Objective To observe the therapeutic effects of Cheneau orthosis with different pressure areas on the patients with adolescent idiopathic scoliosis. Methods Thirty patients with adolescent idiopathic scoliosis who had apex vertebraes below T6 were included, and randomly divided into group A in which patients wore orthosis spanning across 3 vertebra, and group B in which patients wore orthosis spanning across 5-6 vertebra. All patients were assessed with regard to Cobb degree, apex vertebrae rotary degree and pain index after wearing the orthosis. Results The rectified ratio of Cobb degree were 17% and 29% in groups A and B, respectively. There was a statistical difference between the 2 groups(P<0.05). No statistical difference was found between the 2 groups in terms of apex vertebrae rotary degrees(P>0.05). Pain index was 5.00 in group A, and 2.07 in group B, which was statistically different (P<0.05). Conclusions Increasing pressure points' areas of Cheneau orthosis properly can improve more effectively the lateral curvature angle of the patients with adolescent idiopathic seoliosis.
3.Efficacy observation of acupuncture and tapping therapy in the treatment of type 2 diabetes of yin deficiency pattern combined with stasis in the patients.

OBJECTIVETo explore the clinical efficacy and mechanism of acupuncture and tapping therapy in the treatment of type 2 diabetes of yin deficiency pattern combined with stasis in the patients.
METHODSOne hundred and twelve patients were treated with acupuncture, followed by tapping method. The reinforcing method with lifting, thrusting and rotating needle technique was used at Taixi (KI 3), Shenmen (HT 7), Taibai (SP 3), Taiyuan (LU 9), Sanyinjiao (SP 6) and Zusanli (ST 36); and the even-needling technique was used at Taichong (LR 3), Hegu (LI 4), Xuehai (SP 10) and Fenglong (ST 40). The needles were remained for 30 min. After needle removal, the tapping method with plum-blossom needle was applied to Geshu (BL 17), Yishu (EX-B 3), Ganshu (BL 18), Shenshu (BL 23) and Pishu (BL 20), once every two days. The treatment of 3 months was required. The changes were observed before and after treatment in the relevant indices of fat-islet axis such as fasting plasma glucose (FPG), fasting leptin (FLP), fasting insulin (FINS), insulin sensitivity index (ISI), insulin resistance (Homa IR), insulin secretion index (Homa-beta) and the indices of obesity and blood lipids in the patients.
RESULTSAfter treatment, the total effective rate was 84.8% (95/112), in which the efficacy on the stasis predominated syndrome was better than that of yin deficiency predominated syndrome [92.9% (52/56) vs 76.8% (43/56), P < 0.05]. Before treatment, FPG, FLP, FINS, Homa-IR, total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), body mass, body mass index (BMI), fat percentage (F%) and obesity degree in the patients were higher obviously than those in the normal group (all P < 0.01); high-density lipoprotein cholesterol (HDL-C) and ISI were lower than those in the normal group (both P < 0.01). After treatment, FPG, FLP, FINS, Homa-IR, TC, TG, LDL-C, body mass, BMI, F% and obesity degree were all reduced obviously as compared with those before treatment (all P < 0.01). ISI and HDL-C were increased apparently (both P < 0.01). The efficacy on mild obesity was superior to that on moderate and severe obesity, and the efficacy in the young patients was better than that in the elderly, the efficacy in the males was better than that in the females, but the differences were not significant (all P > 0.05). The efficacy in the cases with 10 years duration of sickness was superior to those with over 10 years duration of sickness (P < 0. 01).
CONCLUSIONAcupuncture and tapping therapy achieves the significant efficacy on type 2 diabetes of yin deficiency pattern combined with stasis and its effect mechanism is related to the positive regulation of glucose, lipid metabolism and fat-islet axis.

4.Patterns of Frequency Domain Indexes of Surface Electronic Signals of Adolescent Idiopathic Scoliosis Patients' Paraspinal Muscles

Objective To investigate the patterns of frequency domain indexes of surface electronic signals of adolescent idiopathic scoliosis (AIS) patients' paraspinal muscles by using surface electromyographic (sEMG) techniques .Methods 25 AIS patients enrolled, 7 males, 18 females, ages range from 11 to 21 years old. All of the enrolled patients undertook the Biering Sorensen test(BST) and the object-lifting test, a Finland made type ME3000P sEMG instrument was applied to record the electronic activities of paraspinal muscles(convex /concave) of all subjects, and the frequency domain indexes: median frequency(MF),mean power frequency(MPF), zero crossing rate(ZCR)were analyzed.Results The wave amplitudes and scales of paraspinal muscles electronic frequency domain indexes(MF, MPF, ZCR) were lower than the other tested positions when recorded at the zone of apex vertebrae, and MFslope, MPFslope and ZCRslope all showed a linear degressive tendency as the exercise time was extended.Conclusion Paraspinal muscles at the zone of apex vertebrae have low fatigue durabilities and more likely to be exhausted. sEMG ought to be one of the objective examinations used to evaluate the differences of electronic activities of paraspinal muscles(convex /concave) of AIS patients, and may have a promising value in clinical practice.
5.Anterior cervical intervertebral space decompression under microscope for cervical spondylotic myelopathy in elderly patients.

