1.Clinical observation of low dose thyroxine combined with irbesartan and hydrochlorothiazide combined with psychological intervention in the treatment of severe heart failure in the elderly

Objective To study the clinical efficacy of low dose thyroxine combined with irbesartan hydrochlorothiazide in the treatment of elderly patients with severe heart failure.Methods 100 cases of elderly patients with severe heart failure treated in our hospital from January 2013 to December 2016 were selected as the research object in this study.They were randomly divided into control group and experimental group, with 50 cases in each group.Patients in the control group were treated with conventional therapy, while the experimental group was treated with low dose thyroxine combined with irbesartan and hydrochlorothiazide.The clinical efficacy of the two group were compared and analyzed.Results After the corresponding treatment, the total effective rate of the experimental group was 88.0%,significantly higher than that of the control group(70.0%), the difference was statistically significant(P<0.05).After treatment, the experimental group of patients with left ventricular ejection fraction index, four triiodothyronine and three triiodothyronine levels were significantly higher than those before treatment, the control group of patients with left ventricular ejection fraction were significantly higher than before treatment, the difference was statistically significant(P<0.05).Conclusion Low dose thyroxine combined with irbesartan hydrochlorothiazide in the treatment of elderly patients with severe heart failure has better clinical effect, can improve the therapeutic effect in a certain extent, with further clinical promotion and application significance.
2.Discussion on the Necessity and Feasibility of Setting Up the Qualification of Licensed Assistant Pharma-cists

OBJECTIVE:To provide reference for improving licensed pharmacist qualification system and the legislation of Li-censed Pharmacist Law in China. METHODS:The implementation of licensed pharmacist system in China was introduced;based on the experience of licensed assistant physician system in China and pharmacy technician qualification system in USA,the necessi-ty and feasibility of setting up licensed assistant pharmacist were interpreted. RESULTS&CONCLUSIONS:At present,it is neces-sary to draw lessons from licensed assistant physician system in China and pharmacy technician qualification system in USA and set up licensed assistant pharmacist because of the shortage of licensed pharmacist,low threshold of qualification admittance,non-stan-dard licensed qualification of primary(assistant)pharmacy technician,absence of Licensed Pharmacist Law. It is recommended to promote the legislation of Licensed Pharmacist Law through clearing the definition and responsibility of licensed assistant pharma-cist,confirming examination scope and the threshold of qualification admittance,etc.
3.pEGFC1-IGFBP7 induces apoptosis of malignant melanoma cell line SK-MEL-28

Objective: To construct the insulin-like growth factor binding protein 7 (IGFBP7) expression plasmid (pEGFC1-IGFBP7) and to investigate the effect of IGFBP7 on the apoptosis of SK-MEL-28 (human malignant melanoma cell line) cells. Methods: The pEGFC1-IGFBP7 plasmid was constructed; pEGFC1-IGFBP7 and empty plasmids were transfected into SK-MEL-28 cells separately. The transfection efficiency was observed under fluorescence microscope. Apoptosis of SK-MEL-28 cells after transfection was detected by Annexin-FITC/PI staining. Results: The pEGFC1-IGFBP7 plasmid was successfully constructed and was effectively transfected into SK-MEL-28 cells by Effectene reagent, with the transfection rate being 61%. The results of flow cytometry showed that pEGFC1-IGFBP7 significantly induced apoptosis of SK-MEL-28 cells, with the apoptotic rates of pEGFC1-IGFBP7, empty vector, and non-transfected plasmid groups being (28.4±2.57)%, (5.8±0.44)%, and (6.4±0.71)% 24 h after transfection, respectively (F=406.138, P<0.05). Conclusion: pEGFC1-IGFBP7 can effectively induce apoptosis of malignant melanoma SK-MEL-28 cells, which provides an experimental basis for IGFBP7 gene-based therapy of malignant melanoma.
4.Effect of different tips of posteriors in extraction treatment on three-dimensional forces of teeth with clear aligners

Objective:
To investigate the three-dimensional forces in real-time at different tips of the posteriors created by clear aligners on maxillary teeth during en-masse retraction of anteriors after 2 first premolars extraction treatment.
Methods:
Four groups of maxillary models with different tips of posteriors(T1-upright of posteriors, T2-posteriors with distal inclination of 5°, T3-posteriors with mesial inclination of 5°, T4-posteriors with mesial inclination of 10°) undergoing 0.25 mm en-masse retraction treatment using clear aligners were simulated on a multi-axis force/torque transducer measurement system. When each clear aligner was inserted, a computer connected to the transducer collected and recorded the three-dimensional force of every tooth in real-time. Each group included 12 aligners.
Results:
Incisors received smaller lingual and extrusive forces; canines received greater distal forces; smaller mesial forces were observed on the posteriors, and molars received greater buccal and extrusive forces in T2,compared with T1,which showed statistically significant differences(P<0.05). Incisors received smaller lingual and extrusive forces; canines received greater distal forces; greater mesial forces were observed on the posteriors, and molars received greater buccal and extrusive forces in T3 and T4,compared with T1,which showed statistically significant differences(P<0.05).
Conclusion
Posteriors with distal inclination contributed to the torque of the anterior and anchorage of posteriors. In group T2 posterior teeth experienced larger buccal forces and extrusive forces than in group T1. When the posteriors crown inclined mesially, the anterior teeth were more prone to lingual tip and caused overbite. The more mesially inclined the posterior teeth were, the more serious overbite was, and the posterior teeth were more prone to anchorage loss.
5.Development of a fed-batch process for TNFR-fc producing GS-CHO cells.

