1.Seed cells for tissue-engineered heart valves:source and application

BACKGROUND:Application of mechanical valve and bioprosthetic valve replacement is an effective means for the treatment of end-stage valve disease. However, their clinical application is constrained by multiple factors. Tissue-engineered heart valves with biological activity hold the potential to overcome the defect of the mechanical valves and bioprosthetic valves. Choosing appropriate cells for cellseeding is a significant aspect for the research of tissue-engineered heart valves. A growing number of somatic cells and stem cells have been used for construction of tissue-engineered heart valves, however, not yet achieve the desired result.
OBJECTIVE:To review the studies on seed cells for tissue-engineered heart valves and the cellseeding methods in vitro on the basis of the cellular components of heart valves.
METHODS:First author searched PubMed and Wanfang database with computer for related articles published from January 2000 to December 2012. The keywords were“tissue engineering, heart valves, cel”. Preferred articles concerned seed cells of tissue-engineered heart valves with the relevance and authority. A total of 39 articles published in the prestigious journals were reviewed.
RESULTS AND CONCLUSION:Endothelial cells and interstitial cells were mainly valvular cellular components. In the early stage, endothelial cells and fibroblasts were used to construct tissue-engineered heart valves. With deep stem cellresearch, the application of pulsatile bioreactor cultivation mesenchymal stem cells holds a great potential to produce tissue-engineered heart valves.

Results: An individualized artificial mandible substitute was designed and prepared. Conclusion: Rapid prototyping is feasible in individualized bone substitute preparation.

Results: The shape and size of the manufactured artificial mandibe were almost the same to those of the original natural one. Conclusion: Rapid prototyping may be used in individualized bone substitute preparation.
4.Effect of Soft Tissues Injury around Atlantoaxial Joint on Memory Abilities of Mice

Objective To observe the effect of the soft tissues injury around atlantoaxial joint on memory abilities of mice.Methods 44 mice were divided into the model group and control group with 22 animals in each group.The soft tissues around atlantoaxial joint were injured in the model group by surgery between the first and the second cervical vertebrae.The changes of learning and memory of mice were observed with step-down avoidance test,water-maze test and hole-board test;the expression of caspase-3 in hippocampus was observed by immunohistochemical staining.Results Compared with the control group,escape latency was longer and wrong times were more in water-maze test of the model group(P<0.01). In step-down avoidance test of the model group,the reaction period prolonged and wrong times increased obviously(P<0.01).In hole-board test,the latency period prolonged and the times of extending the holes decreased significantly(P<0.01).Degenerated or dead neuron in hippocampus of surgery-treated mice was found scatteredly or continuously with HE staining,the expression of caspase-3 protein was showed positive with immunohistochemical staining.Conclusion The soft tissues injury arround atlantoaxial joint can induce abnormality of the stability of cervical vertebra,which aggravated the aging process of the mice and decreased abilities of learning and memory.

Objective To explore the association of-11377 site single nucleotide polymorphism in promoter region of adiponectin gene and carotid intima-media thickness(CIMT)in patients with type 2 diabetes mellitus.Methods The adiponectin gene-11377C→G polymorphism was identified by PCR-restriction fragment length polymorphism(RFLP)in 504 patients with type 2 diabetes mellitus(250 patients with increased CIMT and254 Datients with normal CIMT). 6.Therapeutic evaluation of gliclazide-MR on blood glucose excursion by self-monitoring of blood glucose in type 2 diabetic patients

A total of 123 type 2 diabetics was randomised into 3 groups to receive gliclazide-MR, gliclazide or glibenclamide treatments for 16 weeks.All the subjects took self-monitoring of blood glucose (SMBG) during the trial.The effect of therapy was similar in 3 groups.The subtraction value between maximum and minimum blood glucose, mean postprandial maximum blood glucose, postprandial 2 h blood glucose, postprandial 2 h serum insulin and hypoglycaemia events were lower in gliclazide-MR group than those in glibenclamide group (all P<0.01).The data suggest that SMBG is an useful method to evaluate blood glucose excursion.
7.Influence of up-regulated Foxp3 expression on regulatory T cells function and kidney transplantation chronic rejection reaction in rat model

