2.Percutaneous Endoscopic Gastrostomy on Patients with Central Nervous System Injuries

ObjectiveTo explore the percutaneous endoscopic gastrostomy on patients with central nervous system injuries in neurological rehabilitation and to establish enteral nutrition.MethodsThirty-two patients with central nervous system were enrolled for percutaneous gastrostomy, using the Pull technique.ResultsThe successful rate of percutaneous gastrostomy as Pull technique was 100%, 2 cases of stomach bleeding through endoscopy. Straining the fixation gasket and partial compression stoped bleeding. One patient appeared swelling and secretions in abdominal incisions, and recovered after dealing with local iodophor and changing fresh dressing. Hypostatic pneumonia occurred in one patient, and improved after anti-infective treatment. No serious complications occurred.ConclusionPercutaneous gastrostomy is a simple and safe for the patients with central nervous system injuries.
3.Evaluation of Reverse CT Scan in Eliminating Respiratory Motion Artifacts in Patients with Chronic Obstructive Pulmonary Diseases

Objective To study the value of reverse CT scan in eliminating the respiratory motion artifacts in the thoracic CT of the patients with chronic obstructive pulmonary diseases(COPD).Methods Fourty patients with COPD were randomly selected and underwent chest CT examinations with the technique of GR-Helical including directive and reverse CT scans.The images were blindly evaluated by three experienced doctors.Results In 40 cases,the respiratory motion artifacts were present in 17 cases,among them,70.59%(12/17) artifacts was in lower lung field,and 64.7%(11/17) artifacts occurred in the people over 60 years of age.The rate of artifact was 35% at directive scan,while it was reduced to 7.5% at reverse scan,the image quality was improved at 27.5%.There was statistical significance in eliminating respiratory motion artifacts between two scanning method (P<0.05).Conclusion Reverse CT scanning can effectively eliminating or reducing the respiratory motion artifacts in lower lung field,it is the best choice of scanning mode in elderly patients with COPD.

Objective:To study the effect of immune regulation of the rat Leydig cells in testis infection.Methods:UU was injected into the bladders of rats,which mimics the ascending infection pathway,while using culture medium injection as control.After 1,2,3 w respectively,the rats were sacrificed to observe the pathological alterations in testis by histological examination.Leydig cell were also separated from testis,the comparation of levels of IL-1,IL-6,TGF-?,Fas and FasL mRNA expression among control and UU infected groups was made by RT-PCR.Results:Compared with control,in UU infected group,the levels of IL-1,IL-6,TGF-? increased,the level of Fas decreased,the level of FasL increased.Conclusion:Leydig cell can play the role of immune regulation,with the expressing changes of IL-1,IL-6,TGF-?,Fas and FasL in the infection of testis in rat.
5.The preliminary research of relationship between Toll-like receptor and the immunoloregulation of Sertoli cell

Objective:To investigate the relationship between Toll-like receptor and the immune regulation about inflammation by Sertoli cell in vitro.Methods:Here we examined the expression and potential functions of TLR family in rat Sertoli cells.Using our well-characterized urealyticum(UU) induced model we tested the expression changes of TLR2 and TLR6 at 12~(th),24~(th),36~(th) hours after UU infection in vitro.Results:We demonstrated that TLR2-8 are highly expressed;TLR9 and TLR10 are expressed at relatively low level;the expression of TLR1 and TLR5 are not detected in normal rat Sertoli cells.Comparing with control group,Sertoli cells express more TLR2 and TLR6 after infected by UU.Conclusion:There is some relationship between the activation of TLRs and the immune regulation about inflammation by Sertoli cell.

7.Impact of mobile population on transmission of schistosomiasis in transmission-interrupted area

The historical surveillance results showed, there were 10 schistosomiasis cases in Huangshan City from 1994 to 2006. The survey in 2007 showed, the positive rates of blood examination for schistosomiasis in migrant workers and immigrant workers were 0.49% and 0.47% , respectively, but no schistosome-infected patients were detected by using the stool examination. An area with snails of 3 000 m~2 was found in the residence of the immigrant workers, but no infected snails were found. It is indicated that the mobile population has some impact on the transmission of schistosomiasis in the transmission-interrupted area. The surveillance and health education for the mobile population should be strengthened, and the imported infectious source should be prevented.
8.Operative strategy and short-term efficacy of recurrent groin hernia.

OBJECTIVETo explore the appropriate operative strategy in recurrent groin hernia repair.
METHODSClinical and follow-up data of 82 patients with recurrent groin hernia undergoing operation at Department of Pancreatobiliary Surgery, Xiangya Hospital of Central South University from April 2010 to April 2017 were analyzed retrospectively. The operative approaches included laparoscopic transabdominal preperitoneal (TAPP) hernia repair, Lichtenstein repair and hybrid repair. Surgical method selection was based on the basis of European Hernia Society guidelines, combined with hernia histories, preoperative examination results and intra-operative results: (1) When an anterior approach (Lichtenstein, Bassini or Shouldice surgery) was adopted in the previous operation, TAPP was preferred for the recurrent groin hernia. (2) When the previous operation was an posterior approach [TAPP or total extraperitoneal hernioplasty (TEP)], Lichtenstein method was preferred. Moreover, Lichtenstein surgery with local anesthesia or nerve block was also selected when the patient could not tolerate general anesthesia. (3) When extensive preperitoneal adhesions were found in patients with previous anterior approach repair during laparoscopic exploration, especially in patients who had relapsed after multiple operations or had previous biochemical glues injection, hybrid surgery was preferred.
RESULTSAll 82 patients completed operations smoothly. TAPP, Lichtenstein and hybrid operation were applied in 74, 4 and 4 patients, respectively, with median operative time of 70 minutes (40-130 minutes) in TAPP, 60 minutes (40-90 minutes) in Lichtenstein and 120 minutes (70-150 minutes) in hybrid operation, respectively. The median numerical rating scales (NRS) score was 2 (0-6) on postoperative day 1. The incidences of postoperative seroma, pain and urinary retention were 4.9% (4/82), 2.4% (2/82) and 1.2% (1/82) respectively. The median postoperative hospital stay was 2 days (1-6 days). Seventy-two patients were followed-up from 11 to 87 months. The median follow-up period was 27 months. The median inguinal pain questionnaire (IPQ) score was 2 (0-8) month after operation. One recurrent case was reported 1 year after operation. No incision or mesh infection and long-term inguinal chronic pain were observed.
CONCLUSIONSFor recurrent patients with previous open anterior approach, TEP and TAPP repair are equivalent surgical techniques, and the choice should be tailored to the surgeon's expertise. For those with previous TAPP or TEP repair, Lichtenstein technique is recommended. For those with adhesions both in anterior transverse fascia and pre-peritoneum, hybrid operation may be the preferable choice according to adhesion conditions.
