Objective To investigate the relationship between serum 25-hydroxyvitamin D level and gestational diabetes mellitus (GDM).Methods The fasting plasma glucose,fasting insulin,25-hydroxyvitamin D,and high sensitive C-reactive protein (hs-CRP) levels in the normal glucose tolerance group and gestational diabetes mellitus (GDM) group were measured.The homostasis model assessment insulin resistant (HOMA-IR) and homostasis model assessment β cell (HOMA-B) were calculated for assessing the insulin resistance and β-cell function.The relationship between 25-hydmxyvitamin D and hs-CRP,HOMA-IR,and HOMA-B were analyzed.Results The level of 25-hydroxyvitamin D in the GDM group [(2.16 ± 0.38) nmol/L] was significantly lower than that in the normal glucose tolerance group [(4.13 ±0.79) nmol/L](P =0.001).Serum 25-hydroxyvitamin D level was negatively correlated with hs-CRP (r =-0.448,P =0.002) and HOMA-IR (r =0.396,P =0.004),but was positively related with HOMA-B (r =0.495,P =0.001).Conclusions The serum 25-hydmxyvitamin D level remarkably decreases in GDM patients.Vitamin D deficiency may cause the functional damage of beta cell of islet,increase insulin resistance,and thus contribute to the development of GDM.

Objectives To study the incidence of Trichomonas vaginalis(TV)infection in Chinese male patients with nongonococcal urethritis(NGU),to evaluate the sensitivity and specificity of urine-based and urethral swab polymerase chain reaction(PCR)detection,and to set up a method of non-invasive detection for male TV infection.Methods One hundred and five male NGU patients were collected from the STD clinic.Two urethral swabs were obtained from each patient,one for InPouch TV culture system,the other for PCR;one urine specimen(first-void urine or first-catch urine)was also collected for PCR detection.Culture was considered as the"gold standard".Compared with the culture,the sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)of two PCR detection were calculated.Results The positive rate of InPouch TV system detection was4.76%(5/105);the positive rate of urine-based PCR and urethral swab PCR detection was3.81%(4/105)and4.76%(5/105),respectively.In comparison with the culture,the sensitivity,specificity,PPV and NPV were80%,100%,100%and99%for urine-based PCR and80%,99%,80%and99%for urethral swab PCR.Conclusions TV is one of the etiological agent for male NGU,the positive rate of TV is4.76%in our study.The urine-based PCR detection has a higher sensitivity and specificity and provides a noninvasive method which is feasible in practice.

After years of respiratory medicine clinical practice teaching , we have discovered that there exist some problems such as teachers' job burnout, teachers' lowprofessional knowledge level of clinical teaching and lack of teaching ability, medical students' fear, low self-esteem, psycho-logi-cal fear and emotional weariness breeding as well as the patients and their family members ' refusal of practice because of their worrying about being a test and so on. To solve these problems , we have put forward some proposals such as recognizing and rewarding teachers to affirm their teaching levels , organizing teachers to participate in academic conferences, seminars, etc. to expand their professional knowledge in order to improve their overall quality. And at the same time, we have adopted PBL teaching method, applied multimedia technology and network platforms, put emphasis on basic skills training, the improvementof departmental rotation assessment, the establishment of teaching files and doctor-patient communication and other ways to improve clinical teaching quality and develop interns' clinical thinking ability to help them transitto clinicians.

