1.Ginkgo biloba extract clinical observation on treatment of elderly unstable angina

Objective To explore the ginkgo biloba extract curative effect of treatment of elderly unstable angina.MethodsFrom January 2012 to December 2015 encounters during the period of 100 cases of elderly patients with unstable angina as the research object, they were dinided into two groups according to the different methods of treatment.Observation group and control group adopts western medicine aspirin enteric-coated metformin hydrochloride, isosorbide mononitrate zyban and combination treatment with atorvastatin calcium tablet.Observation group plus treated with ginkgo biloba extract tablets.ResultsObservation group the treatment of patients with total effective rate (94.0%) is significantly higher than control group (78.0%), the difference is statistically significant (P<0.05);Before the treatment, two groups product of systolic blood pressure, heart rate and heart rate in patients with systolic blood pressure, CRP index close, there was no statistically significant difference;But after treatment, the observation group of patients with indicators better, statistically significant difference (P<0.05);Blood lipid and blood rheology after treatment, the observation group improved significantly (P<0.05), but the control group to improve the situation is not obvious, the difference between groups with statistical significance (P<0.05);Two groups of adverse reactions in patients with no statistically significant difference.ConclusionGinkgo biloba extract in the treatment of the elderly unstable angina has significant clinical effect, can significantly improve the patient′s blood lipid and blood rheology indexes, and the safety of the drug is good.
2.Application effect of clinical nursing pathways on acute myocardial infarction patients:A Meta-analysis

Objective To evaluate the application effect of clinical nursing pathways in acute myocardial infarction patients by using the Meta-analysis.Methods Published randomized controlled trials (RCT) in acute myocardial infarction patients were searched and screened in CNKI,VIP,Wanfang database under present standards.The quality of the included studies was evaluated by certain standards.The Review Manager 5.0 software was taken for analysis.Results Totally 18 studies including 1877 cases were eligible to the criteria (894 in the experimental group and 983 in the control group) altogether.The Meta-analysis showed there was significant difference between the experimental group and the control group in hospitalization days,patients' satisfaction degree and rate of mastering knowledge.Significant difference existed between the experimental group and the control group in bedridden time,complication rate and the recurrence rate of myocardial infarction.Conclusions Application of clinical nursing pathway in acute myocardial infarction patients can shorten hospitalization days,increase patients' satisfaction degree and rate of mastering knowledge,decrease bedridden time,complication rate and the recurrence rate of myocardial infarction.
3.Studies on the variation of serum cytokines and biochemical parameters in post kidney transplanted patients

Objective The clinical significance of serum cytokine in kidney transplanted patients have been evaluated by observing the variation of cytokines and biochemical parameters. Methods The serum IL-1?、IL-1?、IL-2、IL-4、IL-6、IL-8、IL-10、VEGF、IFN-?、TNF-?、MCP-1、EGF and 22 items of biochemistry parameters have been followed in 10 patients during 90 days around operation time. Cytokine had been measured by biochips methods in evidence 180 automatic biochips analyzer. The serum biochemical parameters had been measured by Roche diagnostic company reagents kits in HITACHI 7600DDP analyzer.Results Serum IL-6 and IL-8 had reached the highest level [(42.5?42.8) ng/L and (343.5?493.1) ng/L] at 7 days after kidney transplantation,whereas IL-2 and IL-10 were reached the highest level [(16.3 ? 5.7) ng/L and (16.6 ? 24.2) ng/L] respectively at 14 days. Then they decreased gradually, but IL-6 and IL-8 were still higher than normal control. The decreased VEGF, EGF and MCP-1 in kidney failure patient had increased gradually after transplantation. But EGF was still lower than normal control at 90 days after operation. 4.Application of FDG PET-CT in chemoradiotherapy for esophageal cancer

Esophageal cancer is one of the most common gastrointestinal cancers, and chemoradiotherapy is an important part of the multidisciplinary treatment for this disease.In recent years,18Fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET-CT) is widely used in esophageal cancer for delineation of gross tumor volume, local boost irradiation in the late stage of radiotherapy, and assessment of the pathologic remission rate after neoadjuvant chemoradiotherapy, response to definitive chemoradiotherapy, and prognosis.In this article, we review the application of FDG PET-CT in the chemoradiotherapy for esophageal cancer.
5.Training the trunk and pelvis of hemiplegic stroke patients

Objective To observe the effects of reinforced trunk and pelvis control training on the motor function of stroke patients. Methods Sixty-two stroke patients were randomly divided into a therapy group and a control group with 31 cases in each group. All were treated with routine medication and regular rehabilitation training.The patients in the therapy group also received trunk and pelvis control training. The Fugl-Meyer assessment scale (FMA) , Barthel index (BI) and functional ambulation categories (FAGs) were used to assess the motor function of the patients before and after treatment. Results After 2 months of treatment there were significant improvements in FMA, BI, FAC and gait in both groups, but the average walking ability in the therapy group was significantly better than that in the control group. Conclusions Reinforced control training of the trunk and pelvis combined with routine rehabilitation improves motor function in hemiplegic stroke patients significantly better than standard rehabilitation alone.
6.Relationship between HBV DNA level, expression of PD-1 on CTL and liver functions in patients with hepatitis B virus cirrhosis

