1.A multicenter study of effect of pidotimod on the immune function in children with respiratory tract infection

Objective To explore the clinical curative effect of pidotimod on patients with respiratory tract infection and effect on immune function. Methods 120 children with recurrent respiratory tract infection in the Third Hospital of Qinhuangdao,the Third Staff Hospital of Baogang Group,the Third Hospital of Wulanchabu were selected,and were divided into two groups according to random number table.60 cases in control group were treated with routine treatment of anti-infection,relieving cough,eliminating phlegm,antipyretic;60 cases in experimental group were treated with pidotimod on the basis of routine treatment,oral with boiled water,0.4g per times,2 times a day,with a course of 60 days.Clinical curative effect after treatment and serum immunoglobulin (IgG,IgA,IgM)levels,T lymphocyte subsets (CD3+,CD4+,CD8+)levels and NK cells relative activities before and after treatment were compared between two groups.Results After treatment,the total effective rate of experimental group (95.00%)was significantly higher than that of control group (81.67%),and the difference was statistically significant (P<0.05);the immune indexes before treatment had no significant difference,and levels of serum immunoglobulin and T lymphocyte subsets were improved,and levels of serum immunoglobulin (IgG,IgA,IgM)and T lymphocyte subsets (CD3+,CD4+,CD8+)of experimental group were more higher than those of control group,and the difference was statistically significant (P<0.05 );relative activity of NK cells in both groups improved after treatment,but relative activity of NK cells in experimental group was significantly higher than that in control group,and the difference was statistically significant (P<0.05 );adverse reactions according minor rashes and anemia were observed in two groups,and there was no significant differece in the incidence of adverse reactions,and ADR was tolerable after symptomatic treatment.Conclusion Pidotimod could significantly improve the clinical curative effect of patients with respiratory tract infections and effectively improve the immune function of patients with recurrent respiratory tract infections with high security,which has a clinical significance.
2.Investigation for anti-tumor effects of CIK cells induced by TLR7 agonist instead of IFN-γ

Objective:To investigate the immune effects of CIK cells induced by Toll like receptor 7 agonist (Tlr7a) instead of IFN-γon killing lymphoma cells in vitro .Methods: Mononuclear cells were isolated from healthy human peripheral blood .CIK were induced by Tlr7a in vitro instead of IFN-γ.Two groups were divided as follows:CIK group,Tlr7a-CIK group.Then the main investigation on immune effects included immune phenotype was detected respectively , and cytotoxicity of the effectors was analyzed .Results: In Tlr7a-CIK group,the amount of CD56+cells was more than CIK group (P<0.05),and the cytotoxicity was also stronger (P<0.05). Conclusion:Tlr7a instead of IFN-γcould promote the immune effects of CIK cells on killing tumor cells in vitro .
3.Efficacy of nalmefene antagonizing postoperative respiratory depression induced by opioids:a multicenter,randomized,double-blind,positive drug-controlled study

Objective To evaluate the efficacy of nalmefene antagonizing postoperative respiratory depression induced by opioids.Methods Two hundred and forty ASA Ⅰ orⅡpatients aged 18-64 yr with body weight fluctuating within 20% of the standard body weight were included in this multicenter,randomized,double-blind,positive drug-controlled study.Anesthesia was induced with etomidate 0.3 mg/kg and TCI of sufentanil(effect-site concentration 0.4.ng/ml).Tracheal intubation was facilitated with vecuronium 0.1 mg/kg or rocuronium 0.6mg/kg.The patients were mechanically ventilated.PETCO2 was maintained at 35-45 mm Hg.Anesthesia was maintained with sevoflurane+ sufentanil TCI(Ce=0.1-0.4 ng/ml).Patients undergoing neurosurgery and liver or kidney operation were excluded.The operation time was within 3 h.The residual effects of muscle relaxants were reversed after operation.The patients were randomly divided into 2 groups(n=120 each):group Ⅰneloxone andgroup Ⅱ nalmefene.Naloxone 0.1 mg or nalmefene 0.25 μg/kg was injected iv over 30 s and was repeated 5 min later if necessary until the respiratory rate>10 bpm,PETCO2<45 mm Hg and apnea time<15 s.The total amount of naloxone was≤0.4 mg while that of nalmefene≤1 μg/kg.BP,HR,SpO2,PETCO2,respiratory rate and apnea time were recorded immediately before and at 2 and 5 min after haloxone/nalmefene administration and then every 5 min until 5 min after extubation.The recovery of spontaneous breathing within 30 min after naloxone/nalmefene administration,extubation time and Ramsay sedation score at 5 min after extubation were recorded.The patients were also observed for adverse reactions.Results Spontaneous breathing recovered within 30 min after naloxone/nalmefene administration in all patients in both groups.The extubation time was significantly shorter in nalmefene group than in naloxone group.There was no significant difference in Ramsay sedation score,BP,HR,SpO2 and incidence of adverse reactions between the 2 groups.Conclusion Nalmefene is better than naloxone in antagonizing opioid-induced postoperative respiratory depression.
4.Correlation study between insulin resistance and cortisol level in patients with pituitary adenoma