OBJECTIVETo investigate the clinical effects of anterior cervical intervertebral space decompression under microscope in treating cervical spondylotic myelopathy in elderly patients.
METHODSFrom June 2009 to March 2012, 43 patients with cervical spondylotic myelopathy were treated with anterior cervical intervertebral space decompression and intervertebral fusion under microscope. There were 26 males and 17 females, aged from 60 to 72 years old with an average of (64.9±3.7) years. Japanese Orthopaedic Association System (JOA) score was from 7 to 12 points with an average of (9.5±1.8) points before operation. The function of nerves was assessed before and after operation according to JOA.
RESULTSAll patients were followed up from 10 to 18 months with an average of (14.7±1.6) months. Postoperative JOA score was (13.81±1.44) points (ranged, 10 to 16), had significantly higher than preoperative (P<0.01). According to the rate of the improved JOA score, 9 cases got excellent results, 26 good, 7 fair, 1 poor.
CONCLUSIONAnterior cervical intervertebral space decompression under microscope for cervical spondylotic myelopathy in elderly patients is safe and effective.

8.Clinical analysis of 56 cases with vaginal intraepithelial neoplasia after hysterectomy

Objective · To investigate the relationship between the results of thin prep cytologic test (TCT), high-risk human papillomavirus (Hr-HPV) detection and vaginalintraepithelial neoplasia (VAIN) after hysterectomy. Methods · A retrospective study was conducted of 56 patients with VAIN after hysterectomy. The analysis included TCT and Hr-HPV examination, clinical data and the relationship between Hr-HPV examinenation and histopathological examination of colposcopy. Results · Postoperative follow-up of TCT and Hr-HPV parallel pathological examination showed that 56 patients had vaginal stump lesions, including cervical factor hysterectomy accounted for 5.81% (45/775) and non cervical factor hysterectomy accounted for 0.19% (11/5933). The difference was statistically significant (P=0.000). In 56 cases of patients with vaginal stump lesions, Hr-HPV infection were 40 cases (accounting for 71.43%), uninfected patients were 16 cases (accounting for 28.57%). The Hr-HPV infection rates of cervical hysterectomy and non cervical factor hysterectomy patients were 80.00% (36/45) and 36.36% (4/11) respectively, and the difference was statistically significant (χ2= 6.248, P=0.012). TCT results showed that the incidence of squamous intraepithelial lesions were 42.22% (19/45) and 9.09% (1/11) respectively. Conclusion · The Hr-HPV infection rate and the morbidity of VAIN of the patients undergone hysterectomy due to the cervical lesionfactors is higher than those who had none cervical lesion factors. In order to identify VAIN early, patients who have the history of hysterectomy should undergo careful evaluation of cervical and vaginal circumstance before surgery and the routine examinations of TCT and Hr-HPV in the follow-up.
9.Expression and clinical significance of serum Golgi protein 73 in infantile hepatitis syndrome

