1.Clinical value of MRI in treatment choice of anterior disc displacement with reduction

Objective To investigate the clinical value of MRI in treatment choice of anterior disc displacement with reduction. Methods 1.5 T superconducting MR was used to scan bilateral temporomandibular joints in 72 consecutive patients who were diagnosed by MRI as unilateral(66 patients)/bilateral(6 patients) anterior disc displacement with reduction at closed and open mouth. MRI sequences included oblique sagittal T2 weighted image and proton density weighted image, and 78 joints' images were acquired. According to the severity of clinical symptoms, all joints were divided into severe symptom group (45 joints) and mild symptom group (33 joints). Treatment was performed in severe symptom group , while follow up was conducted in mild symptom group. Disk position(angle between posterior margin of the disc and the condyle vertical line), disk morphology(biconcave, biplanar, biconvex, rounded, folded, thick posterior band), and joint effusion (none effusion, mild effusion, moderate effusion, marked effusion) were evaluated by two radiologists. The observer agreement for image evaluation was calculated using Kappa statistics. Group difference in disk position was compared using t-test and Chi-square test was used to compare group difference in disk morphology and joint effusion. Results The Kappa value between two radiologists were 0.816 and 0.832 (P<0.01) on evaluation of disk morphology and joint effusion. Statistical results indicated that the angles between posterior margin of the disc and the condyle vertical line in severe symptom group (54.2 ± 10.9)° were larger than those in mild symptom group (46.4 ± 9.0)° (t=3.37, P<0.05). There was no significant difference in disc deformation incidence between the two groups (χ2=1.18,P=0.277). The common deformation in sever symptom group was thick posterior band (χ2=5.65, P<0.05), and in mild symptom group was biplanar (χ2=5.67, P<0.05). No statistical difference in joint effusion incidence was observed(χ2=1.02,P=0.312). Moderate and marked effusion were more common in sever symptom group than that in mild symptom group (χ2=6.55,P<0.05). Conclusions MRI is a useful tool for making treatment choice in anterior disc displacement with reduction. Treatment should be given when the following occurred on MRI:moderate anterior disc displacement, disc deformation (excepting for biplanar), moderate or marked joint effusion.
2.Relationship between Ambient Air Pollution and Children's Respiratory Health in China

Objective To analyze the relationship between children's respiratory health and ambient air pollution in China.Methods The studies on ambient air pollution and children's respiratory health in China published from 1980 to 2008 were collected and 12 of them were selected.Pearson correlation and linear regression analysis were used to find the correlations between levels of air pollutants and children's history prevalence of respiratory diseases and symptoms.Results Strong associations between the levels of TSP and the history prevalence of cough, long-term cough, sputum, long-term sputum, bronchitis for children were found, and 0.50%,0.12%,0.43%,0.09% and 0.51% increased respectively as per 10 ?g/m3 increased for TSP;there were significant associations between the levels of PM10 and the history prevalence of cough, sputum and bronchitis for children, and 2.64%, 2.27%,2.17% increased respectively as per 10 ?g/m3 increased for PM10.Significant associations were also found between the levels of PM2.5 and the history prevalence of cough, sputum and bronchitis for children, and 4.56%,3.49%, 3.74% increased respectively as per 10 ?g/m3 increased for PM2.5.Pearson correlation analysis indicated a significant increase of the history prevalence of wheeze for children with increase of levels of SO2;there were significant associations between the increase of the levels of SO2 and increase of the history prevalence of cough and sputum for children as the levels of SO2 lower than 0.15 mg/m3 , and 1.65% and 1.50% increased respectively as per 10 ?g/m3 increase of SO2.Significant associations between the levels of NOx lower than 0.10 mg/m3 and the history prevalence of long-term cough, long-term sputum, bronchitis for children were found, and 0.86%, 0.51% and 3.21% increased respectively as per 10 ?g/m3 increase of NOx.Furthermore, the associations between air pollutants and the history prevalence of children's respiratory health in north China were more significant.Conclusion The air pollutants in China are risk factors of children's respiratory system health, and impacts of air pollutants on children's respiratory system health in north China is more significant than that of the whole regions.
3.Continuous Positive Airway Pressure Treatment for Epilepsy Complicated with Obstructive Sleep Apnea Syndrome

