1.Pharmacokinetics study on characteristic ingredients of different-dose herbs of shaoyao-gancao decoction.

The paper is to report the observation of pharmacokinetic changes of the characteristic ingredients in the combinations of different-dose herbs of Shaoyao-Gancao decoction. After the establishment of HPLC analytical method of plasma effective constituents, rats were orally administered with different-dose herbs of Shaoyao-Gancao decoction. Blood samples at different times after administering these decoctions were collected, and then were analyzed by HPLC fingerprints technology. Pharmacokinetic parameters of characteristic peaks were analyzed by SPSS 15.0 software and DAS 2.0. At last, we looked for the correlation of those pharmacokinetic parameters and the dosage of Gancao. The best dose of Shaoyao-Gancao decoction was at the ratio of 4 to 4, which was consistent with the dose commonly used in ancient times. The absorption of characteristic peaks from Shaoyao-Gancao decoction was related with the dosage of Gancao, and there existed interaction between each characteristic ingredients. There existed the right dose-ratio of Shaoyao and Gancao to get the best effect. The absorptions of effective constitutents were mutual waxing and waning in order to increase biological effects together. It's demonstrated the compatibility connotation at a right dose-ratio of Shaoyao-Gancao decoction through the angle of pharmacokinetics.
2.Clinical study on pulmonary protective effect of large dose of ambroxol hydrochloride injection in postoperative patients with severe hypertensive intracerebral hemorrhage

Objective To study the pulmonary protective effect of large dose of ambroxol hydrochloride injection in postoperative patients with severe hypertensive intracerebral hemorrhage.Methods One hundred and sixty postoperative patients with hypertensive intracerebral hemorrhage were selected,and the patients were divided into conventional dose group and large dose group by random digits table method with 80 cases each.The therapeutic effect,partial pressure of oxygen,oxygenation index,acute physiology and chronic health evaluation Ⅱ score,antibiotic application time and hospitalization period in intensive care unit were compared between 2 groups.Results After treatment,the partial pressure of oxygen and oxygenation index in large dose group were significantly better than those in conventional dose group:(94.7 ± 7.2) mmHg (1 mmHg =0.133 kPa) vs.(86.5 ± 8.1) mmHg and (420.3 ± 35.5) mmHg vs.(356.0 ± 28.7) mmHg,the acute physiology and chronic health evaluation Ⅱ score was significantly lower than that in conventional dose group:(8.2 ± 3.4) scores vs.(11.6 ± 3.5) scores,the antibiotic application time and hospitalization period in intensive care unit were significantly shorter than those in conventional dose group:(8.5 ± 3.7) d vs.(13.4 ± 5.8) d and (7.3 ± 2.5) d vs.(9.7 ± 5.2) d,and there were statistical differences (P< 0.05).The total effective rate in large dose group was significantly higher than that in conventional dose group:92.5% (74/80) vs.83.8% (67/80),and there was statistical difference (P < 0.05).Conclusion The pulmonary protective effect of large dose of ambroxol hydrochloride injection is significant in postoperative patients with hypertensive intracerebral hemorrhage.
3.Slow synaptic transmission mediated by 5-HT_(1P) receptor in sympathetic ganalion of guinea pig

AIM To investigate the effects of several 5-hydroxytryptamine(5-HT) subtype receptor antagonists on late slow excitatory postsynaptic potential(LS-EPSP) of the neurons of guinea pig inferior mesenteric ganglion(IMG). METHODS Intracellular recordings were made from neurons of the isolated guinea pig IMG. RESULTS Cyproheptadine and BRL 24924 suppressed LS-EPSP of 5-HT sensitive neurons reversibly, while mianserin, MDL 72222 and spiperone showed no significant effect. Continuous superfusion of IMG with MCPP suppressed LS-EPSP of 5-HT sensitive neurons by 5-HT 1P receptor desensitizated. CONCLUSION LS-EPSP of 5-HT sensitive neuron is mediated by 5-HT 1P subtype receptor.
4.Asymmetry of soft tissue thickness of upper eyelids: a clinical study of Shanghai undergraduates undergoing double eyelid surgery

