Objective To investigate the clinical therapeutic effects of neuromuscular electric stimulation (NMES) on patients with swallowing disorders after stroke. MethodsA total of 98 patients with a swallowing disorder after stroke were randomly divided into an NMES group (38 cases),a rehabilitation group (30 cases) and a control group (30 cases).In the control group the patients were treated with medication only; the in rehabilitation group they were treated with medication and swallowing rehabilitation training; in the NMES group they were treated with medication and NMES.The treatments lasted for 3 weeks in all groups.The swallowing function of patients in each group were observed and assessed with dysphagia scoring pre-and post-treatment.Complications of the swallowing disorders,side effects of treatment and the stomach tube leaving rate were observed. ResultsAfter treatment,the dysphagia scores of patients in the NMES and rehabilitation groups both increased,but the average scores in the NMES group were significantly higher than in the rehabilitation and control groups.The difference in therapeutic effect between the three groups was statistically significant.After treatment,the incidence of aspiration pneumonia in the NMES group was significantly lower than that in the control and rehabilitation groups.The average stomach tube leaving rate in the NMES group was significantly lower than that in the control group,but there was no significant difference in stomach tube leaving rates between the rehabilitation group and the NMES group.There was no significant difference among the three groups in malnutrition.No side effect was observed in any group.Conclusions NMES could alleviate post-stroke dysphagia and decrease the incidence of aspiration.The short-term therapeutic effect of NMES was better than that of swallowing rehabilitation training,but the long-term therapeutic effects were about the same in the two groups.

Objective The clinical significance of serum cytokine in kidney transplanted patients have been evaluated by observing the variation of cytokines and biochemical parameters. Methods The serum IL-1?、IL-1?、IL-2、IL-4、IL-6、IL-8、IL-10、VEGF、IFN-?、TNF-?、MCP-1、EGF and 22 items of biochemistry parameters have been followed in 10 patients during 90 days around operation time. Cytokine had been measured by biochips methods in evidence 180 automatic biochips analyzer. The serum biochemical parameters had been measured by Roche diagnostic company reagents kits in HITACHI 7600DDP analyzer.Results Serum IL-6 and IL-8 had reached the highest level [(42.5?42.8) ng/L and (343.5?493.1) ng/L] at 7 days after kidney transplantation,whereas IL-2 and IL-10 were reached the highest level [(16.3 ? 5.7) ng/L and (16.6 ? 24.2) ng/L] respectively at 14 days. Then they decreased gradually, but IL-6 and IL-8 were still higher than normal control. The decreased VEGF, EGF and MCP-1 in kidney failure patient had increased gradually after transplantation. But EGF was still lower than normal control at 90 days after operation. OBJECTIVE:To observe clinical efficacy and safety of rivaroxaban in the treatment of pulmonary embolism. METHODS:100 patients with pulmonary embolism were selected and randomly divided into observation group and control group, with 50 cases in each group. Both groups were given nursing intervention for rehabilitation. Observation group was treated with riva-roxaban 15 mg,bid,for consecutive 3 weeks,3 weeks later adjusting to 20 mg,qd;control group was given low molecular weight heparin 1 mg/kg,bid,combined with warfarin sodium 5 mg,qd,for more than 5 days,stopped taking low molecular weight heparin until the INR reached the target 2-3 and given warfarin alone orally. Treatment course of 2 groups lasted for 3 months. Clinical efficacy of 2 groups were observed,and PaO2,PaCO2 and D-dimer levels were observed in 2 groups before and af-ter treatment;the improvement of clinical symptoms as cough,pectoralgia,dyspnea,and the incidence of ADR were recorded in 2 groups. RESULTS:The total effective rate of observation group (90.0%) was significantly higher than that of control group (74.0%),with statistical significance(P<0.05). After treatment,PaO2,PaCO2 and D-dimer levels of 2 groups were increased signif-icantly,compared with before treatment;the observation group was higher than the control group,with statistical significance(P<0.05). The remission rate of clinical symptoms as cough,pectoralgia,dyspnea in observation group was significantly higher than in control group,with statistical significance (P<0.05). The incidence of ADR in observation group (12.0%) was significantly lower than in control group (30.0%),with statistical significance (P<0.05). CONCLUSIONS:Rivaroxaban supplemented by reasonable nursing intervention can effectively improve clinical efficacy of patients with pulmonary embolism with low incidence of ADR.

