1.Clinical characteristics of neonatal convulsion

Objective To explore the etiologies, clinic symptoms and the methods of emergency therapy for neonatal convulsion. Methods Eighty-four patients with convulsion in pediatric department in our hospital were reviewed,analysed and summarized their clinic informations,first-aid measures. Results The etiologies were HIE(hypoxia-ischemic encephalopathy)41 cases(48.8%),ICH(intracranial hemorrhage)16 cases(19.0%),and hypocalcemia 9 cases(10.7%),the quantity of these patients added up to 2/3 of all. Conclusion The primary measures to reducing incidence of the diease were as follows:do well health protection during perinatal period,treat betimes aiming at etiologies,prevent fleetly paroxysm of convulsion.
2.Losartan in the Treatment of Essential Hypertension

To investigate the efficacy and safety of losartan in the treatment of essential hypertension, 81 patients were randomly assigned to two groups. Patients in the first group(n =41) were given losartan 50 mg to 100 mg once a day; those in the second group( n = 40) were given benazepril 10 mg to 20 mg once a day. The treatment course lasted for a weeks. Ambulatory blood pressure, hepatic and renal functions, blood glucose were examined before and after therapy. Our results showed that the total efficacy rate in losartan group was 93% while that in benazepril group was 90%. The efficacy was similar between the two groups. However, effect of 24 h blood pressure control by lossrtan was superior to than of benazepril. The adverse reaction of losartan was milder that that of benazepril. It is concluded that losartan is a safe, long-acting antihypertensive agent for mild and moderate hypertension with good patient tolerance and less adverse reactions.
3.Features of human umbilical cord mesenchymal stem cells cultured on silk fibroin porous scaffolds in vitro

Objective To observe the attachment,shape,function and activity of human umbilical cord mesenchymal stem cells cultured on silk fibroin porous scaffolds in vitro,and to provide experimental foundation for the choice of scaffold in the study of adipocyte tissue engineering.Methods These silk fibroin porous scaffolds were subsequently seeded with hUCMSCs and cultured in vitro.The growth and function of hUCMSCs were observed and measured with fluorescence inverted microscopy,scanning electronic microscopy and MTT methed. Results hUCMSCs were fixed on silk fibroin porous scaffolds 1 or 2 days later,and multiplicative growth was observed on the 5th to 7th day.After about 10 days,the microholes of the scaffolds were overlayed by hUCMSCs.Scanning electronic microscopy and fluorescence inverted microscopy showed that cells adhered to scafold well and there was a lot of extra cellular matrix surrounding cells. Conclusion Silk fibroin porous scaffolds are ideal for attachment,growth,function maintenance and activity of hUCMSCs.and the scaffolds can be used as natural scaffolds for hUCMSCs in 3D culture.
4.Mechanisms of hydroxypropyl methylcellulose for the precipitation inhibitor of supersaturatable self-emulsifying drug delivery systems.

Hydroxypropyl methylcellulose (HPMC) propels self-emulsifying drug delivery systems (SEDDS) to achieve the supersaturated state in gastrointestinal tract, which possesses important significance to enhance oral absorption for poorly water-soluble drugs. This study investigated capacities and mechanisms of HPMC with different viscosities (K4M, K15M and K100M) to inhibit drug precipitation of SEDDS in the simulated gastrointestinal tract environment in vitro. The results showed that HPMC inhibited drug precipitation during the dispersion of SEDDS under gastric conditions by inhibiting the formation of crystal nucleus and the growth of crystals. HPMC had evident effects on the rate of SEDDS lipolysis and benefited the distribution of drug molecules across into the aqueous phase and the decrease of drug sediment. The mechanisms were related to the formed network of HPMC and its viscosities and molecular weight. These results offered a reference for selecting appropriate type of HPMC as the precipitation inhibitor of supersaturatable SEDDS.
5.Tissue-engineered skin constructed by adipose-derived stem cell sheet

BACKGROUND:Wtih hyaluronic acid and col agen by cellautocrine as endogenous scaffolds, cellsheet technology can eliminate the disadvantages of exogenous scaffolds. Human adipose-derived stem cells combined with cellsheet technology used in the study of ful-thickness skin defects have become the current hot spot.
OBJECTIVE:To summarize the research progress in adipose-derived stem cellsheet.
METHODS:Articles related to adipose-derived stem cells, cellsheet technology and tissue-engineered skin were retrieved in VIP information and PubMed database from January 1993 to April 2014. The key words were“adipose-derived stem cell, wound healing, tissue-engineered skin, cellsheet”in English and Chinese, respectively. Total y 27 relevant articles were analyzed.
RESULTS AND CONCLUSION:Adipose-derived stem cells derived from discarded human adipose tissue are immmunocompatible and multi-potent, rendering them ideal for regenerative medicine applications. cellsheet technology seemingly provides several advantages over traditional tissue reconstruction with biodegradeable scaffolds. Adipose-derived stem cells combined with cellsheet technology can enhance tissue vascularization, promote wound healing, the secreted extracellular matrix provides the microenvironment for the celldifferentiation and growth, so it is expected to build a real three-dimensional tissue-engineered skin.
6.Effect of Acyclovir on the Neurological Function and Cytokines of Children with Viral Meningitis

