1.Repair of soft tissue defects with artificial dermis in 20 cases

In order to study and appraise the application of artificial dermis in repairing skin defect or organization damage, 20 patients underwent artificial dermis transplantation at the Department of Burn and Plastic Surgery, Beijing Jishuitan Hospital from January 2008 to April 2009 were collected. Patients with skin damage or depth portion organization (tendon, joint and bone appears externally) was perform debridement and artificial dermis transplantation, followed by split thickness autoskin transplantation. The survival of artificial dermis and split thickness autoskin was observed. After transplantation, the depth portion organization of patients were covered efficiently, all grafts survived without obvious scar proliferation. The results demonstrated that the prompt application of artificial dermis can reduce the surgery risk, relieve the surgery to damage, and reduce the scar production in repairing organization damage or depth portion organization expose.
2.Application of enteral nutritional emulsion (TPF-D) or enteral nutritional emulsion (TP) in patients with chronic wound and diabetes

Objective To compare the effectiveness of enteral nutritional emulsion (TPF-D) and enteral nutritional emulsion (TP) in patients with chronic wound and diabetes (CWD). Methods Totally 20 CWD patients in Beijing Jishuitan Hospital from June 2008 to June 2010 were enrolled in this study. Enteral nutritional emulsion (TP) was used for the first 5 days ( TP group) and enteral nutritional emulsion (TPF-D) was used for the second 5 days (TPF-D group). Changes of mean amplitude of glycemic excursions (MAGE), insulin dosage, and prealbumin (PA) were compared between TPF-D group and TP group. The adverse effects and post-operational complications were also observed. Results The every-day MAGE was (2. 56 ±0. 35) mmol/L in TPF-D group, which was significantly lower than that in TP group [ (3.23 ± 0. 42) mmol/L] ( P = 0. 01 ). The mean insulin dosage was (9.6 ± 1.7) U in TPF-D group, which was significantly lower than that in TP group [ ( 12. 2 ± 2. 5 ) U ] ( P =0.03 ). The increase of PA showed no significant difference between TPF-D group [ ( 12.7 ± 3. 3) mg/L] and TP group [ ( 13.4 ± 2. 8 ) mg/L ] ( P = 0. 08 ). No enteral nutrition-related adverse effect or post-operation complication was noted. Conclusion Compared with TP, TPF-D is more suitable for the CWD patients.
3.Effect of low molecular weight heparin on the expression of endothelial protein C receptor and protease activated receptor 1 in abdominal aortic endothelial cells of septic rats

Objective To investigate the effect of low molecular weight heparin (LMWH) on the expression of endothelial protein C receptor (EPCR) and protease activated receptor 1 (PAR1) in abdominal vascular endothelial cells (VECs) of septic rats. Methods VECs were cultured by tissue-sticking method, and the purity was determined with flow cytometry (FCM). VECs were randomly divided into three groups: control group, septic group (LPS 1 μg/ml) and LMWH group (LPS 1 μg/ml+LMWH 5 μg/ml). The VECs were collected at 1st, 3rd, 5th days after stimulated. The expression of EPCR and PAR1 were assessed by FCM. Results The expression of EPCR and PAR1 of septic group decreased significantly compared with control group at each time point (P<0.05 or P<0.01), and the expression decreased most obviously on day 5 (26.53±7.21 vs 39.26±2.62,q=6.45,P<0.01;53.21±15.10 vs 86.54±11.34,q=6.94,P<0.01). In LMWH group, the levels of EPCR and PAR1 expression were higher than setpic group at each time point (P<0.05). Compared to control group, the expression of EPCR had a significantly decrease on day 1 (40.86±1.63 vs 45.41±2.82,q=3.51,P<0.05), which had no significantly different on day 3 and 5 (41.20±3.32 vs 42.83±2.66,P>0.05;39.23±3.33 vs 39.26±2.62,P>0.05), and the expression of PAR1 were not significantly decrease compared with control group at each time point (P>0.05). Conclusions LMWH could improve the inhibition status and the expression of EPCR and PAR1 on VECs in septic rats.
4.The expression and meaning of bcl-xL in superficial bladder tumor

Objective To investigate the relations of the expression of bcl-xL in superficial bladder tumor and prognosis. Method The expression of bcl-xL was detected by envision system in 80 cases of superficial bladder tumor. Results In patients with high expression of bcl-xL, the recurrence rate was higher than that of normal expression [71.4%(30/42) vs 50.0%(19/38) ,P < 0.05], and the recurrence of time as early as normal expression [(16.0 ± 1.2) months vs (36.0 ± 4.5) months](P < 0.05). Conclusion The expression of bcl-xL could effectively predict the prognosis of patients with bladder tumor, those who with high expression of-bcl-xL have bad prognosis.
5.Nutritional risk screening in hospitalized patients with respiratory disorders

