1.Application of two-dimensional speckle tracking imaging in diagnosis of apical hypertrophic cardiomyopathy evaluated by ROC curve

Objective:Evaluation of two-dimensional speckle tracking technique in the diagnosis of apical hypertrophic cardiomyopathy (AHCM) using ROC curve.Methods:100 patients with AHCM admitted to Kunshan First People's Hospital from June 1, 2014 to June 30, 2018 were selected as the observation group, and 50 healthy volunteers who underwent physical examination at the same time were selected as the control group. All subjects underwent two-dimensional echocardiography. Circumferential strain (CS) and longitudinal strain (LS) of each segment of the left ventricular wall were obtained. End-rot, epi-rot, bulk-rot, mural-tor and G-tor were measured at mitral and apical levels. To screen effective indicators and evaluate the diagnostic effect of each indicator by receiver operating characteristic (ROC) curve.Results:There were significant differences in end-diastolic volume, end-systolic volume, diameter of left atrium in end-systole, the height of the early diastolic flow-velocity peak (E), the height of the late diastolic peak (A), E/A and apical thickness in end-diastole between the two groups ( P<0.05). There were significant differences in CS at mitral valve level, apical level, papillary muscle level, global CS, LS and global LS (GLS) between the observation group and the control group ( P<0.05), the diagnostic value of combined CS and LS at apical level was higher, and the area under curve (AUC) was 0.821(0.791, 0.904); There were significant differences in mitral valve level and apical level of endo-rot, epi-rot, bulk-rot, mural-tor and G-tor between the observation group and the control group ( P<0.05); G-tor has higher diagnostic value among them, and the AUC was 0.844 (0.771, 0.918). Conclusions:Two-dimensional speckle tracking technique plays an important role in the diagnosis of apical hypertrophic cardiomyopathy. It can observe the ability of myocardial movement and deformation, as well as the changes of left ventricular myocardial rotation and torsion. The combined detection of CS and LS at apical level and G-tor can better differentiate AHCM.

Bronchial asthma is a chronic inflammation airway disease.The imbalance of T helper cell (Th) 1/Th2 immunity plays an important role in the pathogenesis of allergic asthma.Recent years a new T cell lineage has been recognized as Thl7 cells because they can produce cytokines interleukin-17.The new studies find that interleukin-17 is a negative regulator of asthma,and interleukin-17 or Th17 cells may represent an interesting therapeutic target in asthma.
3.Clinical application of interferon gamma release assays for diagnosis of latent tuberculosis infection in children

Latent tuberculosis infection (LTBI) is defined as a patient is infected with Mycobacterium tuberculosis, without signs and symptoms or radiographic evidence of tuberculosis (TB). Until recently, the only test to identify latent tuberculosis infection was the tuberculin skin test (TST). But in BCG-vaceinated or non-mycobacterium tuberculosis infected individuals TST may be false positive and some babies or immunocompromised individuals have negative results on TST even they were proved to be with TB disease. So it is in need of a new method with higher sensitivity and specificity to diagnose LTBi. More recently, a new diagnostic method of interferon gamma release assays (IGRA) was paid close attention, and in some countries it replaced TST and became the final diagnostic method of LTBI. IGRA has high specificity for Mycobacterium tuberculosis without being influenced by BCG vaccinated or non-mycobacterium tuberculosis infection. In people with normal immune systems, the sensitivity of IGRA is same as TST for TB disease, and in immunocompromised individuals the sensitivity of IGRA is higher than TST. IGRA will have important application in diagnosis of LTBI although it cannot absolutely take the place of TST.
4.Clinical study of vaccinating BCG vaccine in bronchiolitis developed into child bronchial asthma in the future

Objective To observe the incidence of vaccinating BCG vaccine according to plan in bronchiolitis developed into child bronchial asthma in the future.Methods Using the method of retrospective study and clinical follow-up,chosen 280 children diagnosed with bronchiolitis from January 1995 to December 2002,135 cases were vaccinated BCG vaccine according to plan (vaccinated BCG group),145 cases were not vaccinated BCG vaccine according to plan (non-vaccinated BCG group),followed up for 3-10 years,the incidence of bronchial asthma between two groups were compared.Results The incidence of bronchial asthma in vaccinated BCG group was lower than that in non-vaccinated BCG group [17.04%(23/135) vs.40.69%(59/145)],there was significant difference (P< 0.01).Conclusions It's easy for bronchiolitis developing into bronchial asthma in children.Vaccinating BCG vaccine in time in occurrence of bronchial asthma show a protective effect in the future.