Objective To explore the insulin resistance(IR),serum cortisol.level and circadian secretion rhythm in the patients with pituitary adenoma(PA).Methods Sixty-seven patients with PA in this hospital from October 2016 to April 2017 were selected as the observation group,including 23 cases of pituitary Cushing syndrome,15 cases of growth hormone adenoma,16 cases of prolactinoma and 13 cases of non-functioning adenoma.Contemporaneous 49 individuals undergoing healthy physical examination were selected as the control group.The biochemical indexes of fasting plasma glucose(FPG),fasting insulin(FINS) and serum cortisol were measured and the homeostasis model assessment for insulin resistance(HOMA-IR) was determined.The correlation between insulin resistance and serum cortisol was analyzed.Results The incidence rate of cortisol circadian rhythm disorder in the observation group was 58.21%,the level of fasting serum cortisol in abnormal secretion rhythm of PA group was higher than that in normal secretion rhythm of PA group and control group(P<0.05);the levels of FINS and HOMA-IR in the pituitary Cushing syndrome and growth hormone adenoma group were higher than those in the control group,the correlation analysis showed that HOMA-IR in PA patients was positively correlated with fasting serum cortisol level and FINS level(P<0.05).Conclusion The IR occurrence exists in pituitary Cushing syndrome and growth hormone adenoma,which may be closely correlated with serum cortisol increase.
5.Bias analysis on dimension measurement ruler results of penis and testis by cylinder boy penis and testicles measurement

Objective? To analyze the bias of dimension measurement results of penis and testis by the self-design cylinder boy penis and testicles measurement (hereinafter called cylinder ruler) and it influencing factors. Methods? From June to December 2018, we selected 152 children of Department of Urinary Surgery in Hu'nan Children's Hospital as subjects by case sampling. Children's penis and testicles dimension were measured repeatedly by their patients and nurses with the self-designed cylinder ruler and three kinds of common clinical measurement tools (vernier caliper+tape measure+testis measurer, vernier caliper+straightedge, vernier caliper+round head probe), and the bias were counted based on medical reference value. Results? Measurement bias of length and diameter of penis in a resting state, length of penis in a traction state, length and width of testis in group of cylinder ruler were lower than those in other groups with statistical differences (P<0.05) which not influenced by boys' ages and measurement time (P> 0.05). Bias of length of penis in a traction state, length and width of testis measured by nurses were lower than those by children's patients with statistical differences (P< 0.05). Interaction effect analysis showed that there were no statistical differences in dimension of penis and testis measured by different measurers with the cylinder ruler (P>0.05); however, there were statistical differences in dimension of penis and testis measured with the cylinder ruler by different measurers at different times of a day (P<0.05). The differences of interaction effects between measurement time and measurers were statistical (P<0.05). Conclusions? Compared with other measurements, the self-designed cylinder ruler for measurement of penis and testis in boys has higher accuracy. The main influencing factors of bias of dimension of boys' penis and testis are ages of boys and measurement time. Bias of length of penis in a traction state, length and width of testis measured by nurses are lower than those by children's patients. Measurers and measurement time are not suggested as independent factors when analyzing the influencing factors of dimensions measurement bias of penis and testis. Dimensions of penis and testis are measured with the cylinder ruler at different times which have certain changes.
6.A systematic review of risk prediction models for diabetic foot infections

Objective To systematically evaluate the risk prediction models of diabetic foot infections,and to ex-plore its implications for clinical nursing care.Methods Relevant studies in the databases of CNKI,VIP,Wanfang,CBM,CINAHL,PubMed,Web of Science,Cochrane Library,Embase,ProQuest,and Ovid were searched for the time-frame of database construction to December 2023.Literature was screened independently by 2 investigators and the area under the model curve(AUC)and its 95%confidence interval(CI),calibration method and predictors were ex-tracted,and the quality of the model was evaluated using the Predictive Modeling Study Risk of Bias Assessment Tool.Meta-analysis of the predictive value of model predictors was performed using RevMan 5.4 software.Results 7 studies were included,containing 8 risk prediction models,all of which reported the AUC(0.748-0.922),and 6 models underwent model calibration.The overall fit of the included studies was good,among which l model was low fit,and all 7 studies were at high risk of bias,mainly focusing on predictors and analysis.Meta-analysis showed that the area under the curve of the combined models was 0.831(95%CI:0.780~0.883);the disease duration(OR=2.460,95%CI:1.850~3.270),vascular disease(OR=3.110,95%CI:1.070~9.000),and neuropathy(OR=3.550,95%CI:1.920~6.560)were effective predictors of diabetic foot infection.Conclusion The current diabetic foot infection risk prediction model is still in the developmental stage,and further optimization and external validation of the ex-isting model is recommended for the future.
7.A Case Report of Primary Hypertrophic Osteoarthropathy

