1.EXPERIMENTAL EVALUATIONS OF THE EFFICIENCY OF OCEAN 79-1 SPONGE HEMOSTATS

Hemostatic experiments were taken by melting the Fugu gelatin with calcium chloride powder on the femoral artery bleeding of the laboratory animals. The powder prescription Gel-B showed better and more effects on stopping the femoral arterial hemorrhage than that of A-25, A-7 hemostats and gauge hemostat, with repetiton rate 87.03% at 6 minutes. There were no allergeic and poisoning reactions on experiments animals.
2.SOMATOTOPICAL LOCALIZATION OF THE RAPHE—SPINAL PROJECTION IN THE RAT A RETROGRADE HRP STUDY

Retrograde HRP technique was used to study the somatotopical organization of the raphe—spinal projection of the rat. HRP or wheat germ agglutinin conjugated HRP was injected into unilateral gray matter or dorsal horn of the cervical cord or lumbar cord. The following conclusions have been reached.The caudal part of NRM projects to areas ventral to the dorsal horn. Projection to the dorsal horn was found to originate from the rostral part of NRM, which is somatotopically localized. Cells projecting to the cervical dorsal horn are distributed more rostrally than those to the lumbar dorsal horn, although an extensive overlapping of these two parts is evident. Somatotopical localization in the form of "gap" as suggested by Watkins et al. could not been verified in our experiment.The functional significance of the sornatotopical organization of NRM is discussed.

The reform for medical insurance payment was the important method for standardizing medical service provider behavior,controlling costs and improving efficiency,which was also the important measurement for further improve and implement the reform of drug and health system.Since the new medical reform in 2009,the central and local government had paid more attention on the role of medical insurance payment reform.Local medical departments of different places implemented reform pilots.Through summarizing the foreign and domestic researches around capitation,bundle payment and global payment,the experiences and effectiveness of these pilots were evaluated so as to provide corresponding policy implications.
4.Three Cases of Cutis Marmorata Telangiectatica Congenita.

We report 3 cases of unusual cutis marmorata telangiectatica congenita (CMTC). The ages of the first two cases of CMTC were premature babies, born at intrauterine pregnancy of 31 weeks and 34 weeks. the other patient was a 45-year old female. Compared to the age of other cases reported in the literature our patients ages are unusual. It is not surprising that the first two cases of premature babies had typical signs because CMTC is a congenital disorder, usually seen at birth, but the fact that the skin lesion of CMTC appeared already in the early third trimester may provide some clues to the pathogenesis of CMTC. The third case seems to be very rare because most reported cases show that the skin lesion usually disappear gradually over a period of months to years. The site and size of the lesion in the last case was told to have been fixed for 45 years without any change.

8.A clinical analysis of 32 patients with diffuse alveolar hemorrhage in diffuse connective tissue diseases

Objective To provide clues to diagnosis and treatment for diffuse alveolar hemorrhage (DAH)in patients with diffuse connective tissue diseases(CTD).Method To analyze restropeetively the data of clinical features,pulmonary images and bronchoalveolar lavage(BAL)in 32 patients hospitalized in Peking Union Medical College Hospital from April 2004 to June 2007.Results The data from 1O of the 17 (58.8%)patients with microscopic polyangiitis(MPA),19 of the 1267(1.5%)patients with systemic lupus erythematosus(SLE),2 of the 56(3.6%)patients with Wegener's granulomatosis(WG)and 1 of the 570(0.2%)patients with rheumatoid arthritis(RA)were consistent with the diagnosis of DAH.DAH in SLE occurred in younger patients(mean age at the time of diagnosis 27.3±13.1 years)and early in the course of disease(median duration of SLE from onset was 16.7±18.3 months),while these figures in MPA patients with DAH were 50.1±20.7 years and 10.6±18.7 months.At the time of DAH in SLE.the median systemic lupus erythematosus disease activity index(SLEDAI)score was 17.1±6.7 and anti-ds-DNA antibody titer elevated markedly,while the median C3 level was low.The levels of erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP)of patients with DAH in MPA,WG and RA showed marked elevation.The titer of antieutrophil cytoplasmic antibody(ANCA,MPO/PR3)in patients with MPA and WG was highly positive.The main clinical manifestations of DAH were hemoptysis,dyspnea and rapid decrease of hemoglobulin and hematoerit(HCT)in peripheral blood.Most patients presented with difluse alveolar infiltration on chest X-ray and hish resolution CT.DAH could be confirmed by bloody bronchoscopic lavage.20 patients(62.5%)had secondary pulmonary infections at the time of DAH;fungus and combined bacterial infection were most frequently seen.The mortalitv 0f CTD with DAH was 59.4%(19 out of 32).12 patients (63.2%)with SLE,5 patients(50%)with MPA and both of the 2 patients with WG died.12 of the lethal cases(63.2%)died of respiratory failure.Conclusions CTD patients presenting with hemoptysis and dyspnea with rapid decrease of hemoglobulin,and diffuse alveolar infiltration on chest X-ray or high resolution CT should be seriously considered to be suffering from DAH.A bloody BAL may confirm the diagnosis of DAH.DAH in CTD is an acute,serious and frequently life-threatening situation resulting in respiratory hilure and pulmonary infection.It is important for CTD patients with DAH to be diagnosed early and treated vigorously.
9.Investigation of the minimum required months for imprecision analysis

