2.Application of fiberoptic bronchoscopy in neonates with weaning failure

Objective To evaluate the safety and efficacy of fiberoptic bronchoscopy in the diagno-sis of 41 neonates with weaning failure.Methods A retrospective study on the results of fiberoptic broncho-scopy was performed in 41 neonates with weaning failure.Results A total of 41 neonates of throat abnor-malities were examined by fiberoptic bronchoscopy,including 38 cases with laryngomalacia,8 cases with seri-ous hyperemia and swelling of the vocal cords,2 cases with scar stenosis of subglottic,and one case in each of congenital laryngeal web,glottic insufficiency and congenital epiglottis cyst;trachea-bronchial abnormali-ties,including 31 cases with tracheomalacia,2 cases with tracheal stenosis,4 cases with partial bronchial ste-nosis,1 cases with bridging bronchus and 1 cases with tracheoesophageal fistula.Thirty-eight neonates under-went fiberoptic bronchoscopy to obtain bronchoalveolar lavage fluid and culture,bacterial culture positive in 15 cases(39.4%),of which 5 cases of Klebsiella pneumoniae,3 cases in each of Pseudomonas aeruginosa and Acinetobacter baumannii,2 cases of Staphylococcus aureus,1 case in each of Escherichia coli and Strep-tococcus mitis;fungal culture positive for 6 cases(15.7%),of which 3 cases of Aspergillus fumigatus,1 case in each of Candida glabrata,Candida albicans and Candida parapsilosis.The most common complications of fiberoptic bronchoscopy were hypoxia and heart rate transient decline(26 cases,63.4%),3 cases fever af-ter operation.Conclusion Fiberoptic bronchoscopy can not only find congenital tracheal deformity and tra-cheal mucosa lesions,but also complete the etiology from deep discharge.Fiberbronchoscopy is safe and relia-ble given adequate preparation and if it is performed by skilled personnel.
3.Correlation study on self-differentiation and career adaptation in nursing undergraduate

Objective To investigate the current status of nursing undergraduate students′ self-differentiation and career adaptability and analyze the correlation of the two, so as to provide a reference for nursing educators to offer employment guidance. Methods The Undergraduate Self-Differentiation Scale and The College Students′Career Adaptability Questionnaire were used to investigate 341 nursing undergraduates. Results The mean scores of the self-differentiation and the career adaptability were (3.70±0.60) points and (3.60±0.42) points, respectively. It showed that the total score of self-differentiation and its each dimension were positively correlated with career adaptability, the Pearson correlation coefficients were career adjustment 0.26, career interpersonal 0.38, career curiosity 0.32, career confidence 0.39, career concerns 0.27, career control 0.46, and the coefficients between two total scales were 0.46 (all P<0.05). Conclusions The self-differentiation level of nursing undergraduates could affect their career adaptation level, and clinical nursing educators can take corresponding measures to improve their career adaptability based on the characteristics of nursing students′self-differentiation.
4.Values of Swansea criteria on diagnosis and severity evaluation of acute fatty liver of pregnancy

Objective To explore the value of Swansea criteria on diagnosis and severity evaluation of acute fatty liver of pregnancy (AFLP).Methods Fifty-two AFLP patients were admitted to Shandong Provincial Hospital Affiliated to Shandong University between January 1,2000 and December 31,2011.All these cases were retrospectively reassessed by Swansea criteria.According to the severity,prognosis and whether continuous blood purification treatment was needed,these cases were classified as mild and severe cases.Differences between groups were detected by x2 or t test.Bivariate correlation analysis was used for Swansea criteria compliance and postnatal hemorrhage and days in hospital.Results After reassessing by Swansea criteria,31 cases could be diagnosed as AFLP (20 cases met seven or more criteria,11 cases met six criteria)and the other 21 cases could not (16 cases met five criteria,three cases met four,and two cases met three).For the 16 cases that met five Swansea criteria,they were confirmed as AFLP based on postnatal follow-up.The five cases that met four or three criteria were confirmed as AFLP because no other disease could explain their status.Among the patients who met seven or more Swansea criteria,the incidence of intrauterine fetal death was 40% (8/20),and 65% (13/20) needed continuous blood purification.These were higher than in patients who met six or fewer Swansea criteria [9% (3/32) and 28% (9/32),x2=6.921 and 6.857,P=0.014 and 0.011].Postpartum hemorrhage was positively correlated with Swansea criteria compliance (r2=0.286,P=0.040).Conclusion Patients who meet five Swansea criteria can be diagnosed as AFLP.Swansea criteria can be applied to the severity evaluation of AFLP.
5.Diagnostic value of fiberobronchoscopy in endobronchial tuberculosis

Objective To assess the value of fiberobronchoscopy in the diagnosis of endobronchial tuberculosis. Methods A total of 67 children suspected of endobronchial tuberculosis in our ward from March 2010 to December 2012 had been tested by chest CT and ifberobronchoscopy, mycobacteria culture, liquid-based interlayer vessel technique and FQ-PCR from bronchoalveolar lavage fluid and biopsy. Results 50 patients were clinically diagnosed as endobronchial tuberculosis. The positive rate of FQ-PCR (60%) and liquid-based interlayer vessel technique (42%) are higher than that of mycobacteria culture (20%), which showed no signiifcant difference with biopsy. The positive rate of the combined detection (FQ-PCR and liquid-based interlayer vessel technique) is significantly higher than that of mycobacteria culture and biopsy. Conclusions Fiberobronchoscopy is effective for the diagnosis of endobronchial tuberculosis.
6.Quality standard for Kudingcha Tablets