Objective To explore the nutrition risk,undemutrition rates,and nutrirlon support in the hos-pitalized patients with respiratory disorders.Methods Totally 100 hospitalized patients in department of respirato-ry medicine of our hospital from October to December 2008 were enrolled in this study.The nutrition risks were screened on the next day after admission and then two weeks later using Nutrition Risk Screening(NRS2002).Body mass index(BMI)lower than 18.5 ks/m2,serum albumin(sALB)lower than 35 g/L,or pre-ALB was lower than 20 g/L Wag regarded as undemutrition.Results Among all these 100 patients evaluated by NRS2002,59 patients were judged at the risk of malnutrition and 54 patients developed undemutrition two weekB later The nutrition risk and undernutuition rate among non-ambulatory patients were 87.7%and 82.9%,which were signifi-cantly higher than in ambulatory patients(32.2%and 30.5%)(P<0.001).The nutrition risk and undernutu-ition rate were also significantly higher in patients who stayed in the hospital for more than 2 weeks than those for less than 2 weeks(75.0% vs 9.4%and 66.2% vs 21.9%,respectively)(P<0.001).Twenty-two patients received nutritional support.The ratio of parenteral nutrition and enteral nutrition wag 1.2:1.Conclusions NRS2002 can be used in the nutrition assessment in hospitalized patients with respiratory disorders.Non-ambulato-ry and a hospital stay of more than 2 weeks increase the nutritional risk and undemutrition rate.Only a small per-centage of patients receive nutritional support.
6.Role of peritoneal transport characteristics and nutritional status in evaluation of peritoneal dialysis adequacy

Objective To evaluate the influence of peritoneal transport characteristics on nutritional status of peritoneal dialysis patients and estimate the role of peritoneal transport characteristics and nutritional status in evaluation of peritoneal dialysis adequacy. Methods Peritoneal transport properties (D/Pcr) were evaluated by Short Peritoneal Equilibration Test and then 53 cases were divided into high transport group (D/Pcr ≥0. 65) and low transport group (D/Pcr < 0. 65). Urea kinetics (Kt/v) and total clearance of creatinine (TCcr) were calcu- lated. Serum albumin (ALB), prealbumin, transferring (TF), hemoglobin (Hb), lean body mass (LBM) and LBM% were examined or calculated. Subjective global assessment (SGA) was used to evaluate the nutritional sta- tus of patients. Results There were 30 cases in high transport group and 23 cases in low transport group. D/Pcr (0. 82±0.15 vs. 0. 55±0. 08, P < 0. 01), TCcr (62. 93%±25. 98% vs. 49. 69%±16. 92% , P < 0. 05) and age were significantly higher in high transport group than those in low transport group, while ALB, TF, Hb, LBM% , and SGA were significantly lower than those in low transport group (P < 0.05). ALB was negatively cor-related with Kt/v (r= -0.2708, P<0. 05). Conclusions High peritoneal transport patients have better solute clearance but worse nourishment status compared with low transport ones. Nutritional status is one of the factors to evaluate dialysis adequacy. Nutrition management and guidance should be strengthened for the continuous ambula- tory peritoneal dialysis patients, especially for the elderly ones.
7.Mechanism of supplementation of glutamine dipeptide on improving wound healing after operation on burns

Objective To study the mechanism of glutamine dipeptide on wound healing after operations on bums. Methods Totally, 30 burned patients were randomly divided into study group and control group (15 cases in each group). All patients received parental nutrition support after operation. Study group patients were supplemented with glutamine dipeptide at a dose of 0. 5 g ·kg-1· d-1. The plasma free hydroxyproline level was measured by a standard amino acid analyzer 1 day before operation and 7 days after operation and the wound healing time was recor- ded. Results The plasma free hydroxyproline levels of both groups were higher than the normal values before opera- tion but without significant difference [control group: (2. 24±0. 84) μg/ml, study group: (2. 32±0.92) jig/ml, normal value: (1.27±0.44) μg/ml]. On the post-operative 7 day, the plasma free hydroxyproline level of study group [ (4. 31±1. 05) ng/ml] was significantly higher than that of control group [ (3. 04±1. 01) μg/ml] (P = 0.002). The wound healing time of study group [ (29. 7±5.3) d] was shorter than that of control group but with- out significant difference [ (33. 3±7.5) d, P = 0. 14 ]. Conclusion The intravenous supplementation of glutamine dipeptide may increase the plasma hydroxyproline level after operation and thus promote wound healing.
8.Values of the integrated visual and auditory continuous performance test in diagnosing children with attention deficit hyperactivity disorder in clinic and school in China

