Objective To compare the difference of the microleakage repaired by direct method and indirect method of the funnel shaped root canal in the anterior teeth. Methods A total of 22 middle teeth at the similar size were collected, prepared into funnel root canal after finishing the root filling. The teeth were divided into A and B groups randomly, and there were 11 test pieces in each group. The root canal was repaired by direct method in group A and indirect method in group B. After 500 thermal-cold cycling and 240 000 dynamic cyclic loading, one tooth was selected in each group randomly. The microstructure were observed by scanning electron microscope (SEM). One piece of each group was randomly selected and the image of the core and the dentin was scanned by electron microscope. The remaining 20 pieces were dyed by India ink, dealt with transparent, compared and observed under the microscope. The degree of microleakage was recorded and analyzed in two groups. Results Under electron microscope, a gap can be observed in the bonding interface of group A. The gap was significantly bigger than that of group B. The leakage was significantly serious in group A than that in group B (P<0.05). Conclusion The level of microleakage of anterior teeth with flared canals, which is restored by fiber post core with the indirect method, is significantly better than that with the direct method. The indirect method has a good edge seal action and is more suitable for clinical application.

Objective To investigate the incidence and risk factors of acute kidney injury(AKI)after different types of cardiac valve replacement surgery. Methods A single cohort of 1113 patients who received cardiac valve replacement surgery from April 2009 to March 2010 in Zhongshan Hospital,Fudan University were prospectively analyzed.Multivariate Logistic regression analysis was used to evaluate possible risk factors associated with post-operative AKI.Akl was defined as a relative 50% increase or an absolute increment of 26.4 μmol/L in Scr within 48 hours and/or urine volume <0.5ml·kg-1·h-1 up to 6h.Results Of the 1113 patients, the incidence of AKI was 33.24%.In-hospital mortality of AKI patients was 6.49%,which was 5.373 times higher than that of non-AKI patients(P<0.01).The incidence of AKI in patients who simultaneously received cardiac valve replacement and coronary artery bypass grafting was 75.00%,which was significantly higher as compared to other types of valve replacement surgery(P<0.01).Unconditional multivariate Logistic regression analysis revealed that male, old age, long extracorpeal circulation (CPB)time(≥120 min)and combined with coronary artery bypass grafting surgery were the independent predictors of AKI episodes, and the corresponding OR values were 1.455,2.110,1.768 and 2.994 respectively. Conclusions AKI is a common and serious complication after cardiac valve replacement surgery.Patients who received combined cardiac surgery as valve replacement and coronary artery bypass grafting have higher incidence of AKI.Old age, male, long CPB time(≥120 min)and combined with coronary artery bypass grafting surgery are the independent risk factors of post-operative AKI for patients undergoing cardiac valve replacement surgery.

Objective To observe the clinical effect of acupoint injection of salmon calcitonin in the treatment of senile osteoporosis. Methods From February 2016 to July 2017,136 elderly patients with osteoporosis admitted to Wenzhou Hospital of Traditional Chinese Medicine were included. The patients were randomly divided into two groups, with 68 cases in each group. The acupoint-injection group was given 50IU salmon calcitonin on day 1, day 2 100IU, one time per day, continuously used for 15 days for a course of treatment, and with an interval of 90 days, and repeat the course of treatment. The patients were given 0. 9% sodium chloride diluted to 2mL,inject 0. 5mL into bilateral Shenshu and Zusanli these two points. The intramuscular injection group was given the same dose of salmon calcitonin gluteus maximus injected intramuscularly. At the same time, both two groups were given calcium Erqi D tablets one tablet a day, alendronate once a week, each time 70mg fasting taken orally. The type Ⅰ collagen β-CTx,PINP,NBAP,femoral neck, femur trochanter, lumbar (L1 -L4) lumbar BMD and osteoporosis symptom score before and after treatment were compared. Results After treatment, the NBAP and BMD of the two groups were significantly higher than those before treatment (all P<0. 05),and the β-CTx,PINP and symptoms scores were lower than before treatment (P<0. 05). After treatment,NBAP in the acupoint injection group was (32. 36 ± 3. 03)μg/L, which was higher than (29. 66 ± 3. 20) μg/L in the intramuscular injection group(t =10. 477,P <0. 05). The β-CTx and PINP in the acupoint injection group were decreased to (0. 10 ± 0. 05)μg/L and (28. 78 ± 5. 23)μg/L, which were significantly lower than those in the intramuscular injection group[β-CTx(0. 20 ± 0. 05)μg/L,PINP (35.77 ±6. 49)μg/L(t =5. 983,2. 662,all P <0. 05)]. After treatment, the BMD of the femoral neck, femur trochanter and lumbar vertebra (L1 -L4) in the acupoint injection group were (0. 690 ± 0. 032)g/m2,(0. 620 ± 0. 010)g/m2and (0. 822 ± 0. 012)g/m2,respectively,which were higher than those in the intramuscular injection group[(0.652 ±0.012)g/m2,(0.572 ±0.022)g/m2and (0.801 ±0.011)g/m2(t=5.055,6.133,1.956,all P<0. 05)]. After treatment, the improvements of TCM syndromes such as weakness in the waist and knees, tinnitus and deafness, loss of appetite, release of teeth, relaxation of two feet, weak tongue, weak pulse in the acupoint injection group were better than those in the intramuscular injection group (all P<0. 05). Conclusion The acupoint injection of salmon calcitonin at Zusanli and Shenshu points is more effective than the intramuscular injection in the treatment of senile osteoporosis. It can significantly increase the serum NBAP,decrease the serum β-CTx,PINP,improve the BMD and improve the patients'clinical symptoms, which means acupoint injection is a safe and effective way to treat senile osteoporosis.

