1.Analysis of risk factors and prognosis on endophthalmitis after cataract surgery

AIM: To study the risk factors of endophthalmitis after cataract surgery and prognosis of endophthalmitis after vitrectomy.
METHODS: One thousand nine hundred patients ( 1 900 eyes) of receiving cataract surgery in our hospital were selected and divided into infection group and control group according to the incidence of endophthalmitis. The influence of clinical index in the endophthalmitis after cataract surgery as well as related risk factors based on single factor and multi- factor Logistic regression was analyzed. All patients with endophthalmitis were received vitrectomy, and the prognosis of endophthalmitis after vitrectomy was observed.
RESULTS: Based on single factor analysis, the risk factors of endophthalmitis after cataract surgery including age, hypertension, diabetes mellitus, vitreous loss, operation time, common operating room and nonuse of anesthetics in the required time lead to the ocurrence of endophthalmitis after cataract surgery (P<0. 05). Based on multi-factor Logistic regression, the independent risk factors of endophthalmitis after cataract surgery included age (≥70 years old ) , hypertension, diabetes mellitus, vitreous loss, operation time (≥ 10min ), common operating room and nonuse of anesthetics in the required time (P<0. 05). After vitrectomy, the vision of patients was significantly improved (P<0. 05).
CONCLUSION:The risk factors of endophthalmitis after cataract surgery include age, hypertension, diabetes mellitus, vitreous loss, operation time, common operating room and nonuse of anesthetics in the required time. Vitrectomy can significantly improve the vision of patients.

3.Clinical study of ulinastatin on the treatment of systemic inflammatory response syndrome in severe acute pancreatitis

Objective To evaluate the clinical value of ulinastatin on the treatment of systemic inflammatory response syndrome in severe acute panereatitis. Method Eighty-four patients with severe a-cute pancreatitis were randomly divided into two groups. In the treatment group (42 cases),on the base of routine treatment, ulinastatin was administered intravenously for seven days after hospitalization, while in the control group only routine treatment was given (42 cases) to. Inflammatory factors in serum, the change of liver function and renal function were measured in two groups before and after the treatment, and the clinical efficacy were observed. Results There was significant difference, in the serum level of tumor necrosis factor-α, interleukin-1, interleukin-6, alanine aminotransferase, aspartate aminotransferase, blood urea nitrogen and creatinine on the 7th day between two groups (P < 0.05 or < 0.01 ) ,there were significant differences in the incidence of complications, hospitalization time, incidence of multi-organ failure between two groups [14.3%(6/42) vs 38.1%(16/42), (29.4 ± 1.5)d vs (34.4 ± 1.8)d, 28.6%(12/42) vs 47.6%(20/42), P<0.01 or <0.05 ]. Conclusions Ulinastatin can suppress the secretion and release of some inflammatory factors, protect the important viscera functions and reduce the incidence of complications, and it has favorable clinical efficacy.
4.Effects of moxifloxacin solution for inhalation on inflammatory mediators and blood gas analysis in patients with chronic obstructive pulmonary disease with respiratory failure

Objective To analyse effects of moxifloxacin solution for aerosol inhalation on inflammatory mediators and blood gas analysis index in patients with chronic obstructive pulmonary disease with respiratory failure.Methods 60 patients who were diagnosed with chronic obstructive pulmonary disease with respiratory failure in our hospital were collected.All patients were randomly divided into experimental group and control group,30 cases in each group.On the basis of conventional treatment, control group were given sulfate for inhalation shating amine alcohol solution 2mg per times, 3 times per day atomization inhalation treatment,and experimental group was given 2 mg/times, 4 times per day atomization inhalation treatment,two were treated for 2 weeks.After the treatment, the serum levels of CRP,TNF-α,IL-10,IL-18 and arterial blood PO2 ,PCO2 levels were detected in all patients.Results Compared with control group post-treatment, the serum levels of CRP, IL-18 and TNF-αwere lower in experimental group (P<0.05), and the serum level of IL-10 was higher;the arterial blood level of PO2 was higher,and the arterial blood level of PCO2was lower in experimental group (P<0.05). Conclusion Effect of moxifloxacin solution for aerosol inhalation can significantly reduce the serum CRP,TNF-αand IL-18 levels, improve serum IL-10 level in patients with chronic obstructive pulmonary disease with respiratory failure, ameliorate PO2 and PCO2 .
5.Expression of caspase-3 and TRAIL receptors in CD4~+ and CD8~+ T cells of systemic lupus erythematosus patients

