Objective To develop a more effective method to treat proximal tubal obstruction. Methods In a total of 89 cases, there were 113 oviduct lesions diagnosed by hysterosalpingogram (HSG) and then by laparoscopy as proximal tubal obstructions. Intubations and recanalizations were performed by means of laparoscopy combined with hysteroscopy. Results Out of the 113 oviduct lesions in 89 cases, 73 (64.6%) were recanalized. In follow-up of 20 days～58 months, there were 20 cases of successful pregnancies and labours, 2 inevitable abortions and 1 ectopic pregnancy, with the rate of pregnancy 25.8% (23 of 89). Conclusions It is an effective approach to treat proximal tubal obstruction by using laparoscopy combined with hysteroscopy. It can also be used to identify the true proximal tubal obstruction from the false one.

Objective To study the efficacy of laparoscopic operations in endometriosis associated with infertility. Methods Video laparoscopic operations of ovariocystectomy, adhesiotomy, electrocoagulation, salpingoplasty and hydrotubotion were completed in 378 cases of endometriosis associated with infertility. Results A total of 198 cases were followed up for 4～50 months after operations, with the pregnancy rate 61 1%(121/198) and remission rate 92 4%(183/198). Recurrence was found in 5 cases. Conclusions Laparoscopic operations, characterized by minimal invasion, rapid postoperative recovery and better clinical effect, can be used in the treatment of endometriosis associated with infertility.

Objective To compare the clinical effect of proximal gastric malignant tumor treated by the total gastrectomy and the proximal gastrectomy.Methods 278 patients with gastric cancer were randomly divided into two groups by double blind method(1 ∶ 1).Patients in the two groups received total gastrectomy (TG)and proximal gastrectomy(PG) treatment,respectively.The surgery effect,postoperative complications,survival rate and tumor recurrence rate were compared between the two groups.Results In PG group,the average blood loss was (151.6 ± 26.4)mL,operation time was (124.5 ±36.2) min,hospital stay was(8.9 ± 3.1) days,which were significantly lower than those in TG group (231.5 ± 15.3) mL,(162.4 ± 24.8) min and (12.6 ± 6.2) days (t =45.352,36.521,11.243,all P ＜0.05).The lymph node excision rate of PG group was (13.2 ± 3.1)％,which was significantly lower than (16.4 ±4.6) ％ in TG group(t =9.654,P ＜ 0.05).The incidence rate of postoperative complications between the two groups had statistically significant difference(x2 =25.321,20.254,all P ＜ 0.05).The survival rates of 3 years,5 years in TG group were 62.16％,42.32％,those in PG group were 60.39％,34.23％.The 3 years survival rate had no significant difference between the two groups (x2 =6.321,P =0.079),5 years survival rate had statistically significant difference between the two groups (x2 =31.265,P =0.000).The recurrence rate in group TG was 22.30％,which was significantly lower than 34.53 ％ in PG group (x2 =29.365,P =0.000).Conclusion Average blood loss,operation time,hospitalization time and postoperative complications of PG group are less,,but the lymph node resection rate is low,tumor recurrence rate is high,the 5-year survival rate is lower than the TG after surgery.TG can be recommended for the proximal gastric malignancy.

BACKGROUND:The residual alveolar ridge in edentulous patients with osteoporosis always presents with a poor shape,which increases the difficulty in complete denture restoration.Until now,there are no predictors for residual alveolar ridge resorption or guidelines on the frequency of denture relines.OBJECTIVE:To investigate the correlation between the frequency of complete denture relines and serum bone turnover markers,osteocalcin (OC) and cross-linked carboxy-terminal telopeptide of type Ⅰ collagen (CTX),in elderly edentulous patients with osteoporosis,in order to provide reference for complete denture repair.METHODS:According to inclusion criteria,50 elderly edentulous patients with osteoporosis who had complete dentures were recruited in the study randomly.The frequency of complete denture relines was surveyed by related questionnaire and medical record,and the serum OC and CTX levels were detected by electrochemiluminescence immunoassay.Related data were analyzed statistically via Pearson correlation analysis.RESULTS AND CONCLUSION:It showed a strong positive correlation between the frequency of complete denture relines and the serum OC level (r=0.517,P ＜ 0.01).No significant correlation between relines frequency and the serum CTX level was observed (r=0.278,P=0.051 ＞ 0.05),but it showed a similar tendency between them.These findings indicate that to detect the levels of serum bone turnover markers may be conducive to evaluating curative effect of complete dentures in elderly edentulous patients with osteoporosis as well as to making subsequent diagnostic and therapeutic strategies.

