1.Clinical application of INRatio coagulometer for fast measuring international normalized ratio

ObjectiveTo compare the correlation and agreement of international normalized ratio (INR) measured by INRatio coagulometer and standard laboratory venous blood measurement in order to judge the clinical application value of measuring INR by INRatio coagulometer.MethodsOne hundred who received warfarin were chosen randomly.Venous blood and finger capillary blood were taken simultaneously for measurement of INR by laboratory coagulation analyzer SYSMEX CA-7000 and INRatio coagulometer respectively.The correlation and agreement of INR results tested by the two methods were analyzed.Results When INR ≤5,there was a good correlation between INRatio coagulometer and laboratory coagulation analyzer SYSMEX CA-7000 (r =0.898,P =0.000).The mean bias of the two methods was 0.36 and INR measured by INRatio coagulometer were higher than those tested by laboratory coagulation analyzer SYSMEX CA-7000.By Bland-Altman,it showed a good agreement for 91.2% (62/68) of the difference INR located 95% the limited of agreement when the mean INR 1.60 - 3.50,and a bad agreement for only 50.0%(1/2) of the difference INR located 95% the limited of agreement when the mean INR > 3.50.ConclusionsIn the therapeutic range,INR measured by INRatio coagulometer shows a good correlation and agreement with INR measured by the established laboratory method.INRatio coagulometer can be used to monitor INR in the patients received oral anticoagulant therapy.
2.Genetic detection of Plasmodium falciparum with Chelex-extracted DNA from thin blood smears

Objective To investigate the feasibility of Chelex DNA extraction from thin blood smears for genetic analysis, and to develop smear-based nested polymerase chain reaction (PCR) of the 18S RNA of Plasrnodium falciparum. Methods Chelex-100 which was chelating ion exchange resin was used to extract DNA from Giemsa-stained or unstained thin blood smears of different concentrations of Plasmodium falciparum. With the extracted DNA as the template, 18S RNA gene was amplified by nested PCR to test the susceptibility of Chelex method. Results Positive band of 205bp appeared in nested PCR with DNA extracted from Giemsa-stained or unstained thin blood smears of patient with falciparum malaria. Using the Chelex method, the detection limits of the smear-based nested PCR were 1.5 × 101 parasite/μL blood for Giemsa-stained and 1.5×10-1 parasite/μL blood for unstained thin blood smears. Conclusions Chelex DNA extraction is a simple and efficient method for extracting trace amount of DNA from thin blood smear. The smear-based nested PCR developed in this study is feasible to identify the gene from reserved thin blood smears and will provide a new approach for clinical diagnosis and study of molecular epidemiology.
3.The value of six-minute walking test on chronic bronchitis needing lung function screening

Objective To study the relationship between the six-minute walking test (6-MWT) and pulmonary function in the diagnosis course of chronic bronchitis converting into chronic obstructive pulmonary disease (COPD). Methods Pulmonary function was detected in 360 patients with chronic bronchitis.Based on the results and diagnosis standard of COPD by Chinese Medical Association in 2002, the patients were divided into COPD group and pulmonary function normal group.Tbe walking distance in 6-MWT (6-MWD) was measured, and the mean value of twice tests was calculated to study the correlation between 6-MWT and pulmonary function, and 50 healthy volunteers were selected as control group. Results Most 6-MWD in COPD group were less than 420 meters(95.95%, 142/148 ), but those in pulmonary function normal group were more than or equalled to 420 meters mostly (97.17%, 206/212 ). However in patients with the 6-MWD more than or equalled to 420 meters , pulmonary function indexes such as the ratio of forced expiratory volume in one second and forced vital capacity( FVC ), FEV_1%, maximal ventilatory volume(MVV)were all significantly greater than those in patients with 6-MWD less than 420 meters, and there were significant differences (P<0.05). Compared with chronic bronchitis, the results of 6-MWD in control group was more than or equalled to 420 meters, but there was no significant difference (P>0.05 ). Conclusions 6-MWT is simple and feasible. However, it is a better indicator of the patients pulmonary function. The test would be an effective selecting examination for chronic bronchitis patients who are not willing to do puhnonary function test.
4.Association between Air Pollutants Exposure during Pregnancy and Preterm Delivery in Taiyuan

