1.Signal transduction mechanism of antidepressant action

The mechanisms of antidepressants are still unclear. There a re two classical theories on monoamine neurotransmitter or on neurotransmitter rec eptors, but both of them can not fully explain the delayed therapeutic action of antidepressants. Recently, many researches have focused on the postreceptor int racellular signal transduction as the mechanism of antidepressant action. G protein is the molecular basis of antidepressants. Neurotransmitter receptors and G protein are the two sectors of their therapeut ic action. They will ultimately influence intracellular signal transduction and result in relative effects such as phosphoration, the induction of neurotrophic factors and neurogenesis. This mechanism suggests a reasonable explanation for t he clinical delaying of antidepressants and it will do great help for the develo pment of antidepressants. It makes the design of novel, safe and more efficaciou s antidepressants possible and provides significant information for the elucidat ion of biology of depression.
2.EpCam and Wnt / β-catenin pathway expression in colon cancer

Objective EpCam and Wnt/ β-catenin pathway in colon carcinoma and its clinic-pathological significance of the distribution,studying the relationship between EpCam and Wnt / β-catenin.Methods Retrospective analysis detected by immunohistochemistry 60 cases of colon cancer,20 cases of adjacent atypical hyperplasia,60 normal colon tissue EpCam and Wnt / β-catenin protein expression.Results (1)normal colon tissue,cancer tissue and cancer tissue showed positive expression EpCam clear upward trend,were 23.5%,62.3%,96.5% ; with normal colonic mucosa to cancer transformation,β-catenin in the membrane expression of the positive rate decreased,while the cytoplasm is followed by increased expression rate in poorly differentiated carcinoma or even nuclear expression,EpCam strong positive expression of Wnt / β-catenin cytoplasmic-positive rate of histological type,depth of invasion,lymph node metastasis; (2)the EpCam with the Wnt / β-catenin expression showed a positive correlation (r =0.653,P <0.05) ;(3)high expression EpCam and Wnt / β-catenin in patients with colon significant increase in cancer recurrence rate and 5-year survival rate was significantly reduced.Conclusion EpCam and Wnt / β-catenin pathway in colon cancer positively is correlated,EpCam and Wnt / β-catenin is connected with high expression and tumor invasion,metastasis and prognosis.
3.Clinical application of ultrasound-guided femoral nerve and lateral femoral cutaneous nerve block for patients undergoing hip fracture surgery

