1.Over-treatment and under-intervention for acute pancreatitis

There was never a time in history that so many disciplines and specialists participate and try to lead the clinical management for acute pancreatitis. Chaos remains at bedside, and episodes of under-intervention and especially over-treatment frequently happened. Physicians are easily confused by questions and controversies in management for acute pancreatitis. We have reviewed the articles in acute pancreatitis and made our recommendations based on the latest evidences.
2.Treatment and prognostic predictor of 28 cases with malignant melanoma of the head and neck

Purpose:To determine the treatments and prognostic predictors of malignant melanoma of the head and neck. Methods:Surgical treatment and outcome was analyzed for 28 patients with malignant melanoma of the head and neck at department of the head and neck of our hospital (1984-1994). Results:23 patients underwent local excised as the first treatment, and local recurrence was demonstrated in 8 (34.8%). Margin of resection of primary melanoma was divided into narrow excision (≤2 cm) and wide excision (>2 cm), and 5-year survival was 58% and 40%(P<0.01). According to surgical methods, 5-year survival of WE, WE/END and WE/TLND were 80%, 80% and 39%. Conclusions:An excision with 2 cm margin is safe procedure for primary tumor of malignant melanoma of the head and neck. Neck dissection may not be practiced until neck lymph node is positive. Neck dissections differ from the positions of the primary tumors, and the flaps must be raised preferably superficial to the platysma.
3.Promoting innovative basic research in eye science by utilization of modern biomedical tools, technologies and strategies

Efforts are being made to build up platforms to support basic research of ophthalmology by organizing capable staff,adequate instruments,technical assistance,and by applying practical evaluation and cultivating healthy cultural environment.While significant progress has been achieved with scientific publications and international academic exchanges,the ophthalmology society still has a long way to go to enhance the basic research in the field of eye science.Utilization of modern biomedical tools,technologies and strategies will undoubtedly further improve the efficacy and production in the area of basic research of ophthalmology in this country.

5.Surgical Intervention Combined with Steroid Therapy on Persistent Jaundice in Infantile Hepatitis

Objective To investigate the effect of surgical intervention combined with steroid therapy on infantile hepatitis(INS) with persistent jaundice.Methods Twenty-two patients (19 males,3 females,aged 2-6 months) with persistent jaundice(therapy group) were admitted into hospital in the period of Jan.2007-Dec.2008.The patients were performed with surgical intervention after they were confirmed with diagnosis as INS.Then,sodium chloride,gentamicin and Dexamethasone were used to irrigate the biliary tract during and after the operation for 14 days.Three days after operation,20 mg,15 mg,10 mg,5 mg of methylprednisolone were administered intravenously to the patients every 3 days,followed with 4 mg/(kg?d) prednisone by oral for 2-3 months.The 17 cases of INS with persistent jaundice were treated with medicine as control(control group).By following-up,the jaundice free and 2 years survival rate of 2 groups were compared by counting the cases of jaundice free and recording the survival time.Results Two cases of 22 patients performed with surgical intervention were diagnosed as biliary atresia and others were INS,90.0% patients were free of jaundice in surgical intervention combined with steroid therapy group,which was higher than that in control group(52.9%,P

7.Expression and clinical significance of PTEN, Fas and DCR3 in gastric cancer

Objective To study the expression of PTEN,Fas and DCR3 in human gastric cancer and evaluate their effects on carcinogenesis,cell proliferation,invasion and metastasis of gastric carcinoma,as well as clinical significance.Methods The expression of PTEN,Fas and DCR3 were detected by immunohistochemical method (streptavidin-peroxidase) in the tissues of 75 primary gastric carcinomas and 15 gastric ulcers.Results The positive expression rate of DCR3 in gastric carcinoma tissues was significantly higher than that in normal tissues,and the expression of PTEN and Fas was obviously lower than that in normal tissues (P<0.01).The expression of DCR3 was negatively correlated with Fas and PTEN respectively (r=-0.720,P<0.001; r=-0.336,P<0.001).There was a positive correlation between PTEN and Fas.The expression of PTEN,Fas and DCR3 was closely correlated with lymph node metastasis,clinical stage,histological differentiation grade and tumor depth of gastric cancer,but not related to sex,age and tumor size (P>0.05).Conclusion PTEN,DCR3 and Fas have mutual restrictive relationships in the process of invasion and metastasis in gastric cancer.Detecting the expression of PTEN,Fas and DCR3 in gastric cancer is helpful to judge the malignant biological behavior of gastric cancer at a different angle,which may be considered as reliability index in clinic diagnosis,judging curative effect and monitoring the prognosis of gastric carcinoma.
9.Research on visualization of ultrasonic rhinitis therapeutic apparatus based on V4L2.

This paper reports the study and design of a visualization system of the ultrasonic rhinitis therapeutic apparatus with the function of endoscopic sinus. On Linux operating platform, we captured the video data of a video capture card that connected the endoscopic sinus using Video4Linux (V4L2) that was provided by the operating system. The video images were displayed by Qt. The visualization system solved the problem that the current ultrasonic rhinitis therapeutic apparatus had to rely on the large and expensive endoscopy equipment. And this simplified the doctors' operation, met the need of monitoring nasal cavity in the process of operating, greatly reduced the costs of treatment and would contribute to the promotion of the instrument. As a result, it has been tested that the nasal endoscopic image achieved by the system is clear and smooth, and fully meet the clinical needs of ultrasonic treatment of rhinitis.

10.Updated treatment of erectile dysfunction after prostatectomy.

The incidence rate of erectile dysfunction (ED) is reportedly as high as 30-90% after radical prostatectomy for prostate cancer, which seriously affects the patients' quality of life. Penile rehabilitation is defined as the use of any drug or device at or after radical prostatectomy to maximize erectile function recovery. A variety of treatment options are available for post-prostatectomy ED patients, including oral phosphodiesterase 5 inhibitors (PDE5I) , intracorporal injections, vacuum erection device, and penile prosthesis. This article presents an overview of the currently used methods for the drug treatment and penile rehabilitation of the ED patients after radical prostatectomy. It seems proper to recommend daily use of a vacuum erection device plus oral PDE5I in the early postoperative period. For those who fail to respond to this therapy, intraurethral alprostadil, intracorporal injections, or a penile prosthesis could be considered.
