1.Opportunity and challenge of big-data driven health service management

Our research is focused on the emerging public health management supported by big-data,which has just come into horizon in this field.We thoroughly analyzed and compared the big-data technology as well as the latest development on theories and methods in public health management. Stream processing,batch processing,distributed database and non-relational database were applied to solve problems and meet demands and challenges in public health management.Furthermore,it should become the strategic choice of innovative public health management driven by big-data.
2.Optimal allocation of hospital resources in China based on reasonable average das of stay:research and prediction

Objective To measure and predict the hospital resources to be saved by reducing one day of stay in average in China,predict its future impacts,and to probe into the importance of less average days of stay for hospital resource deployment.Methods A mathematical model was used for quantitative analysis of the actual impacts of one less day of stay in China′s hospitals for the hospital resources,and the gray forecasting model was used to predict the hospital resources so saved by achieving so during 2013 to 2025.Results One hospital day less could save 9.41% of a hospital′s total bed resources.According to the growing trend of hospital beds in China and the deployment relationship,it is predicted that more hospital resources will be saved by one hospital day less in average from 2013 to 2025.Conclusions Shorter average days of stay and optimal deployment of hospital resources,should be based on quality of care.Less ineffective waiting time to shorten average days of stay can be breakthrough to improve the comprehensive efficiency of health resources.
3.Analysis of human resources deployment and the policies in China

By means of a comprehensive coverage of the numbers,distribution,makeup and efficiency of health human resources in China,a comparative study was made as to the deployment for the present stage.It reveals the following findings:sufficient human resources in general,with its growth lagging behind economic development; out-of-balance deployment; irrational makeup as shown in the inverse ratio between doctors and nurses; good efficiency of the health professionals.Based on these findings,the researchers put forward the feasible policy suggestions as to optimizing the mechanisms of the introduction,training,evaluation and incentives for these professionals.
4.Analysis and suggestions of pediatric health resources allocation and services supply

Objective To analyze the status of pediatric health resources allocation and services supply,evaluating the equity of pediatric health resources allocation and services supply.Methods Cross-sectional data in 2011 and time series data from 2005 to 2011 are used to comparative analysis of children's hospitals,pediatricians,pediatric beds,pediatric outpatient visits,and pediatric inpatient visits status quo.Suggestions were raised to pediatric health resources allocation and services supply.Results Setbacks found in China in this regard are lack of children hospitals,shortage of backup personnel pediatricians,low proportion of pediatric beds in resource,imbalance in the supply of pediatric outpatient services,and pediatric inpatient services which fall short of demand.Conclusion Pediatrics health resources allocation should highlight provincial and regional equity and efficiency step by step,easing the significant conflicts between the deployment and service supply of pediatric health resources by a variety of means.
5.A clinical study of distal radial fractures and carpal instability

Objective To investigate carpal instability after distal radial fractures have been treated with cast immobilization or operation and its effect on clinical functions.Methods 124 patients with distal radial fracture were treated with cast immobilization,external fixation or plate internal fixation.They were followed up.Their carpal instability was evaluated according to X-ray manifestations before and after treatment;their clinical carpal functions were assessed according to Gartland and Werley scoring system.Results The follow-ups revealed five types of carpal instability:palmar or dorsal shift of the carpus,VISI(volar intercalated segment instability),DISI(dorsal intercalated segment instability)and scapholunate dissociation.The incidence of carpal instability was 58.1%before treatment,21.8%after treatment and 22.6%at the follow-ups.Functional results were excellent and good in 80.6 %of the total patients,in 84.5%of those in the stability group,and in 75.0%of those in the instability group.Conclusions Distal radial fractures usually lead to carpal instability.Operations can correct radius-carpal instability effectively,but it is difficult to correct scapholunate dissociation.The function of carpal joint is evident ly poorer in patients with carpal instability than in those with carpal stability,for it is mainly affected by the integrality of radius-carpal joint.
6.Analysis of medical resources allocation in four municipalities and equity evaluation