AIM: To establish a quality standard for Kudingcha Tablets(Cratoxylum Prunifolium Dyer). METHODS: TLC was adopted for identification of Kudingcha Tablets.RP-HPLC was used to determine the contents of ursolic acid,quercetin and kaempferol in the preparation.The Nova-Pak C_(18) column(3.9 mm?150 mm,4.0 ?m) was used to determine the ursolic acid of content.The mobile phase was methanol-water-H_3PO_4(85∶15∶0.1).The detection wavelength was at 220 nm.The flow rate was 0.8 mL/min and the temperature was at 30 ℃.The Nova-Pak C_(18) column(3.9 mm?300 mm,4.0 ?m) was used to determine the contents of quercetin and kaempferol.The mobile phase was methanol-0.4%H_3PO_4(50:50).The detection wavelength was at 360 nm.The flow rate was 0.7 mL/min.The temperature was at 40 ℃. RESULTS: A good linear relation was obtained when the sample size of ursolic acid was in the range of 0.334-6.672 ?g.The linear range of quercetin was(0.012 99-0.649 6) ?g and the linear range of kaempferol was 0.013 34-0.667 2 ?g.The average recoveries of ursolic acid,quercetin and kaempferol were 97.70%,96.89% and 97.41%,respectively.The RSD were 0.87%,(1.06%) and 1.40%(n=6),respectively. CONCLUSION: This standard is accurate and reliable,and can effectively control the quality of Kudingcha Tablets.
7.Investigation on Current Situation of Workplace Violence the Intern Experienced and Their Professional Identity

Objective:To understand the current situation of hospital violence medical students experienced during the internship period and their professional identity.Method:With the method of cluster sampling,a questionnaire survey was conducted among 500 medical students from a medical college in Ganzhou,to understand the characteristics of hospital violence and compare the difference between the medical students who experienced violence and those not.Results:The effective recovery rate of questionnaire was 90.8%,of which 61.4% of medical students had witnessed hospital violence,8.1% of students had experienced hospital violence;the hospital violence that medical students experienced were characterized by low frequency and emotional violence.The overall score of personal identity was (117.05 ± 19.65),was in a medium level.Among the comparison of personal identity between the medical students who had experienced violence and those not,there was a significant difference in occupational cognition dimension (P < 0.01),but there was no significant difference in the total score and other four dimensions (P > 0.05).Conclusion:We should pay attention to the current situation of hospital violence that students experienced and their professional attitude,carry out targeted training and psychological counseling to improve their ability to respond to hospital violence and enhance their professional identity.
8.Wear matching performance of dental restoration materials oppositing to dentine

Objective:To compare the wear resistance between dentin and dental restoration material in vitro.Methods:The friction and wear behaviors of natural tooth dentin respectively against highly polished polymer ceramic,glass ceramic and zirconia were investigated in an artificial saliva test environment by UMT-2 friction and wear testing machine.Worn surface morphology was observed by scanning electron microscopy(SEM),roughness was measured by roughness instrument,the rigidity value was weighed by electronic balance.Results:The roughness of the 3 materials was similar to that of dentin(P > 0.05),the rigidity of dentin was less than that of the 3 materials (P < 0.05).After friction and wear test,the abrasion quantity of dentin respectively against the 3 materials was less than that of the materials (P < 0.05).There was positive correlation between the wear loss and the hardness of the 3 materials and dentin(r =0.846).The mass loss of the dentin against Poly Ceramic after grinding abrasion was the closest among the 3 materials.Conclusion:Different materials have varying degrees abrasion against dentine.The Polymer Ceramic has closer abrasion performancez with dentin than the other 2 materials.
9.Diagnostic evaluation of the temporomandibular joint osteoarthritis using cone beam computed tomography compared with conventional radiographic technology

Objective: Osteoarthritic lesions of Temporomandibular joint(TMJ) were assessed by dental cone beam computed tomography(CBCT),and compared with conventional radiographic technology. Methods: The study was conducted retrospectively on 511 joints of 350 patients, who had undergone dental CBCT ,panoramic radiography,transcranial projection and transpharygeal projection of TMJ . The results were compared and the accuracy with CBCT was assessed. The types of osseous condylar abnormalities were observed.Results: (1) The occurrence of osteoarthritis in male and female were 59.04% and 69.66%, respectively, with no significant difference. (2) Compared with CBCT, panoramic radiography,transpharygeal projection examination showed no significant difference, with the accuracy being 90.64% and 94.10%,respectively;However, transcranial projection indicated a significant difference in comparison with CBCT and the accuracy was 86.97%. (3) A higher occurrence of osteoarthritic lesions of the condyle was sclerosis (39.86%). Bony proliferation or osteophyte (28.18%) and ill-defined cortical bone (18.90%) were followed. Conclusion: Cone beam CT, which reproduces multiple images including axial, coronal and sagittal planes of the joint, provides a complete radiographic investigation of the bony components of the TMJ. It is one of the best choices of imaging diagnosis of TMJ osteoarthritis. Panoramic radiography and transpharygeal projection examination are also good choices for showing osseous condylar abnormalities in the clinic, but transcranial projection examination is inferior.
10.Study on Pharmacokinetics of Xuesaitong Injection

OBJECTIVE:To determine the pharmacokinetic parameters of Xuesaitong injection in rabbits.METHODS:Using ginsenoside Rg1 in Xuesaitong injection as a marker,the determination was carried out with HPLC.The detection conditions were:mobile phase,acetonitrile-water(3∶7);chromatgraph column,Hypersil C18(250mm?4.6mm,5?m);detection wavelength,210nm and flow rate,1 ml/min.RESULTS:After intravenous injection,the plasma concentration time data was found to be in accordance with the 2-compartment model in rabbits.The pharmacokinetic parameters were obtained with PKBP-NI.CONCLUSION:The method is simple,accurate and reliable.It could be used to determine the concentration of ginsenoside Rg1 in plasma.
