The author's retinoscopy with a double-door type of illumination is presented in this paper. It is an objective method for the measurement of refractive errors by using an elect ic retinoscope. Spherical lens are used to neutralize the illumination from the pupil of the Patient u til the size, intensity and rapidity of movement of the double-door illumination from the pupil becomes almost identical. When a lineal back slit srace has been revealed in the middle of the double-door illumination, with a point of re(?)lex light on the center corneal surface, the neutral point is determined. The main points of using this method are also discussed in this paper.

BACKGROUND:The artificial joint has gone through nearly a century of history from the beginning of design to the clinical application. The forerunner of artificial joints is artificial hip joint. With the skiled technological application in clinic, the design, application and improvement of wrist, shoulder, knee and other artificial joints have been promoted. OBJECTIVE: To summarize the application progress and performance characteristics of artificial joint materials in exercise-induced articular cartilage injury. METHODS: PubMed (http://www.ncbi.nlm.nih.gov/PubMed), Wanfang database (http://wanfangdata.com.cn/) and China National Knowledge Infrastructure (http://www.cnki.net/) during 1980 to 2015 were retrieved by the first author. “Sports; Cartilage Injury; Artificial Joint” were taken as the key words in English and Chinese, respectively. Retrieved data were analyzed and summarized. The artificial joint materials commonly used at home and abroad were introduced and analyzed one by one. The clinical advantages and disadvantages of various materials were analyzed from the view of materials science. The surface treatment methods commonly used for artificial joints were described. The direction of the artificial joint research was pointed out from a development perspective. RESULTS AND CONCLUSION:The surface modification of titanium aloys can obtain surface ceramic layer with good performance, and can effectively improve the anti-wear properties of titanium aloys. The filing and modification of ultra-high molecular weight polyethylene can obtain joint composites with good anti-wear properties that can effectively reduce the production of ultra-high molecular weight polyethylene wear particles and the biological response induced by ultra-high molecular weight polyethylene wear particles. Further research and improvement of ceramic artificial joints wil be the future direction of development of artificial joints.

Objective To prove the prophylactic function of absorbable gelatin sponge and bioprotein glue for prevention of epidural and nerve root scar adhesions after laminectomy. Methods We excised the L6 vertebral plates of 36 adult New Zealand male rabbits,to make vertebral plate excision model and to find nerve root. Then divided them into 3 groups based on different materials:gelatin sponge and bioproten glue (group A),gelatin sponge (group B) and saline solution (group C). The morphological changes of the scars were observed grossly and histologically at 2 and 8 weeks after operation. We also measured the orbit of the nerve root. Objective:To analyze the characteristics of high-frequency average hearing loss in both ears of noise exposed workers in Tianjin in 2020, and quantitatively analyze the influencing factors of high-frequency hearing loss in both ears of workers.Methods:In March 2023, Collect and organize basic information about noise-hazardous enterprises and personal information of workers exposed to noise. Data from the Tianjin Occupational Disease and Health Hazard Factors Information Monitoring System from January 2020 to December 2020, and analyze the impact of basic information of employees, enterprise size, regional distribution, industry category, and economic type on the high-frequency average hearing loss of workers during work. Apply logistic regression to quantitatively analyze the influencing factors of abnormal high-frequency average hearing threshold of noise exposed workers.Results:The size, economic type, industry category, and regional distribution of enterprises, as well as the gender, age, length of service of workers, have an impact on the abnormal high-frequency average hearing threshold of noise exposed workers (χ 2=733.56、3 497、27、1352.84、1197.62、2570.59、22.30、506.60, P<0.001) . Quantitative analysis using a logistic regression model showed that in the basic information of workers, noise exposed workers were male ( OR=2.500, P<0.001) and aged 30-39, 40-49, and 50-59 years ( OR=1.33, P<0.001; OR=1.68, P<0.001; OR=1.52, P< 0.001) , with a length of service of 4 to<10 years and≥10 years ( OR=1.08, P<0.001; OR=1.615, P<0.001) being the influencing factors for high-frequency hearing loss in both ears of noise exposed workers; In terms of enterprise characteristics, medium-sized, small and micro enterprises ( OR=1.12, P<0.001; OR=1.75, P<0.001; OR=2.09, P<0.001) , enterprises located in the fourth district around the city ( OR=1.268, P<0.001) , and enterprises with economic types of collective economy, other economy, private economy, Hong Kong, Macao and Taiwan investment, shareholding system, and other industry economies ( OR are all >1, P<0.001) are all factors affecting high-frequency hearing loss in noise exposed personnel. Conclusion:Noise is a common occupational hazard factor in Tianjin's enterprises, especially for workers in micro enterprises who face a high risk of hearing abnormalities. Therefore, enterprises need to strengthen the management and intervention of noise operations to prevent the occurrence of hearing loss in workers.

Objective To discuss risk factors of catheter-related infection and coping strategies about power peripherally inserted central catheter ( POWER PICC) in patients with breast cancer, and to provide the evidence-based nursing basis for standard operational procedure of POWER PICC. Methods A total of 323 patients with breast cancer underwent chemotherapy by three directional valve PICC, and a total of 322 patients did by POWER PICC. Risk factors of catheter-related infection were analyzed by univariate analysis and logistic multiple regression analysis. Results The catheter-related infection rate of POWER group was higher than that of three directional valve group[11.49% (37/322) vs 5.26% (17/323);χ2=8.152,P<0.01]. Four risk factors of catheter-related infection in the POWER group were screened by univariate analysis, including material of catheter, position of puncture, treatment for wound surface and sort of connector. Unconditional multiple analysis was conducted, regarding bacterial colonization as dependent variable and other factors as independent variable. The result demonstrated that individual care strategy for PICC was the risk factor of catheter-related infection of POWER PICC. Conclusions Understanding of the property of POWER PICC material and applying individual care strategy to enhance advantage and avoid disadvantage can prevent catheter-related infection of POWER PICC effectively.