TNFR-Fc is an important fusion protein that has great potential in therapeutic and diagnostic applications. We developed an efficient fed-batch process for GS-CHO cells to produce TNFR-Fc. The rationale of this fed-batch process relies on the supply of sufficient nutrients to meet the requirements of cell metabolism. The optimal feed medium was designed through ration design. A metabolically responsive feeding strategy was designed and dynamically adjusted based on the residual glucose concentration determined off-line. In this process, the maximal viable cell density and antibody concentration reached above 9.4x10(6) cells/mL and 207 mg/L, respectively. Compared with the batch process, the newly developed fed-batch process increased the cell yield by 3.4 fold and the final antibody concentration by 3 fold. This fed-batch process would therefore facilitate the production of therapeutic antibody by GS-CHO cells.

6.Telomerase activity in condyloma acuminatum tissue with different HPV types.

The telomerase activity in condyloma acuminatum (CA) tissue with human papillomavirus (HPV) types of 6/11 and 16/18 was detected to investigate the function of telomerase in the occurrence, development and carcinogenesis of genital CA. Forty-two biopsies from patients with genital CA and 30 control tissue samples were tested for telomerase activity, HPV presence and types. The telomerase activity was determined by modified telomerase repeat amplification protocol (TRAP) assay and HPV typing by polymerase chain reaction (PCR) with typing-specific primers. Results showed that HPV-DNA was negative and the expression rate of telomerase was 16.7% in all normal skin samples. All CA samples were positive for HPV (6/11 type was found in 32 cases, 16/18 in 3 and mixed type in 7). Telomerase activity was detectable in all CA patients. The telomerase activity in CA of 16/18 type was apparently higher than in CA of 6/11 type. It was concluded that the hyperplasia in CA might be increased as a result of HPV infection, suggesting that the activation of telomerase by HPV, especially by 16/18 type may play a role in the etiology and carcinogenesis of genital CA.

7.Impact of Anticoagulation on Deep Vein Thrombosis Occurrence in Patients After Right Heart Catheterization

Objective: To explore deep vein thrombosis (DVT) occurrence in patients after right heart catheterization and the effect of anticoagulant therapy. Methods: A total of 171 consecutive patients with electrophysiological study (EPS) and/or radiofrequency catheter ablation (RFCA) in our hospital from 2015-01 to 2016-05 were enrolled. All patients had supra-ventricular tachycardia and completed a venous surgery, they were randomly divided into 2 groups: Anticoagulation group,n=87 and Non-anticoagulation group,n=84. Lower extremity vascular Doppler ultrasonography was performed at (24-48) h post-operation to compare the incidence of DVT between 2 groups. Results: There were 13/171 patients were excluded for not completing post-operative lower extremity vascular Doppler ultrasonography including 9 patients in Anticoagulation group and 4 in Non-anticoagulation group. 158 patients finished post-operative examination and follow-up study. Anticoagulation group had 7/78 (8.97%) patients suffered from DVT, Non-anticoagulation group had 41/80 (51.3%) patients suffered from DVT,P<0.001. Conclusion: The incidence of DVT was higher after right heart catheterization without anticoagulation; heparin treatment may reduce DVT occurrence in relevant patients.
8.Bilateral hunter′s bow syndrome combined with epilepsy: a case report

Bow hunter′s syndrome, also known as vertebral basilar artery insufficiency, is a rare disease characterized by post?circulation blood supply insufficiency caused by mechanical or stenosis of the vertebral artery when the head and neck rotate or over?extend. To date, few cases regarding the bilateral hunter′s bow syndrome concurrent with epilepsy have been reported. A 29?year?old man was admitted to the First Hospital of Jilin University due to seizures. Results from transcranial Doppler ultrasound and carotid ultrasound showed that the patient had bilateral hunter′s bow syndrome. Further imaging examination showed that the syndrome was caused by congenital bone fusion which resulted in mechanical occlusion of C1-C2 vertebral artery. This case indicates that seizures are closely related to hunter′s bow syndrome.
9.A case report of sporadic spinocerebellar ataxia type 23

Spinocerebellar ataxias (SCAs), formerly known as autosomal dominant cerebellar ataxia, are a group of hereditary heterogeneous neurodegenerative disease that contains many subtypes. Spinocerebellar ataxia type 23 (SCA23), one type of SCAs, is caused by mutant prodynorphin (PDYN) gene. A 22-year-old patient was diagnosed with sporadic SCA23 due to gene detection, with a novel identified mutation, PDYN c.647C>T (p.P216L). Located in the dynorphin A-coding-region of PDYN gene, the pathogenic mechanism of the mutation may be relevant to the pathological changes caused by the variant including neurological dysfunction and death of cells. Mild improvement with the patient has been witnessed after active balance and speaking exercise.
10. Advances in Research on Early Predictive Value of Biomarkers for Severity of Acute Pancreatitis

Acute pancreatitis (AP) is a commonly seen disease of digestive system and with the characteristics of acute onset and high mortality. The existing scoring systems and biochemical indices for early prediction of AP severity are complicated and lagging in time. With the deepening of research, the emergence of new biomarkers, such as immature granulocyte percentage, red blood cell distribution width, can timely and effectively assess the severity of AP. This article reviewed the progress of research on biomarkers for predicting the severity of AP.