OBJECTIVE:To observe the effect of acyclovir on neurological function and cytokines of children with viral menin-gitis. METHODS:Totally 70 children with viral meningitis were randomly divided into control group and observation group. All children were given routine treatment,including defervescence,reducing intracranial pressure and regulating water and electrolyte balance,etc. Based on it,the control group was treated by Ribavirin glucose injection 15 mg/kg,iv,bid;observation group was treated by Acyclovir glucose injection 5 mg/kg,iv,tid. The course for both was 7 d. The clinical data was compared,including the vascular endothelial growth factor (VEGF),matrix metalloproteinase-9 (MMP-9) in cerebrospinal fluid (CSF) and serum,insu-lin-like growth factor-Ⅱ(IGF-Ⅱ) and insulin like growth factor binding protein-3(IGFBP-3) in CSF before and after treatment and the incidence of adverse reactions. There were no obvious adverse reactions during the treatment. RESULTS:After treatment, the VEGF and MMP-9 in serum and the VEGF,MMP-9,IGF-Ⅱ and IGFBP-3 in CSF in 2 groups were significantly lower than before,and observation group was lower than control group,with significant differences(P<0.05). There was no adverse reactions in 2 groups during the treatment. CONCLUSIONS:Compared with ribavirin,acyclovir can more obviously improve the neurological function and cytokines of children with viral meningitis,with similar safety.
7.A report of seven cases of pulmonary sequestration complicated by aspergillosis and literature review

Objective Pulmonary sequestration (PS) is a rare disease,and its clinical symptoms are usually related to subsequent pulmonary infections.We analyzed the clinical characteristics of PS complicated by aspergillosis,and reviewed related literature,so as to disclose the association between these two diseases.Methods Sixty-nine patients with surgery-confirmed PS in Peking Union Medical College Hospital between January 1990 and December 2013 were retrospectively analyzed,including seven cases complicated by aspergillosis.Clinical manifestation,imaging and surgery of these patients were analyzed.Literature focusing on PS complicated by aspergillosis in pubmed data base was reviewed.Results In these seven patients,four cases were male,and three cases were female.Age at diagnosis ranged from 29 to 58 years old.The interval from onset to definite diagnosis ranged from two weeks to 20 years.Clinical symptoms included productive cough in seven cases,hemoptysis in three cases,chest pain in two cases,and fever in one case.All cases were intralobar PS with four in the left lower lobe and three in the right lower lobe.Consolidation in chest CT was noted in six cases.Cavitation was positive in three cases.Surgery of lung lobe resection was performed in all patients.Aberrant arteries were found during operation with the origin from aortic artery in four cases,phrenic artery in two cases,and intercostal artery in one case.Aspergillosis was diagnosed by pathology in six cases and by lung tissue culture in one case.Conclusions PS complicated by aspergillosis is extremely rare,but the trend of an increase in recent years has been noted.Strict and cautious examinations for microorganisms and pathology will help find relatively insidious aspergillosis.

10.Approximate entropy of oxygen saturation and saturation impairment time index for the assessment of hypoxemia severity in obstructive sleep apnea hypopnea syndrome

Objective To explore the clinical significance of two new indexes including approximate entropy of oxygen saturation (SpO2ApEn) and saturation impairment time index (SITi) in assessing the severity of hypoxemia in obstructive sleep apnea hypopnea syndrome (OSAHS) patients.In addition,to investigate the correlation betewwn new indexes and other parameters.Methods The six hour polysomnography (PSG) of 500 patients with OSAHS and snorers in our hospital was retrospectively analyzed.According to the level of apnea hypopnea index (AHI),subjects were divided into four subgroups,namely 113 healthy controls,121 mild OSAHS,118 moderate OSAHS,and 148 severe OSAHA patients.SpO2 ApEn and SITi values among the four groups were compared.the correlations between SpO2 ApEn,SITi and other parameters were analyzed,including AHI,oxygen desaturation index (ODI),the lowest oxygen saturation(LSpO2) and the duration of oxygen saturation lower than 90% (T < 90%).Next,taking AHI as the gold standard,the significance of SpO2ApEn and SITi in assessing the severity of hypoxemia in OSAHS patients was explored by ROC curve analysis.Results SpO2ApEn and SITi between two each groups were significantly different (P < 0.001) and increased synchronously with AHI.Spearman correlation analysis showed that SpO2ApEn and SITi had a good correlation with AHI (correlation coefficient r =0.765,r =0.678,P < 0.O1).SpO2ApEn and SITi also had a good correlation with ODI,LSpO2and T < 90%.According to the ROC analysis,the Boundary values of SpO2ApEn for mild,moderate and severe OSAHS patients were 16.70,17.81,and 20.03,respectively,and the corresponding SITi values were 3.685,4.055,and 4.445.Conclusion In this study,SpO2ApEn and SITi increased synchronously with AHI,SpO2ApEn and SITi had good correlations with AHI,ODI,LSpO2and T <90%.SpO2ApEn and SITi have important clinical significance for assessment of hypoxia severity in OSAHS patients.
