1.CLINICAL SIGNIFICANCE OF PLASMA FIBRONECTIN DETERMINATION IN THE PATIENTS WITH CHRONIC COR PULMONALE

Plasma fibronectin(FN) level was measured by single radial immunod if fusion method in 58 patients with chronic Cor pulmonale during acute, remission and deterioration stages, 20 cases of chronic bronchitis, 10 cases of rheumatic heart disease, and 60 healthy people as controls.We analysed the relationship between the changes of plasma FN level and the degree of the respiratory failure, pulmonary infection and acid-base disturbance in experimental groups.The results showed that plasma FN levels of the controls, and the patients with chronic bronchitis and rheumatic heart disease were 322.4?60mg/L,263?40mg/L and 333.51 48mg/L, respectively.Plasma FN level was significantly decreased in patients with Cor pulmonale during acute stage(l46?67mg/L), gradually restored at remission stage (286?62mg/L) and markedly decreased in deterioration stage (101?39mg/L).It was ako markedly decreased in the patients with severe respiratory failure, pulmonary infection and acid-base disturbance.The authors suggest thai: placma FN may be used as a valuaole parameter to evaluate the clinical severity and prognosis of the patients with Cor pulmonale.The decrease of plasma FN level in patients with chronic Cor pulmonale may be due to its increased consumption and decreased synthesis in acute and deterioration stages.
2.Clinical Observation of Tegafur Combined with Three Dimensional Conformal Radiotherapy for Advanced Esophageal Carcinoma

OBJECTIVE:To observe clinical efficacy and safety of tegafur combined with three dimensional conformal radio-therapy for advanced esophageal carcinoma. METHODS:Totally 88 cases of advanced esophageal carcinoma admitted into Xuchang central hospital during Aug. 2013-Mar. 2016 were selected and divided into control group and observation group according to random number table,with 44 cases in each group. On the basis of anti-inflammatory,relieving asthma and other symptomatic treatment,two groups accepted three dimensional conformal radiotherapy. Control group was given fluorouracil 80 mg/m2,ivgtt,d1+nadeplatin 70 mg/m2,ivgtt,d1-d5. Observation group was given Tegafur capsules 60 mg,bid,continuous use 5 days,rest for 2 days. A treatment course lasted for 3 weeks,and they received 3 courses of treatment. Clinical efficacies of 2 groups were observed as well as serum inflammatory factors(TNF-α,IL-6,IL-8),lab indexes(VEGF,CA125,CA199,CEA),QLQ-C30 scores be-fore and after treatment. The occurrence of ADR was recorded in 2 groups. RESULTS:The total response rate of observation group was 88.64%,which was significantly higher than 68.18%,with statistical significance(P<0.05). Before treatment,there was no statistical significance in serum inflammatory factor levels,lab index levels and QLQ-C30 scores between 2 groups(P>0.05). Af-ter treatment,serum inflammatory factor levels and lab index levels of 2 groups were significantly decreased,while QLQ-C30 scores were decreased significantly;the observation group was significantly better than control group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Tegafur combined with three dimensional conformal radiotherapy shows good therapeutic efficacy for advanced esophageal carcinoma,can significantly reduce serum inflammatory factor levels and lab index levels and improve life quality but do not increase the occur-rence of ADR.
3.Evaluation on performance of MAKER IS 1200 automated chemiluminescence analyzer

Objective To evaluate the performance of the MAKER IS1200 automated chemiluminescence analyzer .Methods A series of experiments were designed to evaluate the precision ,reagent open bottle stability ,linearity range and reference interval of the MAKER IS1200 automated chemiluminescence analyzer in 8 items of hepatitis B 5 indexes ,AIDS ,hepatitis C and syphilis ac‐cording to the requirements of CLSI documents ,the detection results were performed the methodological comparison with those de‐tected by Abbott i2000 automated chemiluminescence analyzer .Results Except the total precision of HBEAG low value was slight‐ly higher than 15% of the judgment standard ,other indexes conformed to the requirements ;the open bottle stability of various items of reagent was better ;the quantitative item HBSAB had good linearity within the detection range (r2 >0 .95);in the verification of reference interval ,no outlier was found in each item ;in the methodological comparison ,the other items had good correlation with those detected by the ABBOTT i2000 except anti‐HCV .Conclusion The MAKER IS1200 automated chemiluminescence analyzer has good detection performance and meets the requirements of immunoassay detection in laboratory work .

