1.Research in status of community nursing among migrant infants of 0~6 months

Objective This paper aimed to analyze the health situation and its influencing factors of the migrant infants in Gongshu district Hangzhou city,and discuss the best children's health care community management mode.Methods A random sampling method was taken to select the children's health management files who had took physical examinations in the community health service center in Gongshu district,the total migrant infants were 71 cases and local infants were 439 cases.Indicators such as rate of anemia,suffering from a disease,timely adding of vitamin D,and breast feeding were analyzed.Results The teething rate was 18.3%,anemia rate was 7.0%,obesity rate was 4.2%,overweight rate was 8.5% in migrant infants.Compared with the local infants,the teething rate,anemia prevalence,overweight rate of migrant infants were lower than the local infants,and obesity rate was higher than the local infants,but there were no statistically significant differences between them.As a whole outdoor activities time of migrant infants was shorter than local infants.They both did not go out within 42 days primarily,outdoor activities time primarily were 1~2 h/d on the third month,more than 2 h/d on the 6th month.Conclusions To carry out health education for migrant parents actively,strengthen migrant infants care,timely addition of assist food,to combine vaccination information and children's health care information effectively are effective measures to improve the infants health service quality.
2.Application of PBL teaching method in occupational health and occupational medicine teaching and its evaluation

Objective To observe the validity and feasibility of PBL teaching method in occupational health and occupational medicine theory teaching.Methods Sixty-one five-year undergraduates majored in preventive medicine were selected and self-control was applied.Two teaching contents which had similar teaching requirements were selected,one applied PBL teaching method and the other applied traditional teaching method.Students' results and passing rate in examination were analyzed and compared.SPSS 16.0 software was used for statistical analysis and data were manifested as ((x-) ± s).T test was used to do comparison between average results and chi-square test was used to do comparison between rates.SAS 9.0 software was employed and Fisher's exact probability analysis was performed on percent of pass.P<0.05 stands for statistically significant differences.Results Test scores of the students taught by PBL teaching method were significantly higher than those of students taught by traditional teaching method (P<0.05).Both female and male students' test scores were improved (P<0.05) and increase of female students' test scores were more obvious than male students'.Passing rate of female students taught by PBL teaching method was significantly higher than female students taught by traditional teaching method (P<0.05).Conclusion PBL teaching method applied in occupational health and occupational medicine theory teaching can significandy improve the quality of teaching.
3.Identification of mutation of the X-linked juvenile retinoschisis gene

Objective To analyze the pathogenesy and mutation of X-linked juvenile retinoschisis (XLRS) 1 gene in XLRS families, and to provide the theory basis in directing gene diagnosis. Methods The mutation of XLRS1 gene code in two XLRS families were detected and screened by polymerase chain reaction (PCR) and DNA direct sequence determination. Results Pro193Ser mutation was detected in family 1. Conclusion Pro193Ser mutation could be found in XLRS families, which can be used for genetic consultation and prenatal gene diagnosis.
4.The clinical application research of enzymatic cycling assay for detecting serum Hcy concentration

Objective To discuss the value of clinical application with Hcy enzymatic cycling assay.Methods To compare the results between enzymatic cycling assay and FPIA.The precision test and correlation test of enzymatic cycling assay were evaluated at Hitachi 7600-DDP automatic Biochemistry analyzer.Serum Hcy concentration of patients with diabetes, hypertension, coronary heart disease, myocardial encephalon infarction or chronic renal function failure had been detected in parallel with enzymatic cycling assay and FPIA and the results had been compared to the healthy.Results The within-run imprecision (CVs) of patient serum is 3.07%(low value ), 3.99%(mid value) and 4.21%(high value ). The between-run imprecision (CVs) is 4.00%(low value), 4.79%(mid value) and 4.58%(high value). The correlation of enzymatic cycling assay and FPIA was excellent(r=0.994,n=53).The averages of Hcy concentration in healthy, diabetes, hypertension, coronary heart disease, myocardial or encephalon infarction or chronic renal function failure respective were (8.15?1.89) ?mol/L, (8.88?1.36) ?mol/L, (10.66?1.60) ?mol/L, (14.02?3.42) ?mol/L, (14.45?3.83) ?mol/L and (15.21?3.50)?mol/L. There is no significant statistic difference between enzymatic cycling assay and FPIA.Conclusion The variation profile of Hcy concentration in different diseases detected by enzymatic cycling assay were similar to FPIA.The results indicated that en{ymatic cycling assay could be used to detect serum concentration of Hcy for clinical laboratory rapidly.

