1.Prevalence of chronic obstructive pulmonary disease among residents at ages of 40 years and older in Qidong City

Objective:
To investigate the prevalence and influencing factors of chronic obstructive pulmonary disease (COPD) among residents at ages of 40 years and older in Qidong City, Jiangsu Province, so as to provide insights into COPD prevention and control.
Methods:
Permanent residents at ages of 40 years and older were sampled using a multistage stratified random sampling method in Qidong City in 2020, and subjects' demographics, history of environmental contact, and medical and family history of respiratory disorders were collected. Pulmonary function test was performed, and the forced expiratory volume (FEV1)/forced vital capacity (FVC) ratio of <70% was defined as COPD. Factors affecting the prevalence of COPD were identified using a multivariable logistic regression model.
Results:
A total of 8 000 questionnaires were allocated, and 7 538 valid questionnaires were recovered, with an effective recovery rate of 94.23%. The respondents included 3 104 men (41.18%) and 4 434 women (58.82%), and had a mean age of (62.40±14.68) years. The prevalence of COPD was 15.06%. Multivariable logistic regression analysis showed that male (OR=1.818, 95%CI: 1.712-1.940), age (50 to 59 years, OR=1.142, 95%CI: 1.111-1.181; 60 to 69 years, OR=1.176, 95%CI: 1.142-1.219; 70 to 79 years, OR=1.300, 95%CI: 1.249-1.361; 80 years and older, OR=1.582, 95%CI: 1.490-1.691), smoking (OR=3.760, 95%CI: 2.658-4.879), alcohol consumption (OR=1.213, 95%CI: 1.077-1.348), use of solid fuels (OR=1.676, 95%CI: 1.591-1.773), history of occupational exposure to dust (OR=1.859, 95%CI: 1.752-1.982), medical history of asthma or dyspnea (OR=2.068, 95%CI: 1.044-3.125) and frequency of persistent cough of more than four times annually (OR=1.225, 95%CI: 1.121-1.345) were statistically associated with the prevalence of COPD among residents at ages of 40 years and older in Qidong City.
Conclusion
The prevalence of COPD was 15.06%, and was associated with gender, age, smoking, use of solid fuels, occupational exposure to dusts, medical history of asthma or dyspnea and frequency of persistent cough among residents at ages of 40 years and older in Qidong City.
2.Association of better atomoxetine response with polymorphisms of CYP2D6 gene in the treatment of attention-deficit/hyperactivity disorder

Objective:To explore the association between CYP2D6 gene polymorphism of cytochrome and clinical response to atomoxetinein children with attention-deficit/hyperactivity disorder.Methods:Totally 111 Han Chinese boys meeting criteria for ADHD in the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edi-tion (DSM-IV)were recruited.Atomoxetine treatment was given in titrated doses in order to achieve optimal re-sponse.Behavior changes were measured with the ADHD Rating Scale (ADHD-RS)at baseline and after optimal doses were reached.The decrease of ADHD-RS scores were primary measures of the treatment effect,the scores of the items in the ADHD-RS was less than or equal to 1 for the remission criteria.The three SNPs of CYP2D6 genes(rs1080985,rs1065852,rs16947)were genotyped by polymerase chain reaction (PCR).Results:There was no asso-ciation between single polymorphisms andatomoxetine after adjusting for baseline rating scores (P >0.05).In con-trast,haplotype analysis revealed that some patients with GAC haplotype achieved significant remission [remission vs.non-remission haplotype frequency (rate):5 /56 (8.9%)vs.3 /164(1.8%),P <0.05 ].After multiple testing correction still existed significant trend(P =0.082)and also found correlation trends after correlation analysis be-tween GAC haplotype and clinical response to atomoxetine (P =0.078),8 patients with GAC haplotype carriers were all effective response to treatment,non-responders without this haplotype.Conclusion:These results suggest association between polymorphisms of CYP2D6 gene and ADHD better clinical improvement with atomoxetine treatment,it is worth further exploration in a large sample.
3.Effect of micellar polymer PEG-PLL-OPG on osteoporosis and arteriosclerosis in mice

Objective By using PEG-PLL and OPG-expressing plasmid to synthesize a nanocompound PEG-PLL-OPG (PPO), to investigate its effects on osteoblasts (OB) and arteriosclerosis (AS) in mice. Methods The incidences of osteoporosis (OP) and AS were studied in people. Collect people′s serum , and divide them into different groups According to the results of diagnosis, the patients were grouped into the groups of Control, AS, OP, and AS. PPO was used to deal with OB in different groups. The cell apoptosis, cell activity, the adhersion and calcification to scaffold PCL were determined by flow cytometry, MTT, alizarin red stain and SEM,respectively. PPO was also injected intp the ApoE-/-RANKL+/+ mice via caudal vein to demonstrate its effects on BMD and AS. Results PPO could increase the cell activity , inhibit OB apoptosis and promote cell adhersion and calcifica-tion on scaffold PCL in vitro. PPO could also decrease the area and calcification of atheromatous plaque , and in-crease the BMD of collum femoris in vivo. Conclusion PPO may be a new drug for OP and AS treatments.
4.Correlation between preoperative CRP/Alb ratio and lymph node metastasis in pa-tients with gastric cancer

