1.Immunomodulation of polypeptides from Chlamys farreri(PCF) in mice

Objective To investigate the immunomodulation of PCF on the immunosuppressed mice. Methods The immunosuppressed model was established by injection of dexamethasone (DEX) intraperitoneally. Effects of PCF on the immunity were studied by the observation of morphology of immune organ, the analysis of T lymphocytes subsets and the measurement of lymphoproliferative ability. Results PCF could markedly improve the low immunoreaction of immunosuppressed mice caused by DEX. It was showed that the white pulp of spleen was enlarged, the quantity of T lymphocytes of the peripheral blood was increased and the response of lymphoproliferation to ConA was enhanced in the test group compared to those in the model group. Conclusion PCF had the abilities to up-regulate the immunity in vivo,which may be due to its influences on the peripheral mature immunocytes.
2.Pharmacokinetic of berberine in combination of Cortex Phellodendri and Rhizoma atractrylodis in rat

AIM: To use seven parts cortex phellodendri and one part Rhizoma atractrylodis to extract mixedly,and study mouse's pharmacokinetic parameter of berberine derived from extract. METHODS: Evodiamine was taken as the marker,HPLC was used to determine berberine content in rat plasma. RESULTS: The linear range of rat's plasma sample was within 0.006 6-0.211 2 ?g/mL,RSD was little than 10%,detection limit was 0.0033 ?g/mL.The main pharmacokinetic parameters consisted of 2 h(T_max),0.183 mg/L(C_max),10.87 h(MRT_0-24),21.562?4.448(MRT_0-∞). CONCLUSION: Berberine in combination of cortex phellodendri and Rhizoma atractylodis can be quickly absorb and achieve the peak value,but metabolize slowly.
3.Berberine-induced apoptosis in leukemia Molt-4 cell line and expression of NF-κB/p65 and Caspase-3

[Objective] To study the inhibition effect of berberine on cell proliferation in acute T lymphocytic leukemia cell line Molt-4,and its mechanisms.[Methods] The logarithmic growing cell lines were incubated with 0,10,100 μg/ml berberine for various hours.The apoptosis induced by berberine was detected by flow cytometry,eletrophoresis and microscopy.The protein pro-Caspase-3 and p65 were determined by Western bolt assay.[Results]Treated with berberine,Molt-4 cells were found with apoptosis bodies by microscopy.By flow cytometry,it was found that number of G2/M cells was increased and number of S cells was reduced.Necrosis band was found in DNA ladder electrophoresis.The expression of pro Caspase-3 was activated partly and p65 was down-regulated in Molt-4 cells treated with berberine.[Conclusion]Berberine can induce Molt-4 cells apoptosis, and NF-κB、Caspase-3 have been shown to regulate the apoptosis programme.
4.Cognitive study on the practicing environment of physicians in Shanxi province

Objective To analyze the views and perceptions of practicing physicians on the practicing environment in the province,in order to provide policy reference for improvement.Methods By means of equi-probability and multi-stage sampling, 3 570 valid questionnaires were collected and analyzed from site survey which consist of the subj ective evaluation,and measures to identify and prevent medical professional risk.Results Overall assessment of the practice environment of physicians in Shanxi province is low,as those holding the environment asniceandacceptableaccounting for 4.8% and 21.7%,only 47.0% of physicians in tertiary hospitals hold the environment as very bad,while only 6.1% of those in level-1 hospitals share this view.92.4% of the physicians surveyed hold compliance of laws and regulations and technical specifications as key to prevention of medical dispute,yet most of them do not expect the media to be obj ective,fair and accurate in their coverage of events.Conclusions It is recommended to strengthen communication with the media for obj ective coverage of the limits of medicine,and explore the mechanism to take care of medical disputes by physicians,and improve their medical risk control capabilities,for a better practicing environment.
5.Post-operative complications and measures on complex cyanotic congenital heart diseases and pulmonary ;shunt

