression often suggests unfavorable prognosis of NSCLC.

BACKGROUNDSubclinical apoplexy of pituitary functional adenoma can cause spontaneous remission of hormone hypersecretion. The typical presence of pituitary growth hormone (GH) adenoma is gigantism and/or acromegaly. We investigated the clinical characteristics of patients with spontaneous partial remission of acromegaly or gigantism due to subclinical apoplexy of GH adenoma.
METHODSSix patients with spontaneous remission of acromegaly or gigantism were enrolled. The clinical characteristics, endocrinological evaluation and imageological characteristics were retrospectively analyzed.
RESULTSIn these cases, the initial clinical presences were diabetes mellitus or hypogonadism. No abrupt headache, vomiting, visual function impairment, or conscious disturbance had ever been complained of. The base levels of GH and insulin growth factor-1 (IGF-1) were normal or higher, but nadir GH levels were all still > 1 µg/L in 75 g oral glucose tolerance test. Magnetic resonance imaging detected enlarged sella, partial empty sella and compressed pituitary. The transsphenoidal surgery was performed in 2 cases, and the other patients were conservatively managed. All the patients were in clinical remission.
CONCLUSIONSWhen the clinical presences, endocrine evaluation, biochemical examination and imageology indicate spontaneous remission of GH hypersecretion in patients with gigantism or acromegaly, the diagnosis of subclinical apoplexy of pituitary GH adenoma should be presumed. To these patients, conservative therapy may be appropriate.