Background The pathogenesis of allergic conjunctivitis has not been clearly established.Current researchers indicate that interleukin-4 (IL-4), IL-5 and IL-13 may play an important role in allergic conjunctivitis.But whether the roles of these inflammatory factors are same in different types of allergic conjunctivitis remains unclear.Objective This study was to investigate the expressions of IL-4, IL-5 and IL-13 in ocular surface with different types of allergic conjunctivitis.Methods A prospective cohort study was designed.Eighty individuals were recruited in Shanxi Eye Hospital from April 2013 to September 2014, including 20 patients with vernal keratoconjunctivitis (VKC), 20 patients with seasonal allergic conjunctivitis (SAC), 20 patients with perennial allergic conjunctivitis (PAC) and 20 normal healthy subjects.Surficial tissues were binocularly scraped using disinfected scraper from upper eyelid conjunctiva, and 4 μl of tear fluid was obtained with capillary tube.The expressions of IL-4, IL-5 and IL-13 protein and mRNA in the conjunctival epithelial cells were detected by immunohistochemistry and real-time fluorescence quantitative PCR.The IL-4, IL-5 and IL-13 concentrations in tear fluid were assayed by Luminex method.This study complied with Declaration of Helsinki and the research protocol was approved by the Shanxi Eye Hospital Ethics Committee.Written informed consent was obtained from each subject prior to entering the cohort.Results IL-4, IL-5 and IL-13 were positively expressed in cytoplasm of conjunctival epithelial cells in the VKC group,SAC group and PAC group,but the expressions of IL-4,IL-5 and IL-13 were absent in the normal control group.The relative expression levels of IL-4 mRNA were 4.11±1.24,2.71±0.71 and 2.00±0.80;the relative expression levels of IL-5 mRNA were 4.02±0.43,2.07±0.45 and 1.47±0.50;and the relative expression levels of IL-13 mRNA expression levels were 6.44±0.66,4.35±1.26 and 2.39±0.86 in the VKC group,SAC group and PAC group, showing significant differences among the 4 groups (F =51.32,220.18,162.49, all at P＜0.01).The relative expression levels of IL-4,IL-5 and IL-13 mRNA were significantly higher in the VKC group than those in the SAC group and PAC group;and those in the SAC group were significantly elevated in comparison with the PAC group (all at P＜0.05).No IL-4, IL-5 and IL-13 were detected in the tear fluid in the normal control group;while the concentrations of IL-4,IL-5 and IL-13 in the tear fluid were (14.06±3.50), (10.88±1.82) and (34.28±8.42) pg/ml in the VKC group,and (7.71 ±0.65), (5.10± 1.33), (23.77±6.29) pg/ml in the SAC group as well as (3.30± 1.50) pg/ml, (2.43± 1.28) pg/ml and (17.67 ± 4.28) pg/ml in the PAC group, showing significant differences among the 3 groups (F =200.29,260.49,128.23, all at P＜0.01).IL-4, IL-5 and IL-13 concentrations in the tear fluid were significantly higher in the VKC group than those in the SAC group and PAC group,and those in the SAC group were significantly raised in comparison with the PAC group (all at P＜0.01).Conclusions IL-4,IL-5 and IL-13 participate in the pathogenesis of multiple allergic conjunctivitis,but their expressions in the ocular surficial tissue are discriminatory in different types of allergic conjunctivitis.

Objective To study the population's iodine nutritional status of Yanping area of Nanping City,Fujian Province,and to provide scientific basis for prevention and control of iodine deficiency disorders (IDD).Methods According to 2014 Key Population Iodine Nutrition in Fujian Province and the Related Health Monitoring Programme,five subareas were selected according to the five directions as east,west,south,north and centre in Yanping area,and one township was selected in each subarea.In addition,one subdistrict administrative office was chosen in urban area,and one neighborhood committee was selected as a monitoring site.Water iodine detection:two water samples from centralized water supply pipe were collected in selected townships and subdistrict administrative office,and arsenic cerium catalytic spectrophotometry was used to detect the level of water iodine.Salt iodine detection:four administrative villages were selected in each monitoring township,15 households were selected in each village,and a total of 300 households were selected;and 31 households were selected in urban area.The General Test Method for Determination of Iodine in Salt Industry (GB/T 13025.7-1999) was used to detect the level of edible salt iodine.Urinary iodine detection:①children aged 8-10 years old:one primary school was chosen in each monitoring township,30 children aged 8 to 10 were selected in each primary school,and a total of 150 children (half males and half females) were selected;one primary school was chosen in monitoring urban area,and 100 children (half males and half females) aged 8 to 10 were selected;②pregnant women and nursing mothers:20 pregnant women in each monitoring village were selected,and a total of 100 pregnant women were selected;50 pregnant women and 50 nursing mothers were selected in urban area;③adults aged 18 to 45 years old:50 adults (half males and half females) were selected in urban area.Urine samples at random times during the day were collected,and Arsenic Cerium Catalytic Spectrophotometry for Test of Urinary Iodine (WS/T 107-2006) was used to detect the level of urinary iodine.Goiter examination of children aged 8 to 10:palpation was used to examine goiter of all children.Results A total of 12 drinking water samples were collected,the levels of water iodine were all ＜ 3.5 μg/L.A total of 331 edible salt samples were collected,the median of salt iodine was 23.3 mg/kg,iodized salt coverage rate was 100.0％ (331/331),and qualified iodized salt consumption rate was 93.1％ (308/331).A total of 250 children were examined by the thyroid palpation,the rate of goiter was 0.8％ (2/250);the median of urinary iodine was 174.0 μg/L;the medians of children in rural and urban areas were 192.3 and 155.7 μg/L,respectively,the difference was statistically significant (Z =-3.582,P ＜ 0.01),and urinary iodine level of rural children was higher than that of urban children.Totally 150 pregnant women urine samples,50 nursing mothers urine samples,52 adults urine samples were collected,respectively,the medians were 143.7,116.2 and 115.6 μg/L,respectively,urinary iodine of pregnant women was lower than national standard (≥ 150 μg/L).Conclusions The population's iodine nutrition level is appropriate.At the same time,there are special people (pregnant women) at risk of iodine deficiency.