Objective To investigate the effect of continuous positive airway pressure (CPAP) on seizure frequency in epilepsy patients complicated with obstructive sleep apnea (OSA). Methods Subjects were divided into CPAP group (20 subjects) and medication group (22 subjects) according to whether they can tolerate CPAP. CPAP group were treated with CPAP combined with antiepileptic drugs. Medication group were treated with antiepileptic drugs alone. Seizure frequency and apnea-hypopnea index (AHI) were compared between groups before and after 4 weeks and 24 weeks of treatment. Results Baseline seizure frequency and apnea-hypopnea index (AHI) were compared between groups(P>0.05). A sig?nificant reduction of seizure frequency was observed in CPAP group after 4 weeks and 24 weeks of treatment compared to that before treatment (P<0.01). In medication group, no significant difference in seizure frequency was noted betweem 4 weeks and 24 weeks after treatment compared to that before treatment(P>0.05). A significant reduction of AHI was ob?served in the CPAP group after 24 weeks of treatment compared to that before treatment (P<0.01), but no change of AHI by treatment was observed in medication group (P>0.05). Conclusion Treatment of OSA in patients complicated with epilep?sy may improve seizure control in short and long term, but longer observation time and more samples are needed for further research.
4.Research of integrative medicine therapy for severe systemic lupus erythematosus from medical history analysis

Objective To ivestigate integrative medicine therapy for severe systemic lupus erythematosus.Methods In 2005,we successfully salvaged a severe SLE patient,who was an inpatient from the TCM Rheumatology department of China-Japan relationship hospital.The mainly clinic manifestations included severe pulmonary hypertension,gangrene and pancytopenia.We analyzed this ease.Results By integrative medicine therapy,we successfully salvaged this patient.Conclusion The key points for integrative medicine therapy for severe systemic lupus erythematosus included:1.Using strong measures to control the disease progress,including hormones,immunosuppressive agents; 2.Positive and effective symptomatic treatment aiming at pulmonary hypertension and severe gangrene; 3.Emphasizing the effect of Chinese medicine,using integrative medicine therapy.In sum,in this case,we applied integrative medicine therapy and got very perfect clinic effect at last.
5.Effect of wet compress of low temperature lidocaine on local pain caused by fructose infusion in children

Objective To observe the effect of local pain alleviation by wet compress with room temperature and low temperature lidocaine and cold wet compress in fructose infusion in children. Meth-ods 120 children patients with pain during fructose infusion were randomly divided into the wet compress group, the room temperature lidocaine group and the low temperature lidocaine group with 40 cases in each group. The analgesic effect was observed in the three groups. Results Analgesic effect of the low temper-ature lidocaine group was significantly better than the other two groups. Conclusions The wet compress with low temperature lidocaine can relieve the local pain in fructose infusion in children.
6.Enzyme-catalyzed synthesis of ASGPR ligand-targeted modifier in non-aqueous medium.

The asialoglycoprotein receptor (ASGPR) was used to mediate drug carrier for hepatic targeted drug delivery, this article showed the enzyme-catalyzed esterification of galactose and vinyl stearate and a kind of ASGPR ligand-targeted which was used to insert the surface of liposome has been synthesized. The structure of product has been confirmed by TLC, ESI-MS and 1H NMR. The factors of types and quantity of enzyme, organic solvents, molar ratio of substrate, temperature and time of reaction have been studied. Results showed when using acetone as reaction medium, the quantity of Novozym 435 immobilized lipase was 30 mg mL(-1), molar ratio of galactose to vinyl stearate was 1:5, and reacted at 60 degrees C for 12 h, the transformation of vinyl stearate reached more than 70%. This study provides a novel and efficient route to the synthesis of ligand-targeted modifier.
7.Modulatory function of high-dose hepatitis B surface antigen vaccine to cellular immune responses in mice

AIM: To observe the effects of high-dose hepatitis B surface antigen (HBsAg) vaccine on cellular immune response in BALB/C mice. METHODS: The mice were immunilized separately with low-dose and high-dose HBsAg vaccine by intramuscular injection two times. The specific proliferative activities of T lymphocytes were measured by [ 3H]-TdR incorporation assay. IL-2 as well as IFN-? levels in the culture supernatant of T cells and anti-HBs IgG2a lever in sera were detected by enzyme-linked immunoabsorbent assay. RESULTS: After first vaccination with high-dose HBsAg, the proliferative activities of T cells in the experimental group were significantly stronger, both levels of IL-2 and IFN-? were markedly higher than that in the control group and the percentage of mice to produce serum anti-HBs IgG2a was significantly higher compared to that of mice immunilized by low-dose HBsAg. All data in experimental groups were further increased after second dose of vaccine. CONCLUSION: Vaccination of mice with high-dose HBsAg can induce cellular immune responses tended to Th1(T helper 1 subset) response.
8.A multicenter study effect of berberine hydrochloride on serum HIF-1αand Caspase -3 levels in patients with acute cerebral infarction