Objective To investigate the relationship between air pollutant exposure and preterm delivery (PTD) by Logistic regression model, as the effect of air pollutant exposure to PTD made great concern today. Methods The present study consisted of 31 145 qualified birth cases, 80.09% of the total amount, from 1 Nov, 2005 to 1 Aug, 2007. The covariant items included maternal age, nation, education, residence, income, occupation, occupation exposure, parent-drinking, father-smoking, heating-type, folic acid supplement, season of pregnancy, prenatal health care, etc. The Logistic regression analysis was employed to control the confounding factors mentioned above, then the pollutant was introduced step by step to build the single, double and triple pollutant Logistic regression model to examine the association between three pollutants (SO2, NO2, and PM10) and PTD during the first and third trimesters of pregnancy. The air monitoring data (daily average of SO2, NO2, and PM10) were collected from Taiyuan Environmental Monitoring Station. Results In the first trimester, a quartile increase of NO2 exposure caused the risk for PTD increased by 22.7%. As for the effect of PM10, no significant differences were seen. In the third trimester, a quartile increase of NO2 exposure caused the risk for PTD increased by 19.1%. Conclusion The NO2 exposure during the pregnancy can increase the risk of PTD. The susceptible exposure-window choice is critical for the further study in this field.
5.Comparison of Phonetic Articulation between Hearing-impaired and Normal Children in 4.5~5.5 Years Old Groups

Objective To study the characteristics and differences of phonetic pronunciation between hearing-impaired and normal children in 4.5~5.5 years old groups. Methods Phonetic articulation of consonants and vowels were tested by three-grade assessors for 20 hearing- impaired children and 20 normal children in 4.5~5.5 years old group using Question Bank for Hearing-impaired Children's Rehabilitation Evaluation of Hearing and Speech. Results and Conclusion For hearing-impaired children, the consonants of /b. m. d. l. j/ were the most articulative, followed with /h. f. r. p. t. n. g. x/, and then /zh. z. k. c. sh. q. s. ch/; for normal children, the consonants of /b. p. f. g. m. d. h/ were the most articulative, followed with /k. j. x. t. r. l/, and then /q. zh. ch. sh. n. z. s. c/. For both groups, the single final was the most articulative, followed with the compound final, and then the nasal. The articulation of these 3 types of vowels were poorer in hearing-impaired children than in normal children (P<0.01)
6.Clinical Observation of Brucea Javanica Oil Injection Combined with Oxaliplatin and Xeloda in the Treat-ment of Elderly Patients with Advanced Gastric Cancer

OBJECTIVE:To observe the clinical short-term efficacy and safety of brucea javanica oil injection combined with oxaliplatin and xeloda in the treatment of elderly patients with advanced gastric cancer. METHODS:82 elderly patients with ad-vanced gastric cancer were randomly divided into control group and observation group. Control group was treated with Oxaliplatin injection 130 mg/m2 added into 5% Glucose injection 250 ml for 2 h,iv,d1+Xeloda tablets 1 000 mg/m2,orally,twice a day, d1-14;based on the treatment of control group,observation group was additionally treated with 10% Brucea javanica oil injection 30 ml added into 0.9% Sodium chloride injection 250 ml,iv,once a day,d1-14. 21 days were as a treatment course,and it lasted 3 courses. The short-term efficacy,life quality and toxicity reactions were evaluated in 2 groups. RESULTS:The short-term efficacy in observation group was significantly higher than control group,life quality was significantly better than control group,and the in-cidences of leucopenia and liver damage were significantly lower than control group(P<0.05). CONCLUSIONS:Brucea javanica oil injection combined with oxaliplatin and xeloda has better efficacy than oxaliplatin combined with xeloda in the treatment of elder-ly patients with advanced gastric cancer,with good safety.
7.Endothelial dysfunction and lipid oxidation status after coronary artery bypass grafting procedure under cardio-pulmonary bypass