Primary hypertrophic osteoarthropathy(PHO)is a rare disease also known as pachydermo-periostosis.We reported a painless case whose diagnosis was confirmed by genetic test.A 24-year-old male presented a series of symptoms that first began at 14.He suffered from progressive clubbed-fingers accompa-nied by swelling of the wrist and ankle joints.Facial skin concentric thickening and alar nose broadening ap-peared simultaneously and increased progressively.He was also prone to acne and hyperhidrosis.X-rays showed thickening of the metacarpal and phalangeal bones,as well as symmetrical periosteal ossification of both the tibia and fibula.Clinical diagnosis of PHO is difficult because of the variable features.With acromeg-aly excluded,the diagnosis was confirmed by a genetic test.Whole exome sequencing revealed a heterozygous SLCO2A1 c.611C>T(p.Ser204Lue)and SLCO2A1 c.1602C>A(p.Asn534Lys)mutation from each par-ent.It suggests that primary hypertrophic osteoarthropathy should be considered for young limb hypertrophic patients especially when periosteal thickening signs were showed in X-ray.A confirmatory diagnosis can be made through the genetic test.
8.Effect of continuous nursing intervention on nursing quality and postoperative function in elderly patients after femoral intertrochanteric fracture

Objective To explore the effect of continuous nursing intervention on limb function and nursing quality after proximal femoral nail antirotation (PFNA) internal fixation for femoral intertrochanteric fracture in the elderly. Methods From February, 2017 to November, 2018, 100 elderly patients with femoral intertrochanteric fracture who underwent PFNA internal fixation in our hospital were randomly divided into control group (n = 50) and observation group (n = 50), who accepted routine nursing and continuous nursing respectively for three months. They were assessed with Harris score and visual analogue scale for pain (VAS) before and after the intervention. The postoperative nursing effect was compared. Results The Harris score increased in both groups after the intervention (t > 45.98, P < 0.001), and increased more in the observation group than in the control group (t = 15.03, P < 0.001). The VAS score decreased in both groups after the intervention (t > 16.33, P < 0.001), and decreased more in the observation group than in the control group (t = 9.749, P < 0.001). The effect of nursing was better in the observation group than in the control group (Z = -2.272, P = 0.023). Conclusion Continuous nursing intervention can significantly improve the limb function and nursing satisfaction of elderly patients with femoral intertrochanteric fracture after PFNA.
9.Individualized red-cell transfusion strategy for non-cardiac surgery in adults: a randomized controlled trial.

BACKGROUND:
Red-cell transfusion is critical for surgery during the peri-operative period; however, the transfusion threshold remains controversial mainly owing to the diversity among patients. The patient's medical status should be evaluated before making a transfusion decision. Herein, we developed an individualized transfusion strategy using the West-China-Liu's Score based on the physiology of oxygen delivery/consumption balance and designed an open-label, multicenter, randomized clinical trial to verify whether it reduced red cell requirement as compared with that associated with restrictive and liberal strategies safely and effectively, providing valid evidence for peri-operative transfusion.
METHODS:
Patients aged >14 years undergoing elective non-cardiac surgery with estimated blood loss > 1000 mL or 20% blood volume and hemoglobin concentration <10 g/dL were randomly assigned to an individualized strategy, a restrictive strategy following China's guideline or a liberal strategy with a transfusion threshold of hemoglobin concentration <9.5 g/dL. We evaluated two primary outcomes: the proportion of patients who received red blood cells (superiority test) and a composite of in-hospital complications and all-cause mortality by day 30 (non-inferiority test).
RESULTS:
We enrolled 1182 patients: 379, 419, and 384 received individualized, restrictive, and liberal strategies, respectively. Approximately 30.6% (116/379) of patients in the individualized strategy received a red-cell transfusion, less than 62.5% (262/419) in the restrictive strategy (absolute risk difference, 31.92%; 97.5% confidence interval [CI]: 24.42-39.42%; odds ratio, 3.78%; 97.5% CI: 2.70-5.30%; P <0.001), and 89.8% (345/384) in the liberal strategy (absolute risk difference, 59.24%; 97.5% CI: 52.91-65.57%; odds ratio, 20.06; 97.5% CI: 12.74-31.57; P <0.001). No statistically significant differences were found in the composite of in-hospital complications and mortality by day 30 among the three strategies.
CONCLUSION:
The individualized red-cell transfusion strategy using the West-China-Liu's Score reduced red-cell transfusion without increasing in-hospital complications and mortality by day 30 when compared with restrictive and liberal strategies in elective non-cardiac surgeries.
