1.Falls in a Nursing Healthcare Facility-special reference to the changes after the enforcement the start of nursing care insurance law

In April 2000, the nursing care insurance law was enforced in Japan. From April 1999 to March 2005, 162 residents including those receiving short-stay services in the Kamitsuga geriatic health services facility visited the hospital because of injuries resulting from falls. Thirty-one cases were diagnosed as fractures of femur, 13 as fractures of spine, 10 as fractures of upper extremities, 10 as fractures of rib and 3 as subdural hematomas. Fracture accounted for 48% of all cases, and femur accounted for 40% of all fracture cases. Annual hospital visits and hospitalization frequencies kept increasing up to 2001, then turned flat after 2002, and decreased in 2004. After 2002 we took the following measures, (1) individualized care, (2) small group care and (3) analysis of the behavior of inmates with an action chart for the first seven days. These measures resulted in the decrease of falls. After 2002 injuries to lower extremities decreased, but injuries to trunk increased. Generally Fractures of lower extremities prevails in the elderly of high-ADL (activity of daily life), and fractures of trunk prevails in the elderly of low-ADL. The above-mentioned three measures were effective for those of high-ADL, but ineffective for those of low-ADL. Other measures for those of low-ADL are necessary in the future.

2.Vitronectin regulates osteoclastogenesis and bone remodeling in a mouse model of osteoporosis

Vitronectin (VN) is an extracellular matrix protein with a crucial role in regulating bone remodeling. In this study, we aimed to investigate the effect of VN deficiency in a mouse model of osteoporosis induced by ovariectomy (OVX).The findings revealed that the absence of VN led to an increase in the activity of tartrate-resistant acid phosphatase (TRAP), a marker for osteoclasts, in the plasma of OVX-operated mice. TRAP staining further demonstrated that VN deficiency resulted in a higher number of osteoclasts within the femurs of OVX-operated mice. X-ray micro-computed tomography analysis of the femurs in OVX-operated mice indicated that VN deficiency significantly suppressed the OVX-induced increase of marrow area and total volume of bone. Additionally, we assessed structural model index (SMI) and degree of anisotropy (DA) as indices of osteoporosis. The results showed that VN deficiency effectively attenuated the OVX-induced increase in SMI and DA among OVX-operated mice. In summary, our study demonstrates the vital role of VN in regulating osteoclastogenesis and bone remodeling in the mouse model of osteoporosis.

5.An Open Study of Sulforaphane-rich Broccoli Sprout Extract in Patients with Schizophrenia.

OBJECTIVE: Schizophrenia is a mental disorder characterized by severe cognitive impairment. Accumulating evidence suggests a role for oxidative stress in the pathophysiology of schizophrenia. Sulforaphane (SFN) extracted from broccoli sprout is an agent with potent anti-oxidant and anti-inflammatory activity. In this study, we attempted to evaluate the effect of SFN on cognitive impairment in medicated patients with schizophrenia. METHODS: We recruited a total of 10 outpatients with schizophrenia, all of whom gave informed consent. Participants took 3 tablets of SFN, consisting of 30 mg of SFN-glucosinolate per day, for 8 weeks. Clinical symptoms using the Positive and Negative Syndrome Scale (PANSS) and cognitive function using the Japanese version of CogState battery were evaluated at the beginning of the study and at week 8. RESULTS: A total of 7 patients completed the trial. The mean score in the Accuracy component of the One Card Learning Task increased significantly after the trial. However, we detected no other significant changes in participants. CONCLUSION: This result suggests that SFN has the potential to improve cognitive function in patients with schizophrenia.

6.The albumin to globulin ratio is associated with clinical outcome in Japanese patients with ulcerative colitis

Purpose:
The albumin-to-globulin ratio (AGR) is a recognized chronic inflammation marker. No evidence regarding the relationship between AGR level and ulcerative colitis (UC) exists. The aim of this study was to evaluate the association between AGR and clinical outcomes among Japanese subjects with UC.
Methods:
The study subjects consisted of 273 Japanese individuals with UC. AGR was divided into 4 categories (low, moderate, high, and very high). The definition of complete mucosal healing (MH) was based on the Mayo endoscopic subscore of 0. Clinical remission (CR) was defined as no rectal bleeding and no abnormally high stool frequency (<3 times per day).
Results:
The percentage of MH was 26.4%. High AGR and very high AGR were significantly positively correlated with CR (adjusted odds ratio [OR], 5.85; 95% confidence interval [CI], 2.52–14.18 and adjusted OR, 4.97; 95% CI, 2.14–12.04) and complete MH (adjusted OR, 4.03; 95% CI, 1.56–11.51 and adjusted OR, 5.22; 95% CI, 1.97–14.89), respectively after adjustment for confounding factors (P for trend=0.001). Only in the low C-reactive protein (CRP) group (≤0.1 mg/dL), very high AGR was significantly positively correlated with complete MH but not CR (adjusted OR, 4.38; 95% CI, 1.06–21.77; P for trend=0.017). In the high CRP group, no correlation between AGR and complete MH was found.
Conclusion
Among Japanese patients with UC, AGR may be independently positively correlated with complete MH. In particular, among UC patients with low CRP, AGR might be a useful complementary marker for complete MH.
