1.The detection and relevance of DTT-FRA in highly sensitized renal recipients

To further analyze immunoglobulin (Ig) properties of panel reactive antibodies (PRA) and their potential harmful effects on allogeneic renal transplants. Methods Since dithiothreitol (DTT) is able to crack disulphuric bond of Ig and to depoly-merize IgM with heavy molecular weight, PRA study was performed with treatment of DTT. Thus, a DTT-ERA method was established, which can identify immunoglobulin class in sera of high PRA patients. Results Among 701 recipients, whose sera were positive by the standard PRA, the positive rate of DTT-PRA was 33.2% . Besides, positive PRA patients could be divided into three groups according to their sera's sensitivity to DTT: Group 1 consisted of patients whose sera contained exclusively IgM antibodies; Group 2 consisted of patients whose sera contained of IgG antibodies only; Group 3 consisted of patients whose sera were found to contain both IgM class antibodies and IgG antibodies. Conclusion IgM class antibodies are not associated with posttrans-plant rejection, while IgG antibodies alone and mixture of IgG and IgM antibodies may mediate acute, or even hyperacute rejection.
3.MR Study of White Matter Hyperintensity in Patient with Cerebrovascular Disease

Objective To quantify the white matter hyperintensity (WMH) with serial MRI in elderly people with cerebrovascular disease, and to evaluate the risk factors that may have impact on their progression.Methods One hundred and thirty-eight patients with cerebrovascular disease underwent twice MRI scans with a 1.5T MR scanner at least one year apart (mean = 13.8 months). The clinical data of all patients, including age, gender, systolic blood pressure, blood glucose level, serum lipid level, alcohol consumption and smoking were recorded at baseline, as well as the historical informations concerning hypertension, diabetes mellitus and hypercholesterolemia. Besides, the overall severity of cognitive impairment and neural deficit of patients were rated by Mini-Mental State Examination (MMSE) and the National Institute of Health Stroke Scale (NIH). MRI measures included volume of gray matter infarction, volume of white matter infarction, and baseline volume of WMH. The volumetric changes of WMH between the twice scans were assessed using a semi-automated software. The influence of risk factors on changes of WMH volume was analyzed. Correlation coefficients were calculated between clinical scales and the change of WMH volume. Results The Baseline WMH volume was(13155?18782) mm3, and the volumetric change of WMH was(7687?9079) mm3. Multiple regression analyses revealed that the occurrence of infarction in cortex and in white matter was significantly associated with the volumetric change of WMH, as well as the baseline WMH volume. Objective To explore the relationships between serum adiponectin,IL-6 and hypoxic-ischemic encephalopathy(HIE) degree in newborn infant.Methods Enzyme-linked immunoabsorbent assay was used to detect the serum adiponectin and IL-6 in 58 neonatal HIE and 26 neonates without HIE.And all the data were processed by SPSS 10.0 software.Results The level of serum adiponectin in moderate and severe HIE at acute stage were significantly lower than that of mild and control groups(Pa0.05).The level of serum IL-6 in moderate and severe HIE at acute stage were significantly higher than that of the mild and control groups(Pa0.05).The level of adiponectin were significant negative correlation with IL-6(r=-0.852 P

7.Transumbilical single-site single-port versus single-site double-port laparoscopic varicocelectomy for varicocele in adolescents.

OBJECTIVETo compare the effect of transumbilical single-site single-port with that of transumbilical single-site double-port laparoscopic varicocelectomy in the treatment of varicocele in adolescents.
METHODSWe randomly assigned 80 varicocele patients aged 10 - 16 years to two groups of equal number to receive transumbilical single-site single-port and single-site double-port laparoscopic varicocelectomy, respectively. We compared the operation time, postoperative hospital stay, incisional pain, complications and satisfaction with the abdominal cosmetic outcomes between the two groups.
RESULTSAll the operations were successfully performed. The double-port group showed a significantly higher score on the Visual Analogue Scale than the single-port group (4.8 +/- 1.4 vs 3.6 +/- 1.1, t = -4.986, P < 0.01), but there were no significant differences between the two groups in the operation time ([29.8 +/- 4.2] vs [31.2 +/- 4.6] min, t = 1.383, P = 0.171), postoperative hospital stay ([1.95 +/- 0.7] vs [1.82 +/- 0.8] d, t = -0.784, P = 0.436), complications (0 vs 0) and scores on the satisfaction with abdominal cosmetic outcomes (4.6 +/- 0.6 vs 4.8 +/- 0.5, t = 1.253, P = 0.214). No recurrence, umbilical hernia, hydrocele and orchiatrophy were found in the two groups of patients at 6 months after operation, and no visible scar was observed on the abdominal surface.
CONCLUSIONWith strict surgical indications, single-site single-port and single-site double-port laparoscopic varicocelectomies have similar clinical effects in the treatment of varicocele, which leave no scar on the abdominal surface. Single-site double-port laparoscopy needs no special instruments and therefore is worthier of wide clinical application.

8.MRI findings in children with vanishing white matter disease

Objective To describe the MRI features of vanishing white matter disease (VWM).Methods Ten patients diagnosed as VWM between 2008-2013 were enrolled,with the mean age of(40 ±23)months.The clinical materials and MR images of them were reviewed retrospectively.The findings of MRI were analyzed by two experienced neuroradiologists in this study,including the location (cerebrum,cerebellum,brainstem,corpus callosum and internal/external capsule),size and signal intensity of the lesions,as well as the presence of cystic degeneration,cerebrum atrophy and the abnormal signal of the basal nuclei.Results MRI of brain typically showed extensive abnormal signal in the cerebral central white matter (10/10) and the focal lesion in subcortical white matter (9/10).The involvement of U-fibers was present in 7 cases.Cystic degeneration of the affected white matter was found in 7 cases.Abnormal signal also appeared in the white matter of posterior limb of the internal capsule (8/10),the external capsule (7/10),the inner rim (the callosal-septal interface) of the corpus callosum (8/10).The lesions of the thalamus and globus pallidus were detected in 2 cases.The white matter of the brainstem and cerebellum were involved in 6 and 7 cases respectively.Conclusion VWM typically shows a diffuse and symmetrical abnormal signal in the cerebral white matter with cystic degeneration on MRI,which is usually diagnostic.

10.Isolated peripheral neuropathy as an unusual presentation for an extramedullary relapse of acute leukemia

A 23-year-old man in remission from acute myeloblastic leukemia after allogeneic peripheral blood stem cell transplantation developed peripheral neuropathy presenting as sciatic and peroneal nerve deficits. Electrophysiological tests localized the lesions to the left sciatic and common peroneal nerve. Magnetic resonance imaging revealed nerve thickening and enhancement, while a positron emission tomography-computed tomography scan demonstrated increased fluorodeoxyglucose uptake tracking along the nerve, suggesting peripheral nerve infiltration. This report demonstrates an unusual presentation of acute leukemia relapse presenting as focal neuropathy