Objective To study the asymmetry of the soft tissue thickness of upper eyelids in Shanghai female undergraduates undergoing double eyelid surgery.Methods Data were collected in 565 female students from Shanghai universities.Before surgery,oblique sagittal view images of the upper eyelids by 3.0T MRI were obtained and the following parameters were measured: soft tissue thickness at upper tarsal plate margin and upper central tarsal plate,sub-eyebrow and intra-orbital septum fat pad areas,and whole upper eyelid soft tissue areas.The resected orbicularis and intra-orbital septum fat were weighed by a highly accurated electronic balance.The eyelid asymmetry index (EAI) was calculated.Results The MRI (U/U2) central tarsal plate soft tissue thickness were:4.46±0.90 in the right side and 3.78±1.01 in the left; the intra orbital septum fat areas were:172.33±49.29 in the right and 136.34±37.42 in the left; the whole tissue areas were: 697.13±146.99 in the right and 500.66±158.87 in the left (P<0.01 for all).The weight of the resected orbicularis oculi muscle and intra orbital septum fat pad were (0.18±0.05) g and (0.17±0.06) g for the right side,and (0.15±0.04) g and (0.06±0.05) g for the left side (P<0.01 for all),respec tively.The orbicularis EAI was 0.17±0.06,and the intra orbital septum fat EAI was 0.41 ±0.08.Conclusions The asymmetric phenomenon of the upper eyelids' soft tissue thickness is commonly found in the Shanghai female undergraduates,and the main manifestation is that orbicularis and intra orbital septum fat in right side are thicker than that in the left.
5.Ultrasound-guided core needle biopsy for diagnosing thyroid nodules:a meta analysis

Objective To evaluate the value of ultrasound-guided core needle biopsy(US-CNB) in diagnosing malignant and benign thyroid nodule by meta-analysis.Methods A computer-based online retrieval on the accurate tests of US-CNB for diagnosing thyroid nodule was conducted in PubMed,Cochrane Library (Issue 5,2016),EMbase,CBM,CNKI,VIP,Wangfang Data databases.The retrieval time limit was from the database establishment to June 8,2016.The quality of included articles was assessed by using the quality assessment of diagnostic accuracy studies (QUADAS).The puncture success rate,diagnosis sensitivity (Sen),specificity (Spe),positive likelihood ratio (+LR),negative likelihood ratio (-LR),diagnostic odds ratio (DOR) and area under the curve (AUC) of summary receiver operating characteristic curve (SROC) were served as the outcome indicators.The included data were performed the statistical analysis by using STATA12.0 and Meta-Disc 1.4 softwares.Results A total of 29 diagnostic accuracy trials were finally included,involving 4 213 patients with 4 391 thyroid nodules.The meta-analysis results suggested that the US-CNB puncture success rate was 98.7 % [95% CI (98.3 %,99.0 %)];Sen of US-CNB for diagnosing thyroid nodule was 0.94 [95%CI(0.93,0.95)],Spe=0.98[95 %CI(0.98,0.99)],+LR=33.96[95%CI(20.31,56.78)],-LR=0.06[95%CI(0.04,0.10)],DOR=581.44[95 %CI(335.08,1 007.87)],and the AUC of SROC was 0.99.Conclusion The current evidences indicate that UG-CNB has high diagnostic accuracy in benign and malignant thyroid nodules,and is worth promotion and application in clinic.
6.Total tumor perfusion value of CT in hepatocellular carcinoma with the tumor volume and Child-Pugh classification

Objective To investigate the correlation between tumor perfusion parameters and tumor volume and Child-Pugh classification in CT of hepatocellular carcinoma (HCC).Methods Fifty-six patients with hepatocellular carcinoma were selected to perform CT perfusion imaging.The parameters of the total tumor perfusion such as hepatic artery perfusion (HAP),portal vein perfusion (PVP)and hepatic perfusion index (HAPI)were calculated according to the degree of hepatic encephalopathy,albumin,bilirubin, clotting time,ascites for liver Child-Pugh classification.The relationship between the tumor or peritumoral perfusion parameters with tumor volume and Child-Pugh classification were analyzed.Results (1)There was no correlation between tumor or peritumoral perfusion with the tumor volume.(2)The difference of HAP,PVP and HAPI between the different Child-Pugh classification groups was statistically significant (P<0.000 1).(3)With the reduction of Child-Pugh classification,the tumor body HAP and HAPI values gradually decreased, while the PVP value increased gradually.Conclusion There is no correlation between the tumor volume of hepatocellular carcinoma with total tumor perfusion parameters.The differences in perfusion measurements between different Child-Pugh classification can intuitively and quantitatively reflect the reserve function of the liver.
7.Transarterial detachable coil embolization combined with ipsilateral intermittent carotid oppression for traumatic carotid-cavernous fistula with small fistula.

