1.Clinical effects of combining kinesio taping with water-filtered infrared-A in treating scapulohumeral periarthritis

Objective:To explore the clinical effect of combining kinesio taping with filtered infrared-A irradiation in the treatment of scapulohumeral periarthritis.Methods:Eighty-six patients with scapulohumeral periarthritis were randomly divided into a control group ( n=43) and an observation group ( n=43). Both groups received 20 minutes of irradiation with water-filtered infrared-A per day for 14 days, while the observation group was also given kinesio taping after the irradiation on 6 days of each week for the two weeks. Before and after the 2 weeks of treatment, both groups were evaluated for pain, their active and passive range of motion (AROM and PROM) of the shoulder, as well as the range of motion (ROM) of the shoulder joint, and their Constant-Murley score (CMS). The subjects also evaluated the functioning of their own shoulders using a visual analogue scale (VAS). Results:After the treatment, the average VAS scores of both groups were significantly higher than before the treatment, but the observation group′s average was significantly higher than that of the control group. There was significant improvement in the average AROM and PROM in both groups, but in the observation group the average AROM in anteflexion, rear protraction, abduction and extorsion and the average PROM in anteflexion and abduction were significantly better than the control group′s averages. The average total CMS scores of both groups had also improved significantly, but with significantly greater improvement in the observation group leading to a significant difference between the two groups after the intervention.Conclusions:Supplementing water-filtered infrared-A irradiation with kinesio taping is very effective in relieving pain, as well as improving ROM and life quality for persons with scapulohumeral periarthritis.
2.Extracorporeal shock wave therapy combined with autologous bone marrow stem cells transplantation for treating early-stage osteonecrosis of the femoral head

Objective To observe the therapeutic effects of extracorporeal shock wave therapy (ESWT) combined with autologous bone marrow stem cells (BMSC) transplantation in treating early-stage osteonecrosis of the femoral head (ONFH).Methods Forty patients diagnosed as stage Ⅰ or Ⅱ according to the classification of osteonecrosis of the femoral head put forward by the Association Research Circulation Osseous were randomly divided into a treatment group and a control group with 20 cases in each group.Both groups were treated with BMSC transplantation,while the treatment group was further treated with ESWT the next day.Harris scores were assigned before the treatment and 3,6,12 and 24 months after the treatment to evaluate hip function,the degree of pain and the effects of the treatment.Frog-bit X-rays of the anteroposterior pelvis were taken to assess the stages.Results There were significant differences in the Harris scores and pain degree of the two groups before and at the different time points after the treatment.After 6,12 and 24 months,significant differences were observed in the two values between the two groups.After 12 months the number of hips rated excellent,good and medium level were 8,18 and 1 in the treatment group and 5,13 and 8 in the control group,showing significant differences.Conclusion Extracorporeal shock wave therapy along with autologous bone marrow stem cell transplantation can be used to treat patients with early stage osteonecrosis of the femoral head,relieving their pain and improving their joint function.

Aim To observe the effects of low concentration ouabain(OUA)on intracellular calcium concentration ([Ca2+]i) were investigated in guinea pig ventricular myocytes.Methods The guinea-pig ventricular myocytes were obtained by enzymatic dissociation technique, then were incubated with Fluo3-AM. The Fluo3-AM fluorescent signal was detected with confocal laser scanning microscopy. The change of [Ca2+]i was represented by the percentage of fluorescence intensity change [(FI-FI0)/FI0,%] (FI: fluorescent intensity after addimg OUA, FI0: control). Results In normal Tyrode,s solution and Ca2+-free Tyrode′s solution.OUA (1?10-9~1?10-6 mol?L-1)elevated [Ca2+]i in a concentration-dependent manner, in normal Tyrode′s solution by 16.7?6.8(P0.05).Genistein (GST) (1、10、50、100 ?mol?L-1)abolished the OUA-induced increases in[Ca2+]i in a concentration-dependent manner by 17.5?3.1、14.2?8.9、0.8?7.6(P
4.The expression of p53 and k-ras in gallbladder diseases

Objective To detect the expression of p53 and k-ras in benign and malignant gallbladder diseases.Method ABC immunohistochemistry was used to detect p53 and k-ras protein in 101 specimens of chronic cholecystisis with cholecystolithiasis(group A),20 with cholesterol polyp(group B),22 with adenoma of gallbladder(group C),25 with primary gallbladder cancer(PGC)(in which there were 5 with atypical hyperplasia and carcinoma in situ)(group D)in this study.The relationship between p53 or/and k-ras expression and clinicopathologic data were analyzed.Resuit p53 and k-ras expressions were detected in both benign and malignant specimens,with the positive rates increasing from cholesterol polyp,to chronic cholecystisis to adenoma of gallbladder to carcinoma in situ and PGC(p53 was 62.8% in patients with benign lesions,and 92.0%in malignant lesion x2=8.16,P=0.004;k-ras was positive in 66.4% patients with benign lesions,and 92.0% in malignant lesions x2=6.65,P=0.01).In benign lesions the p53 expressions in group C was higher than that in group A and B(x2=6.90 and 6.02,P=0.009 and 0.014).There was higher expression of p53 in chronic chylecystisis with a history more than 10 years than those less than 10 years(80.0%vs.52.9%,x2=8.28,P=0.004).k-ras in cases less than 60 years old was higher than those after 60 years of age(70.4%vs.46.8%,x2=5.79,P=0.016).Conclusion The higher expression rate of p53 in cholecyst adenoma may predict carcinogenesis,while k-ras is negatively correlated with patients'age,which may bespeak the development of carcinoma in patients younger than 60 years old.
5.Expression of NF-κB,COX-2 and the MMP-9 in hypopharyngeal carcinoma

