4.Curative Effect of Ribavirin Injections Combined with Tanreqing in the Treatment of Children with Epi-demic Parotitis

Objective:To study the efficacy of ribavirin injections combined with Tanreqing in the treatment of children with epi-demic parotitis. Methods:From January 2012 to September 2013, 128 children with mumps in our hospital were selected and random-ly divided into the treatment group and the control group with 64 cases in each. The control group was intravenously given ribavirin in-jections, 5-7. 5 mg·kg-1 , ivd, bid, and the treatment group was given Tanreqing injections additionally, 0. 25-0. 4 ml·kg-1 ·d-1 , ivd,qd. After the 5-day treatment, the extinction time of parotid swelling and pain, defervesce time and the decrease of TNF- α and CRP were observed in the two groups. Results: The total effective rate in the treatment group was 93. 8%, which was significantly higher than that in the control group (84. 4%) with significant difference (P<0. 05). The extinction time of parotid swelling and pain and defervesce time in the treatment group were significantly shorter than those in the control group (P<0. 05). After the treatment, TNF- α and CRP in the two groups were significantly decreased (P<0. 05), and the decrease in the treatment group was much notable than that in the control group (P<0. 05). Conclusion:Ribavirin injections combined with Tanreqing in the treatment of children with mumps can significantly shorten the extinction time of parotid swelling and pain and defervesce time, and inhibit TNF-αand CRP with significant effect, which is valuable in clinical promotion and application.

7.Application of case - based learning combined with problem -based learning teaching mode in the clinical teaching of ophthalmology

?AIM:To improve the efficiency and quality of teaching, and to cultivate students’ ability to analyze and solve clinical problems, the case - based learning ( CBL ) combined with problem-based learning ( PBL ) teaching mode is introduced into the clinical teaching of ophthalmology.?METHODS:Two classes ( total 131 students ) of 2013 grade major in clinical medicine, Kunming Medical University, were randomly selected as experimental group using CBL combined with PBL teaching mode, and control group using traditional teaching mode. The part of acute angle-closure glaucoma was taught. The scores of the experimental group were compared with that of control group at the end of term.?RESULTS:The mean scores of CBL combined with PBL teaching group were significantly higher than that of traditional teaching group.?CONCLUSION: The application of CBL combined with PBL in the clinical teaching of ophthalmology has obvious advantages in improving the learning interests and promoting students’ self-learning ability. It is helpful in improving the students’ ability of independent-thinking, analyzing and solving problem.
8.Clinical investigation of causes of vertical root fracture in pulpless teeth

Objective To investigate the cause of vertical root fracture in pulpless teeth, and advance the measures of prevention. Methods The clinical data of 20 vertical root fracture in pulpless teeth (17 patients) were collected. The status of the teeth and periodontium before therapy were reviewed, the position and direction of the root fracture were observed, the type and position of the restoration and the occludent station of the teeth were analyzed. Results It was revealed that vertical root fracture frequently occurred in the molars (16/20). Vertical root fracture were frequently found in front teeth(4/20) and premolars (5/20)among those with coronal integrity. Decrease of alveolar level of various degrees was found in 19 cases, among which 8 were mild, 7 moderate and 4 severe. Vertical root fracture mainly took place in molars after coronal restoration (6/7). Vertical root fracture would likely to occur no matter whether they would be worked as the abutment teeth of the removable partial denture or fixed prostheses. Early contact occurred in lateral bite (6 cases) was more susceptible to vertical root fracture than that occurred in centric bite (1 case). Conclusion The causes of vertical root fracture in pulpless teeth are complicated. Due to unfavorable outcomes, it is important to take precautions for vertical root fracture.
9.Clinical effect of neovascular glaucoma treated by vitrectomy and cyclophotocoagulation

AlM: To observe the postoperative intraocular pressure ( lOP) and operation safety in the eyes of the neovascular glaucoma pateints treated by intraocular cyclophotocoagulation which needed vitrectomy at the same time.
METHODS: A total of 12 neovascular glaucoma cases ( 14 eyes ) secondary to diabetic retinopathy, retinal detachment surgery and trauma were reviewed in our study. This procedure mainly used intraocular photocoagulation catheter to highlight the ciliary processes until the ciliary became white atrophy or plosion after vitreous surgery treatment. The intraocular photocoagulation catheter was performed at a power of 300-500mW, for a duration of 0. 1-0. 2ms. Postoperative follow-up was at least for 6mo. The observation of 14 postoperative neovascular glaucoma was performed at 1wk, 1, 6mo observing the lOP and complications.
RESULTS:lOP of the 11 eyes was significantly declined and controlled in normal. After cyclophotocoagulation, average lOP at 1wk was 16. 7±14. 4mmHg, 15. 7±8. 8mmHg at 1mo and 12. 9±4. 5mmHg at 6mo, which compared with untreatment ( 39. 6 ± 10. 0mmHg ) was statistically significant different (P<0. 01). ln follow up time 3 cases were relapsed which were supplied with transscleral or endoscope cyclophotocoagulation. During the follow-up period no endophthalmitis and complications such as eyeball atrophy were found.
CONCLUSlON: The intraocular cyclophotocoagulation and vitrectomy simultaneously can deal with the primary disease and secondary neovascular glaucoma. The operation can be accurately performed under direct cyclophotocoagulation and it is a safe and effective way for neovascular glaucoma which needs vitreous surgery.
10.Descriptive Study of Relationships between Depression, Family Support, and Rehabilitation-motivation in Patients with Cerebrovascular Accident.

PURPOSE: This study was conducted to identify relationships between depression, family support, and rehabilitation-motivation in patients with cerebrovascular accident. METHOD: The data were collected from 'S' hospital in Seoul from Feb. 1, 2004 to Apr. 30, 2004. The data were analyzed using a SPSS 10.0 program for descriptive statistics, Pearson correlation coefficients and stepwise multiple regression. RESULT: The results were as follows: The mean rehabilitation-motivation was 119.89, which was slightly high. The rehabilitation-motivation was influenced by religion, education level, occupation, economic status, treatment fee, duration of hospitalization, level of language ability and ADL. Rehabilitation-motivation has showed correlation with family support (r=.538, p=.000) and depression (r=-.417, p=.000). The variables related to rehabilitation-motivation were identified as family support, level of ADL, occupation, depression, and education level, using stepwise multiple regression. They were accounted for 57% of predictability of the rehabilitation-motivation, with the family support the highest at 29.3%. CONCLUSION: With increasing post stroke patients in Korea, we need to pay more attention to the rehabilitation of them. This study indicates that health professionals need to focus on the intervention especially in increasing family support and in decreasing depression of the patients with cerebrovascular accident.
