1.Application of glucose descriptive chart in monitoring of stress hyperglycemia

ObjectiveTo study the application of glucose descriptive chart in monitoring of stress hyperglycemia,and provide continuous,dynamic change of blood glucose levels for clinic. Methods80 patients with stress hyperglycemia who adopted intensive insulin therapy were randomly divided into the experimental group and the control group with 40 cases in each group.The experimental group used glucose descriptive charts to record blool glucose,the control group used the traditional registration form to record blood glucose.The incidence of hypoglycemia and evaluation of medical staff for the glucose descriptive chart were compared between two groups. ResultsThe incidence of hypoglycemia in the experimental group was significantly lower than the control group.The medical staff considered that glucose descriptive chart helped understand the dynamic change of blood glucose,and provided a more comprehensive basis for regulating the use of insulin. ConclusionsApplication of glucose descriptive chart in monitoring of stress hyperglycemia can provide continuous and dynamic change of blood glucose,which facilitates the medical staff to monitor the blood glucose and decrease the incidence of hypoglycemia.
2.Nursing Care of the Multisite Cardiac Pacemaker Implantation

Objective:To study the nursing care of multisite cardiac pacemaker implantation in treat cardiac arrhythmia and dilated cardiomyopathy.Methods:On the different basis of partient health problems,gave nursing care by apply nursing process.Carry out health edurcation in different period,so that improve the effect of health education and prevent the electrode break off.Results:(1)Au the pacemaker electrodes were at the good place,no broke off or moved away.The sense and the threshold were normal.(2)The patients' cardic function were improved and cardiac arrhythmia were disappear or improved.(3)The wound had no infection.(4)All the patients knew self-supervise and attention knowledge well.Conclusion:Applying nursing proless,keeping right position,keeping intensive observation and giving effective health education are the assurance for the mplantation.
3.Clinical epidemiological characteristics of neonatal respiratory failure in Children's Hospital of Hebei Province

Objective To investigate the incidence,treatment and prognosis of neonatal respiratory failure (NRF) in the Neonatal Intensive Care Unit of Children's Hospital of Hebei Province.Methods The medical data of 626 NRF newborns admitted to the Neonatal Intensive Care Unit of Children's Hospital of Hebei Province from January 1st,2008 to December 31st,2008 were collected,and the pathogenesis of NRF,management,risk factors of their deaths and cost for the disease were studied.The results were compared with the national study in the same period and the local study of 2004-2005 by Chi-square test.Results During the 12 consecutive months,the incidence of NRF in neonatal intensive care unit was 38.9% (626/1608).The mortality in hospital was 22.5% (141/626),among which,most deaths (134,95.0%) occured after early discharge required by the parents.The mortality decreased by 14.7% compared with that during 2004 2005 (37.2%,113/304),and was also lower than that (24.7%,1683/6864) in the national study at the same period.Pneumonia/sepsis ( 34.8 %,218/626 ),neonatal respiratory distress syndrome ( 31.6 %,198/626 ),and meconium aspiration syndrome (10.7%,67/626) were leading primary causes of NRF.The major complications were pneumonia/sepsis (5.4 %,34/626),intracranial hemorrhage (4.6 %,29/626) and persistent pulmonary hypertension (3.2%,20/626 ).The percentage of NRF babies requiring pulmonary surfactant rose from 14.1% (43/304) during 2004-2005 to 23.6% (149/626) in 2008,which almost reached the national level of 26.8% (1840/6864).Pulmonary surfactant treatment was accepted by 70.2% (139/198) patients with neonatal respiratory distress syndrome.12.1% (24/198)of neonatal respiratory distress syndrome infant was administered immediate intubation-surfactant-extubation.Seventeen (2.7%) patients accepted nitric oxygen treatment.Nasal continuous positive airway pressure was given to 76.5%(479/626) patients,which was higher than before (47.1%,143/304).Conventional mechanical ventilation rate decreased from 72.7% (221/304) to 49.8% (312/626),and high frequency oscillatory rate increased from 0.7% (2/304) to 10.5% (66/626),which were similar to the national level.Of the 480 patients discharged after recovery,the mean time of hospital stay was (15.1 ± 4.0) days,and the mean medical cost was (12752 ± 5148) CNY.Conclusions The patient number increases and the medical skills and treatment methods of our hospital improve a lot.Utilization of pulmonary surfactant,nasal continuous positive airway pressure and high frequency oscillatory ventilation increase significantly.However,the burden of NRF is still heavy.
4.Effect of High Frequency Electric Therapy Combined with Movement Training on Osteoarthritis of Knee