Background The visual evoked potential (VEP) is an important functional index to assess visual cortex plasticity.Monocular form deprivation(MD) of rat is a classic model to study visual cortex plasticity change.Utilizing VEP technique record the shift of ocular dominance in adult rats model is of an important significance for explore the treating opportunities of amblyopia.Objective The present study was to observe the pattern VEP changes in different ages of Long-Evens rats and the adult rats after binocular form deprivation.Methods Thirty-six healthy SPF Long-Evens rats were divided into PW3,PW4,PW5,PW6 groups according to the postnatal weeks,and PW7 rats included 36 SPF Long-Evens rats.The left eye lids of the rats were sutured in PW3,PW6 and PW7 rats for 3,5,7 days respectively,and PVEP in both left and right eye were recorded to assess the rat age that visual cortex plasticity ended.Bilateral PVEP were recorded in PW7 rats,and the bilateral eyelids were sutured to establish the bilateral form deprivation models.The right eyes were opened in 7,10,14 days,and the left eyes were opened in the next 3,5,7 days respectively and the PVEPs were recorded again to find the shift of ocular dominance and whether binocular form deprivation induce the visual cortex plasticity in adult rats.Results The P100amplitudes of the left eyes were gradually declined and those of the right eyes were raised from 3 to 7 days after MD of the left eyes in rats of PW3 group in comparison with before MD ( P＜0.01 ).In PW6 groups,no significant changes in P100amplitudes of both the right and left eyes were found in the third day after MD,but significant raise in the right eyes and lowing in the left eyes were seen in the 5 and 7 days after MD in comparison with before MD(P＜0.01 ).No any P100changes in both eyes were found in 3-7 days after MD compared with before MD in PW7 rats (P＞0.05).The C/I ratio (contralateral VEP amplitude to ipsilateral VEP amplitude in occluded eyes) in the rats of PW3,PW4 and PW5 groups were 1.07±0.15,1.16±0.16 and 1.14±0.15 respectively in 3 days after MD,showing the significant lowing in comparison with before MD (2.69±0.45,2.58±0.4,2.62±0.32) (P＜0.01),but those of PW6 and PW7 were unchanged (2.80±0.48 vs 2.90±0.46,2.59 ±0.36 vs 2.90±0.46,P＞0.05 ),indicating the absence of ocular dominance plasticity in the adult rats.In rats of PW7 with binocular form deprivation for 14 days,a significant decrease was observed in the C/I ratio in rats with next MD for 3 days,demonstrating that the visual cortex plasticity was reactivated after 14 days of binocular form deprivation in PW7 rats( 1.33±0.18 vs 2.70±0.45,P＜0.01 ).Conclusions PVEP can be recorded in Long-Evens rats.It is a major index for identifying the shift of ocular dominance in the Long-Evens rats.Binocular form deprivation can reflect the visual cortex plasticity in the adult rats.