7.Protective Effects of Glutamine on Cardiac Muscle Cell in Septic Model Rats

OBJECTIVE:To study protective effects of glutamine (Gln) on cardiac muscle cell in septic model rats. METH-ODS:Rats were randomly divided into sham operation group (normal saline),model group (normal saline) and Gln low-dose, medium-dose and high-dose groups(0.5,0.75,1.0 g/kg)with 10 rats in each group. In these groups,septic rat model was induced by cecal ligation and puncture except sham operation group received sham operation. They were given relevant medicine intrave-nously 10 min after operation,and the characteristics and apoptosis of cardiac muscle cell were observed 12 h after operation. The serum contents of CK,LDH and TnⅠ,and the expression of Bcl-2 and p53 mRNA were all detected. RESULTS:Compared with sham operation group,myocardial necrosis of model group was found,and the serum content of CK,LDH and TnⅠ and apoptotic index increased,and mRNA expression of Bcl-2 in cardiac muscle cell decreased while that of p53 increased,with statistical signifi-cance(P<0.05). Compared with model group,myocardial injury relieved significantly in Gln high-dose and medium-dose groups, and serum contents of CK,LDH and TnⅠ and apoptotic index decreased;mRNA expression of Bcl-2 increased in cardiac muscle cell while that of p53 decreased,with statistical significance (P<0.05). CONCLUSIONS:Gln can improve myocardial injury of septic model rats significantly,by a possible mechanism of down-regulating the expression of p53 gene and up-regulating the ex-pression of Bcl-2 gene.
8.The role of fiberoptic bronchoscopy in diagnosis and treatment of mycoplasma pneumoniae pneumonia associated with atelectasis in children

Objective Mycoplasma pneumoniae is one of the main pathogens of community-acquired pneumonia in children and teenagers , which may cause mycoplasma pneumoniae pneumonia ( MPP) .Atelectasis is one of the most common manifestations of MPP.The aim of the article was to evaluate the diagnostic and therapeutic value of fiberoptic bronchoscopy ( FB) in children with MPP associated with atelectasis . Methods Retrospective analysis were made on clinical data and FB records of 99 cases of hospitalized children with MPP associated with atelectasis from August 2010 to December 2012 . Results The positive rate of MP-DNA was 92.9%(92/99) in bronchoalveolar lavage fluid (BALF), and the positive rate of MP-Ab-IgM in serum was 68.7%(68/99).All pa-tients showed obviously bronchial mucosa congestive edema during FB examination .The results are as follows:bronchial phlegm bloc-king in 31 cases (31.3%), lumen inflammatory stenosis in 22 cases (22.2%),mucosal ulcer change in 7 cases(7.1%) and mucous nodular change in 3 cases (3.0%).4 weeks after being treated with bronchoalveolar lavage (BAL), only 3 cases remained in atelec-tasis, the partial and complete re-expansion rate reaching 96.5%(82 of 85). Conclusion The positive rate of BALF MP-DNA de-tection technology for the diagnosis of MP infection is higher than that of serum MP-Ab-IgM examination .Early interventional therapy by FB can shorten the course of disease and promote the lung re-expansion in children with MPP associated with atelectasis .
9.The clinical significance of advanced oxidation protein products in patients with systemic lupus erythematosus complicated with atherosclerosis praecox