Objective To investigate therapeutic efficiency of high frequency electric therapy combined with movement training on osteoarthritis of knee joint.Methods Eighty-five patients were randomly divided into the high frequency electric therapy group (22 cases including 31 knees), movement training group (33 cases including 45 knees) and high frequency electric therapy combined with movement training group (30 cases including 40 knees). All of the patients were assessed with Visual Analogous Scale (VAS) and Lysholm Knee Score Scale (LKSS) before and after treatment.Results Every group had significant improvements compared with selves after treatment. There were significant differences among the three groups (P<0.01).Conclusion Three ways have different therapeutic effect on osteoarthritis of knee, the high frequency electric therapy can slacken the soft tissue, the movement training has reliable effect in the near future; the therapy by high frequency electric therapy combined with movement training is reliable.
5.Role of Svcam-1 in the pathogenesis of intrahepatic cholestasis of pregnancy

Objoctive To investigate circulating leveh of soluble vascular cell adhesion molecule-1(sVCAM-1)in the peripheral of intrahepatic cholestasis of pregnancy(ICP).To reveal the pathogenesis of ICP.Methods Maternal scram sVCAM-1 levels were determined in 49 women which 15 eases were normal pregnant women and 34 with ICP.The latter were composed of 14 women with mild,20 with severe ICP.sVCAM-1 was measured by ELISA.Results The levels of serum sVCAM-1 was significantly elevated in women with mild and severe compared with control subjects(P<0.01),and markedly decreased to normal 5 days after dilively(P>0.05).The sVCAM-1 level were negatively correlated with weight of neonatal(r=-0.643,P<0.01).Conclusion Elevated levels of sVCAM-1 indicate that a primary endothelial cell dysfunction play an important role in the ICP.And it influences the neonatal.

7.Reliability of 36-item Short Form Health Survey (Chinese Version) in Assessing the Quality of Life in Patients with Knee Osteoarthritis

ObjectiveTo determine whether the Chinese version of 36-item Short Form Health Survey (SF-36) can be taken as an effective assessment to evaluate the quality of life in patients with knee osteoarthritis. Methods30 patients with knee osteoarthritis were tested twice in 3 d with the Chinese version SF-36 by two standard-trained testers. The Cronbach's α coefficient and intraclass correlation coefficient (ICC) were investigated. ResultsThe ICC of total scores and parts scores in the Chinese version SF-36 were 0.54~0.91, while the Cronbach's α coefficients were 0.70~0.95. ConclusionThe Chinese version SF-36 is reliable in evaluating the quality of life in patients with knee osteoarthritis.
8.Effect of Guishen Pain-Killer on easing pain in micro-wave heating among tissue to treat hemorrhoids

ObjectiveTo observe the easing pain effect of being used Guishen Pain-Killer, a Chinese medicine, for improving the anaesthesia project which was used in using micro-wave heating among tissue to treat hemorrhoid.MethodsOn the basic of the original anaesthesia project, patients drunk the Chinese medicine Guishen Pain-Killer 50ml which was heated to 40℃ by microwave stove for pre-anaesthesia drug 10 minutes before the operation. The effect of adding drinking Guishen Pain-Killer and of merely using the original anaesthesia project were estimated with the McGill Questionnaire and stated the data with u test.ResultsThe pain estimation of those patients who drunk Guishen Pain-Killer before operation was (1.71±1.05) and (1.32±0.83), in operation and just after operation,while that of patients who didn't drink Guishen Pain-Killer before operation was (2.58±1.03) and (1.93±1.11),P<0.01.ConclusionGuishen Pain-Killer can be used as the pre-anaesthesia drug in micro-wave heating among tissue to treat hemorrhoids.
9.Hydroelectric bath therapy of low frequency current modulated by medium frequency applied to treat patients with sequela of sprain of ankle

ObjectiveTo observe the effect of low frequency current hydroelectric bath therapy modulated by medium frequency on sequela of sprain of ankle. Methods46 patients with sequela of sprain of ankle were treated with ZM C intellectual faculties intermediate frequency therapeutics instrument. Medium frequency was modulated with full square wave. The frequency of carrying wave was 4KHz. The frequency of modulated wave was 100Hz. The depth of modulation was 80%. The water temperature was 40℃.ResultsAfter treatment, all patients got satisfactory effect and shorter treating course without adverse reactions appearing. Conclusions The low frequency current hydroelectric bath therapy modulated by medium frequency is a simple, safe and better treating method for sequela of sprain of ankle.
10.Therapeutic Effect of Warm Magnetic Field Combined with Medium Frequency Electric Current on Lumbar Intervertebral Disc Protrusion of Old Patients

Objective To observe the therapeutic effect of warm magnetic field combined with medium frequency electric current on lumbar intervertebral disc protrusion of old patients.Methods By retrospective analysis, 120 old patients with lumbar intervertebral disc protrusion were divided into the therapeutical group (treated with warm magnetic field combined with medium frequency electric current) and control group (treated with ultrashort wave combined with medium frequency electric current), and therapeutic effects of two groups were compared.Results There was no significant difference between the therapeutic effects of two groups ( P>0.05).Conclusion Warm magnetic field combined with medium frequency electric current is same effect as ultrashort wave combined with medium frequency electric current on lumbar intervertebral disc protrusion of old patients.
