The potential advantage of Traditional Chinese Medicine(TCM) theories of disease prevention,especial the thinking mode of "health preservation from both physique and spirit "has been generally recognized and widely understood by international society increasingly.In TCM,methods of disease prevention are various and the "health preservation from both physique and spirit" is praised the highest.This article demonstrates the theory that preserving the spirit firstly and then combining with physique,also the methods of keeping an undisturbed spirit and protecting the physique from any harm,for the purpose to make new contribution to the mankind’s health-care industry.

Peripherally inserted central catheter (PICC) has become internationally widespread in various healthcare settings (hospital, clinics ,homecare, et al), mainly used for administration of chemotherapy drugs, hypertonic parenteral nutrition solution and infusion therapy in two extreme age patients (neonates, especially in premature infants;elderly), which provides necessary vascular access for medium to longer term transfusion therapy. The risks of complications related with PICC are still existed, and PICC site with impaired skin integrity is prevalent, but described by only paucity of literature. International evidence-based guidelines provide little strategy. Our study was to review PICC site with skin injury in order to better understand the etiology, evolution and familiar with the international nursing dynamics, and identify directions for future research in this field, promote to overcome the nursing conundrum, and provide a basis for construction of guidelines for the skin management of PICC.

Objective To evaluate the expression of IL-6,TGF-β,matrix metalloproteinase-7 (MMP-7) in the peritoneal fluid of different stage of endometriosis (EMs) patients.Methods Peritoneal fluid samples from female without EMs (n =35) and 45 patients with EMs (36 cases with stage Ⅰ,Ⅱ;9 cases with stage Ⅲ,Ⅳ)were obtained for detectionof IL-6,TGF-β,MMP-7byELISA.Results TheconcentrationofIL-6,TGF-β,MMP-7 was (736.5 ± 526.6) ng/L,(936.5 ± 523.5)ng/L and (5.63 ± 1.98) μg/L in EMs patients,which were higher than those in control group (P ＜ 0.01).The concentration of IL-6,MMP-7 was (948.6 ± 485.4) ng/L,(8.69 ± 1.23) μg/L in stage Ⅲ,Ⅳgroup,and (453.6 ± 256.8) rng/L,(4.23 ± 1.08) μg/L in stage Ⅰ,Ⅱ group,there were significant differences between the two groups (P ＜ 0.05).The concentration of MMP-7 was positively correlated with IL-6(r =0.789,P ＜0.01),and was negatively correlated with TGF-β(r =-0.458,P ＜0.05).Conclusion IL-6,TGF-β,MMP-7 may play roles in microecology immune imbalance and the development of EMs.

Objective To explore the evolution of HBV polymerase gene as a result of adefovir dipivoxil treatment in liver transplant patients with lamivudine resistance. Methods Eight patients with HBV reinfection which was lamivudine resistance received adefovir therapy (10mg/day). Sequential serum samples were obtained from 2 consecutive patients with adefovir failure, and the polymerase gene of HBV was amplified by PCR and TA cloned, and sequencing and analysis were performed in 10 random clones. Results Neither rtN236T nor A181V/T mutation, which were associated with adefovir (ADV) resistance, was found. The rtL180M mutation associated with lamivudine resistance presented before ADV treatment, and several previously unreported amino acid substitutions were observed in the reverse transcriptase domain. Conclusion Possibly, the rtL180M mutation in the B domain of HBV polymerase reduced the susceptibility to ADV. ADV resistance mutation may occur in other sites besides rtN236T and A181V/T amino acid substitutions or do not happen early after treatment.

OBJECTIVE:To determine the content of chrysophanol in Diedazhitong cataplsam by HPLC.METHODS:The samples were separated on a Kromasasil C 18 ,methol-0.1%phosphoric acid(85∶15)was taken as the mobile phase with a flow rate at1.0ml/min and detection wavelength at254nm,the sample size was10?l.RESULTS:Good linear relationship was achieved when the concentration range of chrysophanol was0.6?g～4?g(r=0.9994);The average recovery of chrysophanol was99.23%(n=5,RSD=2.45%).CONCLUSION:This method is accurate,reliable,and reproducible.

Objective To evaluate content validity and construct validity of International Classification of Functioning,Disability and Health(ICF)Core Set for patients with organ transplantation.Methods Patients with kidney,liver,heart,or lung transplantation were assessed with ICF Core Set questionnaire,Functional Independence Measure(FIM)and Barthel Index(BI).Content validity was evaluated with frequency and percentage of patients with a problem for each ICF category,while construct validity was evaluated with Spearman correlation between ICF categories with FIM and BI.Results A consecutive sample of 102 patients completed this study.In body functions,10 categories were reported as a problem by more than 30% of the patients,of which 22 categories correlated significantly with FIM or BI.In body structures,structure areas of skin(s810)were reported as a problem by 30% of the patients,of which 3 categories correlated significantly with FIM or BI.In activities and participation,3 categories were reported as a problem by 30% of the patients,of which 15 categories correlated significantly with FIM and BI.In environmental factors,8 categories were reported as a facilitator by 30% of the patients,of which 8 categories correlated significantly with FIM or BI.Conclusion The content validity and construct validity of ICF Core Set for patients early after organ transplantation were good.

