5.Individualized thyrotropin suppression in patients with differentiated thyroid cancers

The epidemic characteristics of differentiated thyroid cancer(DTC)are changing, thyrotropin(TSH)suppressive therapy is also improving in recent years. The risks of recurrence and the death of patients, as well as the adverse effects of levothyroxine sodium treatment should be fully considered during thyroid hormone administration for patients of DTC. Hence, the degree of TSH suppression should be individualized in patients with DTC.
6.Relation of Dynamic Error Analysis System and Pulse Wave Velocity Measure

Obiective To detect the carotid-femoral pulse wave veloeity(CF-PWV) and evaluate relation in pulse pressure (PP) and pulse wave velocity ('PWV), and prove the effect of dynamic error analysis system on pulse wave velocity measure. Methods 486 cases with essential hypertension were detected with carotid-femoral pulse wave velocity. The dynamic error was controlled in three kinds of condition: 15%>error>10%, 10%>error>5% and 5%>error, which PWV values were tested repeatedly with the effect of different errors observed. At the same time the blood pressure was measured and the pulse pressure was calculated. Results The pulse wave velocity which pulse pressure ≥60mm Hg (1mm Hg=0.133 kPa) was significantly higher than pulse pressure <60turn Hg [(14.2± 2.34)m/s and (8.5±1.81)m/s, P<0.001] . Pulse wave velocity and pulse pressure and age showed a significant positive correlation (pulse pressure r=0.529, P=0.000; age r= 0.331 P=0.003).When dynamic error is more than 10% and less than 15%, the diversity of PWV values showed the significance of statistics. Conclusion The effect of dynamic error analysis system on pulse wave velocity measure, less error test pattern is advantageous to enhance accuracy. The pulse pressure is closely related to pulse wave velocity. Arterial pulse wave velocity is a new evaluation of the indicators of arterial stiffness, and it's more scientific, accurate and eonvenient.
7.Risks and prevention for ethical review of drug clinical trials

The authors presented in the paper the following subjects: a description of risk exposures commonly found in drug clinical trials; risk types in terms of the personnel organization of ethical review boards, and the composition of the ethical review board members, as well as ethical training, ethical review procedures and supervising of the ethical review. To avoid these risks, an analysis is made on the causes of ethical review risks, and recommendations are proposed on international certification, reform of ethical review board setup, greater efforts on training, introduction of ethical supervision and ethical acceptance procedure.
8.Functions of human periodontal myofibroblast in vitro.

OBJECTIVETo investigate the functions of human periodontal myofibroblast (MFB) in vitro.
METHODSHuman periodontal fibroblast (hPDLFs) was cultured and induced to MFB by transforming growth factor-β1 (TGF-β1). MFB was denoted as the experimental group, whereas the hPDLFs was the control group. The groups were continuously cultured and harvested at 0, 12, 24, 48, and 72 h. The MFB marker α-smooth muscle actin (α-SMA) was examined by immunocytochemistry. The expression of fibronectin (FN) between MFB was examined by immunocytochemistry to detect the MFB contact relationship. The mRNA expression levels of α-SMA, collagen (Col) I, and Col III were measured by reverse transcription-polymerase chain reaction (RT-PCT) to analyze extracellular matrix secretion. The protein expression levels of α-SMA and Col I were also assessed by Western blot.
RESULTSThe experimental group had significantly higher α-SMA expression than the control group at 0 h (P < 0.001). A positive expression of FN was found between MFB. The experimental group had significantly higher expression levels of Col I and Col III than the control group at 24 h (P < 0.001).
CONCLUSIONHuman periodontal MFB presents a continuous, high expression of α-SMA. MFB could interact through FN. MFB is significantly capable of extracellular matrix secretion.

9.The monitoring of intra-abdominal pressure in critically ill patients

Objective To monitor intra-abdominal pressure (IAP) in critically ill patients.Methods A prospective cohort study was conducted.IAP was measured through the bladder technique.Patients were screened for intra-abdominal hypertension (IAH,IAP ≥ 12 mmHg,1 mmHg=0.133 kPa) upon ICU admission.The patients with IAH/abdominal compartment syndrome (ACS) were given appropriate treatment and management for IAH and/or ACS.Mean arterial pressure (MAP),IAP,abdominal perfusion pressure (APP),filtration gradient (FG) and serum creatinine (Cr) were determined in patients with or without IAH,as well as in survivors and non-survivors.Results The entire protocol of IAP measurement was completed in 88 patients.Number of IAH and ACS patients was 25 (28.4%) and 2 (2.3%),respectively.The number of survivors was 80 (90.9%),with 8 (9.1%) non-survivors.Compared with non-IAH patients,IAP and SCr were increased in IAH patients [IAP (mmHg):14.16 ± 2.43 vs.8.13 ± 2.28,t=10.984,P=0.000; SCr (μmol/L):126.72 ± 83.02 vs.73.41 ± 37.59,t=3.087,P=0.005],with a lower FG (mmHg:59.32 ± 17.08 vs.70.24 ± 15.03,t=-2.956,P=0.004).There were no significant differences in MAP and APP between IAH group and non-IAH group [MAP (mmHg):79.18 ± 12.33 vs.88.71 ± 17.34,t=-1.368,P=0.190; APP (mmHg):73.40 ± 16.11 vs.78.37 ± 14.32,t=-1.415,P=0.161].Compared with survivors,non-survivors showed significantly lower APP and FG [APP (mmHg):60.88 ± 14.58 vs.78.56 ± 14.06,t=3.382,P=0.001 ; FG (mmHg):50.38 ± 16.18 vs.68.81 ± 15.44,t=3.208,P=0.002],and higher SCr (μmol/L:129.12 ±83.62 vs.84.36 ± 55.15,t=-2.082,P=0.040).There was no significant difference in IAP and MAP between survivors and non-survivors [MAP (mmHg):71.00 ± 25.46 vs.84.38 ± 13.53,t =1.224,P=0.238 ; IAP (mmHg):10.62 ±5.34 vs.9.76 ± 3.40,t=-0.647,P=0.519].Conclusions Earlier IAP measurements in critically ill patients are essential for the detection of IAH/ACS and renal injury.With appropriate management of IAH/ACS,significant decrease in morbidity and mortality of patients has been achieved.
10.Studies on Relationship of Serum Insulin like Growth Factors and Catch-up Growth in Rats

Objective To investigate relationship of serum insulin like growth factors(IGFs) and catch up growth. Methods The model of rats intrauterine growth retardation (IUGR) was built by clamping bilateral uterine arteries and veins of pregnant rats. The growth parameters (body weight and length), organs weights (brain, lungs and liver) and serum concentrations of IGF Ⅰ, Ⅱ of all neonatal rats was measured at birth and postnatal day 3 and day 5, respectively. Results (1)At birth, the serum levels of IGF Ⅰ, Ⅱ and the average growth parameters, organs weights in IUGR rats were significantly lower than those in control group( P 0.05). The serum of IGF Ⅱ in high catch up group was significantly higher than that at birth( P 0.05). Conclusion Both IGF Ⅰ, Ⅱmay play an important role in the regulation of catch up growth and the decrease of IGF Ⅰ, Ⅱlevels may be the internal factors contributing to the absence of postnatal catch up growth in IUGR rats.