Purpose To investigate the value of bedside ultrasonography for emergent patients in the diagnosis of vasa previa and umbilical cord prolapse. Materials and Methods The bedside ultrasonography data of 12 emergent cases of vasa previa and 18 emergent cases of umbilical cord prolapse were retrospectively analyzed for detection rate. Results Nine out of 12 emergent patients with vasa previa were detected by bedside ultrasonography;2 were misdiagnosed as umbilical cord prolapsed;and 1 patient who had missed diagnosis suffered fetal demise during vaginal labour. Fifteen out of 18 emergent patients with umbilical cord prolapse were detected by bedside ultrasonography;2 were misdiagnosed as vasa previa; and 1 missed diagnosis (no death case was reported). The detection rates for both groups of patients had no significant difference (P>0.05). Conclusion Bedside ultrasonography for emergent patients can visualize the traveling of vasa previa and umbilical cord prolapse so as to promptly provide evidence for clinical diagnosis and reduce perinatal mortality.

Objective:To investigate the relationship between ADAM 33 polymorphisms and susceptibility of asthma of Chinese in Qingdao.Methods: The polymorphism of ADAM33 gene, they were rs2280090, rs487377 and rs2787094, were detected with SnaPshot method.Results: There was no significant difference between asthma group and control group in genotypic frequencies.Conclusion:The study shows that the rs 2280090 , rs487377 and rs2787094 were not associated with susceptibility of asthma in Han nationality in Qingdao .

Objective To investigate the value of imaging-pathology fusion guided targeted cryoablation for localized prostate cancer.Methods Between June 2014 and February 2017,64 patients undergoing targeted cryoablation for localized prostate cancer were retrospectively collected and analyzed.The average age was 77.8 years old,ranging 45-87 years old.The preoperative valne of PSA ranged from 5.1 to 19.8ng/ml,with mean of 15.3 ng/ml,including 6 case of cT1c,35 cases of cT2a,and 23 cases of cT2b,and 35 cases of Gleason score 6,8 cases of 3 + 4 and 21 cases of 4 + 3.All patients had good continence and 11 cases had good erectile function before operation.All the patients received MRI or contrast-enhanced ultrasonography examinations before biopsies,and the sites and range of cryoablation were determined by MRI or contrast-enhanced ultrasonography and biopsy pathological results.Results All the patients underwent the surgery successfully.The average postoperative hospital stay was 1.6 days,ranging 1 to 3 days,including 22 cases discharged within 24 h after surgery.The operative duration ranged from 85 to 152 min,with mean of 99.6 min.The satisfaction rate of the patients was 100％.Seven patients had Clavien Ⅰ or Ⅱ postoperative complications,and recovered after symptomatic treatment.The follow-up duration ranged from 0.5 to 32.2 months,with median of 11.8 months.All patients were alive without new metastasis.Eight patients developed biochemical recurrence,5 of those underwent intermittent endocrine therapy,resulting in stable PSA levels.Conclusion Imaging-pathology fusion guided targeted cryoablation was safe and effective for localized prostate cancer with the advantages of few complications,high satisfaction rate and quick recovery.