Objective To investigate the value of transcranial Doppler(TCD)in the prediction of the outcome and determination of brain death in patient with spontaneous intracerebral hemorrhage. Methods The waveforms and the cerebral blood flow velocities of the bilateral middle cerebral arteries(MCA)were monitored and analyzed using EME TCD in 68 comatose patients with Glasgow scores ≤7. Results The results showed that persistence of retrograde diastolic flow or small systolic forward flow in bilateral MCA lasted for more than 1 hours were observed in 28 cases of comatose patients, which was a reliable indicator for predicting or confirming brain death. Asymmetrical TCD waveforms were characteristically associated with the localized brain damage. Conclusion TCD monitoring is a useful technique for predicting outcome and guiding treatment of patient with spontaneous intracerebral hemorrhage.

Objective:To explore TCM syndrome variation regularity on acute stage of ischemia stroke.Methods:To collect clinical dynamic state information of ischemia stroke from 72h to 14days by using prospective clinical research method and investigate TCM syndrome variation regularity in acute stage of ischemia stroke.Results:Wind,fire and phlegm syndromes were the most frequent syndromes in acute stage of ischemia stroke.They appeared varied regularity.There were forty-four combination patterns about six basic syndromes.The wind syndrome and fire-phlegm syndrome were increasing in acute stage of ischemia stroke.The wind-phlegm syndrome was decreasing in the first three days,the wind-fire syndrome peaked on the third day,the phlegm was decreasing on the fifth day.The wind-fire-phlegm was the most frequent and undulate syndrome on the fifth day.The wind-phlegm-stagnation syndrome was increasing for the first three days.Conclusion:There were fastly morbility,changeable and complicated syndrome change in acute stage of ischemia stroke.It was very necessity and urgency for complementing diagnostic criteria of stroke differentiation.
8.THE HISTOCHEMICAL STUDY OF MUCO-POLYSACCHRIDE IN THE TISSE OF GASTRIC CARCINOMA

Twenty-fine samples of stomach and 25 samples of adenocarcinoma stomach were employed for this study. The former have been collected from the patients of duodenal ulcer. Specimens were taken from the large curvature and pylorus. In the adenocarcinoma of stomach, materials were taken from the cancer focus and the cancer margins, fixed in 10% formalin-GaCl_2 Solution, embedded in paraffin and cut at 5? in thickness. Then they were examined with defferent reactions of histochemistry; PAS, Alcian blue (AB), Toludine blue (TB) and Barnett's method. The surface of gastric mucosa obtained from patients of duodenal ulcer after subtotalgastrectomy was covered with a substance which consists of neutral and acid mucopolysaccharide and protein. The epithelial cells of the mucosa contains large amount of neutral muco-polysaccharide. The cells of the neck of gastric gland contain quite a bit of acid muco-polysaccharide, but the cells of the deep segments of the glands do not show any of it. The cells of adenocarcinoma of stomach contain a different deal material mixture which consists of acid muco-polusaccharide and protein. The surface of the cavity in the well-differenlialed cancer glands was covered by a positive substance in TB, AB method, which forms a thin layer of membrance. An evenly distributed substance in the cavity of carcinoma gland was shown with positive reactions of PAS, AB, TB and protein. The degenerated cells show a positive reactions of PAS and protein. The cells of low-differentiated cancer were surrounded with a superficial layer of the acid mucopolysaccharide. The interstitia in the invaded margines often showed clear, distinct positive PAS and TB reactions. It indicates that it cortains more chondroitin sulfuric acid and the hyaline acid. In the superficial layers of mucosa of the cancer tissue, the epithelial cellswere often to have less positive substance than that of the membrane in the parts of cancer margine, it means a lesser amount of neutral-polysaccharide. But the acid muco-polysaccharide in the surface of cancer and its metaplastic margin clearly increases in amount than the metaplastic area far away from the cancer focus. This report discussed the significance of the increased muco-polysaccharide in the tissue of the gastric cancer.
9.Effect of breathing filter on prevention of respiratory tract infection following general anesthesia in elderly patients

