Abstract: Objective To analyze the resistance and spatial distribution of Mycobacterium tuberculosis (MTB) to six commonly used anti-tuberculosis drugs in Qinghai Province from 2016 to 2019, so as to provide a reference for tuberculosis treatment and drug-resistant tuberculosis control. Methods A total of 1 182 identified strains of Mycobacterium tuberculosis in Qinghai Province from 2016 to 2019 were collected, and 6 anti-tuberculosis drugs were subjected to drug susceptibility tests and strain confirmed by the proportional method. By means of ArcMap10.7 and SaTScan10.1 software, map visualization, spatial autocorrelation analysis and spatial scanning of MTB drug resistance were performed to identify MTB drug resistance clusters in Qinghai Province. Results From 2016 to 2019, the total drug resistance (TDR) rate of 1 182 Mycobacterium tuberculosis strains in Qinghai Province was 23.77% (281/1 182), with a mono-resistance (MR) rate of 11.08% (131/1 182), a poly-resistance (PDR) rate of 3.89% (46/1 182), a multi-drug resistance (MDR) rate of 8.80% (104/1 182), and an extensive drug resistance (XDR) rate of 0.85% (10/1 182). The rates of MDR, XDR and TDR all showed a decreasing trend year by year (P<0.01). The drug resistance spectrum displayed 21 combinations. The TDR rate and MDR rate in the retreatment patients were higher than those of the initial treated patients, and the difference was statistically significant (χ2
TDR=22.784, χ2MDR=45.082, P<0.01). In terms of demographic characteristics, the TDR rate in males was higher than that in females, and the middle-aged group was higher than other age groups, and the differences were statistically significant (χ2=7.541, 10.825, P<0.05). The results of global spatial autocorrelation analysis showed that there was no statistical significance in the autocorrelation and obvious spatial clustering of MTB drug resistance in Qinghai Province from 2016 to 2019 (P>0.05), which indicated a random distribution. The results of spatiotemporal scanning showed that there was a kind of clustering area, but the clustering effect was not significant (P>0.05), indicating a random distribution. Conclusions The TDR of MTB in Qinghai Province from 2016 to 2019 showed a downward trend year by year. In comparison with the national average, the rate of multi-drug resistance and extensive drug resistance was still high, and most of the multi-drug resistance resulted from rifampicin and isoniazid. The drugresistant population mainly consisted of retreatment, males, and young and middle-aged pop

Objective To evaluate traditional surgical treatment, intraluminal strategy and hybrid operation on revascularization of atherosclerosis obliterans (ASO) of the lower extremity. Methods Clinical data of 197 ASO cases receiving revascularization from January, 1998 to December, 2008 were retrospectively analyzed. Seventy-seven cases underwent surgical treatment, 82 cases received intraluminal therapy, and 38 cases were treated by hybrid operation. The indications, clinical effect, complication and perioperational mortality of these three strategies were evaluated. Results 71% patients (164 cases) were followed up from 2 to 112 months. Surgical and intraluminal method had no statistical difference on long-term patency of aortic-iliac and femoral-popliteal artery (57% vs. 51%;48% vs. 42%). Hybrid procedure led to higher patency on multi-level lesion and concurrent thrombosis. The complications after surgery was higher than intraluminal on aortic- iliac and femoral-popliteal artery (31% vs. 12%;31% vs. 11%), and higher than intraluminal and hybrid on multi-level lesion (36% vs. 12% vs. 15%). The perioperative mortality of surgical group was 1.5% and 2.0% on aortic-iliac and multilevel lesion and 0% on other site;and that of intraluminal and hybrid procedure was 0%. Conclusion For aortic-iliac and femoral-popliteal artery revascularization, surgery was preferred in cases of long occlusive lesion and intervention was preferred for cases with short non-occlusive lesion. Hybrid procedure was the best for multi-level and concurrent thrombosis.

Objective:To evaluate the analgesic effect of continuous infusion of low-dose dexmedetomidine combined with scalp nerve block after craniotomy in preschool children. Methods:This single-center, prospective, double-blind, randomized controlled study included 120 pediatric patients, aged 3-7 yr, of American Society of Anesthesiologists Physical Status classification Ⅰ-Ⅲ, with body mass index of 12-25 kg/m 2, who underwent elective craniotomy at Beijing Tiantan Hospital from May 2022 to December 2023. The patients were divided into 2 groups ( n=60 each) using a random number table method: IV infusion of low-dose dexmedetomidine combined with scalp nerve block group (Dex+ SNB group) and scalp nerve block group (SNB group). Dexmedetomidine 0.2 μg·kg -1·h -1 (in 50 ml of normal saline) was intravenously infused over 24 h in Dex+ SNB group, while the equal volume of normal saline was given instead in SNB group. The electronic intravenous analgesia pump was routinely used after operation, the solution contained sufentanil 2 μg/kg + ondansetron 0.3 mg/kg in 100 ml of normal saline, and the pump was set up to deliver a 2 ml bolus dose with a 30-min lockout interval and no background infusion. The FLACC scale was used to evaluate the level of pain. The FLACC scale was used to evaluate the degree of pain. When the FLACC pain scale score ≥ 4, the analgesia pump was first pressed by the nursing staff in the post-anesthesia care unit, intensive care unit or ward. If that did not work, oral acetaminophen was given at a dose of 15 mg/kg for rescue analgesia. The consumption of sufentanil was recorded within 1, 2, 4, 18, 24 and 48 h after operation. The FLACC score, Richmond agitation-sedation score (RASS score) and Cornell pediatric delirium score (CAPD score) were assessed at 1, 2, 4, 18, 24 and 48 h after operation. The occurrence of moderate or severe pain (pain FLACC score≥4), rescue analgesia, excessive sedation (RASS score≤-3), agitation (RASS score≥2), delirium (CAPD score > 9) and poor sleep quality (sleep quality score≤2) within 48 h after operation were recorded. The occurrence of drug- and surgery-related adverse reactions within 48 h after operation was recorded. Results:Fifty-six children were ultimately included in SNB group, and 55 children in Dex + SNB group. Compared with SNB group, the consumption of sufentanil was significantly reduced within 2, 4, 18 and 24 h postoperatively, the FLACC score for pain was decreased at 18 h postoperatively, the incidence of poor sleep quality was decreased on postoperative day 1 ( P<0.05), and no significant change was found in the incidence of moderate or severe pain, excessive sedation, agitation, delirium and adverse reactions after surgery in Dex+ SNB group ( P>0.05). Conclusions:Compared with scalp nerve block alone, combination of IV infusion of low-dose dexmedetomidine and scalp nerve block can reduce the amount of opioids and improve sleep quality when used for postoperative analgesia in preschool children undergoing craniotomy.