Objective:To investigate the relationship between Creactive protein(CRP)/albumin ratio (CAR) and lymph node metastasis of gastric cancer. Methods:A total of 96 cases of gastric cancer were included in the study. The clinical pathological stage and lymph node detection in patients with gastric cancer were analyzed, with the preoperative CAR as the dependent variable. Results:1) The pa-tients with preoperative CAR>0.04 has higher transfer rate and metastasis than the patients with preoperative CAR≤0.04, and the dif-ference was significant (P<0.05). The lymph node stage of patients with preoperative CAR>0.04 was significantly higher than that of the patients with preoperative CAR≤0.04, and the difference was significant (P<0.05). 2) The mean CAR of patients with Borrmann typesⅠ,Ⅱ,Ⅲ, andⅣgradually increased;with the progression of the pathological stage of gastric cancer, the mean value of CAR in-creased. 3) During the operation, the total number of lymph node dissections was high and the mean value of CAR before the opera-tion was high. Conclusion:The correlation between preoperative CAR and lymph node metastasis in patients with gastric cancer may reflect the degree of lymph node metastasis to a certain extent.
5.Data Mining Analysis and Experience Summary of LAO Shaoxian in the Treatment of Gastric Stuffiness(Weipi)

Objective To analyze the medication rules of Professor LAO Shaoxian in the treatment of gastric stuffiness based on syndrome differentiation.Methods The effective prescriptions for patients with gastric stuffiness treated by Professor LAO Shaoxian from March 2017 to March 2022 were collected,and the general information,chief complaints,diagnosis,syndrome differentiation and prescriptions of patients were extracted.Excel software and the ancient and modern medical records cloud platform(V 2.3.7)were used to construct the prescription database.Data mining function was used to carry out analysis of the syndrome type of prescription,analysis of the frequency and property of Chinese herbs,as well as association rule analysis and cluster analysis.Results A total of 272 prescriptions were included,involving 164 kinds of medicinal herbs.The main traditional Chinese medicine(TCM)syndrome types are damp-heat syndrome and qi stagnation syndrome.The frequency of 25 herbs was more than or equal to 30 times.The representative herbs is Pinellinae Rhizoma Praeparatum,Glycyrrhizae Radix et Rhizoma,Citri Reticulatae Pericarpium,Perillae Caulis and Aucklandiae Radix.The medicinal properties are mainly warm and flat.The medicinal flavors are spicy,bitter and sweet.The drug meridians mainly included the spleen,stomach and lung meridians,followed by the liver meridian.There were 23 core drug pairs obtained by association rules,such as"Aucklandiae Radix-Perillae Caulis","Citri Reticulatae Pericarpium-Perillae Caulis",and"Pinellinae Rhizoma Praeparatum-Citri Reticulatae Pericarpium".Clustering analysis of drugs can be divided into three combinations,which have the effects of regulating qi and relieving distension,resolving dampness,and clearing heat and detoxifying.Conclusion The core prescription of Professor LAO Shaoxian in the treatment of gastric stuffiness is Aucklandiae Radix,Perillae Caulis,Citri Reticulatae Pericarpium,Pinellinae Rhizoma Praeparatum,Glycyrrhizae Radix et Rhizoma,Kaki Calyx,Aurantii Fructus Immaturus and Arecae Pericarpium.It focuses on regulating qi movement of middle jiao and treating spleen and stomach simultaneously.The main therapeutic method is regulating qi and relieving distension.At the same time,attention should be paid to the application of dampness-dispelling and stagnation-removing,heat-clearing and detoxifying drugs.The clinical therapy of Professor LAO Shaoxian on gastric stuffiness is significant,which can be used as a reference for diagnosis and treatment.
6.Two types of digestive tract reconstruction after proximal gastrectomy for early gastroesophageal junction adenocarcinoma: a randomized controlled study.

OBJECTIVETo investigate the better method of digestive tract reconstruction in proximal gastrectomy for early gastroesophageal junction adenocarcinoma.
METHODSA total of 153 cases of early gastroesophageal junction adenocarcinoma who were planned to receive radical proximal gastrectomy from January 2003 to December 2011 were prospectively enrolled and randomly divided into two groups by table of random number according to methods of digestive tract reconstruction, including 3S anastomosis group (80 cases, 3S jejunal interposition) and traditional anastomosis group (73 cases, esophageal remnant gastric posterior wall anastomosis). Postoperative complications, operative time, mortality, nutritional parameters and postoperative quality of life were compared between these two groups.
RESULTSThere were no significant differences between two groups in postoperative complications, operative time and mortality (all P>0.05). 3S anastomosis group was better in nutritional parameters than traditional group six months after operation (P<0.05). As compared to traditional group, incidence of reflux esophagitis decreased [20.0%(16/80) vs. 46.6%(34/73), P<0.01] and gastric emptying time prolonged obviously [(160.8±8.1) min vs. (61.1±10.8) min, P<0.01] in 3S anastomosis group 18 months after operation. Postoperative QLQ-C30 rating scale revealed quality of life was significantly higher in 3S anastomosis group as compared to traditional group.
CONCLUSIONJejunal interposition is a better method of digestive tract reconstruction in proximal gastrectomy for early gastroesophageal junction carcinoma.