Objective To explore application of case management for hypertension in the elderly.Methods In total, 106 elderly patients with hypertension treated at outpatient and inpatient departments of the First Affiliated Hospital of Xian Jiaotong University were recruited during January to October 2007, and divided randomly into two groups, one with conventional management and the other with case management.Patients with conventional treatment accepted routine diagnostic and therapeutic procedures and were followed-up passively, and patients with case management accepted integrated therapy and active interview provided by a team with a cardiovascular specialist as core, were followed-up actively and their medical records were kept in files. Percentage of the patients with blood pressure lowered to meet criteria of effectiveness and their improvement in depression and anxiety status and quality of life were evaluated two years after management for the two groups. Results Percentage of patients with their blood pressure lowered to meet effective criteria increased significantly in both of the groups, from 32. 8% to 67.5% in patients with conventional management and from 33.7% to 83.4% in those with case management ( P = 0. 007 ),respectively. Scores of self-rating anxiety scale ( SAS) and self-rating depression scale (SDS) decreased in patients of the both groups, from 57 ± 10 and 56 ± 11 before management to 40 ±9 and 48 ± 10 after it,respectively in patients of conventional management ( P = 0. 032 ), and from 56 ± 10 and 57 ± 10 before management to 35 ±9 and 40 ± 9 after it ( P = 0. 008 ), respectively in those with case management; and scores of quality of life improved in patients of the two group, from 65 ± 13 before management to 85 ± 14 after it in those with conventional management, and from 64 ± 13 before management to 98 ± 14 after it in those with case management ( P = 0.017 ), respectively. Conclusions Case management is better for chronic diseases and recommended in management for the elderly with hypertension, which can improve their targets of blood pressure lowering, decrease their scores of SAS and SDS and improve their quality of life,with an integrated, in-depth, continuous management.

Aptamers are short single-stranded nucleic acid ligands that are capable of binding almost any targets with low nano- to picomolar affinities and exceptional specificities. They are selected out of a large combinatorial oligonucleotide library through an in vitro evolution process termed SELEX (systematic evolution of ligands by exponential enrichment). The unique advantages of high-throughput screening technique and aptamers in precise recognition as well as easiness to synthesis and modification bring aptamers a bright prospect in analytical chemistry, biology and medicine research. The recent advances in the SELEX technique and aptamers are reviewed.

OBJECTIVE To investigate the distribution and antimicrobial susceptibility of clinical isolates from the aged patients with urinary tract infection.METHODS Totally 1211 strains of germs were identified and(susceptibility) tests were performed using VITEK-60 system as recommended by the manufacturer.RESULTS The most common pathogens were Escherichia coli,Klebsiella pneumoniae,and Enterococcus faecalis.The infective rate of fungi was higher in the elderly.CONCLUSIONS Our data will be useful for reasonably choosing(antimicrobial) agents in(treatment) of UTI in the aged.

Objective To investigate the obstructive sleep apnea syndrome ( OSAHS) associated with nasal resistance.Methods 30 cases with obstructive sleep apneahypopnea syndrome ( OSAHS ) in the OSAHS group, 30 cases with snoring but without OSAHS in the snoring group and 30 cases with normal group based on polysomnog-raphy were detected the nasal resistance.Results The index of thsnoring index[(42.7 ±27.3),(19.4 ±18.5), AHI(38.7 ±30.2),(5.9 ±1.6)and Resl+r(1.63 ±1.11)Pa· s-1· mL-1,(0.72 ±0.31)Pa· s-1· mL-1] in OSAS group and snoring group were significantly higher than those of the normal group ( t=6.451,7.444,7.698,all P<0.05);The index of lowest oxygen saturation ( SaO2 ) ,Flowl+r in OSAS group and thsnoring group were lower than that in normal group(t=7.551,7.334,all P<0.05);There were significant differences among groups of nasal resistance (t=7.451,8.144,8.608,8.904,all P<0.05);There were positive correlation between nasal resistance and snore index (r=5.658,P<0.01);The index of nasal resistance correlated with AHI index (r=0.002,P>0.05),wake times (r=0.083,P>0.05) and lowest SaO2(r=0.105,P>0.05)were not significantly correlated;The cases with OSAS cephalometric.The results suggest majority snoring symptoms significantly more at supine posi-tion in OSAS.Conclusion Nasal resistance increased,which may be associated with the occurrence in patients with OSAHS.