Objective To investigate the expression and the possible clinical significance of serum Golgi protein(GP73)in infantile hepatitis syndrome(IHS)by different causes.Methods Totally 79 patients with IHS in Guangzhou Women and Children's Medical Center from February 2012 to December 2012 were enrolled in this study,including 15 cases with biliary atresia(BA)group,29 cases with infection(infection group),5 cases with neonatal intrahepatic cholestasis caused by citrin deficiency(NICCD group),and 30 cases with unknown etiology(idiopathic infantile hepatitis group).At the same time,30 healthy infants were enrolled as healthy control group.The serum levels of GP73 were determined by quantitative enzyme-linked immunosorbent assay(ELISA),and the children's liver function[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),direct bilirubin(DBIL),alkaline phosphatase(ALP),γ-glutamyl trans-peptidase(γ-GT),total bile acid(TBA)and albumin(ALB)] were measured by turbidimetric inhibition immuno assay.Then,the corresponding data were statistically analyzed.Results Serum GP73 in BA group,infection group,NICCD group,idiopathic infantile hepatitis group and the healthy control group were(296.6±67.5)μg/L,(185.1±66.4)μg/L,(199.2±87.1)μg/L,(181.7±74.2)μg/L and(65.3±17.0)μg/L,respectively.Serum γ-GT levels in BA group,infection group,NICCD group,idiopathic infantile hepatitis group and healthy control group were(764.7±775.8)U/L,(448.2±352.7)U/L,(239.4±88.7)U/L,(283.3±377.2)U/L and(54.0±72.6)U/L,respectively.The levels of GP73 and γ-GT were significantly higher in infants with IHS,and the levels of GP73 and γ-GT in infants with BA were the highest(F=46.775,9.238,all P<0.05).There was a positive correlation between serum levels of GP73 and γ-GT(r=0.280,P<0.05),and there was no correlation between GP73 and ALT,AST,TBIL,DBIL,ALP,TBA,ALB(r=-0.061,-0.071,0.164,0.123,0.137,0.008,-0.047,respectively,all P>0.05).The receiver operating characteristic curve(ROC)constructed with GP73 showed a sensitivity of 80.0%and specificity of 82.8%with an area under the receiver(AUC)of 0.872 for diagnosis of BA,comparatively,a sensitivity of 66.7%and specificity of 71.9%were showed with a AUC of 0.731 when performed with γ-GT.Conclusions Serum GP73 concentration significantly increased in all liver disease groups,regardless of the etiology.Serum GP73 expression is significantly higher in infants with BA.Serum GP73 shows a superior sensitivity and specificity to γ-GT for diagnosis of BA,which might be useful for early diagnosis of BA and IHS with different causes.
10.The effects of preconditioning with pioglitazone on infarct size and mitochondrial ATP-sensitive potassium channel in rats with ischemia-reperfusion

Objective To observe the effects of preconditioning with pioglitazone on infarct size and mito-chondrial ATP-sensitive potassium channel in rats with ischemia-repedusion, and to explore its possible mecha-nism. Method The whole experiment was divided into experiment Ⅰ and Ⅱ. In experiment Ⅰ, 24 rats were ran-domly divided into four groups (6 rats in each group): (1)Sham-operated (SO) group: the coronary artery of rat was threading without hgation, and the heart was removed by cutting immediately 4 hours later; (2) Isehemia-reperfusion (I/R) group: the rats were administered with 0.9% saline intravenously via caudal vein at 24 hours before iigating the left anterior descending branch of coronary artery for 30 minutes, and followed by reperfusion for 4 hours; (3)5-hydroxydecanoate plus pioglitazone(5HD+Pio) group: the rats were injected with 10 mg/kg 5-hy-droxydecanoate (the blocker of mitochondrial ATP-sensitive potassium channels,) at 24 hours before ligation, and 30 minutes later, 3 mg/kg pioglitazone was given in 5 minutes, and then the rats were subjected to ischemia for 30 minutes, followed by reperfusion for4 hours; (4)pioglitazone treatment group (Pio): the mrs were given 3 mg/kg pioglitazone at 24 hours before occlusion, and then they were treated as done in the 5HD+Pio group. In I/R, 5HD+Pio and Pio group, the hearts were removed by cutting after reperfusion. Western blotting was used to detect the protein expression of P38MAPK, .INK and NFκB P65. In experiment Ⅱ, 30 rats were randomly divided into five groups: SO, I/R, Pio, 5HD+Pio and 5-HD group (rats were treated as done in the rats of I/R group and were injected with 10 mg/kg 5-bydroxydecanoate 24 h before ischemia/reperfusion),and the size of myocardial in-farction and isehemia were measured after reperfusion. Statistical analyses were performed using SPSS10.0 soft-ware. Multiple comparisons were analyzed by one-way analysis of variance (SNK-q test). P<0.05 was consid-ered statistically significant. Results (1) The infarct size in i/R group was(34.93±5.55)%, while pioglita-zone reduced the infarct size to(20.24±3.93)% (P<0.05). There was no significant difference between I/R and 5-HD±Pio or 5-HD groups (P>0.05). Compared with the sham-operated group, the expression of P38MAPKmRNA, JNKmRNA and protein of P38MAPK, JNK and NFκB P65 in I/R increased (P<0.05). Com-pared with the I/R group, pioglitazone inhibited these undue expressions (P<0.05). Conclusions Pioglitazone could protect the heart from ischemia-reperfusion injury evidenced by reducing infarct size. These protective effects of pioglitazone may be related to opening mitochondrial ATP-seusitive potassium channels or downregulation of JNK and p38 MAPK signaling, leading to the overexpression of NFκB p65 activation.