Objective To observe the application of ultrasound-guided femoral nerve block (FNB) and lateral femoral cutaneous nerve block (LFCNB) for patients undergoing hip fracture surgery.Methods 60 patients scheduled for hip fracture surgery undergoing LMA general anesthesia were randomly divided into 3 groups,20 cases in each group.Before transfer patients from bed to operating table,A group received dezocine 5mg iv,B group received fascia iliaca compartment block(FICB),C group received FNB combined with LFCNB.40mL of 0.375% ropivacaine was injected guiding by ultrasound in B group and C group.The time of sufficient sensory block and awake,the dosage of propofol and remifentanil,MAP and HR at pre-block (T1),20min after block (T2),transfer bed (T3),LMA insert (T4),skin incision(T5),LMA remove(T6) and sober(T7) were recorded.Pain was assessed using visual analogue scale(VAS) pre-and post block.The incidence of using vasoactive drugs,agitation,pain and adverse reaction were also recorded.Results The time of sufficient sensory block and awake,the dosage of propofol and remifentanil in A,B and C groups were as following:A group (not measured),(20.3 ± 1.3) min,(835 ± 6.7) mg,(1 285 ± 18) μg;B group (i2.2 ±2.7)min,(13.3 ± 1.4)min,(610 ±9.9)mg,(835 ± 15) μg;C group (9.7 ± 2.4)min,(12.8 ± 1.5) min,(555 ± 6.5) mg,(785 ± 16) μg.The time of awake,the dosage of propofol and remifentanil in B group and C group were significantly lower than those in A group(F =2.62,2.41,2.45,all P < 0.05).The time of sufficient sensory block in C group was lower than that in B group (p < 0.05).The MAP and HR at T2,T3,T5 and T7 in A,B and C groups were:A group (115 ± 4) mmHg,(90 ± 8) beats/min,(135 ± 6) mmHg,(98 ± 8) beats/min,(104 ±6) mmHg,(87 ± 4) beats/min,(120 ± 5) mmHg,(88 ± 8) beats/min;B group (102 ± 3) mmHg,(81 ± 6) beats/min,(112 ± 5)mmHg,(82 ± 8)beats/min,(89 ±6) mmHg,(72 ± 3) beats/min,(100 ±6)mmHg,(76 ± 8) beats/min;Cgroup (100 ± 3) mmHg,(80 x 6) beats/min,(109 ± 6) mmHg,(83 ± 5) beats/min,(86 ± 5) mmHg,(70 ± 3) beats/min,(99 ± 5) mmHg,(75 ± 5) beats/min.The levels of MAP and HR in B group and C group were significantly lower thanthose in A group(F =2.25,2.85,2.87,2.91,all P < 0.05).The VAS scores at T2,T3,and T7in A,B and C groupswere:A group (3.9 ± 0.7) points,(8.2 ± 0.3) points,(6.0 ± 0.8) points;B group (2.3 ± 0.4) points,(4.1 ±0.4) points,(2.2 ± 0.7) points;C group (2.1 ± 0.5) points,(2.4 ± 0.4) points,(1.2 ± 0.4) points.The VAS scoresin B group and C group were significantly lower than those in A group (2.36,2.82,2.88,all P < 0.05).The VASscores at transfer bed and sober in C group were significantly lower than those in B group (F =2.32,2.38,all P <0.05).The incidence of using ephedrine/atropine,urapidil/esmolol,PONV,agitation,pain and incision pain in A,Band C groups were:A group 30%,30%,25%,25%,40%;B group 10%,10%,0%,0%,10%;C group 10%,5%,0%,0%,0%.The number of patients who required vasoactive drugs and adverse reaction in B group and C group were significantly lower than those in A group(x2 =7.58,8.81,9.11,9.11,8.89,all P <0.05).The incidence of incision pain at sober in C group was lower than that in B group(x2 =9.21,P < 0.05).Conclusion The ultrasound -guided FNB and LFCNB can obviously shorten the onset time,reduce the dosage of general anaesthetic and maintain the stability of henodynamics during the perioperative period.It has effective analgesia during transfer of patients from bed to operating table and sober.
4.Influence of neo-adjuvant chemotherapy on progrsssive gastric cancer operation withi multi-disciplinary team

Objective To discuss the influence of the progress and result in gastic cancer operation through application of neo-adjuvant chemotherapy in multi-disciplinary team (MDT).Methods Clinical data of the patients treated in MDT model 45 cases and non-MDT model(78 cases) were respectively analyzed,and the index about pathologic change,histologic transform and operative result between the two groups were compared.Results In the index of pathologic change,the incidence of abdominal adherence and ascites and pyloric obstruction in MDT model group were obviously less than non-MDT model group( P <0.05 ).In the index of histologic transform,texture of mesentery in MDT model group was more fragile than non-MDT model group( P <0.05).In the index of operative results,the operative duration and intra-operative bleeding in MDT model group were less than non-MDT model group (P <0.05).Conclusion Neo-adjuvant chemotherapy has certain influence in gastric caneer operation,but the successful operations were performed by standard and correct procedures.Therefore,optimizing combined therapy in MDT model and constructing preoperative evaluation system with neo-adjuvant chemotherapy.
5.Expression and clinical significance of EpCAM and β-catenin pathway in colon cancer

Objective To investigate the expressions and clinical pathological significance of EpCAM and the β-catenin pathway in colon cancer,and the correlation between EpCAM andβ-catenin.Methods Immunohistochemistry was used to detecte the expressions of EpCAM and β-catenin proteins in colon cancer tissues and corresponding adjacent tissues of 70 cases of colon cancer patients,the clinical and pathological features of colon cancer and their relationship were retrospective analyzed.Results ①EpCAM and β-catenin protein expressions in colon cancer tissue was 52 cases (74.3%) and 55 (78.6%) positive respectively; EpCAM and β-catenin proteins in cancer adjacent tissues in 13 cases( 15.7% ) and 9 cases ( 12.9% ) were positive,the differences were statistically significant.②the EpCAM expression is positively correlated with the β-catenin expression ( r =0.616,P < 0.01 ).③The expressions of EpCAM and β-catenin correlated with the tumor differentiation,invasion depth,lymph node metastasis and TNM staging related ( P < 0.05).Conclusion EpCAM and β-catenin pathways in colon cancer positively correlate,and closely correlate with colon cancer.The expression level of EpCAM and β-catenin can be used as a reference for the colon disease course and healing.
6. Evaluation on the differences of wrist triangular fibrocartilage complex with T2 mapping in healthy adults

