1.Identification role of plasma high sensitive C-reactive protein and lipoprotein (a) levels in the diagnosis of ischemic stroke according to the TOAST classification

Objective To investigate the identification role of plasma high sensitive Creactive protein (hs-CRP) and lipoprotein (a) (Lp [a]) levels in the diagnosis of patients with acute ischemic stroke according to the TOAST classification. Methods The levels of plasma hsCRP and Lp (a) in 82 acute stroke patients ( <24 hours) and 60 healthy controls were detected using immune scatter turbidimetry and immune transmission turbidity, and try to make use of Holter, ultrasonography, magnetic resonance imaging, magnetic resonance angiography/CT angiography/dagital subtraction angiography and other tests. Finally, they were classified according to the diagnostic criteria of the TOAST classification of ischemic stroke. Results There were significant differences in plasma hs-CRP and Lp (a) levels between all the subtypes of the acute ischemic sroke group and the control group (all P <0. 001). The the level of plasma hsCRP in patients with cardioembolism (CE) was highest. Hs-CRP could be used as a biological marker of CE subtype (odds ratio[OR] = 1. 84,95% confidence interval [CI] 1. 18-2. 85, P < 0. 05). When its concentration was > 3. 48 mg/L, the sensitivity and specificity of predicting CE were 89% and 83% respectively. The plasma level of the AT patients was highest, it could be used as a biological marker of AT subtype (OR = 1. 02, 95% CI 1. 01-1. 03, P<0. 05); when its concentration was > 183. 5 mg/L, the sensitivity and specificity of predicting AT were 87% and 85% respectively. Conclusions The plasma hs-CRP and Lp (a) levels of patients with acute ischemic stroke may provide some help for timely and accurate etiological typing.
2.Immuno-therapeutic effect of Interleukin-2 in HIV-1 infection

Antiretroviral therapy(ART)has been started in an increasingly earlier period in clinical practice.However,owing to the failure of immunological reconstitution or maintenance of latent HIV-1 reservoir in present antiviral treatment,interleukin-2(IL-2)is widely used as an adjuvant to ART with a view to intensifying immunological reconstitution and to activating latent HIV-1 infections.Some conclusions could be drawn as follows,1)IL-2 can extend the half-life of CD+4 T cells and increase the number of CD+4 T cells.2)Compared with ART alone,IL-2 and HAART combination can not reduce HIV-1 related opportunistic infection or death.IL-2 clinical usage is mainly focused on purging HIV-1 reservoir.3)IL-2 combined with ART could increase out flow rate of HIV-1 latent reservoir to reduce HIV-1 infected quiescent memory cells in peripheral circulation even to an undetectable level.4)IL-2 is supposed to be used periodically and for a long period of time.
3.Random control test-based studies on treatment of infantile mycoplasmal pneumonia with traditional Chinese medicine:A review

4.Effect of hemofiltration on severe acute pancreatifis

Objective To investigate the therapeutic effect of hemofiltration (HF) on severe acute pancreatitis (SAP). Methods The data of 60 patients with SAP was retrospectively reviewed in terms of life vital signs, biochemical indexes and prognosis within 96 h. 30 cases (the average APACHE Ⅱ score was 16.64 ±4.21 ) underwent HF on the basis of traditional treatment while another 30 cases ( the average APACHE Ⅱ score was 15.05 ± 3.78) were treated traditionally. Results From the beginning to 96 h after treatment, vital signs and the biochemical indexes such as body temperature, respiration rate, heart rate, blood electrolytes, blood pressure, liver function and renal function were improved more obviously in patients who received HF treatment than those who only received traditional treatment. The hospitalization time was positively correlated with the disease onset time. Patients receiving HF treatment need shorter hospitalization time than patients receiving conventional treatment. There was no significant difference in terms of mortality rate between HF treatment group and conventional treatment group ( 16. 7% and 30% respectively). Conclusions HF treatment can improve some biochemical indexes and symptoms in SAP patients. For example, body temperature, respiration rate, heart rate,blood electrolytes, liver function, kidney function, lung function, abdominal pain and abdominal distension. It can shorten hospitalization time. However, HF has no significant effect on mortality rate.