Objective To study the quantitative expression and the correlation of the NF-κB p65,COX-2 and MMP-9 Drotein in the hypopharyngeal carcinoma tissue.Methods FCM method was performed to detect the quantitative expression of the NF-κB p65,COX-2 and MMP-9 protein in 48 cases of hypopharyngeal carcinoma fresh sample and 48 cases of para-carcinoma tissue.Fluorescence Index wasdeftned as the quantitative expression index of the three proteins.Results The quantitative expression of the NF-κB p65,COX-2 and MMP-9 in hypopharyngeal carcinoma tissues(1.16,1.32 and 1.26) was remarkably higher than in para-carcinoma(1.03,1.04 and 1.04).The quantitative expression of three proteins in metastasis group was obviously higher than in non-metastasis group.The expression of NF-κB p65 and COX-2 protein in hypopharvngeal carcinoma tissues was positively related (P<0.05). Conclusion The high expression of NF-κB p65 and COX-2 is closely related in hypopharyngeal carcinoma tissues.NF-κB p65 might improve the expression of COX-2.
6.Research of Telemedicine Financial Operation Mechanism in Hubei

Objective:Telemedicine financial operation mechanism was researched to promote long-term operation of the telemedicine in Hubei. Methods: Telemedicine operator was built to manage telemedicine services; the construction costs of the telemedicine system were analyzed to develop service prices; telemedicine services income distribution programs and cost clearing methods were analyzed. Results: The financial operaton models, including the price-setting, income distribution and the cost clearing methods were developed. Conclusion:The reasonable financial operation system ensured the involved parties ’ interests of telemedicine and it could promote long-term development of telemedicine.
7.Progress of Synthesis α-Substituted Propanoic Acids Drugs by Transition Metal-catalyzed Asymmetric Hydrogenation

Optically pure α-substituted propanoic acids and their derivatives represent as an important kind of organic building blocks and key intermediates,which has been widely used in the synthesis of chiral drugs.Some of them have been used directly as nonsteroidal anti-inflammatory drugs (NSAIDs),such as ibuprofen,naproxen,ketoprofen and so on.Dihydroartemisinic acid (DHAA),the same structure as the α-substituted propanic acids,is a key intermediate for the synthesis of artemisinin,the most effective and current used anti-malarial drug.Therefore,the asymmetric synthesis of α-substituted propanoic acids is always a hot topic for chemical scientists.Asymmetric catalytic hydrogenation attracts more and more attentions because of its atom economy and efficiency.This dissertation will disclose the asymmetric synthesis of α-substituted propanoic acids using transition metal-complex as a chiral catalyst.
8.Imaging findings of breast phyllodes tumors and their correlations with pathological features

Objective To study the radiological appearance and pathological features of breast phyllodes tumors(PTs), and to enhance the knowledge of the tumor.Methods Clinical and imaging findings were reviewed retrospectively in 65 women with PTs confirmed by surgical pathology.61 patients had preoperative MRI and/or sonography, and 44 had preoperative mammography.Then these findings were compared with the pathological results.Results (1)The percentages of benign, borderline and malignant PTs were 32.3%(21/65), 43.1%(28/65) and 24.6%(16/65), respectively,and the mean maximum diameters of these lesions were (4.75±2.62) cm, (5.37±3.11) cm and (4.96±3.82) cm, respectively.There were no statistical difference in lesion''s size among these three types(F=0.247,P=0.782).(2)On the mammograms of these 44 PTs(including 16 cases of benign type, 18 cases of borderline and 10 cases of malignant type), the appearances of radiolucent halo and calcification among these three types showed statistically significant differences(both P<0.05).(3)On the sonography of these 61 PTs(including 19 cases of benign type, 26 cases of borderline and 16 cases of malignant type), the shape among these three types showed statistically significant difference(P<0.05).(4)On the MRI of these 61 PTs(including 21 cases of benign type, 24 cases of borderline and 16 cases of malignant type), internal hypointense septations on T2WI and time intensity curves showed statistically significant differences among these three types(both P<0.05).Conclusion Some radiological appearances of PTs can be used to help determine the histologic subtypes of breast PTs preoperatively.
9.Validity and reliability of the Hong Kong version of the functional test for the upper extremities of hemiplegic stroke patients

Objective To quantify the validity and reliability of the Hong Kong version of the functional test for the Hemiplegic Upper Extremity (FTHUE-HK).Methods Forty-two stroke patients were studied.Patients were assessed twice within one week using the FTHUE-HK,the upper extremity component of Fugl-Meyer movement assessment (FMA) and the modified Barthel index (MBI).The test-retest reliability and inter-rater reliability of the FTHUE-HK were thus quantified.The FTHUE-HK's validity was evaluated according to the correlation between the FTHUE-HK,FMA and MBI results.Results Significant correlations between the three assessments were demonstrated(P≤0.01).The intra-and inter-class correlation coefficients were 0.983 and 0.985 respectively.Conclusion The FTHUE-HK is a simple and useful assessment of the upper extremity function of stroke patients with good validity and reliability.
10.Current integrated management of obstructive sleep apnea in patients with type 2 diabetes mellitus

Obstructive sleep apnea ( OSA) is a common disorder in upper respiratory tract, and high prevalent in patients with Type 2 Diabetes Mellitus ( DM-2 ) . OSA can lead to the development of glucose metabolism disorder and accelerate diabetic complications especially the microvascular. Screening for OSAS and selecting appropriate treatment is very important to optimize the efficacy and improve prognosis in patients with DM-2 and ordinary people. More effective strategy should target both controlling blood sugar and improving ventilation in OSA patients with DM-2. Current therapeutic regimens include: therapy of continuous positive airway pressure, controlling blood sugar with insulin, building good sleep habits, weight loss, and orthognathic surgery.