Objective To explore the influence of up-regulated Foxp3 on Treg function and kidney transplantation chronic rejection reaction in rat model.Method The kidney transplantation chronic rejection reaction rat model was established.The F344 kidney was transplanted to Lewis rats,and retroviruses highly expressing Foxp3 were constructed.The Banff 97 hierarchical diagnostic criteria were used to diagnose chronic renal allograft nephropathy (CAN).The rat models were divided into three groups by random number table.In experimental group,the pSCV-BsdRFP-FoxP3 retroviruses were injected into the rats via the tail vein after operation.In negative control group,the pSCV-BsdRFP retroviruses were injected into the rats via the tail vein after operation.In blank group,the normal saline was injected into the rats via the tail vein after operation.Enzyme-linked immunosorbent assay (ELISA) was used to detect the changes of interleukin-10 (IL-10) and tumor necrosis factor-β (TGF-β) immediate,1,2,3,and 4 weeks after operation.The rats were killed at 4th week after operation,and kidney tissues were taken out for pathological examination.Result The pathological changes of CAN were observed at 4th week.The typical chronic rejection change was seen at 12th week.The levels of IL-10 and TGF-β were increased,and reached the peak at 3rd week.The levels of IL-10 and TGF-β in experimental group were higher than in negative control group and blank group at 1st,2nd,3rd,and 4th week.At 4th week,obviously different degrees of intimal thickening,and mild hyperplasia of interstitial fibers,glomerular sclerosis and infiltration with lymphocytes and plasma cells were observed in the three groups.In the experimental group,the lesions were mildest,and apparent neointimal hyperplasia was found.Conclusion pSCV-BsdRFP FoxP3 retroviruses can reduce the kidney transplantation chronic rejection reaction in rat model,and have the potential treatment effect.

9.Evaluation of brachial plexus injury by CT myelography

Objective To evaluate the diagnostic value of CT myelography (CTM) in brachial plexus injury. Methods Twenty-seven patients with brachial plexus injury were examined by using cervical CTM with spiral scan and bone reconstruction algorithm. CT images were reviewed by the senior radiologists, who determined if the nerve root avulsion was presented. The criteria of diagnosing nerve root avulsion were loss of normal nerve root appearance in the Isovist filled thecal sac in consecutive CTM slices plus companion signs. The sensitivity, specificity, and accuracy of CTM in diagnosing nerve root injuries were calculated with operation findings and follow-up results as gold standard. Results Direct sign of nerve root avulsion was the loss of normal nerve root defect seen in the Isovist filled thecal sac in consecutive CTM slices. Indirect signs included: (1) Pseudomeningocele bulge: The leak of Isovist into nerve root sheath, and extended into foramina; (2) Arachnoid cyst: displacement of spinal cord; (3) Dissymmetry of subarachnoid cavity: deformity of thecal sac, partially lack of Isovist into arachnoid space; (4) Non-integrity of dural capsule wall: one side of capsule cavity was obstructed. Part of the surface of spinal cord was exposed. Brachial plexus injury could be diagnosed by direct sign with one of the indirect signs. Of the 27 patients (128 nerve roots), 91 nerve root avulsions were found on CTM, and 37 was found normal. Compared with operation findings, 84 were true positive, 7 false positive, 34 true negative, and 3 false negative. Based on these results, the sensitivity, specificity, and accuracy were 96.6%, 82.9%, and 92.2%, respectively. Conclusion CTM is accurate in detecting nerve root avulsion of brachial plexus.
10.Clinical Analysis of 142 Cases of Coronary Artery Bypass Grafting under Extracorporeal Circulation

Objective To summarize the clinical experience, operation indicators and curative effect of 142 cases of coronary artery bypass grafting (CABG) surgery. Methods We retrospectively analyzed the clinical data of 142 cases of CABG from January 2012 to July 2013 in the Department of Cardiovascular Surgery of Affiliated Yan 'an Hospital of Kunming Medical University. The patients included 92 cases of male, 50 cases of female, aged 24 to 78 years old, average (59.5±5.8) gears. Surgery was performed within the thoracic artery on the left of the left anterior descending branch and right coronary artery and convolution using great saphenous vein arterial anastomosis, treatment of valvular disease and aneurysm at same period,the average bypass 3.89/person. Results Two patients died after the operation,the motality rate was 1.41%. Postoperative complications included low cardiac arrhythmia, cardiac,renal insufficiency, respiratory failure, cerebral infarction/cerebral hemorrhage, open chest again stop bleeding,poor healing of incision, the incidence was 11.2%. The hospital total cost was (68.9±23.4) thousand yuan per patient,the postoperative follow-up time was (8.9±8.9) months,the follow-up rate was 96%,the postoperative patients were reviewed by echocardiography,suggesting left ventricular end-diastolic diameter and left ventricular ejection fraction were improved by different degrees. Conclusion Application of the mulitivessel transplantation methods can reduce the surgical mortality improve the cardiac function and decrease the hospital costs of patients.
