1.Observation of the therapeutic effect of ganciclovir in the treatment of children with viral encephalitis

Objective To observe the clinical efficacy of ganciclovir in treatment of children with viral encephalitis.Methods 80 children with viral encephalitis were randomly divided into observation group and control group.All patients received the comprehensive treatment including anti-spasm,dehydration,control of body temperature,decreased intracranial pressure and drugs of improving brain metabolism.40 children in the observation group received intravenous ganciclovir.40 patients in the control group received intravenous ribavirin.The clinical effect of the two groups was compared.Results The total effective rate of observation group was 92.5%,that of the control group was 70.0%,the difference was statistically significant (x2 =6.721,P < 0.05).In the observation group,the main clinical symptoms including fever,headache,vomiting,convulsions and the disappearance time of unconsciousness were significantly shorter than the control group(t =5.270,9.730,5.749,5.903,5.421,all P < 0.05).The incidence rate of adverse reactions in the observation group was 7.5%,which was lower than 17.5% in the control group,but the difference was not statistically significant (all P > 0.05).Conclusion Ganciclovir in the treatment of children with viral encephalitis can improve clinical symptoms in a short time,and adverse reactions are mild,which can be used as the ideal choice for treatment of children with viral encephalitis.

4.Complicated tuberculosis infection following renal transplantation in 8 cases

This paper retrospectively analyzed the clinical manifestation,diagnostic method,as well as the clinical treatment of 8 cases,who suffered complicated tuberculosis infection following renal transplantation.Of the 8 cases,2 cases showed blood Tb-Ab positive,1 case was positive to antiacid bacillus of sputum,the rest cases with negative TBAb.3 patients with 2 showed organs infection,6 patients passed the tissue biopsy diagnose,body temperature of 6 cases fall to normal after 3-7 days of antituberculotic treatment,mild liver dysfunction appeared in 2 patients,hyperuricemia occurred in 7 patients,in addition,1 patient developed reject reaction.Among all the cases,7 cases healed,1 died of pulmonary fungal infection.The result showed that minimal invasive tissue biopsy offers reliable basis for early diagnosis and treatment of disease.
5.Application of continuous renal replacement therapy in simultaneous pancreas-kidney transplantation: One case report

OBJECTIVE: To study the effect of continuous renal replacement therapy on acute renal failure and multiple organ dysfunction syndrome following simultaneous pancreas-kidney transplantation. METHODS: A patient was complicated with acute renal failure, severe acute pancreatitis, lung infection, bleeding in anastomosisbetween duodenum and jejunum, and peritonitis following simultaneous pancreas-kidney transplantation. He was treated with immunosuppressor, antibiotics, amylopsin inhibitor, haemostatic and alimentation; at the same time, he was treated with continuous renal replacement therapy for 22 days. The Baxter system was used for continuous venovenous hemofiltration. RESULTS: The vital signs and hemodynamic indicators were stable during continuous renal replacement therapy. Pulmonary edema was well controlled, and acid-base equilibrium of water electrolyte was maintained. The function of vital organs was stableand graft function was normal following continuous renal replacement therapy for 22 days. He was completely cured and out of hospital on day 40. CONCLUSION: Continuous renal replacement therapy plays an important role in treating acute renal failure and multiple organ dysfunction syndrome following simultaneous pancreas-kidney transplantation. Thus, it is a well kidney support for ultaneous pancreas-kidney transplantation.
6.Hereditary nonpolyposis colorectal cancer:a report of 27 cases in 10 families

Objective To study the clinical characteristics of hereditary nonpolyposis colorectal cancer (HNPCC). WT5”HZMethods Ten kindreds of HNPCC in South Zhejiang area were analyzed. ResultsWT5”BZ Among the 10 families, 27 persons had colorectal cancer with age ranging from 19 to 71(average 45 3 years),74 percent of cases being diagnosed before the age of 50. The total of the colorectal cancer foci in 27 cases were 37, 40 percent of focis located proximal to splenic flexure, 30 percent of cases had multiple primary colorectal cancer, among which 14 8 percent of cases had synchronous multiple primary colorectal cancer, 18 5 percent of cases had metachronous multiple primary colorectal cancer. Thirteen cases died during follow-up of 1~23 years, 14 cases were still alive during follow-up of 1~28 years. KG2Conclusion HNPCC has definite charactistics that are of much help to early diagnosis.

