OBJECTIVE: To evaluate the clinical outcomes of medical supporting bone graft following posterior approach and bone cement implantation in the hip joint in treating intertrochanteric fracture.METHODS: A computer-based online search of Science Direct, Ei databases was performed for English articles published between January 1960 and October 2009, with the key words "bone cement, intertrochanteric fracture". In addition, CNKI and CBM were searched for related Chinese articles published between January 1994 and October 2009, with the key words "intertrochanteric fracture, coxa vara, posterior approach of hip joint, bone cement implantation in major screw hole". Moreover, related books were manually searched. Treatment of intertrochanteric fracture, basic and clinical experiment of intertrochanteric fracture treated by bone cement was included.RESULTS: Intertrochanteric fracture treatment includes lateral or anterior approach for dynamic hip plate system, dynamic hip screw, and femoral proximal intramedullary screw internal fixation. Dynamic hip screw is standard internal fixation to treat intertrochanteric fracture, with strong anti-rotation function, and meets the biomechanical requirements. However, screw loosening, extraction and breakage frequently occur. Cancellous bone screw track enhanced by bone cement increases screw retention force, benefits screw compression, enhances bone-screw interface to transfer the stress to screw-bone regions, which significantly increases the anti-flexion and anti-torsion strength of dynamic hip screw and improves fracture stability. The mechanism involves the integral formation of cancellous bone, bone cement and screw by the micro-interlocking of bone trabecula and surrounding cancellous bone.CONCLUSION: Dynamic hip screw filled by bone cement significantly enhances the anti-flexion and anti-torsion strength of internal fixation and improves fracture stability.

Objective:To observe the efficacy of rosuvastatin combined with the conventional treatment in the patients with acute cerebral infarction (ACI) and the influence on the serum levels of MMp2, vasostatin-2 and sICAM-1.Methods:Totally 136 patients with ACI were randomly divided into the routine treatment group and the rosuvastatin group with 68 ones in each .The patients in the routine treatment group were treated with the conventional treatment , and those in the rosuvastatin group were treated with rosuvastatin additionally. The treatment course was 15 days. The efficacy, adverse drug reactions and the changes of MMp 2, vasostatin-2 and sI-CAM-1 in the two groups were compared .Results:The effective rate was 92.65%in rosuvastatin group and that was 77.94%in the routine treatment group, and the difference was statistically significant (P<0.05).After the treatment, the levels of MMp2, vasosta-tin-2 and sICAM-1 for the mild, medium and severe types in the two groups were significantly decreased when compared with those be -fore the treatment (P<0.05), and the decrease for the same type in rosuvastatin group was more notable than that in the routine treat -ment group (P<0.05).The indices for different subgroups with various infarcts in rosuvastatin group were all lower than those for the same subgroup in the routine treatment group (P<0.05).The incidence of adverse drug reactions (ADR) in two group was no signifi-cant difference (P>0.05).Conclusion:Rosuvastatin used in the patients with ACI has better clinical efficacy than the routine treat-ment, which can reduce the levels of MMp 2, vasostatin-2 and sICAM-1 and is worthy of clinical promotion .

With the popularization of rehabilitation and the development of the physiotherapy, some chronic diseases do not only mean the history of the patients, but also the objects of physiotherapy. The exercises will affect the cardiopulmonary function of the patient, and result in a high risk of accident. This article would review the risk factors and risk management in physiotherapy for chronic diseases.

objective To evaluate the clinical effect of touqing capsule associated with Fengchi on the vertebral artery type of cervical spondylosis(CSA)in the attack stage. Methods A total of 100 patients with CSA, according to the doctor order, were randomly divided into a control group and a treatment group respectively, with 50 patients in each. The control group was treated with acupuncture therapy, the treatment group was additionally treated with Touqingcapsule on the basis of the control group, and both groups were treated for 20 days. The hemodynamics was detected by transcranial doppler, the change of TCM syndrome integral was observed before and after the treatment, and the clinical efficacy was evaluated. Results After treatment, the treatment group dizziness (1.10 ± 0.33 vs. 2.01 ± 0.26,t=14.909), neck shoulder ache (0.87 ± 0.16 vs. 1.92 ± 0.69,t=10.266), numbness (0.95 ± 0.14)vs. 1.87 ± 0.16,t=29.844), the total integral (2.16 ± 0.75 vs.3.89 ± 1.14,t=8.756) integral were significantly lower than the control group(P<0.01).Treatment group RVA (40.03 ± 5.89 cm/svs. 34.26 ± 6.26 cm/s,t=4.628), the LVA (42.14 ± 5.42 cm/svs.33.89 ± 6.08 cm/s,t=6.985) rise and PI (0.79 ± 0.14 vs.1.04 ± 0.19,t=7.312) reduction were significantly better than those of the control group (P<0.01). The total effective rate in the treatment group was significantly better than the control group (91.7%(44/48)vs. 74.5%(35/47),χ2=3.863, P=3.863).Conclusion Touqing Capsule associated with Fengchi is effective for CSA. Its functional mechanism is correlated with improvement of haemodynamics of vertebral-basilar artery of the CSA patients.

Objective To evaluate the MRI findings and clinical features of methotrexate-induced acute encephalopathy in children. Methods The clinical data and brain MRI obtained in 13 children with methotrexate-induced acute encephalopathy were retrospectively reviewed. The MRI features were analyzed , including information on the location , the signal intensity and follow-up MRI study was performed. Results Of the 13 patients , 2 patients suffered from seizure.Five patients had dysphasia , of which 4 patients had evidence of hemiparesis , 1 patient had right facial palsy.Five patients had unilateral weakness. And left hemiparesis was observed in 1 patient.DWI revealed well demarcated asymmetrical hyperintensity lesions within the centrum semiovale and/or periventricular white matter in 10 patients, corresponding to areas of hypointensity on ADC maps. One case showed hyperintensity areas in the bilateral supratentorial cortex and subcortical white matter on T 2-weighted images with subtle high-intensity on DWI.In all 10 cases there were resolution of the diffusion abnormality , 8 cases displayed residual FLAIR signal abnormalities involving areas of previously seen diffusion restriction , 5 cases showed decreased range of the lesion , 1 case was progressive, and 2 cases were stable. One case with hyperintensity areas in the supratentorial cortex and subcortical white matter showed small residual hyperintensity on T 2-weighted images and resolution of the diffusion abnormality. Conclusions MTX-induced acute encephalopathy often manifests as stoke-like symptoms.DWI is the imaging modality of choice for the detection of acute MTX neurotoxicity , and asymmetrical restricted diffusion in the deep white matter is the characteristic sign.Cytotoxic edema induced by MTX is transient and reversible .

0.05),hints that the tenocyte's function was not disturbed. The DNA index of cells of GDBM group was 0.96 and 2.1% higher than the control group, indicating that the tenocytes grow and proliferate faster when being combine cultured on GDBM. Conclusion GDBM show good biocompatibility combined with tenocytes and they are promising extracellular matrix scaffold for cell transplantation in tendon tissue engineering.
