Objective To compare the effect of acupuncture and swallowing training on dysphagia in recove-ring stroke patients. Methods One hundred and thirty-two subacute stroke inpatients,73 males and 59 females, with a mean age of(67.9±9.0)years,26 cerebral hemorrhage patients and 106 cerebral infarct patients,participa-ted in the study. The patients were divided into a swallowing training group(ST,n=53)treated with thermal-tactile stimulation, deglutition exercises and food swallowing training;an acupuncture group(AP,n=48) treated with acu-puncture at the Fengchi(GB20),Lianquan(RN23),Bailao(EX-HN15),Jinjing(EX-HN12) and Yuye (EX-HN13)acupoints;and a control group(CG,n=31).The interventions were administered once a day,6 times a week. Before and after treatment, the patients were evaluated with a water-swallowing test and the"Any Two"test, and the arterial oxygen saturation(SpO_2) was monitored by pulse oximetry. Results The ST and AP groups showed significant improvement in water-swallowing and on the"Any Two"test after the treatment regimen, and were significantly better than the control group. There was no significant difference between the ST and AP groups. The average decrease in SpO_2 during water-swallowing was under 2%.After treatment, the baseline SpO_2 and post-water-swallowing SpO_2 of the ST group were significantly higher than those of the control group. The post-water-swallowing SpO_2 of the AP group was also significantly higher than that of the control group. Conclusions Acupuncture and swallowing training have similar effects in treating dysphagic stroke patients.Swallowing function and misaspiration cannot be predicted based on decreases in SpO_2.

Objective To construct and evaluate a nomogram for predicting the risk of peri-operative complications after radical retro-pubic prostatectomy (RRP) with standard classification criteria. Methods The peri-operative complications and clinicopathological data of 240 patients (50-82 years old) who had undergone RRP for prostate cancer from June 2006 to June 2011 were retrospectively collected.Gleason score:95 cases < 7; 145 patients ≥ 7;Clinical staging:cT1 is 1 case (0.4%),T2a 5 cases (2.1%),T2b 7 cases (2.9%),T2c 162 cases (67.5%),T3a 26 cases (10.8%),T3b 39 cases (16.3%).The peri-operative complications (in 30 days after surgery) were classified by Clavien-Dindo Classification system (the occurrence rates are as follows:rectum injury 1.6%,wound infection 2.0%,deep venous thrombosis 1.2%,urinary leak 5.0%,lymphocele 4.5%,myocardial infarction 5.8%,second look operation 1.6%),and a logistic regression model was used to construct the nomogram. Results BMI,N staging and Blood loss more than 200ml during surgery were independent prognostic factors of RRP morbidity in multivariate logistic regression. The nomogram predicting the risk of peri-operative complications showed relative good concordance index (0.633) and good calibration. Conclusions Based on the clinicopathological factors,a nomogram to predict the probability of peri-operative complications in patients undergone RRP was constructed. This statistical tool may be beneficial in judging operation risk and help consulting with patient before or after surgery.

To explore the influence of matrix materials in complicate ingredients on traditional Chinese medicine and investigate the excipients selection model based on balanced release characteristics of multicomponents, the influence of HPMC (K4M, K15M, K100M) and Carbomer (934P, 971P, 974P) was illustrated by testing in vitro release of ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in Panax notoginseng saponins (model drug, PNS). According to in vitro release results of PNS matrix tablets in water and artificial intestinal juice, the release curves were analyzed with Peppas equation and simulating factor (f). Significant differences in k value and n value among ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 existed in various formulations. The release behaviors from various excipients could be described with Non-Fickian transport or super Case II transport pattern. The f2 values for ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in 971P matrix tablet containing 30% Carbomer 971P were 74.91, 53.45, 57.89 in water and 79.35, 55.51, 51.89 in artificial intestinal juice, respectively. The release profiles fit for the regulation of FDA. The result revealed that the balanced release rates of Rg1, Rb1 and R1 in 971P matrix tablet were obtained.
