1.The application of psychological counseling in the thoracic operation

Objective To study the effect of psychological counseling for patients with thoracic surgery recovery period.Methods Ono hundred patients with lobectomy were selected,they were divided into experimental group and control group with 50 cases each according to the treatment method.Two groups of patients were in the ward completed indwelling catheter,inserted into the right double lumen endotracheal tube by high seniority anesthesiologists,postoperative given perfect analgesia.Two group patients were treated with routine operation before the visit,assess the condition and signed the anesthesia informed consent,the test group were given psychological support therapy,explain in detail postoperative recovery period may be uncomfortable,eliminate the anxiety of patients,establish the confidence to overcome the disease,Two groups of patients after surgery by the recovery room nurse to give psychological counseling,complete extubation and record the restlessness,24 h after the patient satisfaction questionnaire.Results In test group patients were able to follow instructions and coordinate nurse smooth tube removal,15 cases of patients need to brake,additional sedatives and dehyed extubation,the difference between the two groups has statistical significance (P < 0.05).The satisfaction questionnaire in test group was significantly higher than that in control group:100% (50/50) vs.82% (41/50),there was statistical difference (P < 0.05).Conclusion Psychological counseling can effectively avoid agitation in recovery period of patients with postoperative chest tube removal,smooth.
2.Qualitative research about cognition of cardiac surgical patients for patient safety

Objective To know the condition of cognition of cardiac surgical patients for patient safety.Methods Carried out a semi-structured depth interviews among 8 cardiac surgical patients,and then cleared up the information by phenomenology method.Results Three themes were extracted:patients' consciousness about patient safety,the lack of knowledge of patients in patient safety management,the lack of emotional support of patients in patient safety.Conclusions Some positive counter-measures should be taken by nursing administrators to resolve patients' difficulty and confusion,and then promote patients take part in the course of safety management of patients,ensure patients' safety.
3.The express of Caveolin-1 and cyclin D1 and their implications in gastric cancer

Objective To investigate the expressions of Caveolin-1 and cyclin D1 and their significance in gastric cancer,and offer effective experiment evidence for the decision of gastric cancer prognosis and multitude pathway therapy.Methods The protein expression of Caveolin-1 and cyclin D1 were detected by immunohistochemistry in 120 cases of gastric cancer and 30 cases of normal gastric tissue.The correlations were analyzed between Caveolin1 and cyclin D1 expression and the clinicopathologic features of gastric cancer.Results The decreased expression of Caveolin-1 in gastric cancer may be negative correlated with tumorous differentiation degree,infiltration depth,lymphoid node metastasis and TNM stage;the increased expression of cyclin D1 in gastric cancer was positively correlated with tumorous differentiation degree,lymphoid node metastasis and TNM stage(r =- 0.297,P =0.001).Conclusion The expressions of Caveolin-1 and cyclin D1 were negatively correlated,and their negative synergy may be closely related to the occurrence,development and evolution of gastric cancer.Caveolin-1 and cyclin D1 were novel prognostic mark ers of gastric cancer,and may play an important role in the treatment of gastric cancer.
4.Effect of salinomycin with umbilical cord blood mesenchymal stem cells in breast cancer MCF-7 cells

BACKGROUND:Mesenchymal stem cel s-released factors can form a microenvironment inhibiting malignant tumor cel proliferation, and moreover, salinomycin also inhibits the growth of tumor cel s. OBJECTIVE:To explore the effect of salinomycin with umbilical cord blood mesenchymal stem cel s in breast cancer MCF-7 cel s and its action mechanism, in order to seek a new target and therapeutic strategy to treat breast cancer. METHODS:Logarithmically growing MCF-7 cells were randomly divided into control, umbilical cord blood mesenchymal stem cell group and combination group. Cells in the control group were given no treatment, while those in the other two groups were given umbilical cord blood mesenchymal stem cell suspension or umbilical cord blood mesenchymal stem cell suspension combined with salinomycin, respectively. RESULTS AND CONCLUSION:Forty-eight hours after the intervention, the proliferation and invasion of MCF-7 cel s and expression of POSTN protein in cel s were significantly reduced in the combination group compared with the other two groups. These findings indicate that the combination of umbilical cord blood mesenchymal stem cel suspension and salinomycin effectively reduces the proliferation and migration of MCF-7 cel s, and this combined use provides a new insight into the treatment of breast cancer.
5.Determination of Diisopropyl Sulfate in Ezetimibe by GC

Objective:To establish a method for the determination of diisopropyl sulfate in ezetimibe. Methods:Diisopropyl sul-fate was determined by GC with a DB-FFAP capillary column (30 m × 0. 32 mm, 0. 5 μm) and an FID. The carrier gas was nitrogen and the flow rate was 2. 0 ml·min-1 . The temperature program was as follows: the initial column temperature was 40 ℃, and then raised to 180 ℃ at a rate of 25℃·min-1 ,and maintained for 2 min. Results:Diisopropyl sulfate had a good linear relationship with-in the range of 4. 040-13. 466 μg·ml-1(r=0. 999 8). The average recovery was 97. 57%and RSD was 2. 37% (n=9). Conclu-sion:The method is specific and reproducible with high sensitivity, which can be used to determine the content of diisopropyl sulfate in ezetimibe.
6.Determination of Residual Organic Solvents in Erlotinib Hydrochloride by Headspace Gas Chromatography