Objectives To evaluate the influence of cardio-pulmonary bypass (CPB) to the endothelial function and lipid oxidative status of the Coronary Artery Bypass Grafting (CABG) patients. Methods Thirty-five coronary patients (female 5 cases, male 30 cases, mean age 59.5±8. 6 years) who needed CABG operation therapy were included. The control group consisted of 20 health volunteers (female 5 cases, male 15 cases, mean age 49. 5 ± 7.3 years).Blood samples were taken at preoperative, 3 and 7 days postoperative. Serum von Willebrand factor (vWF) level, one of the endothelial dysfunction markers,and oxidized low density lipid (OxLDL) level, one of the lipid oxidization status markers were detected with ELISA kits. Results The levels of vWF and OxLDL in CABG patients' serum were elevated significantly compared with those of control group ( 139.60% ± 34. 39% vs 82. 79% ±13. 89% and 63. 7 ±18. 9U/ml vs 25.4± 15.2U/ml respectively). Three and 7 days after CABG significant increases in the vWF level (194. 30% ±37. 74%; 181.07% ±31.05%vs 139.60% ±34.39% at baseline, P<0. 0001, respectively) and OxLDL level (79. 3±26. 7U/ml; 72.4±23.1U/ml vs 63.7±18. 9U/ml at baseline, P<0. 01, respectively) were detected. Conclusion Compared with normal people, the endothelial system of CABG patients is impaired, and their lipids are in a more oxidative status. CABG procedure with CPB can further deteriorate the conditions, which may imply that the patients are predisposed to acute coronary accident shortly are the procedure.
8.Application of MRI in severity grading and activity staging in thyroid associated ophthalmopathy

The influence of MRI on severity grading and activity staging in thyroid associated ophthalmopathy (TAO) was evaluated.The results showed that MRI had a higher detection rate in abnormal extraocular muscle than NOSPECS scores(χ~2 = 19.37,P = 0.000).MRI had higher sensitivity,specificity,and accuracy in detection of active TAO than clinical activity score (100% vs 95.7%,88.2% vs 35.3%,96.9% vs 79.7%,respectively).Therefore,MRI improves early detection of atypical grade 4 cases.Signal intensity elevation in adipose suppressed T2WI is helpful to define active stage.
9.Therapeutic effects of sensorimotor training on patients with knee osteoarthritis

Objective To investigate the effects of sensorimotor therapy on patients with knee osteoarthritis.Methods Ninety-two patients with knee osteoarthritis were divided into a treatment group and a control group by using random digits table. Both groups received conventional rehabilitation therapy, including knee isometric and isotonic contraction training, ultrashort wave diathermy as well as aerobic exercise. The treatment group was also administered with sensorimotor training, in addition to the conventional training,including the muscle strengthening and control training of quadriceps and hamstring muscle using sling exercise therapy system and on the Thera-band mattress. The visual analogue scale (VAS) for pain, manual muscle testing and knee range of motion as well as the Japanese Orthopedic Association knee function evaluation form were used to evaluate the patients. Results There was no statistic difference between the 2 groups before treatment, with regard to the evaluation parameters used in this study. After 2 months of treatment, both groups improved significantly (P <0.05),with the treatment group improved to a significantly greater extent (P <0.05). Conclusions Sensorimotor training in addition to the conventional rehabilitation treatment can further relieve the pain of patients with knee osteoarthritis joints arthritis and improve the function of patients with knee osteoarthritis.
10.Practice and Thinking in the Teaching Reform of Diagnostics

Given that there has been a series of problems in the teaching of diagnostics,appropriated approaches should be made and feasible actions carried out.First of all,the notion of clinical teaching should be transformed and the level of qualified teaching staffs should be further elevated.In addition,the humanity connotation should be integrated into the course of diagnostics,and quality supervision and control should be executed to ameliorate the teaching of diagnostics.