Objective: To explore the feasibility of evaluation on the differences of wrist triangular fibrocartilage complex (TFCC) with T2 mapping in healthy adults. Methods: Totally 81 healthy volunteers underwent T2 mapping of wrist joint. Among them 30 subjects underwent bilateral wrist scanning, and the images were divided into handedness group and non-handedness group according to the handedness questionnaire. All images were postprocessed to obtain T2 mapping pseudo-color images. T2 values of radial cartilage, triangular fibrocartilage disc proper (TFC), fibrous vascular tissue, meniscus and TFC's ulnar attachment were measured. The differences of T2 values in different areas of wrist cartilage were compared between different genders and handedness group and non-handedness group. Results: There were statistically differences of T2 values in different regions of wrist cartilage (F=32.235, P<0.01). There was no statistically difference of T2 values of fibrous vascular tissue ([34.22±6.85]ms) and TFC's ulnar attachment ([32.57±7.23]ms), nor T2 values of radial cartilage ([41.37±7.04]ms) and meniscus ([39.26±8.88]ms) (all P>0.05), while there were statistically differences of T2 values of the remaining areas of carpal cartilage (all P<0.05). There was no significant difference of T2 values of wrist TFCC between different genders nor handedness group and non-handedness group (all P<0.05). Conclusion: T2 mapping can visually display the morphology of wrist TFCC and quantitatively evaluate changes of T2 values in different regions of TFCC, which are not affected by gender and handedness.
7.Simulation-based training in neonatal resuscitation program for residents

Objective To examine the effectiveness of simulation newborn simulator in neona-tal resuscitation training for pediatric residents. Methods From June 2011 to June 2012, 11 residents working in neonatal ward of the Third Hospital of Peking University were enrolled into the study. Eval-uation on the residents was made before the training. Training of simulated teaching using simulation newborn simulator was conducted and evaluation was made after the training. SPSS 18.0 was used for statistical analysis. Comparison was made between pre- and post-training test by paired t test. P<0.05 was considered statistical significant. Questionnaire survey was conduct to acquire residents' feedback. Results A total of 11 participants completed the training and finished the questionnaire. The score of pre-training was 37.82±1.17 versus that of post-training 39.18±0.87(t=4.89, P<0.01). All residents were satisfied with the simulation-based training. Conclusion Simulation training can improve pedi-atric residents' knowledge and skills in neonatal resuscitation.
8.Preparation and in vitro/in vivo pharmacological studies of bFGF/collagen composite sponge

Aim: To investigate a new type wound dressing,basic fibroblast growth factor(bFGF)/collagen com-posite sponge,and conduct its pharmacological studies in vitro and in vivo.Methods: bFGF/collagen composite sponge was prepared using fresh pig skin and bFGF.The sponge's physicochemical properties were studied.MTT assay was used to detect the proliferation effect of the sponge extract on 3T3 cells.Delayed allergy of the sponge was tested for the assurance of its biosafety.Results: Results showed that the physicochemical properties of bFGF/collagen composite sponge with high and low doses of bFGF have no significant difference from those of blank collagen sponge.SDS-page analysis indicated that the composite sponge has apparent strip in 18 kD.It was also found that bFGF/collagen composite sponge was responsible for significant effects on 3T3 cell proliferation in comparison to saline treatement(P <0.01,P <0.05).In the allergy study,during the periods of the induction and stimulation,no allergic reaction was found in bFGF/collagen composite sponge groups with high and low doses of bFGF,while severe reactions and inflammation occurred in positive group(2,4-dinitrochlorobenzene).Furthermore,pathological examination indicated the intact dermal structure and no sign of inflammation.Conclu-sion: The developed sponge has good physicochemical propertis and noticed cellular proliferation without dermal irritation.There is much potential to develop bFGF/collagen composite sponge into a new kind of wound dressing material for clinical use.
9.Cytocompatibility of nano-hydroxyapatite/bacterial cellulose nanocomposites and its residues