8.Study on Analgesic and Antipyretics Effect of Re-Du-Ning Injection

This article was aimed to study the analgesic and antipyretic effect of Re-Du-Ning (RDN) Injection. Heating model was building on SD rats after subcutaneous injection of yeast. And then, RDN Injection was intravenous injected at the dose of 10.16 g·kg-1, 5.08 g·kg-1, and 2.54 g·kg-1, respectively. Observation was made on changes of rats' temperature. RDN Injection was intravenous injected into ICR mice at the dose of 20.30 g·kg-1, 10.15 g·kg-1, 5.08 g·kg-1 for 7 consecutive days. The methods of mice twist, hot plate and jaw pain were used in the testing of analgesic action of RDN Injection. The results showed that RDN Injection at high and middle dose can significantly reduce the temperature of heating rats models (P < 0.05 or P < 0.01). It can also significantly reduce the twisting times of mice by acetic acid (P< 0.05 or P< 0.01). It can also significantly increase the pain threshold of mice by hot plate and jaw pain (P< 0.05 or P< 0.01). It was concluded that RDN Injection had a good analgesic and antipyretic effect in mice.
9.Effects of hypothermia on secondary axotomy of nondisruptive axonal injury after diffuse brain injury in rats

Objective To investigate curative effects of hypothermia on the secondary axotomy of nondisruptive axonal injury (NDAI) after diffuse brain injury (DBI).Methods A total of 16 male Sprague-Dawley rats were randomly and equally divided into hypothermia group (at 32℃ for 6 hours) and control group (at 37.5℃ ).The axonal swelling and axonal balls were detected by means of NF68kD immunochemistry after DBI caused by fluid percussion.The changes of maximal density of axonal swelling and axonal balls in callosum,diencephalon-mesencephalon,pons-oblongata and cerebellum were compared 24 and 72 hours after injury between both groups.Results NF68kD immunochemistry well showed axonal swellings and axonal balls in whole brain.The axonal swelling and axonal balls were significantly decreased 24 hours after DBI in both groups (P<0.05),especially in diencephalon-mesencephalon ,pons-oblongata and cerebellum (P<0.01).While there showed significant decrease of axonal swellings and axonal balls in pons-oblongata and cerebellum in hypothermia group 72 hours after DBI (P<0.05,P<0.01) but insignificant changes in the callosum and the diencephalon-mesencephalon compared with control group (P>0.05 ).Conclusions Hypothermia can retard the progress of mild or severe NDAI at early stage,which would taper with the longer time after injury except for partial mild NDAI.Hypothermia may prevent mild NDAI from secondary axotomy.
10.Blastic plasmacytoid dendritic cell neoplasm: two cases report and review of literatures

Objective To identify the clinical and pathological features of blastic plasmacytoid dendritic cell neoplasm (BPDC). Methods The characteristics of BPDC hematodermic neoplasm were discussed with a report of two new cases and review the literatures. Results Both patients presented with skin nodules and the tumors were CD+4 and CD+56. Lineage specific markers for B- and T-cell were negative and the tumors did not express myeloperoxidase. Systemic chemotherapy resulted in complete remission, but the disease relapsed quickly and were unresponsive to further chemotherapy. The patients died 26 months and 11 months respectively after diagnosis. Conclasion BPDC hematodermic neoplasm is a rare subtype of lymphoma with distinct clinicopathologic and immunophenotypic features. The disease often has a fulminant course with a poor prognosis. More recent studies suggest that there is a derivation from a plasmacytoid dendritic cell precursor.
