1.Progress on the pathogenesis of retinopathy of prematurity

Retinopathy of prematurity(ROP)is a kind of eye disease associated with the premature infant.It is characterized by retinal vascular dysplasia,which can cause retinal detachment and blindness.It is the main reason of children with visual impairment and blindness.ROP is a complex disease,associated with oxygen therapy,low gestational age,low birth weight,cytokines and other factors.At present,erythropoietin and infection can also affect the occurrence of ROP.According to the pathogenesis,early detection and early treatment have become more important.This review summarizes the research progresses of pathogenesis in ROP.
2.Analysis of benefits as a result of the modification of the wards

To genuinely orient our service towards the needs of patients, we modified the wards in our hospital, turning previous wards housing 8 to 10 patients each into medium-sized and small ones, with each one having a separate toilet, balcony, and air conditioner, meeting the diversified needs of patients, and attracting more patients to seek service at the hospital. As a result, in 2000, as compared with 1999, the number of emergency outpatients increased by 14.4%, the number of discharged patients increased by 16.2%, the average ratio of bed usage increased by 9.0%, the turnover rate of beds increased by 6.8%, the average length of stay dropped by 7.0%, income from outpatient service increased by 43.9%, the proportion of income from the sale of pharmaceuticals in the total business income dropped by 10.5%, and the annual business income increased by 18.9%.
3.Professor SHI Yanping's Experience of the Treatment of Adenomyosis

[Objective] To introduce the Professor SHI Yanping's experience in treating adenomyosis with traditional Chinese medicine. [Methods]Through clinical learning with the teacher and systematic reading literatures about adenomyosis, the author summarized the teacher 's academic thoughts and clinical treating experience of the disease from the etiology and pathogenesis,fundamental law of treatment ,and medication characteristics,and one proven case was presented respectively.[Result] Professor SHI thinks the basic pathogenesis of adenomyosis is deficiency of kidney and blood stasis, accompanied by phlegm dampness, the proof is vacuity, treatment should be staged in terms of different periods:non menses-paying attention to eliminating mass and strengthening the body resistance, creatingNei Yi Ting Fang;menses-paying attention to regulating qi and blood by warming drugs, makingXin Gui Zhi Tong Fang;and at the same time,we have to pay attention to the adjustment of life style.All those lead to a good clinical effect. [Conclusion] The rich experience of Professor SHI in treating this disease might be worthy of promotion and learning,which has a good effect on relieving the discomforts and improving the quality of life in patients.
4.Effects of taurine on the expression of neurons SCF mRNA of focal cerebral ischemia in rat

Objective To investigate the effects of taurine on the expression of stem cell factor mRNA of the focal cerebral ischemia in rat.Methods 63 adult male rats were randomly divided into taurine-treated group (Taurine, 80 mg/kg, began at the day of MCAO and continuously intravenous injection for 10 min, daily for 7 days) and controlled group (0.9% normal saline, 1 ml, continuously intravenous injection for 10 min daily for 7 days). The rats received right middle cerebral artery occlusion at 2h and different hours of reperfusion. Expression of SCF was detected by in situ hybridization in the rats subjected to2h,24h,3d,7d, 14 d of reperfusion and sham-operated group (n=3). Results Low level SCF mRNA expressions were found in cortex, striatum and extraventricular zone in sham-operated group. The expression of SCF mRNA in ischemic hemisphere of controlled group increased markedly compared with sham-operated group (t=2.16~25.19, P< 0.05), besides that of 2 h in cortex, 2 h in striatum, 2h and 14d in extraventricular zone. SCF mRNA expression in the treatment group increased markedly in cortex,striatum and extraventricular zone at 3d and 7d(t=5.19~ 26.17,P<0.05). Conclusion Taurine increased the SCF mRNA expression in MCAO rats, therefore it might promote the creation of neural stem cell.

6.The effect of relaxation training with listening to music on negative emotions of patients with maintenance hemodialysis

Objective To explore the effect of relaxation training with listening to music on negative emotions of hemodialysis patients.Methods 70 hemodialysis patients were divided into the intervention group and the control group by random number table,and each group had 35 cases.The control group was given routine care,health education and routine discharge guidance during dialysis by the responsible nurses.The intervention group had relaxation training with listening to music on the basis of routine care.Before the intervention and three months after the intervention the Hamilton Rating Scale for Depression was used as an assessment tool to evaluate negative emotion of patients.Results Before intervention,the HAMD scores of the two groups showed no significant differences.3 months after intervention,HAMD scores had significant difference between two groups.Conclusions Relaxation training with listening to music can significantly improve patients' negative emotions,and is worthy of clinical application.
7.Clinical Observation of Bushenzhuanggutang Combined with Salmon Calcitonin see Calcimar in the Treatment of Renal Osteopathy