Objective To investigate the plasma level of SDF-1a in patients with breast cancer after modified radical mastectomy and chemotherapy, and to evaluate the value of serum SDF-1a half-life for predicting breast cancer recurrence and metastasis. The correlation of SDF-1 a half-life and breast cancer recurrence and metastasis after treatment were retrospectively analyzed. Methods Serum chemokine SDF-1a of 112 cases of breast cancer were detected before and after modified radical surgery, and 1 day beforeeach chemotherapy session. SDF-1a levels and the dynamic changes in the process were observed and calculated. Results In 85 cases with no recurrence and metastasis the plasma level of SDF-1 a decreased rapidly to normal level ,while that in 27 cases with recurrence and metastasis was on high level with the halflife of SDF-1a being longer than that in no recurrence group(P <0. 01 ). Taking SDF-1a half-life ≥14 d as cut off point to predict breast cancer recurrence and metastasis after treatment, the sensitivity is 81.5%,specificity is 70. 6 %, and accuracy is 73.2%. Conclusions Serum SDF-1a half-life is a valuable marker in predicting postoperative breast cancer recurrence and metastasis.

Chinese Journal of Hepatology 2015;23(4):254-257
OBJECTIVETo explore the correlation between serum hepatitis B surface antigen (HBsAg) level and hepatic tissue pathological staging in patients with chronic hepatitis B (CHB).
METHODSClinical data was collected from our hospital's records for 302 CHB patients with HBsAg-positive status for more than 6 months and who had undergone hepatic biopsy. The HBsAg level,HBV DNA level and other clinical data were measured using commercial diagnostic assays. Liver histology was scored using the GS staging system. Correlation between serum HBsAg quantity, HBV DNA quantity, stage of inflammation and degree of fibrosis was assessed statistically.
RESULTSThe correlation of serum HBsAg level and HBV DNA level was notable. The serum HBsAg level was a variable affecting hepatic tissue pathological stage significantly. Serum HBsAg level appeared to be a highly specific and sensitive diagnostic marker of hepatic fibrosis. As the severity of liver fibrosis increased, the quantitative levels of platelet (PLT), HBsAg and HBV DNA gradually decreased, and the APRI index gradually increased; there were significant differences between the groups (all P<0.001). Serum HBsAg and HBV DNA levels in patients with hepatitis B e antigen-positive (HBeAg(+)) status showed strong correlation (r=0.721, P<0.0001) by Spearman analysis. HBeAg(+) patients with moderate to severe fibrosis (S2-4) exhibited significantly lower serum HBsAg and HBV DNA levels compared with patients with no or mild fibrosis (S0-1; t=5.475 and 4.826, P<0.001). ROC analysis suggested that a serum HBsAg cutoff of 4.46 log 10 IU/mL (28 800 IU/mL) would provide a theoretical sensitivity of 76.3%, with theoretical specificity of 70.5% in HBeAg(+) CHB patients. A serum HBV DNA cutoff of 7.13 log 10 IU/mL (1.35*10(7) copies/mL) would provide a theoretical sensitivity of 71.1%, with theoretical specificity of 73.4% in HBeAg(+) CHB patients. Logistic regression analysis showed that the level of HBsAg was an independent prognostic factor of moderate to severe liver fibrosis, with alanine aminotransferase, aspartate aminotransferase, HBsAg, HBV DNA and PLT (P<0.001).
CONCLUSIONHBsAg and HBV DNA levels decrease gradually along with aggravation of liver fibrosis. The cutoff values of 28800 IU/mL for HBsAg and 1.35*10(7) copies/mL ofHBV DNA provide higher specificity and sensitivity for predicting the degree of liver fibrosis in HBeAg-positive CHB patients, and the former is an independent predictor of severe liver fibrosis.
