2.Analysis of 251 cases entered the procedure of medical malpractice identification

Objective To analyze the causes of medical disputes and to find an effective way to reduce medical malpractice and medical dispute.Methods The complete identification files of 251 cases of medical malpractice obtained from corresponding Offices of Technical Identification of Medical Malpractice in three cities at prefecture level in Shandong province were analyzed to compare the cause of medical malpractice identified by patients with that identified by experts.Results The main causes that identified by patients included violation(36.41%),default(35.60%),illegality(13.05%),tort(11.89%),and absence of management(3.05%).Among the litigation claims,58.53%were identified as existed factors causing disputes.Among the identified factors,72.66%should have been prevented and controlled.Conclusion Medical malpractice and medical dispute can be effectively prevented and controlled by putting management site forward,ensuring the implemention of the policies,and strengthening the substantive responsibility of the leadership.
3.Survey of the standardization of medical records writing by physicians

Objective To study the standardization of medical records writing by residents in hospitals. Methods With both quantitative and qualitative analysis, medical records made by residents were sampled in groups to study their standardization. Results Recognition rate of such medical records was 82. 55%, and the excellence rate was 24. 50% in the grade evaluation. For senior residents, their rate of writing excellence falls below those of junior ones; whether the evaluators have medical background bears no significant difference for grade evaluation of medical records writing, yet a significant difference was found with the recognition rate. Conclusion Writing of medical records by residents is found with incompliance now and then, and the recognition rate ought to be improved.
4.ATRA combined with IFN? to induce differentiation and growth inhibition of NB cells.

Objective To study the effect of ATRA and IFN? on growth and differentiation of SH-SY5Y cell line and the possible molecular mechanisms.Methods The expression of TrkA mRNA was detected by RT-PCR.Trypan blue exclusion was used to detect the antiproliferative effect of ATRA,IFN?and ATRA +IFN? on SY5Y cells.Morphologic changes were observed using phase-contrast microscopy.Results TrkA mRNA was upregulated by ATRA and IFN?.ATRA and IFN? could induce morphological differentiation and inhibit growth of SY5Y cells.The combination of ATRA+IFN? enhanced the induced differentiation and growth inhibition of SY5Y cells.Conclusion Synergistic effects of ATRA and IFN? on growth inhibition and inducing differentiation in SY5Y cells are observed and this may result from upregulating the expression of TrkA mRNA.
5.Adaptability of Dean's criteria of dental fluorosis for rural residents in eastern Guangdong Province

Objectives To describe the esthetic acceptance to dental fluorosis in the rural residents of eastern Guangdong Province and to explore whether the Dean's criteria is still suitable for modern society. Methods A survey was conducted in eastern areas of Guangdong Province in 1 237 cases of 9~17 and 35~44 years old rural residents who drank well water containing 0.1~2.5 mg/L of fluoride. Clinical examination and questionnaire investigation were performed to record dental fluorosis and the severity of perceived appearance of their teeth. Results While the study subjects perceived the appearance of their teeth as " no problem", the community fluorosis index (CFI) was 0.5 and prevalence rate of dental fluorosis was 10.1 %. While perceived "a little bit bad" and "bad" and "very bad", the CFI and prevalence rate would be 0.9 and 33.7 %, 1.3 and 75.8 % and 1.7 and 98.2 % respectively. Conclusion The acceptance to the severity was basically in accordance with Dean's criteria. The Dean's criteria about dental fluorosis is still suitable for rural areas of eastern Guangdong Province and therefore it can be used in setting criteria of fluoride concentration in drinking water in these areas.

7.Small Interfering RNA of Cystathionineβ-Synthase/Cystathionineγ-Lyase-Derived Hydrogen Sulfide Synthesis Induces Apoptosis of Rat Hepatic BRL Cell Line