Objective To explore the effect of berberine hydrochloride on serum HIF-1α,Caspase -3 levels in patients with acute cerebral infarction with a multicenter study.Methods 126 patients with acute cerebral infarction from January 2012 to January 2014 were selected in the Affiliated Hospital of Hebei Engineering University,the First Hospital of Handan City,Handan Central Hospital were divided into experimental groups and control group using random number table.Control group had 63 cases,were given 80 mg ozagrel sodium injection intravenously,twice a day,and 800 mg calf blood protein injection intravenously,once per day on the basis of conventional treatment;at the same time,100 mg of oral aspirin tablets were taken,once per night,7 days for a course of treatment.Experimental group were given berberine hydrochloride on the basis of conventional treatment,0.7 g orally,three times a day,7 days for a course of treatment.After a course of treatment,serum HIF-1α,Caspase-3 levels in patients with acute cerebral infarction were observed before and after treatment.Results Serum HIF-1αlevels in two groups were reduced after treatment,and its level in experimental group[(733.49 ±225.01)ng/mL]was lower than that in control group [(975.32 ±311.35)ng/mL],the difference was statistically significant (P<0.05 ).Serum Caspase-3 levels in two groups after treatment were decreased,and its level in experimental group [(1 1.88 ±2.66 )ng/mL]was lower than the control group [(12.51 ±3.77)ng/mL],the difference was statistically significant (P <0.05).Conclusion Berberine hydrochloride could improve the serum HIF-1α,Caspase-3 levels in patients with acute cerebral infarction,reduce the fatality rate,and could be actively used in the recovery of patients with acute cerebral infarction treatment.
9.EML4-ALK and EGFR mutation status and survival analysis in Uygur with stageⅣNSCLC

Objective To investigate the relationship between the echinoderm microtubule associated protein like 4-anaplastic lymphoma kinase (EML4-ALK) and epithelial growth factor receptor (EGFR) mutation status and overall survival (OS) in Uygur patients with stageⅣnon-small cell lung cancer (NSCLC) who did not accept tyrosine kinase inhibitor treat-ment. Methods Totally 97 tissue samples were collected from Uygur patients with stageⅣNSCLC who did not accept tyro-sine kinase inhibitor treatment. EML4-ALK fusion gene and EGFR mutation status were detected by using FISH and ARMS methods. The survival rates were analysed. Results In 97 tissue samples, EML4-ALK fusion genes were found in 6 (6.2%) samples, EGFR mutations were found in 26 (26.8%) samples. The survival analysis showed that there was no significant dif-ference in OS between EML4-ALK fusion gene group and no EML4-ALK fusion gene group (P=0.941). There was no signifi-cant difference in OS between EGFR mutation group and wild-type EGFR group (P=0.607). The values of median OS were 17.7 months, 17.3 months and 16.2 months for EGFR mutant group, EML4-ALK positive group and EML4-ALK negative+EGFR wild-type group, and thre was no significant difference between them (P=0.915). Conclusion Excluding the thera-peutic influence in TKIs, EML4-ALK fusion gene and EGFR mutation status of tumor tissue can not be used as an indepen-dent factor in assessing the prognosis in Uygur patients with stageⅣNSCLC.
10.Application research of cone beam CT in dowel preparation for the mandibular first molar.

OBJECTIVETo measure the root anatomic topography of mandibular first molar using cone beam CT (CBCT), calculate the proper space for dowel preparation.
METHODSThe roots of 121 mandibular first molars from 71 patients were scanned by CBCT, the images from cross section, sagittal plane, and coronal plane were got respectively. The dowel preparation spaces according to established three dimensional images and dowel preparation requirements were analyzed.
RESULTSThree dowels could be prepared. The dowel length for distal-root was (7.19 +/- 0.91) mm, dowel diameter was (0.80 +/- 0.12) mm on the level of 4mm above root apex. The dowel length for mesio-root was (6.92 +/- 1.02) mm, and dowel diameter was (0.71 +/- 0.08) mm on the lever of 4 mm above root apex for buccal side and (0.71 +/- 0.07) mm for lingual side. Dowel should be prepared cone-shaped.
CONCLUSIONMany factors affect dowel preparation, individualized design is needed. CBCT can provide reliable radiographic evidence.