8.CT features of small thyroid carcinoma

Objective To study the CT findings of small thyroid carcinoma. Methods The CT findings of 40 patients with histology-proven small thyroid carcinoma (diameter, 1.0 to 2. 0 cm) were retrospectively reviewed. Results (1)The single lesion was detected in 38 cases and two lesions in bilateral thyroid in 2 cases. Two cases were combined with contralateral nodular goiter and I case with contralateral thyroid adenoma. ( 2 ) Eight lesions showed smooth edge and complete envelope. Thirty-four lesions demonstrated foggy edge and incomplete envelope,but they didn't invade the surrounding soft tissues and important organs. ( 3 ) The density of all lesions were homogeneous or comparatively homogeneous without obvious hemorrhage or necrosis area on non-enhanced CT. Thirty lesions showed varied shape calcifications,with granular calcifications in 20 lesions being the most common. Irregular nodular,eggshell-like or mulberrylike calcifications were also detected. (4)Forty-one lesions showed marked enhancement on post-contrast CT and the amplitude of enhanced CT value was greater than 40 HU(range,90 to 140 HU). Thirty-eight lesions exhibited homogeneous enhancement, and other 3 lesions showed marked enhancement center with a ring-like low density edge and manifested as a characteristic damascene-like appearance. (5)Enlarged cervical lymph nodes were found in 24 cases ( 60. 0% ), which displayed solid, cystic-solid or cystic appearances on nonenhanced CT. They showed markedly homogeneous,irregular ring or wall-node enhancement on post-contrast CT. In 8 cases there were granular, nodular or eggshell-like calcifications within the enlarged lymph nodes.Conclusion A solid thyroid nodule with granular calcification, incomplete envelope and marked enhancement, companied with enlarged lymph nodes with calcification, cystic degeneration and obviously enhanced solid part are the relatively characteristic CT features of small thyroid carcinoma.
9.Aorta-pulmonary septal defect and aortic origin of the right pulmonary artery with interruption of the aortic arch: a clinical analysis of 5 cases

Objective Reviewing the experience in the diagnosis and treatment of cases with aorta-pulmonary septal defect and aortic origin of right pulmonary artery complicated with interruption of the aortic arch. Methods Reviewed clinical data from medical records for5 patients (4 boys and 1 girl, age from 1 to 18 years on admission) who had been treated in Wuhan Asia Heart Hospital between 2005 and 2009. Results All cases had type A interruption of the aortic arch ( according to Celoria and Patron's Classification) , type Ⅱ aorta-pulmonary septal defect( according to Mori Classification) and aortic origin of right pulmonary artery complicated with interruption of the aortic arch. Large patent ductus arteries were observed in all cases,and no cardiac abnormality such as ventricular septal defect was found. All patients received echocardiography and spiral CT examination, and 3 of them over 10 years of age received cardiac catheterization and angiography for the evaluation of the pulmonary artery pressure. Two patients, at the age of 14 and 18 years, had severe pulmonary hypertension and declined to receive the operation. One of the 2 cases had hemoptysis and was given prostacyclin ( Iloprost solution for inhalation, a drug for pulmonary artery hypertention ) for reducing the pulmonary pressure. The patient now has an improved cardiac fu0ction without recurrent hemoptysis. One 14-year-old case gave up the operation because of the financial problem and failed to communicate with us after discharge. Radical surgery was performed in the remaining three cases, one had serious infection in the lung and died 11 days after the operation, in one case ( 2 years old) pulmonary artery hypertension has been persisted even though drug therapy was given for a long time and was found at a poor cardiac function 18 months after the operation. One patient recovered well under routine drug therapy and has been followed-up. Conclusion Aorta-pulmonary septal defect and aortic origin of right pulmonary artery with interruption of the aortic arch ( with intact ventricular septum) are different from the classic triad of inter ruption of the aortic arch, consisted of the interruption of the aortic arch, patent ductts arteriosus and ventricular septal defect.It should be considered as a special kind of cardiovascular malformation syndrome. Echocardiography can be used for the preliminary diagnosis of aorta-pulmonary septal defect and aortic origin of right pulmonary artery with interruption of the aortic arch. The final diagnosis and decision for surgery should be based on the combined information from cardiac catheterization,cardiovascular angiography and spiral CT examination. Pulmonary artery pressure and resistance are the most important factors,because they have great effects on the duration of surgery and the outcome. A radical operation is necessary for all cases, but the mortality rate is high. The prognosis would be great if only the patient can survive successfully during the peri-operative period, a period with a poor cardiac function, potential lung infection and the pulmonary artery hypertension.
10.Optimization of the Formulation of Azithromycin Sustained-release Vaginal Suppository

OBJECTIVE:To optimize the formulation of azithromycin sustained-release vaginal suppository(ASVS).METHODS:The ASVS was prepared using S-40 as suppository base,HPMC as sustained-release material and glycerine as humectant.The formulation of ASVS was optimized by orthogonal design with accumulated release percentage and hardness as indexes and the amount of 8% HPMC and glycerol as factors;meanwhile,a verification test and the fitting of in vitro drug release model were performed.RESULTS:The optimized formulation ASVS was as follows:azithromycin 6 g,8% HPMC 23.52 g,glycerol 29.40 g,ethanol absolute 3 g,ethylparaben 0.59 g,and S-40 294 g.Three batches of suppositories prepared under the optimized formulation reached a mean content of 99.5%,with hardness up to the standard,showing good reproducibility and homogenicity in drug release in vitro.The accumulative release rates of all samples were greater than 98% at 180 min,and the release dynamics in vitro were in line with Higuchi equations.CONCLUSION:The optimized ASVS is feasible in formulation,stable and reproducible in preparation technology and up to the standard for sustained-release preparation.