Objective To explore the inhibitory effects of endogenous hydrogen sulfide, a novel and important gas-eous transmitter generated in mammalian tissues mainly by cystathionine β-synthase (CBS) or cystathionineγ-lyase (CSE) on the apoptosis of the rat hepatic BRL cell line in physiological condition. Methods BRL cells were cultured, and divid-ed randomly into several groups in different phases of the experiment, including negative-siRNA (control) group, CBS siRNA (CBS 1~3) group and CSE siRNA (CSE 1~3) group, which were used to select the most efficient sequences of siRNAs at 48 or 24-hour transfection. Solution group and (CBS+CSE) siRNA group were added to detect the variation of apoptosis. The BRL cell line was observed and evaluated at 0, 4, 8, 12, and 24 hrs after siRNA transfection. When the mechanisms of the apoptosis were detected, CBS/CSE siRNAs were transfected individually or jointly into BRL cells, and compared with nega-tive-siRNA group to examine the variation. The genic and protein expression of CBS/CSE were detected by RT-PCR and Western blot assay. After transfection of CBS/CSE siRNA, the apoptosis of BRL cells was detected by Hoechst stain and flow cytometry (FCM). The mitochondrial membrane potential (MMP) changes were observed by fluorescent staining. Western blot assay was used to examine the protein expression of intracytoplasm cytochrome C (Cyt C) and cleaved-caspase 3. Re-sults CBS and CSE were observed in BRL cells. After transfection of CBS/CSE siRNA, endogenous H2S generation de-creased and the apoptosis of BRL cells increased. Accordingly, the expression of intracytoplasm-Cyt C and cleaved-caspase 3 increased. Conclusion The inhibition of endogenous H2S synthesis induced the apoptosis of BRL cells under physiologi-cal condition, which may be involved in mitochondrial pathway of apoptosis.
8.Application of body strait jacket design in mental patients

Objective To explore the application effect of self-developed strait jacket in mental patients.Methods A total of 200 mental patients meeting indications for protective restraint were selected from a tertiary first-class psychiatric hospital using convenience sampling,and were restrained using self-developed strait jackets,application effects and numbers of adverse events occurring during restraint were observed;patients' satisfaction with restraint tools was investigated as well.Results Cases of adverse events occurring during protective restraint were:3 patients developed dysaemia (1.50%),3 had self-injury (1.50%),4 were injured by others (2.00%),5 developed skin scratching (2.50%),7 threw off restraint by themselves (3.50%),13 presented violent and aggressive behaviors (6.50%),and no patient developed swelling of limbs.According to satisfaction survey,71.00% of the patients demonstrated acceptance of this restraint tool,76.50% felt comfortable,83.50% held that this strait jacket could meet the needs of free activities,76.00% felt safe,and satisfaction to body strait jacket came to 83.00%.Conclusion The manufacture of body strait jacket is simple and it is comfortable to wear,patient's compliance is high,and restraint effect is satisfactory;this body strait jacket reduces the incidence of adverse events,and improve patient's satisfaction to restraint tool.
9.Cytokine levels and the discrepancy of dexamethasone intervening in ALI rats induced by hydrochloric acid and lipopolysaccharide

AIM: To explore the cytokines level and the discrepancy of reaction to dexamethasone (Dex) in ALI rats induced by hydrochloric acid (HCl) and lipopolysaccharide (LPS). METHODS: Ninety-six SD rats were divided into six groups at random (n=16 in each group): NS group, HCl group, LPS group, NS+Dex group, HCl+Dex group and LPS+Dex group. Every group was divided into two subgroups: the bronchoalveolar lavage (BAL) subgroup and no bronchoalveolar lavage (NBAL) subgroup. The total leukocytes, PMN%, macrophage%, lymphocyte%, total protein in BALF and the wet/dry of the lung weight were measured. The concentrations of TNF-?, IL-1?, IL-4 and IL-10 in serum and BALF in every group were compared. 10.Study on neuroendocrine change in patients with ST segment elevation in acute myocardial infarction using thrombectomy

Objective To study neuroendocrine change and clinical value of percutaneous thrombectomy system(GuardWire PlusTM)in treatment of patients with ST-elevation acute myocardial infarction(STEMI). Methods 72 patients with STEMI underwent percutaneous coronary intervention(PCI)were divided into A group(38 patients)with direct stent placement after thrombectomy and B group(34 patients) with primary PCI. The plasma levels of ET, PRA, ALD, AngⅡ, NE, E were measured on the day of operation and the first, second, third and fifth days after PCI. Left ventricular ejection fraction(LVEF) was measured by echocardiography at one week and three months after PCI. Results The stents were successfully implanted in two groups. All the neuroendocrine factors have no difference between the two groups before operation. The first and second day after PCI, the levels of ET, PRA, ALD, AngⅡand E were significantly lower in A gronp than those in B group(P 0.05). Conclusions Deteriorated neuroendocrine changes are significantly improved with thrombectomy, providing potential benefits on heart function.