Objective To offer decision support for reasonable allocation of medical resources by analysis of the present resources allocated.Methods Lorenz curve and Gini coefficient method were called into play.This paper studied such medical resources as the practicing doctors,registered nurses,medical institutions and hospital beds,from both the population distribution and geographical distribution aspects.The purpose is to compare the allocation in these municipalities (Beijing,Tianjin,Shanghai and Chongqing).Results If the Lorenz curve is drawn based on the total population served with such resources,the Gini coefficients indieate relatively balanced allocation of medical institutions,practicing physicians,and registered nurses in the municipalities,and highly balanced allocation of hospital beds.If the curve is drawn based on geographical distribution,the Gini Coefficient indicates relatively balanced allocation of medical institutions allocation in these municipalities,yet considerable difference in the allocation of practicing physicians,registered nurses,and hospital beds.Conclusion The study shows that medical resources allocation in China in these municipalities mirrors the condition of Priority to population density,and neglect of geographical distribution.It also reveals poor equity in the geographical distribution of medical resources,which plagues accessibility of medical resources.
7.Empirical analysis and evaluation of the technical efficiency and impact factors for tertiary hospitals

Objective For an analysis and evaluation of the technical efficiency of tertiary hospitals in Tianjin.Methods Data Envelopment Analysis of different combinations of indicators and Spearman Rank Correlation Analysis were used for a study of the relative efficiency between 28 tertiary hospitals and the correlation between the indictors.Results The study showed that 12 hospitals reached DEA efficient,effective rate of 42.86%,and there is a correlation between the five sets of indicators. Conclusion Regulators of Tianjin hospitals should pay attention to efficiency calculations,make full use of medical resources,strengthen scientific management,and enhance the competitiveness of the hospital.
8.Empirical analysis and evaluation on diachronic dynamic and synchronic compare operating efficiency of hospitals

Objective To probe into the varying efficiency of public hospitals and recommend on efficiency improvement.Methods By means of DEA method including CCR model,BCC model and Malmquist index,to study the static and dynamic changes of operating efficiency of hospitals in Tianjin from 2008 to 2012.Results The overall efficiency of these 24 hospitals is not high,and the efficiency of the tertiary hospitals (0.900)is higher than the second-level hospitals(0.828).There exist problems that the size of the second-level hospitals is either too big or too small,a lot of health resources can be saved by managing the non-DEA efficient hospitals.The trends of the total factor productivity of 24 hospitals in Tianjin from 2008 to 2012 are increased.Thanks to the advanced technology and innovation, the overall efficiency of tertiary hospitals is improved.Through improving organizational management level,the overall efficiency of second-level hospitals is improved.Conclusion Hospital managers should introduce the high-level medical workers,enhance technological innovation,improve and standardize the hospital management and service system,optimize the allocation of medical resources and select the appropriate size of the hospital with the hospital's own situation to improve the operating efficiency of the public hospitals.
9.Equity analysis of health resources allocation with the concentration index method for provinces in China

Objective To analyze allocation of health resources and utilization of health services,and evaluate the equity of health resources allocation and health services utilization.Methods Concentration curve and concentration index(CI)were used to evaluate the equity of health resources allocation in terms of health professionals,hospital beds,medical institutions,outpatient visits and hospitalizations in China.Results The CI for health professionals,hospital beds,and medical institutions is 0.038,-0.012,and-0.116 respectively.That for outpatient visits and hospitalizations is 0.111 and -0.023 respectively.Conclusion Health resources allocation in hospital beds and health professionals is fair,while the equity of medical institutions and outpatient service utilization needs to be improved.
10.Study on efficiency of health resources allocation with data envelopment analysis of provinces in China

Objective To evaluate the efficiency of health resources allocation of 31 provinces,autonomous regions and cities in China in 2011,identify the causes for low allocation efficiency and provide suggestions accordingly.Methods By ways of literature research and data envelopment analysis.Results The average efficiency value of these provinces and cities was 0.973.The health resources allocation of 18 provinces and cities were found efficient in general,and that of the other provinces and cities were found non-efficient.They respectively account for 58.1 %and 41.9%.Conclusion To improve the efficiency of health resources allocation of the 13 provinces and cities,their policymakers should increase government funding support,attract private capital into medical institutions,optimize health investment scale,improve medical management and foster medical and health personnel development.