Objective To analyze the causes and treatment methods of early complications after central systemic-pulmonary shunt in complex cyanotic congenital heart diseases.Methods Two hundred and twelve cases of central systemic-pulmonary shunt in complex cyanotic congenital heart diseases were retro-spectively analyzed in order to explore the early postoperative complications and related treatment measures. Results There were 61 cases(28.77%)of the early postoperative complications,including severe low car-diac output syndrome in 27 cases,acute pulmonary edema in 14 cases,24 h shunt pipe blockage in 12 cases, and supraventricular tachycardia in 8 cases.All patients got followed up,average for(2.49 ±1.21 )years.Af-ter the systemic-to-pulmonary artery shunts,pulmonary vascular had significant growth,8 patients(3.77%) of them who pulmonary hypoplasia were promoted by transcatheter aortopulmonary collateral vessels.At the end of the follow-up,77 patients(36.32%)achieved the standard of radical surgery.Conclusion The factors affecting surgical survival rate include:enhancement of patients cardiac function and strictly handle operation indication before operation a clear operational view;rational surgical methods;treatment of complication with-out delay;strict,intensive care and synthesized treatment.
6.Diagnosis and treatment of elbow varus posteromedial rotational instability

Objective To report the early surgical outcomes of treating elbow varus posteromedialrotational instability (EVPRI),a pattern of traumatic elbow instability which has been recently described but incompletely understood. MethodsFrom December 2009 to April 2011,11 male patients with an EVPRI pattern were surgically treated in our hospital.Their average age was 33.8(from 22 to 40) years.They had 4 left and 7 right elbows affected.All had tenderness at the medial and lateral sides of the elbow and varus angulation of the elbow without dislocation.Their preoperative stress view X-rays and CT scans showed widened humeroradial joint space and fracture of the anteromedial facet of the coronoid process.We confirmed the diagnosis by applying yarus,pronated and axial stresses onto the forearm to evoke elbow dislocation,under fluoroscopy after anesthesia.In the initial operative treatment,the coronoid was repaired with a plate and K-wires applied to the medial surface of the coronoid,and a hinged external fixator was applied at the lateral side without repairing the lateral collateral ligament.Early rehabilitation was encouraged. Results All were followed up for an average of 14.4 (from 6 to 26) months.Each obtained an excellent result according to the Mayo Elbow Performance Index and recovered excellent elbow function.The average flexion was 137.8° ± 4.4° (from 130°to 140°), average extension 5.6°±7.3° (from 0 to 20°), average range of extension-flexion 132.2°±9.7°(from 120° to 140°),average pronation 87.8°±6.7°(from 70° to 90°),average supination 88.9° ± 3.3° (from 80° to 90°),and average range of rotation 176.7° ± 10.0° (from 150° to 180°).No complications such as varus subluxation of the elbow,infection and arthrosis occurred in this group. Conclusions Since EVPRI is a distinct type of elbow fracture-dislocation that must be recognized and adequately treated to restore good elbow function,inadequate or conservative treatment may cause subluxation,arthrosis or a poor outcome.Surgical treatment can achieve an excellent early outcome and avoid severe complications.

8.Comparative study on effect of coronoid fixation with lateral collateral ligament repair versus hinged external fixator in treatment of elbow varus posteromedial rotational instability

Objective To compare the effect of coronoid fixation combined with lateral collateral ligament repair versus hinged external fixator in treatment of elbow varus posteromedial rotational instability.Methods This retrospective cohort research included 34 patients with elbow varus posteromedial rotational instability operated between January 2011 and June 2015.All patients had coronoid process fractures of O'Driscoll type Ⅱ[(six with subtype 1,24 with subtype 2 and four with subtype 3).Fifteen of the 34 patients were operated by coronoid fixation combined with lateral collateral ligament repair (Group A) and other 19 patients were operated by coronoid fixation combined with placing hinged external fixator (Group B).Interval between injury and operation,operation time and blood loss were recorded.At final follow-up,elbow range of motion,Mayo elbow performance score (MEPS) and Hastings and Graham heterotopic ossification classification were measured.Results There were no significant differences in the interval between injury and operation,operation time and blood loss between the two groups (P >0.05).Median period of follow-up was 30 months in Group A and 40 months in Group B.Last follow-up showed flexion of the affected elbow in Group B [145° (135°-150°)] was better than that in Group A [140° (130°-145°)] (P < 0.05),while between-group differences were insignificant in elbow extension,elbow extension-flexion,forearm pronation-supination and heterotopic ossification classification (P > 0.05).MEPS in Group A scored 100 in 10 patients,90 in three,85 in one and 70 in one;MEPS in Group B scored 100 in 13 patients,90 in one,85 in four and 80 in one.There was no significant difference in MEPS between the two groups (P > 0.05).Conclusion For treating elbow varus posteromedial rotational instability,either lateral collateral ligament repair or hinged external fixation after anatomic reduction and coronoid fixation can achieve good results.
9.Retrospective analysis of 104 senile cases with salivary gland necplasms