Objective To investigate the effects ofLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets on diabetic complications in patients with type 2 diabetes mellitus (T2DM). Methods Seventy-eight patients with T2DM were randomized into a TCM treatment group and a conventional treatment group by random number table method, 39 in each group. The patients in the conventional treatment group were regularly using antihyperglycemic drugs or insulin. On the basis of conventional treatment group, the patients in the TCM treatment group were additionally treated withLiuwei-Dihuang Pills and Ginkgo Biloba Leaves Extract Tablets. Intima-media thickness (IMT) of internal carotid artery, vibration perception threshold (VPT) and diabetic retinopathy in both groups were detected before the treatment, 12 and 36 months after the treatment.Results The VPT in the left foot 12 months after the treatment in the TCM treatment group were significantly lower than that in the conventional treatment group (13.98 ±4.38 Vvs. 18.70 ±2.43 V;t=2.764, P=0.008). IMT of internal carotid artery (left: 0.81 ± 0.16 mmvs.0.70 ± 0.10 mm,t=3.120,P=0.003; right: 0.81 ± 0.17 mmvs. 0.73 ± 0.12 mm,t=2.286,P=0.026), VPT (left foot: 14.82 ± 6.45 Vvs. 20.63 ± 9.75 V,t=2.714, P=0.009; right foot: 16.73 ± 7.10 Vvs. 20.73 ± 10.35 V,t=2.001,P=0.048) and incidence of diabetic retinopathy (35.9%vs. 53.8%;χ2=5.804,P=0.016) 36 months after the treatment in the TCM treatment group were significantly lower than those in the conventional treatment group.ConclusionsLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets can effectively reduce the progression of diabetic complications in patients with T2DM.

BACKGROUND:Tracheal injury suture and the development of a new tracheal prosthesis all need to understand the tensile mechanical properties of tracheal cartilage.so as to repair and rebuild tracheal function.Previous researches on the biomechanics of artificial trachea are many reported.while the biomechanics of human tracheal cartilage is reported less.OBJECTIVE:To investigate the mechanical properties of tracheal cartilage using one-dimensional tensile test method.METHODS:Two fresh cadaver specimens of normal human trachea.with the informed consents of their families.were involved.The specimens were thaw at room temperature,and then tracheal cartilage specimens were cut using scalpel into 20 samples at the length of 25 mm,width 5 mm,thickness 1.8-2.2 mm.The 20 tracheal cartilage samples were subjected to one-dimensional tensile test with Shimadzu electronic universal testing machine Japan,at the tensile test speed of 5 mm/min.The tensile maximum load,maximum displacement,maximum stress,maximum strain,elastic modulus and stress-strain curve of the specimens were observed.RESULTS AND CONCLUSION:The maximum load of human cadaver tracheal cartilage was(60 946±10.377)N,maximumdisplacementwas(1.973±0.159)mm,maximum stresswas(6.229±1.125)Mpa,maximum strainwas(32.825±2.776)%.Tracheal cartilage stress-strain curves was changed along with the index,the initial low slope of the curye was due to the direction of imposed tension was similar with the arrangement of collagen structure,the steep slope represented the tensile strength of collagen.To describe the tracheal cartilage stress-strain relationship in a one-dimensional tension,15 stress-strain data of the tracheal cartilage experimental data adopted polynomial by the least square fitting method,to obtain stress(δ)-strain(ε)relationship forrnula:σ(ε)=-0.111 3e~5+1.602 1e~4-7.821 6e~3+17.995 1e~2+3.624e.It is indicated that tracheal cartilage has a strong capacity to bear Joad and resist deformation,reflecting their viscoelastic mechanical properties of both flexibility and viscosity,supporting that the mechanical properties of cartilage is positively related to cartilage collagen content.

OBJECTIVE:To observe the clinical efficacy and safety of caspofungin acetate in the treatment of invasive pulmo-nary fungal infection(IPFI). METHODS:70 patients with IPFI were selected and randomly divided into observation group(40 cas-es) and control group (30 cases). Control group was given Itraconazole injection with initial dose of 250 mg,bid,decreasing to 200 mg,qd,2 days later;observation group was given Caspofungin acetate injection 70 mg on the first day,decreasing to 50 mg, ivgtt,qd,within 1 h. Clinical efficacy,the rate of nacterial smear negative conversion and ADR were observed in 2 groups. RE-SULTS:The total effective rate of observation group was 92.50%,which was significantly higher than that of control group (76.67%);the rate of nacterial smear negative conversion was 72.00% in observation group,which was significantly higher than that of control group(42.10%);the incidence of ADR was 7.50%in observation group,which was significantly lower than that of control group (13.33%),with statistical significance (P<0.05). CONCLUSIONS:Caspofungin acetate is effective for IPFI with low incidence of ADR.
