1.Clinical analysis of massive blood transfusion in elderly patients in third-tier general hospital

Objective To investigate the clinical status of massive transfusion in elderly patients in general hospital in order to improve the clinical use of blood and ensure the security of blood transfusion in elderly patients.Methods 100 patients who required massive transfusion were divided into 2 groups:elderly group (patients aged 60 years and over,n=32) and non-elderly group (patients aged under 60 years,n=68).Clinical blood transfusion indications in all patients met the Technical Criterion of Clinical Blood Transfusion issued by Ministry of Health of the People's Republic of China in 2000.Clinical data were analyzed and compared between the two groups.Results Elderly patients with massive transfusion were mainly in department of surgery,including department of orthopedics,general surgery,hepatobiliary surgery,cardiac surgery and urology surgery and gastroenterology,and the mean age of patients in these departments was 71.5 years,69.9 years,72.0 years,66.0 years,65.5 years,70.8 years respectively,accounting for 50.0%,70.0%,20.0%,75.0%,20.0% and 66.7% in all patients who required massive blood transfusion respectively.The mean blood transfusion volumes were higher in patients in department of gastroenterology,general surgery,hepatobiliary surgery and orthopedic,in which the mean units of red blood cell components transfusion were 14.0 U,7.0 U,5.2 U and 4.1U respectively,the mean units of plasma components transfusion were 23.3 U,8.4 U,8.0 U,0.5 U respectively,and the mean uint of platelet components transfusion was 1.6 U in hepatobiliary surgery.Conclusions The risk of blood transfusion is higher in the elderly than in the non elderly.We should select the effective blood components reasonablely,reduce the unnecessary blood transfusion and decrease the dependence on plasma transfusion to avoid the transfusion overload and adverse reactions.
2.Calcium channels in glioma

Calcium channels express abnormally in glioma,and play a key role in the physiological functions of glioma such as tumorigenesis,proliferation,metastasis and angiogenesis. Researches show that low or high expressions of different types of calcium channels have influence on the development of glioma and cell-cycle redistribution. Such calcium channels may represent new targets in treating glioma,which provide new ideas for controlling glioma.

4.Research progress of mechanism of vitamin D in regulating obesity and insulin resistance on children

Vitamin D has a wide range of biochemical effects. Insufficiency/ deficiency of vitamin D increases the risk of insulin resistance by ways of inflammation,adipocytokines,oxidative stress and mitochondria function,it could lead to metabolic disorder of glucose and lipid,which closely relates to the incidence and development of obesity and metabolic syndrome on children. This paper presents the relationship between vitamin D with obesity and metabolic syndrome on children,states the possible mechanisms of regulation of glucose/ lipid metabolism and insulin resistance by vitamin D.
5.Simultaneous Determination of Three Ingredients in Compound Olmesartan Medoxomile Tablets by HPLC

Objective:To establish an HPLC method for the simultaneous determination of olmesartan medoxomile, amlodipine be-sylate and hydrochlorothiazide in compound olmesartan medoxomile tablets. Methods:The desired chromatographic separation was a-chieved on a Phenomenex C18 (250 mm × 4. 6 mm, 5μm) column. With gradient elution, the optimized mobile phase consisted of ace-tonitrile as solvent A, and 50 mmol·L-1 potassium dihydrogen phosphate buffer (pH 3. 0, containing 0. 05% triethylamine) as sol-vent B. The flow rate was set at 1. 0 ml·min-1 , the column temperature was 30 ℃, the UV detection was carried out at 236 nm and the sample size was 20μl. Results:The compounds were separated well, the linearity between the peak area and the concentration was observed within the range of 4. 0-80. 0 mg·L-1(r=0. 999 9)for olmesartan medoxomile, 1. 0-20. 0 mg·L-1(r=0. 999 9)for amlo-dipine besylate and 1. 25-25. 0 mg·L-1(r=0. 999 9)for hydrochlorothiazide. The average recovery of olmesartan medoxomile, amlo-dipine besylate and hydrochlorothiazide was 99. 1%(RSD=1. 31%, n=9), 100. 3%(RSD=1. 21%, n=9) and 100. 2%(RSD=1. 06%, n=9), respectively. Conclusion:The method is specific and stable in the determination of olmesartan medoxomile, amlo-dipine besylate and hydrochlorothiazide in the tablets.
6.Single port laparoscopic treatment of inguinal hernia in children

Ojective The improve laparoscopic treatment of inguinal hernia in children. Methods 40 children aged from 2 to 14 years were treated by single port laparoscopic surgery for inguinal hernia between January 1999 and December 2000. Twenty four children manifested a right inguinal hernia, 12 a left hernia, and four cases a bilateral hernia. All the patients were treated by purse-string suture around the internal ring under laparoscopy. Results The duration of operation were from 10 to 30 minutes. There was no intra-or postsurgical complication. Conclusions The single port laparoscopic surgery is a simple, conveninet, quick and safe and minimal invasive procedure for the treatment of inguinal hernia in children.
7.How much do community residents know about dermatosis and venereal diseases and its impact factors?