5.Curative effect of Aidi injection solution combined with carboplatin on controlling malignant pleural effusion

Objective To investigate the curative effect of Aidi injection solution combined withcarboplatin on controlling malignant pleural effusion(MPE).Method A total of 86 cancer patients with appearedPME were selected as our subjected who hospitalized in the First People's Hospital of Shangqiu from Feb.2012to Mar.2013.They were randomly divided into the observation group(43 cases) and the control group(43cases).Patients in the oontrol group were treated with carboplatin at dose of 100 mg/per times which was putinto the thoracic cavity,and patients in the observation group was treated with Aidi injection solution at dose of100 mg/per times beside carbeplatin.The treatment period was 4 weeks.The effect information,quality of lifeand adverse reactions were recorded.Results The total effective rate and survival quality improvement rate inobservation group were 86.05% (37/43) and 51.16% (22/43),higher than those in control group (67.44%(19/43),30.23% (13/43)),and the differences were significant(x2 =4.170,3.903 ; P < 0.05).There wasno significant difference in terms of the related adverse reactions between two groups(P >0.05).ConclusionThe therapy of Aidi injection solution plus carboplatin on controlling MPE is proved to be effective and it canimprove patients' quality of life with better safety.Therefore it is worthy of recommendation.
7.Advances in the treatment of aggressive non-Hodgkin's lymphoma

As recent researches suggested,the continuous infusion and novel targeted medicines have been expected to improve the outcome of patients with aggressive B-cell non-Hodgkin's lymphoma.Nevertheless,it does not improve the curative effect by increasingly intensive dosage.Patients with aggressive T-cell non-Hodgkin's lymphoma have present better prognosis recently,which is attributed to better efficacy of the modified regimens in patients with NK/T cell lymphoma.Multiple genetic abnormalities are implicated in poorer prognosis in patients with aggressive T-cell non-Hodgkin's lymphoma.Non-myeloablative allogeneic stem cell transplantation and new targeted drugs have improved the curative effects.Radiotherapy with modern technology also is expected to become an important means to improve outcomes of aggressive non-Hodgkin's lymphoma.
8.Changes of HIF-la and VEGF expression induced by hypoxia in rats with brain injury caused by drowning

Objective: To observe the histomorphology/pathology changes and the expression of hypoxia inducible factor-1 alpha (HIF-1?) and vascular endothelial growth factor (VEGF) in drowning rats brain, in an attempt to explore new pathways for treatment of water drowning. Methods: The animal model of freshwater drowning was established with rats. The rats were randomly divided into the following groups: control group, drowning-to-death group and 2 min-drowning group (including 5 subgroups survived for 3, 6, 12, 24. and 40 hours after being drowned in water for 2 min [n=10]). Brain indices of rats in each group were detected and the pathological changes of cerebral tissues were observed; the expression of HIF-lo and VEGF were detected immunohistochemically and the correlation between them was analyzed. The dynamic blood oxygen saturation was detected in rats survived for 40 h after 2 min-drowning at different points. 10.The study of clinical long-term effect and prognostic factors in nasopharyngeal carcinoma after intensity modulated radiation therapy.

OBJECTIVE:
To retrospectively analyze the long-term curative effects and prognostic factors of 137 cases of NPC patients treated with intensity modulated radiation therapy.
METHOD:
The three-dimensional conformal intensity modulated radiation therapy adopted for the design plan of 137 cases of newly diagnosed and no distant metastatic NPC patients. Evaluation of short-term clinical efficacy was made according to the efficacy evaluation criteria of WHO solid tumor one month after radiotherapy. During the 2 years after radiotherapy, re-examinations were made every 3 months. Two years later, re-examinations were made every 6 to 12 months, and the routine examinations included clinical body check, nasopharyngoscope, CT, B Chao, MRI, chest X-ray, bone scan, so as to understand the condition of cavum nasopharyngeum, cervical lymph nodes and cranial nerves. Life table method was adopted to calculate the overall survival rate, Kaplan-Meier method was used to calculate relapse-free survival rate, distant metasis-free survival rate and disease-free survival rate. Log-rank test was used for univariate analysis of 14 clinical factors which may exert influences on prognosis. Multivariate analysis was performed with the Cox regression model, P < 0.05 is of statistical significance.
RESULT:
(1) Of the 137 cases, the overall survival rates of 1-year, 3-year and 5-year were 98.5%, 90.3%, 74.6% respectively, relapse-free survival rates were 97.0%, 81.9%, 66.7% respectively; distant metasis-free survival rates were 96.3%, 80.5%, 56.0% respectively; disease free survival rates were 95.6%, 76.9%, 43.8% respectively. (2) Univariate analysis revealed that the influences of T-staging,N-staging, 92 Fuzhou clinical staging, combined chemotherapy, skull base bone destruction, injury of cranial nerve, retropharyngeal lymph node metastasis, residual disease, total time of IMRT on prognosis were of no statistical significance(with all the P values lower than 0.05) The influences of gender, nationality, age, pathological type and anemia or on prognosis were of no statistical significance(with all the P values higher than 0.05). (3) T-staging, skull base bone destruction, injury of cranial nerve, retropharyngeal lymph node metastasis, N-staging, combined chemotherapy, residual disease, total time of IMRT, and anemia were drawn into Cox model, and the results showed that N-staging, combined chemotherapy, injury of cranial nerve, residual disease, and total time of IMRT were independent factors that affect prognosis (with all P values lower than 0.05).
CONCLUSION
IMRT obviously has advantages than conventional radiotheraphy in treating NPC patients. N-staging, injury of cranial nerve, combined chemotherapy, residual disease, and total time of IMRT are the main factors that affect prognosis.
