1.STUDIES ON THE CELL KINETICS OF SKIN AND TESTIS IN EXPERIMENTAL ANIMALS

By using double labelling methods with ~3H-TdR and ~(14)C-TdR, autoradiographs were prepared in young (40 days old) and in old (16 months old) male LACA mice and in adult (7~7.5 months old) female Wistar rats to observe the cell cycle time and relevant parameters of dorsal skin and both testes.The following results were obtained:1. In young and old mice, duration of cell cycle (Tc) and relevant parameters of epidermis were as follows respectively: labeling index (LI)=1.0 and 1.5%; mitotic index (MI)=0.4 and 0.4%; duration of DNA synthesis (Ts)=3.5 and 4.2 hours; Tc=20.3 and 17.5 days.In adult female rats, they were 3.0%, 0.3%, 4.7 hours and 8.3 days respectively. Tc of rat epidermis was obviously shorter than that of mouse epidermis (P0.50).
2.Effects of desflurane, sevoflurane and isoflurane on hepatic blood flow and hepatic oxygen delivery and consumption in swine

Objective To observe the effects of desflurane, sevoflurane and isoflurane on hepatic oxygen balance and hepatic blood flow Methods Fifteen pigs were randomly divided into three groups: A desflurane group(n=5), B sevoflurane group(n=5), C isoflurane group(n=5) This study observed the effects of desflurane, sevoflurane and isoflurane on systemic and hepatic blood flow and oxygen balance with different concentrations using continuous thermodilution cardiac output monitor and ultrasonic Doppler blood flow monitorResults ①Hepatic blood flow: total hepatic blood flow(THBF) decreased dose-dependently in three groups without significant differences between groups The effects of desflurnae and sevoflurane on THBF were same as those on cardiac output(CO), while the effect of sevoflurane on THBF was larger than that on CO ②Oxygen balance: both systemic oxygen delivery(DO_2) and hepatic oxygen delivery(HDO_2) decreased significantly with the increasing of inhalation concentrations, systemic oxygen delivery(VO_2) and hepatic oxygen consumption(HVO_2) decreased significantly only at high concentration The effects of desflurnae and sevoflurane on HVO_2 and HDO_2 were same as those on VO_2 and DO_2, while the effect of sevoflurane on HDO_2 was larger than that on DO_2, but the effect of sevoflurane on HVO_2 was still same as that on VO_2Conclusions Because of the dose-dependently decreased hepatic blood flow, hepatic oxygen delivery decreases significantly, but hepatic oxygen consumption can be maintained well by self-regulation of hepatic oxygen extraction ratio,indicating that there is almost no influence on hepatic intracellular respiration and metabolism
3.Effects of two expectoration methods on pulmonary complications and hypoxemia in esophageal cancer patients having undergone radical resection with video-assisted thoracoscopic surgery

Objective To compare the effects of expectoration by high frequency back tapping with both hands and traditional expectoration by tapping back with a single hand on pulmonary complications in esophageal cancer patients having undergone radical resection with video-assisted thoracoscopic surgery (VATS). Methods Sixty patients after radical resection for esophageal cancer with VATS from May 2013 to January 2014 were set as the control group, in which expectoration by tapping the back with a single hand. Another 60 patients after radical resection for esophageal cancer with VATS from February 2014 to July 2014 were set as the observation group, in which the expectoration by high frequency tapping the back with both hands. The two groups were compared in terms of pulmonary complications. Result The incidence of atelectasis and pulmonary infection in the observation group were lower than those with a single hand in the control group (all P<0.05). Conclusion For the patients having undergone radical resection of esophageal carcinoma with VATS, the expectoration with high frequency back tapping with both hands is more effective in lowering atelectasis rate than that with a single hand.
4.Expression of doublecortin-like kinase 1 in colorectal cancers and colorectal adenomas and its clinical significance