8.Effect of Metformin on Sex Hormone,Insulin Resistance and Related Indexes in Women with Polycystic Ovarian Syndrome

OBJECTIVE:To investigate the effect of metformin on sex hormone level,insulin resistance and related indexes in women with polycystic ovarian syndrome(PCOS). METHODS:79 PCOS women were collected as PCOS group,other 86 healthy women as control group. All patients received 0.85 g Metformin tablet,twice a day,for totally 6 months. Boby mass,BMI,waist circumference,hip circumference,ratio of waist to hip,glycated hemoglobin,T,DHEA-S,E2,LH,FSH,PRL,PROG,SHBG, FPG,FINS,LH/FSH,HOMA-IR and FAI before and after treatment in PCOS group were observed and compared with control group. RESULTS:During treatment,2 patients made withdrawal because of pregnancy,3 made withdrawal because of showing gastrointestinal side effects,2 were lost to follow,totally 72 completed it. After treatment,body mass,BMI,waist circumference, hip circumference,glycated hemoglobin,FINS,HOMA-IR,T,LH,LH/FSH,PROG,FAI in PCOS group were significantly low-er than before,SHBG was significantly higher than before,glycated hemoglobin and PROG were lower than control group,while waist circumference,FINS,HOMA-IR,LH/FSH and FAI were higher than control group,the differences were statistically signifi-cant (P<0.05). There was no significant difference in ratio of waist to hip,FPG,DHEA-S,E2,FSH and PRL before and after treatment(P>0.05). CONCLUSIONS:Metformin can effectively improve the sex hormone levels and insulin resistance of patients with PCOS.
9.The Selection of Transdermal Release Condition and Percutaneous Enhancer of Oxaprozin Gel in Vitro

OBJECTIVE: To observe the effect of drug loading, medium composition, transdermal enhancer, lauryl alcohol and azone on percutaneous permeation of oxaprozin gel in vitro.METHODS: Drug permeation test was carried out by using modified Franz-type double compartment diffusion cell and isolated mice skin in vitro as transdermal barrier.RESULTS: Under the condition of an effective area of 5.77cm2, it was found that the result was stable and the reproducibility was well with drug loading more than 1.2g, the receiver solution ethanol to normal saline=7∶ 3(v∶ v);The transdermal enhancing effect of lauryl alcohol was superior to that of azone, the effect of mixed transdermal enhancer 3% AZ+ 10% LA was the best.CONCLUSION: The selection of both the optimum release condition and the best penetration enhancers provided reference for oxaprozin transdermal delivery.The standardization of gel percutaneous test in vitro was discussed preliminarily.
10.Therapeutic Effect of Ulinastatin Combined with Xingnaojing Injection for Acute Cerebral Hemorrhage

Objective To observe the protective effect to Ulinastatin combined with Xingnaojing Injection (XI) for acute cerebral hemorrhage. Methods One hundred and eight cases of acute cerebral hemorrhage were randomly divided into treatment group ( N=56) and control group ( N=52) . All of the patients were given conventional western medical treatment including relieving cerebral edema, regulating blood pressure, maintaining electrolyte balance, anti-inflammation, preventing epilepsy. Additionally, the control group was given intravenous drip of Ulinstatin, and the treatment group was given intravenous drip of Ulinstatin and XI. The clearance of intracranial hematoma in the two groups was observed on treatment day 3, 7, 14, the absorption of cerebral edema was observed after treatment for one treatment course of 14 days, neurological deficit scores ( NDS) were compared on treatment day 14, 30, and the clinical efficacy was evaluated. Results ( 1) After treatment, hematoma area was reduced in the two groups ( P<0.05 compared with that before treatment); on treatment day 3, the difference of hematoma area was insignificant between the two groups (P>0.05); on treatment day 7 and 14, hematoma area in the treatment group was less than that of the control group, the difference being significant (P<0.05) . (2) On treatment day 14, the total effective rate for absorption of cerebral edema was 89.3% in the treatment group, and was 65.4% in the control group, the difference being significnat ( P<0.05) . ( 3) After treatment, NDS of the treatment group was less than that of the control group, and the total effective rate on NDS was 89.3% in the treatment, higher than 71.2% in the control group, the difference being significant ( P<0.05) . ( 4) In the treatment group, 3 cases had slight increase of aminotransferase. Eight cases of the control group had increased aminotransferase, and then the aminotransferase level recovered to normal after suspension. In the treatment group, 2 cases were dropped out for receiving emergency operation due to rehaemorrhagia during the treatment, and 3 cases were death for illness deterioration. In the control group, 7 cases gave up the treatment for illness deterioration and then were dead during the follow-up. Conclusion Ulinastatin combined with XI shows certain protective effect in treating acute cerebral hemorrhage.