Objective:To establish a headspace capillary gas chromatography method for the determination of residual solvents in erlotinib hydrochloride .Methods:A DB-624 capillary column (30 m ×0.53 mm, 3.0 μm) was used and the carrier gas was nitro-gen.The flow rate was 2.0 ml· min-1 and the inlet temperature was 190℃.The FID detector temperature was 230 ℃.The column temperature program was with the initial temperature of 35℃( maitaining 8 min) , risen to 170℃with the rate of 28℃· min-1 ( main-taining 8 min) , and then risen to 200℃with the rate of 32℃· min-1 ( maintaining 7 min) .The headspace vial temperature was 100℃and the time was 30 min.Results:Ethanol, isopropanol, methylene chloride and n-butanol had a good linear relationship within the range of 0.68-409.14 μg· ml-1 (r=0.999 8),0.67-404.88 μg· ml-1 (r=0.999 8),1.71-51.31μg· ml-1 (r=0.999 7) and 0.72-431.12 μg· ml-1(r=0.999 8), respectively.The average recovery was 99.0% (RSD=0.41%, n=9), 100.2%(RSD=0.52%, n=9),97.1%(RSD=1.75%, n =9) and 102.5% (RSD=1.08%, n=9), respectively.Conclusion: The method is simple and accurate , which can be used for the determination of four residual organic solvents in erlotinib hydrochloride .
7.Effect of different routes of keyhole limpet hemocyanin immunization on the T cell dependent antibody response in mice

Objective To study the influence of different routes of keyhole limpet hemocyanin ( KLH) immuniza-tion on the T-cell-dependent antibody response in mice.Methods SPF Kunming mice were divided into four groups: the intravenous injection group, subcutaneous injection group, intraperitoneal injection group and control group.Each mouse was injected 200 μg KLH intravenously, subcutaneously or intraperitoneally daily for consecutive 10 days, respectively. Mice in the control group were given solvent injection only.Serum concentration of IgG stimulated by KLH antigen was measured 7 days after the last dosing.Spleen was isolated to calculate the organ coefficient and examined by pathology u-sing hematoxylin and eosin staining.Results Intravenously, subcutaneously and intraperitoneally administered KLH stimu-lated the generation of secondary lymphoid follicles and germinal center to varying degrees, B cell apoptosis, increased a-mount of cells in the marginal zone and other pathological changes were observed in the spleen.Intravenous and intraperito-neal administration of KLH led to more pronounced pathological changes compared with that in the subcutaneous injection group.All of the three administration routes of KLH induced generation of IgG antibody, significantly higher than that in the control group (P<0.05).Intravenous injection of KLH generated the highest concentration of IgG and organ coefficient among the three administration routes ( P<0.05) .Conclusions Different immunization routes do affect the production of IgG antibody, organ coefficient and pathological changes in the spleen, and these differences should be taken into consider-ation when analyzing the T cell dependent antibody response in mice.
8.Placing subcutaneous drain and preseting triclosan-coated polyglactin 910 suture with delayed suturing to prevent typeⅢabdominal surgery incision infection

Objective To investigate the effect of placing subcutaneous drain tube and preseting triclosan-coated polyglactin 910 suture with delayed suturing to incision infection after typeⅢabdominal surgery. Methords Dividing 504 patients with typeⅢincisions undergone abdominal surgery into 3 groups. The number of group A patients with thoroughly incision washing and primary triclosan-coated polyglactin 910 suture after abdominal surgery was 143. The number of group B with closed anterior rectus sheath, opened skin and subcutaneous fat with preseted triclosan-coated polyglactin 910 suture was 190. The number of group C with subcutaneous drain tube after abdominal surgery was 171. We compared the incidence rates of incision infection and the second phase debridement suture rates among the 3 groups. Results For the group A, B, C, the number of incision infection people was 11, 4, 3 and the incision infection rate was 7.69%, 2.11%and 1.75%respectively. The difference of the 3 groups incision infection rate were statistically significant (P<0.05). The incision infection rate of the group B and group C were lower than that of group A and the difference were statistically significant (P<0.016 7). There were no significant differences in second phase debridement suture rates among the 3 groups. Conclusion Preseting triclosan-coated polyglactin 910 suture with delayed incision sutue and placing subcutaneous drain tube can decrease the incision infection rates for type Ⅲpatients after abdominal surgery, but can not decrease the second phase debridement suture rates of the infectious incision.
9.Clinical characteristics and survival analysis of 37 cases of breast carcinosarcoma

Objective To explore the clinical characteristics and prognostic factors of breast carcinosarcoma.Methods Clinical data of 37 breast carcinosarcoma patients treated at Tianjin Cancer Hospital from 1974 to 2014 was analyzed retrospectively.Kaplan-Meier unvariate analysis was used to calculate the survival rate and compare the survival rates.Multivariate factors for survival were analyzed by COX proportional hazards regression model.Results 37 cases of breast carcinosarcoma were all female.The median age was 55 years.Among them 31 cases complained for painless mass,six cases had breast mass with skin ulceration.21 cases were three-negative breast cancer.Ten cases had axillary lymph node metastasis,ten cases had distant metastasis.The 5-year survival rate was 62.3%.Unvariate analysis showed that axillary lymphnodes metastasis (P =0.045) and therapeutic schedule (P =0.016) significantly influenced the oulcome of the patients.COX multivariate analysis validated that the therapy modality was an independent prognostic factor for breast carcinosarcoma(P =0.041).Conclusions Breast carcinosarcoma is rare and has a poor prognosis.ER,PR and HER-2 expressions in most cases are negative.If there is axillary lymph node metastasis,the prognosis is poor.A reasonable and comprehensive treatment can improve the prognosis.