Objective To explore the expression of caspase-3 and tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) receptors in the CD4+ and CD8+ T cells of systemic lupus erythematosus (SLE)patients. Methods The CD4+ and CD8+T cells from peripheral blood of 20 SLE patients and 10 healthy volunteers were separated using magnetic cell sorting system (MACS), reverse transcription-polymerase chain reaction (RT-PCR) was used to test the expression of caspase-3 and TRAIL receptors in CD4+ and CD8+T cells of SLE patients and healthy volunteers. Results The CD8+T cells from SLE patients had significantly higher caspase-3 (P=0.003) and TRAIL-R2 (P=0.024) expression than those from healthy volunteers. Conclusion The TRAIL-R2 signal pathway may be a possible important pathway to the apoptosis of T cells in SLE patients.
6.Comparison between nuclear matrix protein 22 (NMP 22) test and voided urine cytology for detection of bladder cancer

Objective To compare the sensitivity and specificity of NMP 22 test with voided urine cytology in detection of bladder cancer and to evaluate their clinical values. Methods For 155 patients suspected with bladder cancer NMP 22 and cytology were conducted in the same voided urine samples.Of them 95 patients with bladder transitional cell carcinoma were confirmed histologically.The sensitivity and specificity of NMP 22 and urine cytology were analyzed.60 patients without bladder cancer were selected as control. Results The overall sensitivity and specificity of NMP 22 test were 65.3% and 70.0%,respectively,those of urine cytology were 43.2% and 83.3%,respectively.There was no significant difference between the specificity of urine cytology and that of NMP 22 test;however,NMP 22 was significantly more sensitive than urine cytology in detection of any stages and grades of bladder cancer(P
7.Expression of SLAM gene in CD4~+ and CD8~+ T cells from SLE patients

Objective To explore the expression of SLAM gene in the CD4+ and CD8+ T cells from SLE patients.Method The CD4+ and CD8+ T cells from peripheral blood of 20 SLE patients and 10 healthy blood donors were separated using magnetic cell sorting system(MACS).We detected the expression of SLAM mRNA in CD4+ and CD8+ T cells by RT-PCR.Results The CD4+ and CD8+ T cells from SLE patients showed significantly higher SLAM expression than those from healthy blood donors.Conclusion The SLAM signal pathway may play an important role in contributing to the abnormal activation of T cells in SLE patients.
8.Detection and significance of CD4 and CD8 subsets of T lymphocytes in the peripheral blood of systemic lupus erythematosus patients by magnetic activated cell sorting

Objective To investigate the abnormality of CD4 and CD8 subsets of T lymphocytes in the peripheral blood of systemic lupus erythematosus patients. Methods The CD4 + and CD8 + T lymphocytes from 20 SLE patients and 10 healthy controls were separated using magnetic activated cell sorting (MACS). Results The numbers of CD4 + T cells in the peripheral blood and CD4 +/CD8 + ratio of SLE patients were significantly lower than those in healthy volunteers(CD4,P=0.001;CD4/CD8 ratio,P=0.046). Conclusion There exist abnormalities in the CD4 and CD8 subsets of T lymphocytes in the peripheral blood of SLE,especially the decreased numbers of CD4 + T cells.
10.Effect of Gehua Jiejue Dizhi decoction on the liver fatty deposition and expression of PXR in themousealcoholic fatty liver

Objective To explore the effect of a herbalcompound Gehua Jiejue Dizhi Decoction (GJDD) on the liver fat deposition and the expression of PXR, and the mRNA and protein expression of its target genes CYP3A11 and CYP3A25in the liver tissues of mouse models of alcoholic fatty liver.Methods Twenty-nine healthy male C57BL/6J mice were randomly divided into control group (n=5), model group (n=8), high dose GJDD group (n=8)and low dose GJDD group (n=8).The mouse model of alcoholic fatty liver was prepared according to the National Institute on Alcohol Abuse and Alcoholism (NIAAA) method.Then, the mice were treated with the high dose and low dose GJDD for 9 days.Serum glutamic-pyruvic transaminase (AST) and aspartate aminotransferase (AST) were detected by enzyme-linked immunosorbent assay (ELISA).Liver fat deposition was detected by oil red O staining.Real-time RT-PCR and immunohistochemistry were performed to examine the expressions of PXR, CYP3A11 and CYP3A25.Results Compared with the model group, the liver fat deposition in the intervention groups was significantly reduced in a dose-dependent manner, with a significant increase of the expression of PXR and CYP3A25 (P < 0.01).The serum ALT level was significantly reduced in the model group (P < 0.01), while the transcriptional levels of CYP3A11 mRNA in the groups were similar (P ≥ 0.05).Conclusions Gehua Jiejue Dizhi Decoction has obvious therapeutic effect on the AFLD in mice, which may be related to the activation of PXR and its target genes CYP3A25.