Objective To study the clinical features of senile salivary gland neoplasms. Methods A total of 104 cases of senile (beyond 60 years old) salivary gland tumors treated in the past 15 years were analysed. Results Among the 104 cases of senile salivary gland tumors, benigns(85.6%) was more than malignants; males(76.0%) was more than females, the ratio of M∶F was about 1.5∶1. Seventy-two cases(69.2%) were from the parotid gland, 19 (18.3%) from the submandibular gland, 6 (5.8%) from the sublingual gland, 7 cases(6.7%) from the minor salivary gland. Warthin tumor (34 cases) accounted for 32.7%(34/104) of all senile salivary gland tumors, and 47.2%(34/72) of senile parotid gland tumors, 54.8%(34/62) of benign parotid gland tumors. Conclusions The senile salivary gland tumor were mainly benign tumor; Warthim tumor is the most commonly seen and the parotid gland is the most frequently targeted organ. The incidence in males is higher than in females, patients at 60-70 years old (76.0%) are the main victims. Regional excision of parotid gland (resection of tumor together with its surrounding glands) was efficient for surgical treatment in the elderly.
10.Correlation between serum lipid specific serine protease inhibitors and inflammatory factors in patients with coronary heart disease

Objective To observe the correlation between serum fatty specific serine protease inhibitor (vaspin) and inflammato-ry factors in the patients with coronary heart disease (CHD) .Methods 114 cases of CHD(CHD group) in our hospital from April 2010 to April 2015 were selected and divided into the stable angina group ,unstable angina group and acute myocardial infarction group according to the disease type ,and divided into the 1-vessel lesion group ,2-vessel lesion group and 3 or more vessel lesion group according to the coronary lesion vessels .At the same time 45 healthy persons were selected as the healthy control group .The enzyme-linked immunosorbent assay(ELISA) was adopted to detect the levels of vaspin ,tumor necrosis factor-α(TNF-α) ,interleu-kin (IL)-6 ,IL-10 and C reactive protein(CRP) .Results The vaspin level in the CHD group was significantly lower than that in the healthy control group ,while the levels of TNF-α,IL-6 ,IL-10 and CRP were significantly higher than those in the healthy control group ,the difference between the two groups was statistically significant (P<0 .05).The levels of vaspin in the stable angina pec-toris group ,unstable angina group and acute myocardial infarction group decreased in turn ,and the levels of vaspin in the 1-vessel lesion group ,2-vessel lesion group and 3 or more vessel lesion group decreased in turn ;the levels of CRP、IL-6、IL-10、TNF-αin the stable angina pectoris group ,unstable angina group and acute myocardial infarction group increased in turn ,and the levels of CRP、IL-6、IL-10、TNF-αin the 1-vessel lesion group ,2-vessel lesion group and 3 or more vessel lesion group increased in turn ;the differ-ence between the groups was statistically significant (P<0 .05) .The CHD severity was positively correlated with the levels of CRP , IL-6 ,IL-10 and TNF-α(P=0 .007 ,0 .003 ,0 .004 ,0 .005) ,and negatively correlated with the vaspin level (P=0 .002) .Conclusion The vaspin level is closely related to the inflammatory factors in CHD patients ,and is related with the vascular lesions severity of CHD .