Objective· To evaluate the safety of neoadjuvant therapy which was constituted by docetaxel based systemic chemotherapy and maximal androgen blockage for patients with locally advanced prostate cancer and to summarize the related adverse events and clinical managements.Methods· From June 2015 to February 2017,the clinical data of 55 patients undergoing neoadjuvant chemotherapy combined with complete androgen deprivation were retrospectively reviewed.The patients were given docetaxel and prednisone as DP regimen every 3 weeks and LHRH analogues with bicalutamide as maximal androgen deprivation for a total of 4 cycles.All treatment-related adverse events were observed and then recorded.Results· Two cases with liver function impairment after 2 cycles of treatment were withdrawn from the study.No severe allergic reactions occurred during neoadjuvant therapy.The most common adverse events were hematologic toxicity,while 23.6％ of patients had grade Ⅲ-Ⅳ neutropenia,and about 12.7％ had anemia.Due to a relatively short course of treatment,the skin or mucous damage,peripheral neurotoxicity and fluid retention were rare.However,hot flash,male breast development as well as erectile dysfunction were very frequently observed due to maximal androgen deprivation.The majority of these adverse events were relieved by symptomatic and supportive treatment.Conclusion · After strict selection,4 cycles of neoadjuvant chemotherapy combined with total androgen blockade could be well tolerated by the patients with high-risk locally advanced prostate cancer.Even though the adverse events were controllable,they still need to be closely monitored during treatment in order to reduce the incidence.In addition,the very low testosterone level associated endocrinal metabolic disorders caused by complete androgen deprivation were also of great concern.

OBJECTIVE The antagonism of obidoxi me on sarin induced miosis and visual impair-ment was evaluated and its antagonistic mechanism was investigated.METHODS ① 30 min after sarin (2 μg /0.1 mL per eye)was given as an eyedrop,the ability of the 2.5%,5.0%,7.5% obidoxi me and 1 .0% atropine to reverse effects of sarin on pupil dia meter and light reflex were evaluated at different ti mes.② Another 36 rabbits received sarin and at 30 min afer sarin exposure,the drugs above were ad-ministrated and their effects on pupillary light reflex,as well as the AChE activity of cornea,iris and reti-na were recorded 4h after the treatment.RESULTS ① Miosis and impaired pupillary light reflex oc-curred soon after sarin exposure but the abnormal pupil width and pupillary light reflex had disappeared by 48 h after sarin exposure;Subcequent to 1 .0% atropine treatment,the pupil dilatedinstead while the impaired light reflex did not i mprove significantly;unlike atropine,soon after ad ministration of 2.5%, 5.0%,7.5% obidoxi me,the pupil dia meter and light reflex were significantly increased(P <0.01 )and then had beco me normal totally by 24 h post-dose,much faster than those of the control and atropine treatment group.However,there was no significant difference in the recovery ti me between the different dose groups of obidoxi me.② 4h after treatment,the AChE activity in cornea and irisof sarin-treated group were (42 ±4)%,(26 ±2)%,respectively;the AChE activity in cornea of 2.5%,5.0%,7.5%obidoxi me were (74 ±1 1 )%,(81 ±10)% and (74 ±7)%,respectively,and the AChE activity in iris were(39 ±10)%,(43 ±8)% and (43 ±8)%,respectively ,co mpared with sarin-treated group,AChE activities of cornea and iris as well as light reflex of the obidoxi me-treated group were significantly increased(P<0.01 ).But there was no difference in light reflex and AChE activity between the sarin-treated and atropine-treated groups.CONCLUSION Obidoxi me showed better antagonism of sarin-induced ocular effects than that of the commonly used drug,atropine;the antagonistic mechanism is likely closely related to its rapid reactivation of the inhibited AChE in the cornea and iris.

OBJECTIVETo assess the association of polymorphisms of IL-4 gene (rs2243250, rs2243283) and IL-4R gene (rs1805012, rs1801275, rs1805010) with susceptibility to asthma among ethnic Chinese in Qingdao.

METHODSFor 400 asthma patients and 200 healthy subjects, above polymorphisms were detected with SnaPshot method.

RESULTSFor rs1805012, the frequency of TC genotype in the asthma group was significantly lower than the control group (8.8% vs. 15.5%, χ (2)= 6.498, P= 0.039), and so were the frequencies of TC+ CC genotypes (9.0% vs. 15.5%, χ (2) = 5.522, P= 0.019) and the C allele (4.6% vs. 7.7%, χ (2) = 4.729, P= 0.039). No significant difference was detected between the two groups in the frequency of the remaining four polymorphisms or the haplotypes formed by rs2243250 and rs2243283 (All P> 0.05).

CONCLUSIONThis study has indicated that rs1805012 polymorphism of IL-4R gene is associated with asthma in ethnic Han Chinese from Qingdao region. TC+ CC genotypes have a protective role against asthma compared with TT genotype. However, polymorphisms of IL-4 gene are not associated with susceptibility to asthma.