Objective To investigate the effect of breathing filter(BF) on prevention of the respiratory tract infection in elder patients undergoing general anesthesia.Methods One hundred elderly who underwent the operation of gastrointestinal tumors were involved in the current study.They were randomly divided into control and experiment group and each for 50 cases.Patients in experiment group were given BF while in control group were not given.Bacteria on the tip of endotracheal tube and in the anesthesia machine circuits close to endotracheal tube were checked.The rate of the respiratory tact,infection of operative patients were recorded.Results After operation,a large number of bacterial growth in endotracheal catheter ends in both groups.The level of bacteria in the anesthesia machine circuits of control group was (305.2 ± 12.4) CFU/cm2,higher than that in experiment group ((10.2 ± 2) CFU/cm2 ; t =166.077,P < 0.001).No patients in experimental group got respiratory tract infection in the follow up periods,but 5 patients (10.0%) in control group were infected,and the difference was significant (x2 =5.263,P < 0.05).Conclusion BF can effective filtrate the bacterial in breathing circuit,significantly reduce the incidence rate of respiratory tract infection.
10.Effects of inhaled aerosolized different doses of lidocaine on lung injury in patients undergoing cardiac valve replacement under cardiopulmonary bypass

Objective To evaluate the effects of inhaled aerosolized different doses of lidocaine on lung injury in patients undergoing cardiac valve replacement under cardiopulmonary bypass (CPB).Methods Thirty ASA physical status Ⅱ or Ⅲ patients of both sexes,aged 18-58 yr,weighing 35-70 kg,undergoing elective cardiac valve replacement with CPB,were randomly divided into 3 groups (n =10 each) using a random number table:control group (group C),lidocaine 100 mg group (group L1) and lidocaine 200 mg group (group L2).Anesthesia was induced with iv injection of midazolam,etomidate,fentanyl and vecuronium.The patients were endotracheally intubated and mechanically ventilated.The aemsolized normal saline 10 ml,2% lidocaine 5 ml + saline 5 ml and 2% lidocaine 10 ml were inhaled in C,L1 and L2 groups,respectively,starting from 10 min after induction.At 10 min after induction (T0),1 and 10 min after opening of vena cava (T1,2),and the end of CPB (T3),blood samples were collected from the left radial artery (LRA) and right atrium (RA) for determination of plasma interleukin8 (IL-8),tumor necrosis factor-α (TNF-α) and malondialdehyde (MDA) concentrations (using ELISA) and the expression of CD11 b on polymorphonuclear leukocytes (by flow cytometry).Blood samples were collected from the left radial artery at T0,immediately after beginning of CPB,at T3 and at 2 and 6 h after termination of CPB for blood gas analysis.The oxygenation index (OI),respiratory index (RI) and dynamic lung compliance (Cdyn) were calculated.Results Compared with group C,the ratio between IL-8 concentration in LRA and in RA (concentration of IL-8LRA/RA) was significantly decreased at T2,3,the concentration of MDALRA/RA was decreased at T3 (P < 0.05),no significant change was found in the expression of CD11bLRA/RA at each time point (P > 0.05),and RI was decreased at T3 in L1 and L2 groups (P < 0.05).There were no significant differences in the concentration of IL-8LRA/RA,TNF-αLRA/RA and MDALRA/RA,expression of CD11bLR A/RA,RI,OI and Cdyn at each time point between group L2 and group L1 (P > 0.05).Conclusion Aerosolized lidocaine inhalation can attenuate lung injury and improve lung function in patients undergoing cardiac valve replacement under CPB by reducing inflammatory responses and lipid peroxidation in lung tissues.