Objective To estimate the degree of oxidative stress and atherosclerosis praecox in patients with systemic lupus erythematosus (SLE), and emphasize on exploring AOPPs' clinical significance in premature atherosclerosis in SLE. Methods The levels of AOPPs, Hey, MDA, SOD, baPMV and CIMT were detected by ELISA and spectropholometry in 44 non-menopausal female SLE patients and 31 healthy middle-aged women respectively, and baPWV. The results were compared with AOPPs of the two groups. Then each group was stratified based on disease duration (≥5 years or <5 years) and the disease activity(active and inactive) in SLE patients. The patients' TC, TG, LDL were analyzed. T test, t' test and Pearson correla-tion were selected. Results The levels of AOPPs, Hcy, MDA in SLE patients were significantly higher than those of the healthy controls (P<0.05). The activities of SOD were lower than controls (P<0.05). The levels of AOPPs, Hcy, MDA, SOD had statistical significance between SLE patients disease duration ≥5 years or <5 years, active or inactive groups. There were two cases with CAP in patients with SLE (more than 5 years disease duration),while there wasnone in healthy controls. The levels of baPWV and CIMT in patients with SLE were higher than healthy controls (P<0.05), and had statistical significance in SLE patients disease duration more than or less than 5 years (P<0.05). The oxidative stress targets (AOPPs, Hey, MDA, SOD) had significant correlation with the level of baPWV and CIMT (P<0.01). The level of serum AOPPs had significant positive correlation with the levels of Hcy, TC, TG and LDL (P<0.01~0.05). Conclusion SLE patients have increased oxidative stress , and significantly higher prevalence of atherosclerosis than healthy controls. The disease duration and oxidative stress play important roles in the duration of atherosclerosis. AOPPs probably involves in the accelerated atherosclerosis of SLE patients. It may be a predictor for SLE complicated with atherosclerosis praecox.
10.Study on antibiotic resistance and staphylococcal chromosomal cassette mec molecular epidemiology of methicillin resistant coagulase negative staphylococci in children

Objective To learn about the antibiotic resistance status of methicillin resistant coagulase negative staphylococcus(MRCNS),and to investigate the distribution and resistant feature of different staphylococcal chromosomal cassette mec(SCCmec) genotypes of children in Anhui,so as to guide clinical medication.Methods Resistance phenotype screening was conducted in coagulase negative staphylococcus,which were isolated from clinical strains in children in Anhui from 2010 to 2014 each year in September.MecA gene was detected by using PCR method in order to collect MRCNS.Minimal inhibitory concentrations (MIC) of 16 antibiotics were determined by adopting agar dilution method.Vacomycin-resistant strains were identified with population analysis and the Brain Heart Infusion vancomycin screen agar dilution method recommended by Clinical and Laboratory Standards Institute in 2013.Van gene and SCCmec types were detected by using PCR method.Results A total of 148 MRCNS strains were detected through the resistance phenotype screening and the detection of mecA gene.There were methicillin resistant staphylococcus epidermidis,methicillin resistant staphylococcus haemolyticus,methicillin resistant staphylococcus hominis,and other kinds of MRCNS,and the proportions of them were 44.59% (66/148 cases),25.68% (38/148 cases),19.59% (29/148 cases) and 10.14% (15/148 cases),respectively.The analysis of antibiotic resistance showed the antimicrobial resistant rates of MRCNS to Penicillin,Cefoperazone,Cefotaxime,Ceftriaxone,lmipenem and Meropenem were all 100%,to Erythromycin and Azithromycin,Ciprofloxacin,Clindamycin,Gentamicin,Lewofloxacin,Rifampincin,Chloramphenicol,Teicoplanin and Vancomycin were 92.57%,97.98%,83.78%,79.05%,43.24%,35.81%,24.32%,8.78%,2.03% and 0.68%,respectively.There was 1 heterogeneous Vancomycin-resistant strain,which was resistant to both Vancomycin and Teicoplanin (with MIC 32.00 mg/L and 64.00 mg/L).No vanA,vanB,vanC1 or vanC2/3 gene was detected from heterogeneous Vancomycin-resistant strain by PCR.Ⅰ to Ⅴ SCCmec genotypes were detected from 148 MRCNS strains,and the major SCCmec type was SCCmec type Ⅲ,which was followed by hybrid type.Three subtypes of SCCmec type Ⅳ were identified,including Ⅳa,Ⅳc and Ⅳd.There were 148 MRCNS strains that showed different resistant phenotypes to various antibiotics.Conclusions The MRCNS strains of children in Anhui province showed multiple resistance to antibiotics.It should be on alert when heterogeneous Vaneomycin-resistant strain appeared.There were several different SCCmec types among several kinds of MRCNS,and SCCmec Ⅲ genotype was the major epidemic isolate.There was no significant correlation between the different resistance rates of non-β-lactamase antibiotics and SCCmec genotypes in MRCNS.