Objective To explore the relationship between serum HBV DNA level , expression of PD-1 on CTL and liver functions in patients with hepatitis B virus cirrhosis. Methods 109 HBV hepatic cirrhosis patients with , HBV DNA positive , HBeAg positive and HLA-A2 positive were divided into two groups according to their HBV DNA levels,52 cases in group A, HBV DNA 2-4 log10 copies/mL, 57cases in group B, HBV DNA 5-7 log10 copies/mL , differences of HBV specific CTL surface PD-1 expressions , HBV specific CTL levels and liver functions were compared between two groups. Results HBV specific CTL surface PD-1 expressions of group A were lower than that of group B (t = 11.101, P < 0.01), HBV specific CTL levels of group A were higher than that of group B (t = 24.424, P < 0.01), ALT of group A were lower than that of group B (t = 2.652, P < 0.01), ALB of group A were higher than that of group B (t = 2.347, P < 0.05). Child-pugh rating, grade C of group A was lower than that of group B (χ2= 4.262,P < 0.05). Conclusions HBV specific CTL surface PD-1 expression levels were higher in those hepatic cirrhosis patients with , high level of serum HBV DNA, and HBV specific CTL levels were lower, liver functions damage were much serious. So, lower HBV DNA levels , lower HBV specific CTL surface PD-1 expression levels and higher HBV specific CTL levels can product relatively good effect of anti-viral treatment.

Cobra venom secretory phospholipase A_2 (sPLA_2) is an important component of cobra venom which has a variety of biological activities. Recent studies are mainly focusing on each pharmacological active component of venom, SPLA_2 is one of them. This review summarized the structure, purification and biological activities of cobra venom sPLA_2 with emphasizing its diverse pharmacological effects and toxicity. In addition, some mechanisms of actions of sPLA_2 and possible applications of sPLA_2 were also discussed.
8.Effects of Recreational Therapy on Social Communication of Stroke Patients

ObjectiveTo investigate the effect of the recreational therapy on the ability of social communication in stroke patients. Methods58 cases of stroke patients were divided into recreational group (28 cases) and control group (30 cases). All of them received routine rehabilitation therapy, while the recreational group received the recreational therapy in addition. All patients were evaluated with Functional Independence Measure (FIM) before and after treatment. ResultsThe scores of FIM significantly improved after treatment (P<0.05) in both groups, and the scores in social communication improved more (P<0.01) in recreational group than in control group. ConclusionRecreational therapy may improve the ability of social communication more effectively for stroke patients.
9.Association of serum C peptide level with the severity of diabetic foot ulcers and its healing rate

Objective To investigate the association of serum C peptide concentration with the severity and the outcome of diabetic foot ulcer (DFU). Methods The clinical data of 257 inpatients with DFU were collected, including fasting and postprandial 2h C peptide levels and C peptide area under curve (AUCCP ). The patients were followed up on the outcomes of ulcers and death. The associations of serum C peptide concentration with the Wagner degree, infection severity, and healing rate were analyzed. Results The medians of fasting and 2h postprandial serum C peptide as well as AUCCP were 1. 37(0. 02 ~ 9. 00) nmol/ L, 3. 22(0. 02 ~ 29. 61) nmol/ L, and 511. 65 (3. 60 ~ 2 691. 30)nmol·min-1 ·L-1 respectively, which were lower than general levels. The time of follow-up in our study was 2. 8 (1. 0 ~ 5. 1) years. By the end of study, the wound of 75. 88% patients was healed, 3. 5%undergone major amputation, and 23. 74% died. After adjusting for relative factors, there were no significant associations of serum fasting and postprandial C peptide levels and AUCCP with Wagner degree and infection severity (P>0. 05). Cox regression analysis showed that the fasting plasma C peptide and hemoglobin were the independent protective factors for the healing of ulcers; old age, male, higher infection degree, and diabetes family history were their independent risk factors ( all P < 0. 05). Conclusions The lower plasma fasting C peptide concentration in patients with DFU is not correlated with Wagner degree and infection severity, but closely related with healing rate.
10.Effect of preeclampsia and varying degrees proteinuria on perinatal outcome

Objective To explore the impacts of preeclampsia and the different extent of proteinuria on maternal and perinatal outcomes.Methods The retrospective analysis was conducted according to the perinatal clinical data of preelacmpsia,pregnancy-induced hypertension in pregnant women and normal pregnant women from the Fifth People's Hospital of Shanghai,excluding twins,diabetic mellitus and patients with chronic kidney disease previously.Patients were divided into three groups on the basis of their conditions:① preeclampsia patients (A group,220 cases); ② patients with gestational hypertension (B group,189 cases); ③ normal pregnant (C group,220 cases).Patients with pre-eclampsia according to the degree of proteinuria were further divided into three subgroups:A1:patients with mild proteinuria (n =109); A2:patients with moderate proteinuria (n =72); A3:patients with severe proteinuria (n =39).Results Compared with the other two groups,the patients in A group had higher blood pressure,serum creatinine,uric acid,cesarean section rate,perinatal prematurity,stillbirth,fetal distress and neonatal asphyxia in preeclampsia group.However,the serum albumin level,eGFR,neonatal birth weight,length and Apgar scores were lower in A group compared with B and C group (P < 0.05).In three subgroups,serum creatinine level,uric acid level,cesarean section rate,perinatal prematurity and fetal distress were significantly increased in A3 group compared with A1 group,while the serum albumin level,eGFR,gestational age and neonatal birth weight were obviously lower in A3 group than in A1 group (P < 0.05).In patients with preeclampsia,24 h urinary protein was negatively related with the levels of serum albumin and eGFR (P < 0.05),and positivly related with the blood pressure,serum creatinine and caesarean production rate (P < 0.05).Large amounts of proteinuria was a risk factor of adverse outcome for pregnant patients with preeclampsia (OR =2.899,P < 0.05).Conclusions Preeclampsia patients with large amount of proteinuria have poor maternal and perinatal outcomes.Massive proteinuria is a risk factor of adverse outcome for patients with pre-eclampsia.
