Atrial fibrillation (AF) is one of the common arrhythmias. Catheter radiofrequency ablation is a only effective therapeutic method of AF currently. Contact force sensing catheter like SMART TOUCH can real time monitoring the pressure of catheter tip, which can help operators to understand the contact force between atrial and ablation points, and guide the atrial fibrillation ablation. It is significantly improved the effectiveness and safety of the atrial fibrillation. This article will focus on the current situation of radiofrequency ablation for AF by contact force catheter, in order to provide guidance for clinical work.

Chinese Journal of Postgraduates of Medicine 2016;(2):182-185
Acute ST-elevation myocardial infarction is a common clinical acute and severe disease, and it has a high fatality rate. At present, thrombolysis is still the most applicable and effective treatment method at the primary hospital. In the thrombolysis therapy, the best anticoagulation therapy can improve the patency of infarct-related coronary artery and lower the rate of reinfarction. For acute ST-elevation myocardial infarction, studies on thrombolysis anticoagulation mechanism, common anticoagulant, bleeding complications and prospect of research are necessary.

Objective To determine the relationships between serum leptin levels and maternal weights in late pregnancy and cord blood leptin levels to birth weights, C peptide, insulin and insulin like growth factor (IGF II). Methods Fifty normal pregnant women at 37~38 weeks and their newborns were studied, and 29 non pregnant women were set as control. Venous blood was taken from women and from the cord at delivery. Blood leptin and cord blood C peptide, insulin, and IGF II were measured by radio immunoassay. Results The average leptin level in maternal sera was (13.62?3.68)?g/L, significantly higher than that in the control (6.60?3.04)?g/L and that in cord blood (8.05?4.61)?g/L. Maternal leptin levels were significantly correlated with maternal weights and body mass index (BMI. r =0.33, 0.35, P 0.05). Cord blood leptin levels were significantly correlated with birth weights and BMI ( r =0.54, 0.49, P 0.05). Significant difference of the cord leptin levels was not seen between the males and females. The cord blood C peptide was (0.86?0.35) ?g/L, insulin (8.49?4.76) mU/L and IGF Ⅱ(0.218?0.076) ?g/L. Cord leptin levels were correlated with C peptide levels ( r =0.37, P 0.05). Conclusions Maternal leptin levels in late pregnancy were significantly higher than those in normal non pregnant women and positively correlated with maternal weights and BMI. Cord blood leptin levels were positively correlated with birth weights and BMI of the newborns. The leptin levels of cord blood were correlated with those of C peptide but not insulin and IGF II.

0.05). (2) In the total 12 rabbits, all the cornea grafts survived without rejection reaction, corneal haze or corneal neovascularization. The fresh grafts got transparent after 2 months, and the dehydrated grafts got transparent after 6 months. Histological study demonstrated both fresh and dehydrated stroma grafts had fused with rabbits'corneal stroma very well without lymphocytes infiltrating. (3) As shown in histological observations, the reconstructed cat corneal endothelial tissue was similar to the nature tissue. Cultured cat endothelial cells connected tightly to each other and attached to the Descemet's membrane closely. CONCLUSION: Swine corneal stroma has low immunogenicity and satisfying biocompatibility,it is an ideal biological carrier for cornea reconstruction in vitro.

AIM: To study the effect of high fat diet on the expression of sterol regulatory element biding protein-1 (SREBP-1) and transforming growth factor β_1 (TGF-β_1) in renal tubular cells and rosiglitazone intervention. METHODS: Wistar rats were treated with high fat diet and rosiglitazone for 3 months. The serum glucose, serum insulin and serum triglyceride were detected. Oil Red O staining was used to observe the renal lipid deposit and Masson staining was for the detection of ECM accumulation. SREBP-1, TGF-β_1 and FN protein were determined by the methods of immunohistochemistry and Western blotting. SREBP-1 mRNA was detected by in situ hybridization. RESULTS: Rosiglitazone prevented effectively the increase in serum glucose, serum insulin and serum triglyceride resulted from high fat diet. High fat diet led to lipid droplet formation in renal tubular cells and interstitial ECM accumulation, which was decreased by rosiglitazone treatment. Compared to normal rats, SREBP-1 protein and SREBP-1 mRNA showed high expressions in high fat diet rats that were lowered by rosiglitazone. The precursor segment and mature segment of SREBP-1 protein were decreased by 27.39% and 27.32%. Similarly, the high expressions of TGF-β_1 and FN protein in kidney of high fat diet rats were also prevented by rosiglitazone intervention. Compared to high fat diet rats, the expression of TGF-β_1 in rosiglitazone treatment rats was lowered by 19.14%. CONCLUSION: Rosiglitazone prevents effectively the over-expression of SREBP-1 and TGF-β_1 in renal tubular cells, and decreases lipid accumulation and ECM production in rats fed with high fat diet.