Objective To investigate the expression of doublecortin-like kinase 1 (DCLK1) in colorectal cancer and colorectal adenomas,and it's role in the development and progression of colorectal carcinoma,and explore the relationship of the DCLK1 expression and prognosis in colorectal cancer patients with diabetes medical history.Methods Expression of DCLK1 in 70 cases of colorectal cancers,70 cases of para-carcinoma tissues,and 35 cases of tubulovillous adenomas were detected by immunohistochemical method.Results (1) The DCLK1 high expression rate in colorectal cancers was 41.4% (29/70),and 45.7% (16/35) in tubular villous adenomas,both significantly higher than the para-carcinoma tissues (4.3%;3/70) (P <0.01).(2) DCLK1 high positive expression had statistical significance with pathological grading (P < 0.01),infiltration depth (P < 0.05),lymph node metastasis (P< 0.05),vascular invasion (P <0.05),and distant metastasis (P <0.01).(3) There was no obvious correlation between the Diabetes medical history,expression of DCLK1,and prognosis of patients.Conclusions DCLK1 has high positive expression in colorectal carcinomas,and may have certain significance for the early clinical diagnosis and prognostic judgment of colorectal cancers.The expression of DCLK1 and prognosis of colorectal cancers has no obvious correlation with diabetes medical history.
5.Status quo of job-family and family-job conflicts in nurses in operating room

Objective To investigate the status quo of job-family and family-job conflicts among the nursing of the operating room nurses.Method A total of 190 nurses from an operating room participated in the survey by demographic questionnaire and work-family conflict scale.Results The average score on job-family was (3.15 ± 0.48) and there were significant differences between family-job conflict and job-family (2.75 ± 0.27 vs.3.55 ± 0.61) (t =-10.349,P<0.05).There were significant differences in job-family conflict scores (P <0.05) in view of different age,working years,professional titles,number of night shifts per month,marital status and daily working hours.There were significant differences in family-job conflict subscale scores (P<0.05).Conclusions The job-family conflicts of the nurses in the operating room is at the middle level.The perceived job-family conflict is higher than that of the family-job conflict.Such factors as age,length of work,professional tide,number of night shifts per month,marital status and daily working hours can lead to job-family conflict among the operating room nurses.The nursing administrative should displace nursing personnel and optimize the shifts so as to reduce the rate of job-family conflicts.
6.Effects of PDS-C on immunoregulation in mice with aplastic anemia

Aim To observe the effects of panaxdiol saponins component ( PDS-C) extracted and isolated
from Chinese ginseng herb as new Chinese patent med-icine on the promotion of hematopoiesis and the regula-tion of the immune system in treating mice models with aplastic anemia ( AA ) . Methods For preparation of immune mediated AA models, BALB/c mice were ex-posed to sublethal doses of 5. 0 Gy γ radiation, fol-lowed by transplanted lymphocytes from DBA/2 donor mice. The mice models were divided into six groups in-cluding normal control, AA model, PDS-C treated groups with lower, medium and higher dosages, cy-closporine ( CsA) as positive drug control. Both PDS-C and CsA were administered by gastrogavage for 15 days. The peripheral blood cells counts and bone mar-row pathological examination were tested, the percenta-ges of Th1/Th2/Treg cells from spleen were measured, the protein expression levels of T-bet, GATA-3 and FOXP3 transcription factors in spleen cells were detec-ted. Results Curative effect of PDS-C on treating AA
mice was satisfactory. The peripheral hemoglobin, white blood cells and platelet counts in PDS-C groups with medium and higher doses were significantly higher than those in model control. Meanwhile, PDS-C ele-vated the percentages of Th2 cells and Treg cells, but decreased the percentage of Th1 cells, as well as up-regulated the GATA-3 , FOXP3 and down-regulated the T-bet protein levels. Conclusion PDS-C possesses the activities of promoting hematopoiesis obviously. It can improve marrow myelosuppression, enhance the re-covery of hematopoiestic function, and elevate the pe-ripheral blood cells counts. PDS-C also pays its immu-noregulatory efficacy though recovering from unbal-anced Th1/Th2/Treg cells in treating immune media-ted AA mice.
7.Estimation of the plasma-effect site equilibration rate content(Ke0)in patients before operation

Objective To estimate the value for the plasma-effect site equilibration rate constant(Ke0)of propofol using the time to peak effect(Tpeak)method and two pharmacokinetic models for propofol.Methods The Tpeak after a submaximal bolus dose 1.5 mg?kg~(-1) of propofol was measured with AAI A-line auditory evoked potential monitor in 36 patients scheduled for elective surgery under general anesthesia.Using Tpeak and two sets of pharmacokinetic parameters for propofol reported by Marsh and Shafer,the Ke0 was estimated according to the method described by Minto.Results The mean Tpeak was(142?62)s in 30 patients.The Ke0 was 0.626 min~(-1) with the model by Marsh and 0.914 min~(-1) with the model by Shafer.The corresponding t_(1/2) Ke0 values were 1.107 min and 0.759 min respectively(P<0.01).Conclusion The Ke0 value of propofol estimated depends on the pharmacokinetic models used.The values we estimated in Chinese patients are significantly different from the values reported by foreign authors
8.Analysis of factors influence and posttransfusion effectiveness in matched-type of platelet transfusion

