1.Advances in diagnosis and treatment of hereditary nonpolyposis colorectal cancer

Hereditary nonpolyposis colorectal cancer (HNPCC) is a distinct autosomal dominant syndrome, accounting for 5%-10% of the total colorectal cancer population. To date, we have noticed that patients with HNPCC inherit a germline mutation in one of the genes responsible for repair of DNA mismatch errors. Tumors of HNPCC show a high frequency of microsatellite instability. Identifying individuals afflicted with HNPCC has implications for surgical management, prognosis, follow up, and surveillance of HNPCC patients and family members at risk.
2.Clinicopathologic features and DNA ploidy of sporadic colorectal cancers with microsatellite instability

Purpose:To explore the clinicopathologic features of sporadic colorectal cancer patients with microsatellite instability(MSI) and the correlation between the microsatellite instability and the DNA ploidy.Methods:Detecting microsatellite instability with two sites(BAT25 and BAT26),conduct flow cytometry to analyze the DNA ploidy among 71 sporadic colorectal cancer patients and probe into the relationship between the microsatellite instability phenotype and the clinicopathologic characteristics as well as the DNA ploidy.Results:The positive rate of the microsatellite instability among sporadic colorectal cancer patients was 9.86 percent.The microsatellite instability phenotype was correlated with the tumor site and the histopathologic type and the differentiation grade with the P value less than 0.05,while it was independent of the gender,age,lymph node metastasis and Dukes' stage.The proportion of right colon cancers and poorly-differentiated adenocarcinomas of sporadic colorectal cancers showing microsatellite instability was higher than that of microsatellite stable ones.18 and 50 cases showed diploidy and aneuploidy respectively.5 cases of MSI were diploid,There was statistically significant correlation between the microsatellite instability phenotype and the DNA ploidy(P=0.012).Conclusions:Sporadic colorectal cancers with microsatellite instability were prone to occur in the right colon and to be poorly-differentiated adenocarcinomas and inclined to be diploid.

Stroke is one of the most common diseases among middle and senior - aged people,and depressive disorder,which hinders severely recovery and prognosis,is one of the complications of stroke.This paper analyzes the efficiency of social intervention mode on patients with post - stroke depressive disorder,and comes up with the conclusion that social intervention mode can not only help to ameliorate the occurrence of post - stroke depressive disorder,but also promote the recovery of patients' nervous function.Therefore,the social intervention mode can be regarded as an effective means for medical treatment.
5.Effect of laryngeal mask anesthesia in video-assisted thoracoscopic surgery and nursing strategies

Objective To assess the effect of thoracoscopic surgery under laryngeal mask anesthesia and explore the nursing strategies. Methods Thirty-five patients from April to December in 2014 undergoing video-assisted thoracoscopic surgery (VATS) were given intubation anesthesia and another thirty-five ones undergoing the same surgery in 2015 received laryngeal mask anesthesia. The two groups were compared in terms of surgery conditions, surgery complication and ambulation time. Results No significant differences were found in surgery time or blood loss between two groups (P>0.05). The postoperative waking time in the mask anesthesia group was significantly shorter than that of the intubation anesthesia group (P<0.05), and ambulation time was significantly shortened (P<0.05). The rates of throat discomfort, hoarseness and gastrointestinal reactions were significantly lower. Conclusions Laryngeal mask anesthesia used in small thoracoscopic surgery for airway management is safe and feasible. Combined with training in respiration and limb function, better analgesia and nursing, it can keep away complications related to intubation anesthesia, shorten hospital stay and accelerate postoperative rehabilitation.
6.Effect of Yipi Rougan recipe comined chemotherapy on quality of life in intermediated or advanced-staged primary hepatic carcinoma and its efficacy

Objective To investigate the effect of Yipi Rougan recipe comined chemotherapy on quality of life in the treatment of patients with intermediated or advanced-staged primary hepatic carcinoma and its efficacy.Methods From April 2013 to December 2015 in our hospital, 97 patients with advanced primary liver cancer patients were selected, and randomly divided into traditional Chinese medicine group of 48 patients and chemotherapy group of 49 patients, two groups were treated with transcatheter arterial embolization chemotherapy, while the traditional Chinese medicine group were given Yipi Rougan recipe adjuvant therapy, 21 days for a course of treatment, with a total of four courses.Results There was no significant difference in total effective rate between traditional Chinese medicine group and chemotherapy group ( 83.33%vs.77.55%) .After treatment, the quality of life (QOL) score, Karnofsky performance score (KPS) in traditional Chinese medicine group were higher than those in chemotherapy group (P<0.05). The each symptom score in traditional Chinese medicine group was lower than that in chemotherapy group (P<0.05).Conclusion The efficacy of Yipi Rougan recipe combined with chemotherapy is not significant on patients with intermediated or advanced-staged primary liver cancer chemotherapy, but can significantly relieve symptoms and improve the quality of life of patients .
7.Factors relating to cross infection in hospital of cancer patients