Objective: To find out the influence of stabilization occlusal splint on intra-articular pressure of the temporomandibular joint. Methods: A muti-channel measuring and analysis system for intra-articular pressure of the temporomandibular joint was developed in this study. Twenty-two subjects were selected to detect the intra-articular pressure of the temporomandibular joint before and after wearing stabilization splint. Results: Before splint wearing, the intra-articular pressure of the temporomandibular joint at intercuspal occlusion status was 61.3?48.5 mm Hg (1 mm Hg=0.133 kPa), the negative pressure at open mouth status was 33.2?34.2 mm Hg. After splint wearing, the pressure at intercuspal occlusion status was 39.5?24.5 mm Hg, the negative pressure at intercuspal at open mouth status was 36.3?25.3 mm Hg. The intra-articular pressure of the temporomandibular joint after wearing splint was significantly lower than the pressure before splint was used, the pressure of mouth opening status was stable before and after splint was used. Conclusion: Stabilization occlusal splint can reduce the intra-articular pressure of the temporomandibular joint.

Objective:To discuss the effect of improved supine position on the prevention of pressure injuries of emergency patients under observation and their comfort levels. Methods:A total of 114 patients in our Emergency Observation Ward were randomly divided into the intervention group and control group, 57 patients in each. Patients in the control group were treated with routine supine position and pressure injury prevention nursing; while patients in the intervention group were treated with improved supine position nursing intervention. Then, the incidence of pressure injury, its risk score and its comfort evaluation were compared between patients under observation in the two groups. Results:The incidence of pressure injury of the intervention group was obviously lower than that in the control group ( Z value was -2.426, P<0.05). The scores of sensation, humidity, activity, friction and shear force of patients in the intervention group were 3.36 ± 0.59, 2.92 ± 0.47, 3.21 ± 0.52 and 2.61 ± 0.29 respectively, which were significantly higher than those in the control group, specifically 2.87 ± 0.51, 2.24 ± 0.38, 2.76 ± 0.44 and 2.04 ± 0.32. The difference was statistically significant ( t value was 4.74-9.96, P<0.01). In addition, the scores of the physiological and psychological comfort level of patients in the prevention group were 15.41±2.49 and 27.26 ± 3.42 respectively, which were significantly higher than 11.01±1.97 and 22.14± 3.23 in the control group. The difference was statistically significant ( t value was 10.46, 8.21, P<0.05). Conclusions:The improved supine position nursing method can effectively lower the occurrence of pressure injury in emergency patients under observation and enhance patients’ comfort degree. Therefore, it is applicable to emergency patients under observation.

Objective:To investigate the variations in infrared radiation at Taichong(LR3),Taixi(KI3),and control points before and after menstruation and to examine the infrared radiation patterns associated with Yuan-primordial points of Zang-Fu organs during the physiological menstrual cycle. Methods:Using a point infrared radiation spectrum detection system,we detected the infrared radiation spectra of Taichong(LR3),Taixi(KI3),and the control points located 1 cm away from the two points,in a range of 1.50-18.00 μm wavelengths during the premenstrual, menstrual, and postmenstrual phases in 32 healthy adult women.Subsequently, data mining and analysis were conducted. Results:Before, during, and after menstruation, the infrared spectral shapes of bilateral Taichong(LR3),Taixi(KI3),and their control points were generally consistent, with characteristic infrared spectral wavelengths located at 11.25 μm.Prior to menstruation, the total intensity of infrared radiation at the right Taixi(KI3)was significantly lower than that at the control point(P<0.05),and that at the left Taichong(LR3)was significantly lower than that at the control point(P<0.01).During and after menstruation, the total infrared radiation intensity at both Taixi(KI3)was significantly lower than that of the control point(P<0.05).The wavelength points exhibiting significant differences in the infrared radiation intensity between points and control points were concentrated at the primary peak of 7.50-14.25 μm and the secondary peak of 15.00-17.25 μm. Conclusion:During different menstrual phases, the infrared radiation spectra of Taichong(LR3)and Taixi(KI3)exhibited distinct point specificity, mainly evident in the infrared radiation intensity and wavelength.

Objective To investigate the spinal stability reconstruction and related complications after total spondylectomy for thoracolumbar tumors. Methods From January 1997 to December 2009, 34 cases with thoracolumbar tumors treated with total spondylectomy, including 20 males and 14 females with an average age of 43 years. The tumors were located in thoracic vertebra in 27 cases and lumbar vertebra in 7, including 6 spinal metastases and 28 primary tumors. The total spondylectomy was piecemeal in 23 cases and en bloc in 11. The reconstruction methods included posterior short-segment fixation in 19 cases, multi-segmental fixation in 13, anterior intervertebral fixation in 2, titanium mesh with auto-bone graft in 20 cases, titanium mesh with bone cement in 7, bone cement filling in 2, auto-bone strut graft in 2, titanium mesh with strengthened rings in 1, artificial vertebral body replacement in 2, posterior massive bone bridging graft in 8 and bone particles graft in 26. Results The mean follow-up time was 43 months. Seven patients died, 5 suffered recurrence. Poor grafted bone fusion was found in 5 cases. The intervertebral sagittal Cobb angle of adjacent vertebra was ??after operation, 9.3?at the end of follow-up or before revision operation. The average lost angle was 14.1?for cases with multi-segmental fixation, and 21.5?with short-segment fixation. Complications included internal fixation broken (5) and loosening (2), titanium mesh subsidence (6) and displacement (3). There were 5 revision surgeries. Conclusion There are relative more complications about spinal stability reconstruction after total spondylectomy. To achieve temporary stiff fixation and long-term fusion, the posterior multi-segment fixation and massive bone bridging graft combined with the anterior artificial vertebral body replacement are effective.