Randomized sampling-based questionnaire among 2000 community residents and personal contact with selected populations were conducted.Based on incidence or clinical featuxes of dermatosis and venereal diseases and recognition-related impact factor analyses,we provided evidence for community-based health education and behavior intervention.Our findings showed that itching was the most common skin disease,and fungous infection displayed an increasing trend.Viral diseases increased significantly in recent years.Patients failed to understand the skin diseases,although they partly had some misleading information about venereal diseases.Health care providers should perform more health education and behavior interventions regarding dermatosis and venereal diseases for community residents to improve their health condition or quality of life.
8.Evaluation of clinical blood transfusion management using DRGs

ObjectiveBy analyzing the transfusion-related data using DRGs data to establish a credible evaluation system for clinical use,thereby enhancing the safe and effective clinical transfusion management.Methods Use the transfusion-related data of DRG-s,and statistical methods of EXCEL database to categorize and analyze the number of blood transfusion,hospital name,hospital type,hospital level,DRGs code and name,times of blood transfusion,volume of transfusion,etc.Results Output the statistics of the city's ratio of institutional use of blood,per capita consumption in patients,distribution and ordering of hospital based blood tranfusion,distribution and ordering of DRGs based blood transfusion,distribution and ordering of single DRGs between hospitals,annual consumption growth in different hospitals and major disease groups of blood tranfusion.ConclusionThe use of DRGs can efficiently control clinical indicators for blood transfusion and evaluate the performance of blood transfusion.it can provide a credible management tool for the health administrative departments and hospitals while offering data support for policy making of management objectives at the same time.
9.Analysis of safety on blood transfusion in the elderly patients

Objective To analyze the safety of blood transfusion in hospitalized senile patients,and to prevent or reduce adverse transfusion reactions. Methods The blood transfusion information of patients over the age of 60 years in our hospital from January 2009 to December 2011 were retrospective analyzed. The data of pre-storage type of autologous blood transfusion for elective surgery were also discussed. Results Among 3549 cases of elderly patients,there were totally 23877 times of allogeneic transfusion and 84 cases with blood transfusion adverse reactions of heating or allergic reactions.The adverse reaction rates occured most in the infusion of fresh frozen plasma (0.55%).There was no hemolytic reaction,and transfusion adverse reactions occarred during stored blood autotransfusion. Conclusions Medical institutions should set the scientific and rational principle of blood use and select the appropriate blood components to reduce transfusion adverse events in the elderly patients. The autologous blood transfusion in the elderly patients should be actively pursued.
10.The relationship between airway inflammation typing and asthma status and its response to inhaled corticosteroids treatment in children with mild or moderate asthma

Objective To evaluate the relationship between airway inflammation before treatment and asthma status and response to inhaled corticosteroids treatment in children with mild or moderate asthma. Methods Eighty-seven children diagnosed with mild or moderate asthma were enrolled as study group, 20 healthy children as control group. Sputum induction, cellular differential count, and the assaying of mediators in sputum supernatant were performed before treatment with corticosteroids. Eosinophil cationic protein were measured by enzyme-linked fluorescent assay, interleukin-8 and transforming growth factor-β_1 (TGF-β_1) were measured by enzyme linked immunosorbent assay. Pulmonary function tests were performed for small airway function on the baseline and methacholine bronchial provocation tests were performed to screen airway hyperresponsiveness. SPSS13.0 software was used to analyze the data. Results (1) Among the 87 patients, 64 patients were studied as eosinophil asthma (EA) group, 23 patients as non-eosinophil asthma (NEA) group according to the ratio of eosinophils in sputum. The percentages of inflammatory cells and level of ECP, IL-8 were of significant difference between the two groups (P < 0.05), other indexes as FEV_1% pred, PEF% pred, moderate-severe AHR%. small airway function were also of significant difference between the two groups (P < 0.05). (2) Patients in EA group showed significant improvement in pulmonary function, bronchial hyperresponsiveness and small airway function after treatment with inhaled corticosteroids compared with NEA group. (3) Multiple stepwise regression analysis showed that among the different baseline variants considered only baseline FEVl%pred, sputum eosinophil percentages (EOS%), sputum TGF-β_1 significantly correlated with the response to inhaled cortieosteroids, moreover, sputum eosinophil percentages had the closest correlation (β= 0.583, t = 6.214, P < 0.05). Conclusions There were different patterns of airways inflammation in children with mild or moderate asthma. Sputum eosinophilia was associated with asthma status. Low sputum eosinophils, low FEV_1%pred, high sputum TGF-β_1 before treatment predict poor response to treatment with inhaled corticosteroids. Evaluation of those baseline indexes may be helpful to an individualized therapeutic regime.