BACKGROUND: The nano-hydroxyapatite/bacterial cellulose (nHAP/BC) nanocomposites has a good prospect of application in bone tissue engineering, and the bone tissue engineered materials and its degradation products Should have excellent compatibility. This study further assessed DAN synthesis cycle using flow cytometry on the basis of evaluating cell compatibility by metabolic 3-(4, 5-dim ethylthiazo 1-2-y 1) -2, 5-Dipheny 1-2H-tetrazolium (MTT) assay. OBJECTIVE: To evaluate the cytocompatibility of a new-pattern nHAP/BC nanocomposites and its residues. METHODS: Effects of nHAP/BC nanocomposites and its residues on morphclogicel changes in osteoblasts were observed using in vitro cell culture method. Effects of nHAP/BC nanocomposites and its residues on osteoblast growth and prclifera'don were evaluated by MTT assay. Cell cycle phase changes were detected using flow cytometry to evaluate matsdal effects on cell proliferation on molecular levels. RESULTS AND CONCLUSION: The nHAP/BC nanocomposites and its residues had neither remarkable effects on cell morphology, nor significant inhibition on osteoblast growth and proliferation. Test of MTT cytotoxicity showed that the average cell proliferation rate was over 80% after treated with the material and its residues, with the cytotoxity grade of 1 (non-toxic). Flow cytometry indicated that the rate of G_0/G_1 was reduced, and the rates of S, G2/M were increased, and the synthesis of DNA was increased, the cellular growth and repair in osteoblasts was accelerated. These indicated that nHAP/BC nanocomposites have good cytocompatibility, and it will be safe and prospected scaffolds in bone tissue engineering.
10.Construction of vector SM22α-PAC-IRES2-EGFP used for purification of smooth muscle cells and its expression in mouse embryonic stem cells

BACKGROUND:Embryonic stem cells (ESCs) serve as a major cell source for smooth muscle cells,but the heterogeneity of cells derived from ESCs result in difficulty to obtain high purity smooth muscle cells.OBJECTIVE:To construct a double expression vector of puromycin resistance (pac) gene and enhanced green fluorescence protein (EGFP) gene driven by smooth muscle specific SM22α promoter (pSM22α-PAC-IRES2-EGFP),in addition,to detect its availability and specificity in ESCs.DESIGN,TIME AND SETTING:The observational experiment of gene level was performed at the Cardiovascular Institute,General Hospital of Shenyang Military Region from April 2007 to September 2008.MATERIALS:ESCs line R1 with number SCRC-1011TM was purchased from American ATCC Company.The pSM22α-EGFP vector was constructed by our laboratory.And the pIRES2-EGFP,pSM2C and pSuper.basic vectors were purchased from Invitrogen Company.METHODS:SM22α promoter was cloned from pSM22α-EGFP by polymerase chain reaction.CMV promoter of pIRES2-EGFP vector was replaced by SM22 promoter to establish pSM22α-IRES2-EGFP.Pac gene,excised from pSM2C by HindⅢ/Clal digestion,was sub-cloned into pSuper.basic to establish pSuper-PAC.After BgⅢ/Accl enzyme digestion of pSuper-PAC,pac gene fragment was obtained,which was further sub-cloned into pSM22α-IRES2-EGFP to produce pSM22α-PAC-IRES2-EGFP.ESCs were transfected with pSM22α-PAC-IRES2-EGFP using lipofectamine.Positive clones were selected by G418 and induced to differentiate and further identified by amplification of pac gene by RT-PCR.Differentiated cells were immunostained by SM α-actin,and expression of SM α-actin and EGFP was observed simultaneously under fluorescence microscope.MAIN OUTCOME MEASURES:Sequencing result of pSM22α-PAC-IRES2-EGFP;Amplification of pac gene;EGFP expression;as well as SM α-actin immunostaining.RESULTS:Three segments of 261 bp,664 bp,and 5000 bp were obtained by HindⅢ/Clal digestion,which was coincident with expectation,and the sequencing results showed that pSM22α-PAC-IRES2-EGFP vector was successfully constructed.Amplification of pac gene identified 4 ESCs clones successfully transfected.After induction of differentiation,partial portion of differentiated cells expressed EGFP,accompanied by positively stained by SM α-actin antibody.CONCLUSION:pSM22α-PAC-IRES2-EGFP vector was successfully constructed.ESCs clones transfected with this vector expressed pac gene and EGFP gene,and the expression of EGFP is smooth muscle specific.