Objective To analysis the related influencing factors and explore the approach of improving the effect of platelet transfusion through compared the posttransfusion effect of matched-type with random platelet transfusion.Methods Analysis the results of platelet antibody screening test and matched-type test in our hospital′s inpatients during July 2013 to June 2014,who applied for platelet transfusion.The influence of sex,blood transfusion history,pregnant history on the antibody were analyzed.The factors of sex,times of blood transfusion,pregnant times,platelet component sorts,storage time,combined with other components,platelet antibody (positive or negative) and transfusion matched-type platelet on the effect of platelet transfusion were analyzed.Application in our clinical blood transfusion intelligent management and evaluation system to evaluate the effect of platelet transfusion.Results Totally 812 patients′ platelet antibody were screened.Then we randomly selected 87 antibody positive inpatients were selected and 1 247 U plateletswere transfused,which including matched-type platelet transfusion for antibody screening positive patients,random platelet transfusion for antibody screening positive patients,random platelet transfusion for antibody screening negative patients,there were statistical difference (P<0.05).With Logistics regresion analysis,the history of blood transfusion was an independent risk factor for platelet antibody production (P<0.05,OR=13.104,95%CI:7.784-22.061).Sex (P<0.05,OR=1.629,95%CI:1.236-2.148),transfusion times,different platelet component sorts (leukocyte-reduced platelets aphaeresis and irradiation leukocyte-reduced platelets aphaeresis),different storage time,transfusion combined with other components (RBC)(P<0.05,OR=2.464,95%CI:1.053-5.765),transfusion matched-type platelet(P<0.05,OR=0.576,95%CI:0.389-0.854) were the risk factors for platelet transfusion.Conclusion Matched-type test should be done to improve efficiency when the platelet antibody screening were positive.Sex,times of blood transfusion,platelet component sorts,storage time,combine with other components,transfusion matched-type platelet couldinfluence the effect of platelet transfusion.
9.Application of genetically engineered mice model in the research of gastrointestinal tumor.

The gene knockout is based on the DNA homologous recombination and embryonic stem cell technology. It is an experimental method for modification of specific gene loci, which is one of the most direct means to clarifying gene functions. The similarity of genomes between mouse and human is as high as up to 95%. The life cycle of mice is relatively short, and easy to breed with stronger reproductive capacity. Therefore, the knockout mice are suitable for observing the whole process of disease pathogenesis. With genetic manipulation technology, researchers can produce the simulations of phenotypic effect by human genetic variation. This article focuses on genetically engineered mice models and their application in gastrointestinal cancer.

10.Precise liver resection by intraoperative interventional intravascular segmental vessel balloon catheter occlusion

Objective To evaluate the feasibility of precise hepatic segmentectomy or subsegmentectomy using intraoperative image-guided interventional intravascular segmental vessel balloon catheter occlusion of the segmental hepatic artery and portal vein.Methods 6 patients with liver resection carried out from 2011.3-2011.8 were retrospectively analyzed.Results The mean operating time was (270.83±21.31) min,the median of blood loss was 800 ml,the median of intraoperative transfusion volume was 450 ml.The tumors were mainly located in segments Ⅴ,Ⅵ,Ⅶ,Ⅷ.The mean diameter of tumor was (5.67±1.03) cm.Postoperative liver function in the first postoperative day showed the mean alanine aminotranferase (ALT) was (570.00±157.76) U/L,the mean aspirate aminotrarsferase (AST) was (410.00 ±189.94) U/L,and the mean total bilirubin (TBIL) was (10.83± 1.60) mmol/L.Liver function recovered to normal within 7 days.There was intestinal leakage and wound dehiscence in one patient,pleural and effusion in two patients.Conclusion Imageguided interventional intravascular segmental vessel balloon catheter occlusion was a safe and efficacious maneuver.This technique allowed hepatic segmentectomy or subsegmentectomy to be carried out,decreased intraoperative bleeding,and protected the function of the liver remnant.