Objective To discover and certify the minimum required months of IQC ( Internal Quality Control ) data which were used to quantify the imprecision To identify the impact of test items , different concentrantions and years on the defective rate .Methods IQC data involving 20 analysis items ( including Albumin, Creatine kinase, Total bilirubin, Alanine aminotransferase, Aspartate aminotransferase, Lactate dehydrogenase,γ-Glutamyl transferase, Alkaline phosphatase, Calcium, Chlorine, Creatinine, Glucose, Potassium, Sodium, Phosphorus, Triglyceride, Total cholesterol, Total protein, Uric acid, Urea) and six measurement systems (including Roche, Architect, Hitachi, Dade/Behring, Beckman, Olympus) were collected from hospitals in Shanghaibetween 2009 and 2011.A total of 3 534 groups, referred to one year laboratory′s IQC data of one concentration range , were analysed to find the minimum required months in each group when the cumulative months were compared with the population by using T test.The correlation coefficient of hospital′grades, measurement levels and test items were evaluated by u test, and the percentage of groups of P>0.05 were collected.The cumulative IQC data′coefficient variation ( CV) of six months and eleven months were compared with the total CV, respectively .Imprecision higher than the professional specification was regarded as unqualified .Difference of unqualified rate among test items, concentrations and years were expolored .Results Rates of hospital′grades, measurement levels , items are 94.2%,100%, 100%respectively , 88%of groups′imprecision became stable in ten months .25%groups reach the criteria that the relative bias is ≤5% when calculated the cumulative IQC data′CV of six months, while 88%groups do when calculated the cumulative IQC data′CV of ten months.The unqualified rate is 20%and the most unqualified item is Ca , the unqualified groups of low level are larger .With increase of year , the unqualified rate showed a downtrend .Conclusions Ten months′IQC data is more reliable to quantify imprecision .Unqualified rates are different according to test items , measure levels and years , the establishment of professional specification should consider them.
10. Inhibition of rapamycin on proliferation of human acute promyelocytic leukemia HL60 cells

OBJECTIVE: To investigate the effect of rapamycin on proliferation, autophagy and apoptosis of human acute promyelocytic leukemia HL60 cells. METHODS: The inhibitory effect of rapamycin on HL60 cell was detected by MTT assay. The expressions of LC3-II autophagy-related protein were determined by Western blot. The cell cycles were analyzed by flow cytometry (FCM). Apoptosis were detected by DNA agarose gel electrophoresis. RESULTS: After treatment with rapamycin of 0, 1, 5, 10, 20, 40 nmol · L-1 for 24 h, proliferation of HL60 cells were inhibited in a dose-dependent manner (r=0.97, P < 0.05). Western-blot shows that the expression of LC3-II protein of each concentration group was higher than that of the control group (P < 0.01). Rapamycin has a role in promoting cell autophagy. Compared with control group, more cells were arrested at G0/G1 phases (P < 0.05) and fewer cells were at S phases (P < 0.05). Agarose gel electrophoresis shows no apoptosis DNA fragments. CONCLUSION: In given concentration range, rapamycin can inhibit the proliferation of human acute promyelocytic leukemia HL60 cells by promoting autophagy rather than inducing apoptosis.