Objective To explore the psychometric properties of the Chinese version of integrated visual and auditory continuous performance test (IVA-CPT) ,and to assess the diagnosis value of the Chinese version of IVA-CPT in two samples of children with attention deficit hyperactivity disorder (ADHD) in clinic and school.Methods 112 participants were divided into 4 groups:clinic group, clinic control group, school group and control group from school.The participants were measured by IVA-CPT and the results were compared.Results ①The integrated quotients of IVA-CPT had concordance with the dimensions of the DSM-ⅣV and ASRS.Each integrated quotients of IVA-CPT and each dimension of the DSM-ⅣV and ASRS were negatively correlated(school: r=-0.277, P<0.05;clinic: r=-0.423, P<0.05).The score of HI dimension was negatively correlated with integrated attention quotient and integrated inhibition quotient of IVA-CPT (r=-0.480, P<0.01;r=-0.452, P<0.01).② The results of the IVA-CPT and the comprehensive assessment had consistency for diagnosis of children with ADHD from the total subjects,the clinic ,and the school.And there were no difference between the sensitivity, the specificity, the coincidence rate, the missed diagnosis rate and the misdiagnosis rate of IVA-CPT when used to diagnose children with ADHD in clinic and school(x2=3.396, P>0.05).③The IVA-CPT evaluation system had good stability according to the retest of a random sample of 25 subjects(P<0.01).The pretest and posttest results of each full quotients of IVA-CPT were significantly correlated (P<0.01).Conclusion The IVA-CPT not only can be used as auxiliary diagnostic tools to diagnose children with ADHD in clinic,it can be used as auxiliary diagnostic tools to diagnose children with ADHD in the community.
9.The effect of silencing Sp1 gene on proliferation of colon carcinoma cell SW620 by RNA interference

Objective To inhibit the expression of transcription factor special protein 1(Sp1) through RNA interference (RNAi) technique and to investigate its impact on the proliferation ability of colorectal cancer cell line SW620. Methods The recombinant plasmid of Sp1 RNAi (pGenesil-1-Sp1) was constructed and transfected into SW620 cells by Lipofectamine. The transfcction efficiency was observed under fluorescence confocal microscopy. Expression levels of Sp1 mRNA and protein from SW620 after transfection were examined by real time PCR and Western blot respectively, after transduction of the recombinant plasmid into the SW620. The proliferation ability of SW620 cell line was evaluated by MTT assay. Results The expression plasmid (pGenesil-1-Sp1) against Sp1 was successfully constructed, recombinant vectors could reduce the expressions of Sp1 mRNA and protein in SW620, the ratio of inhibition of the expression of Sp1 mRNA and protein was 68.47 % and 73.82 % in 48th hour respectively. Compared with the control group, the difference was significant (P <0.05). MTT showed that the proliferation ability of SW620 cell was degraded. Conclusion Silencing Sp1 gene by the RNAi technology can actively inhibit the proliferation of SW620 cell. The successful application of Spl SiRNA extends the list of available therapeutic modalitics in the treatment of human colon cancer.
10.Inhibitory effects of AP-2α on proliferation of colon cancer cell of SW620 and its mechanism

Objective To study inhibitory effects of transcription factor activator protein-2α(AP-2α)on proliferation of colon cancer cells in vitro and its mechanism. Methods The peDNA3.1 (+)-AP-2α recombinant plasmid was constructed. Plasmid pcDNA3.1(+)- AP-2α and pcDNA3.1(+)was transfected into SW620 cell by liposome mediation for transient expression, and proliferative activities of SW620 cell were evaluated by MTT assay. The change in the mRNA and protein expression level of ER-β before and after transfection was detected using the methods of Real-Time PCR and Western blotting respectively. Results The mRNA and protein expressions of AP-2α could be enhanced by transfecting of AP-2α gene in SW620 cell. MTT assay indicated: the proliferation velocity of SW620 cell for transfection of the pcDNA3.1(+)-AP-2α plasmid was apparently inhibited. The expression of ER-β in SW620 cell increased significantly after AP-2α gene transfection. Compared with control group, the difference was significant (P<0.05). Conclusion Overexpression of AP-2α inhibits the proliferation of SW620 cell in vitro, which is probably related with activation of ER-β.