7.Promotion of postoperative recovery with fast track surgery for gastric cancer patients undergoing gastrectomy: a prospective randomized controlled study.

OBJECTIVETo study the safety and feasibility of fast track surgery (FTS) in the promotion of postoperative recovery for gastric cancer patients undergoing gastrectomy.
METHODSFrom January to December in 2013, 71 gastric cancer patients were prospectively enrolled and randomized into the FTS group and the control group. Patient in the FTS group received FTS management and those in the control group received routine management. The postoperative recovery and stress were compared between the two groups.
RESULTSFTS was associated with shorter time to bowel function return [(67.8±19.7) h vs. (90.0±20.6) h, P<0.01], shorter hospital stay [(13.5±3.0) d vs. (17.8±7.3) d, P=0.01], lower hospital cost [(23.8±3.7) thousand Yuan vs. (27.8±6.1) thousand Yuan, P<0.05], and less stress response (lower pain score, WBC count, C-reactive protein, all P<0.01). The postoperative complications including ileus, infection, anastomotic leakage were similar (all P>0.05).
CONCLUSIONFast track surgery decreases postoperative stress response and promotes recovery.

8.Analysis of related factors of postoperative fever in patients with oral and maxillofacial tumor radical treatment and simultaneous repair and reconstruction

Objective:To understand the fever rate, disease distribution, fever degree and fever related factors of the patients in the same period of oral and maxillofacial tumor repair and reconstruction.Method:A retrospective analysis was performed on 153 patients who underwent radical treatment of oral and maxillofacial tumors from January 2018 to December 2018 in the affiliated stomatological hospital of sun yat-sen university. The patients were divided into fever group and non fever group, and their fever influencing factors were analyzed.Result:Among the 153 patients, 97 (63.40%) had fever symptoms.It’s mainly moderate fever. The incidence of fever was the highest in gingival cancer and oropharyngeal cancer.The results of univariate analysis showed that there were statistically significant differences in serum albumin, tracheotomy and infection between the two groups ( χ2= 7.74, 7.48, 8.58, P<0.01 or 0.05).Logistic multivariate regression analysis showed that infection and tracheotomy were independent influencing factors for postoperative fever in patients with oral and maxillofacial tumor repair and reconstruction (or = 4.74, 2.47, P < 0.05). Conclusion:Patients with oral and maxillofacial tumors undergoing radical treatment and simultaneous repair and reconstruction are more likely to have fever after surgery. Tracheotomy patients and patients with infection need to be paid more attention. They need to be strengthened tracheotomy care and actively prevent infection, so as to reduce the postoperative fever rate, and make the patients with oral malignant tumors repaired and reconstructed at the same time go through the perioperative period smoothly.
9.Clinical features and prognosis of paraganglioma of the urinary bladder

【Objective】 To explore the clinical features, treatment and prognosis of paraganglioma of the urinary bladder (PUB). 【Methods】 The clinical data of 41 PUB patients treated at our hospital during Sep.2012 and Sep.2022 were collected. The clinical features, surgical records, pathological reports and follow-up records were retrospectively analyzed. Patients’ survival was estimated with Kaplan-Meier estimator. The differences among groups were compared with Log-rank test. 【Results】 Among the 41 patients, 20 were male and 21 were female, with a median age of 52 years. All patients were treated with surgery, including transurethral resection of bladder tumor (TURBT) in 16 cases, partial cystectomy (PC) in 23 cases, and radical cystectomy (RC) in 2 cases. All patients were followed up for 4.0 to 125.0 months, with a median of 59.0 months. Local recurrence occurred in 5 patients, and distant metastasis occurred in 5 patients. Survival analysis showed that the 5-year overall survival (OS) rate and 5-year relapse-free survival (RFS) rate were 95.7% and 84.8%, respectively. Further analysis showed statistically significant differences in OS and RFS among groups with different maximum tumor diameters, growth patterns, and Ki-67 expressions (P<0.05). For patients with a maximum tumor diameter ≤2.8 cm, there was no significant difference in OS and RFS among different surgical groups. 【Conclusion】 PUB is rare, and a definitive diagnosis is based on pathology. In addition, the main treatment is surgery and the prognosis is good.