One case of traumatic carotid-cavernous fistula (TCCF) with small fistula treated by transarterial detachable coil embolization was reported. The intermittent ipsilateral carotid compression was used to identify the final blocking of the residual fistula. The follow-up digital subtraction angiography showed that the TCCF was cured finally. From this case, we conclude that this method may be an effective way to treat TCCF with small fistula.

8.Study on microsurgical treatment by unilateral subfrontal approach in 16 patients with large olfactory groove meningiomas

Objective To study the method and therapeutic effect of microsurgical treatment by unilateral subfrontal approach in patients with large olfactory groove meningiomas. Methods The clinical data of 16 patients with large olfactory groove meningiomas who had underwent microsurgical treatment by unilateral subfrontal approach from January 2010 to December 2017 were analyzed retrospectively. Results Total removal of tumor (Simpson Ⅰ and Ⅱ grade) was achieved in 14 cases and subtotal removal of the tumor (Simpson Ⅲ) was in 2 cases. No patient died from the microsurgery. The treatment effect was satisfactory in all patients during the follow- up. Conclusions The microsurgical treatment by unilateral subfrontal approach in patients with large olfactory groove meningiomas conforms to the concept of minimal invasive surgery, which provides enough surgical space, and the effect of surgery is satisfactory. So the approach is a good choice for the removal of large olfactory groove meningiomas.
9.Pharmacokinetics effect of shaoyao gancao compound with different decocting methods on characteristic ingredients in rat plasma after oral administration.

OBJECTIVETo observe the pharmacokinetic effect of Shaoyao Gancao compound on rats using different decocting methods, and to pharmacokinetically detect the the compatibility of the component compounds of the Shaoyao Gancao compound from the pharmacokinetic viewpoint.
METHODBased on the established HPLC analytical method of plasma effective constituents, rats were orally administered with a single or mixed decoction of Shaoyao and Gancao. Blood at different times after administering these decoctions were collected, and then variance analysis was made for pharmacokinetic parameters.
RESULTThe results of all six rats indicated that the absorption of the mixed decoction of Shaoyao and Gancao was greater than that of the single decoction. While, the absorption of the characteristic peaks at the retention time of 5.2, 11.0, 12.0, 14.6 min had statistical significance.
CONCLUSIONShaoyao Gancao compound was proved better than that of the single decoction from the pharmacokinetic viewpoint.

10.Effects of different deflation modes for endotracheal tube cuff on coughing response and hemodynamics during extubation

Objective To investigate the effects of different deflation methods of endotracheal tube cuff on coughing response and hemodynamics during extubation.Methods Ninety patients un-dergoing elective surgery for endotracheal intubation under general anesthesia were selected as study subjects and randomly divided into study group and control group,with 45 patients in each group.In the study group,the cuff end of the endotracheal tube cuff was connected to a syringe and a non-liq-uid pressure gauge through a three-way stopcock.Before extubation,the cuff was aspirated to de-crease the cuff pressure at a rate of 3 cmH2O/s.In the control group,the gas in cuff was rapidly de-flated by aspirating all the gas inside with a syringe during extubation.The incidence and severity of coughing response during extubation were recorded in both groups.Mean arterial pressure(MAP)and heart rate(HR)were recorded before induction of general anesthesia(T0),before cuff deflation(T1),immediately after cuff deflation(T2),1 minute after extubation(T3),3 minutes after extuba-tion(T4),and 5 minutes after extubation(T5).Adverse events were also recorded in both groups.Results The coughing response during extubation started immediately after cuff deflation in both groups.The incidence and severity of coughing response were lower in the study group than in the control group(P<0.05).Compared with the control group,values of MAP were lower at T2 to T4 and values of HR were lower at T2 to T5 in the study group(P<0.05).The incidence of post-extubation pha-ryngeal discomfort was 6.67%in the study group,which was lower than 26.67%in the control group(P<0.05).There was no significant difference in the incidence of post-extubation hypoventi-lation between the two groups(P>0.05).Conclusion The method of slowly reducing the pres-sure of the endotracheal tube cuff at a constant rate can reduce the incidence and severity of coug-hing,decrease postoperative extubation complications,and stabilize hemodynamics,with better effects than the method of rapidly aspirating all the gas inside the cuff at once.