Purpose:To investigate the susceptible factors and treatment against cross infection in hospital of cancer patients.Methods:174 cases of cross infection in hospital of cancer patients from 1997 to 1999 were analyzed for infective site, infective agent and infectious bacteria. Results:174 cases of cross infection in hospital of cancer patients were found, which was 10.45% of the in paitents of the same period. The majority (108/174) had infection of tissues and organs within irradiation fields after radiotherapy. Respiratory tract infection rate was 76.43%. 16 cases (40.0%) were found to have secondary infection of digestive canal after chemotherapy in the 40 cases of infection of digestive canal. Bone marrow deression or chronic failure and long term decubitus were also causes of infection. In addition, mycotic infection was 43.42% (33/76) in bacteriological tests with bad prognosis.Conclusions:Cross infection in the hospital occurs that patients enfeebled by radiotherapy, chemotherapy, chronic bone marrow failure and long term decubitus. Attention should be paid to the possibility of mycotic cross infection in hospitals, as the prognosis could be quite serious. As soon as the diagnosis is confirmed, early and full dose treatment by anti mycotic drugs should be given.
8.Therapeutic Observation of Salt-partitioned Moxibustion for Belching Due to Chemotherapy

Objective To observe the effect of salt-partitioned moxibustion at Zhongwan (CV12) in treating belching due to chemotherapy.Method Subjects with chemotherapy-induced belching were randomized into an observation group and a control group, 30 in each group. The control group was intervened by intramuscular injection with Ritalin 20 mg, while the observation group was by salt-partitioned moxibustion at Zhongwan for 30 min in addition to the intramuscular injection.Result The total effective rate was 93.3% in the observation group versus 76.7% in the control group, and the difference was statistically significant (P<0.05); the action latency in the observation group was (1.7±1.8)min in the observation group versus (15.6±4.2)min, and the difference was statistically significant (P<0.01).Conclusion Salt-partitioned moxibustion at Zhongwan is significantly effective in treating chemotherapy-induced belching, and it’s easy-to-operate, efficient, and safe.
9.Recent progress in the combination treatment of radiotherapy and PD-1/PD-L1 signaling blockade immunotherapy

Every stage of tumor initiation and development closely relates to immune regulation as tumor cells tend to evade attacks from immune system by employing the programmed death 1 (PD-1)/ programmed death-ligand 1 (PD-L1) interaction.Therefore,targeting the PD-1/PD-L1 pathway has become an attractive approach for cancer immunotherapy.Radiotherapy has long been considered a local tumor treatment modality and it is immune-inhibitory.However,accumulated evidence has shown that radiotherapy might enhance immune function by eliminating the tumor mass and has become a systemic tumor treatment modality.These observations indicate a strong rationale that the radiotherapy and anti-PD1 and anti-PD-L1 immunotherapy may work synergistically to provide a powerful anti-tumor effect.This review discusses current progresses,challenges and perspectives of this novel combination treatment modality.
10.Study of RHD genes among RhD negative individuals in Hainan province,China

Objective To understand the RHD gene profiles of RhD negative individuals in Hainan Han population,and provide reference to establish the right method for RHD genotyping.Methods RhD was tested by anti-globulin test, and RhDel and genuine RhD-negative phenotype were identified by absorption/elution method. RhD negative samples were further tested for RHD exons by PCR-SSP.The RhD negative samples with intact RHD genes were further analyzed by PCR-SSP for RHD introns 2, 10 and RHD?gene. Results Thirty-one (29.25%)cases of RhDel individuals were identified among 106 apparent RhD-negative individuals. All RhDel samples had RHD genes;67 cases of genuine RhD-negative had no RHD genes and 8 cases were partial D. All 31 RhDel samples had Din2 and Din10 but none had RHD?. Additionally, We detected exons 1,3,4,6,7,9 and 10 in one case of ccdEe sample. Conclusion The proportion of RhDel phenotype is high among apparent RhD-negative Hainanese, and total RHD exons can be detected in all RhDel samples. Polymorphisms of RHD gene are present among genuine RhD-negative Hainanese. There is no exon 5 in all 8 cases of partial D, which suggests that exon 5 specific amplification may be very important in RHD genotyping for Hainan Han population.
