1."Observations on the Efficacy of ""Three Yin Points"" Acupuncture plus Medicinal Spreading Moxibustion in Treating Erectile Dysfunction"

Objective To investigate the clinical efficacy of three yin points acupuncture plus medicinal spreading moxibustion in treating erectile dysfunction.Methods Sixty patients with erectile dysfunction meeting the inclusion criteria were randomly allocated to two groups, 30 cases each. The control group took sildenafil at 1 hr before sexual intercourse. The treatment group received three yin points acupuncture plus medicinal spreading moxibustion, 8 days as one course of treatment, for three consecutive courses with 2 days of rest between the courses. The erectile function was scored in the two groups before and after treatment. The clinical therapeutic effects were compared.Results The total efficacy rate was 93.3% in the treatment group and 80.0% in the control group; there was no statistically significant difference between the two groups (P>0.05). The marked efficacy rate was 83.3% in the treatmentn group and 60.0% in the control group; there was a statistically significant difference between the two groups (P<0.05). in the treatment group, there was a statistically significant pre-/post-treatment difference in the erectile function score after 20 days of treatment (P<0.05) and statistically a very significant pre-/post-treatment difference in the erectile function score after 30 days of treatment (P<0.01). In the control group, the overall therapeutic effect was stable after treatment and there was a statistically significant pre-/post-treatment difference in the erectile function score (P<0.05). There was no statistically significant post-treatment difference in the erectile function score between the treatment and control groups (P>0.05).Conclusions Both acupuncture-moxibustion and sildenafil have a therapeutic effect on erectiledysfunction. Sildenafil produces a rapid and stable effect. Acupuncture and moxibustion produce a slower effect but have an advantage in the marked efficacy rate. Acupuncture and moxibustion are safe, of fewer contraindications and suitable for most of the population.

BACKGROUND:The OPG-RANKL-RANK transport system plays a crucial role in bone resorption mechanism of osteoclasts. OBJECTIVE:To observe the expression of OPG-RANKL-RANK signaling system in osteoclasts, osteoporosis, and the targeted therapy of OPG-RANKL-RANK signaling system. METHODS:We retrieved related literatures in the periodicals database with the key words of“osteoprotegerin, RANKL, RANK, osteoclasts, osteoporosis”in English and Chinese. According to the inclusion criteria, the literatures were included in this study after the evaluation of quality. RESULTS AND CONCLUSION:The mature of osteoclasts is mediated via OPG-RANKL-RANK signaling system and its associated signaling pathways. This signal pathway leads to the mechanism of osteoporosis. At the genetic and molecular levels, the targeted therapy of osteoporosis has become the focus. OPG-RANKL-RANK signaling system can provide a research platform for targeted treatment of osteoporosis, and OPG-RANKL-RANK signaling system is an important way to the regulation of osteoclasts and osteoporosis.
3.Slow synaptic transmission mediated by 5-HT_(1P) receptor in sympathetic ganalion of guinea pig

AIM To investigate the effects of several 5-hydroxytryptamine(5-HT) subtype receptor antagonists on late slow excitatory postsynaptic potential(LS-EPSP) of the neurons of guinea pig inferior mesenteric ganglion(IMG). METHODS Intracellular recordings were made from neurons of the isolated guinea pig IMG. RESULTS Cyproheptadine and BRL 24924 suppressed LS-EPSP of 5-HT sensitive neurons reversibly, while mianserin, MDL 72222 and spiperone showed no significant effect. Continuous superfusion of IMG with MCPP suppressed LS-EPSP of 5-HT sensitive neurons by 5-HT 1P receptor desensitizated. CONCLUSION LS-EPSP of 5-HT sensitive neuron is mediated by 5-HT 1P subtype receptor.

AIM To study the role of serot on in (5-HT) in ventrolateral preoptic nucleus(VLPO) on sleep and wakefulness cycl e of rat by microinjection of 5-hydroxytryptaphan (5-HTP , precursor of 5-HT ) , non-special 5-HT receptor antagonist methysergide (MS) and 5-HT retake i nhibitor fluoxetine. METHODS Stereotaxic, microinjection and po lysomnography (PSG) were used in the experiment. RESULTS There was no significant effect by microinjection of 5-HTP(0 5 ?g,0 1 ?l) into VLPO,but microinjection of 5-HTP(1 ?g,0 1 ?l)and fluoxetine lead wake i ncreased and sleep decreased; while microinjection of non-selective serotonin receptor antagonist MS lead to the opposited effect. The chang of sleep-wakefu lness cycle caused by 5-HTP or MS were significantly assiociated with time. CONCLUSION 5-HT involved in the regulation of sleep-wake cycle a nd promoted wake in the VLPO and its role of promotion may involve the gene exp ression of post-synaptic neurons.
6.Clinical features comparison of ST segment elevation and non ST segment elevation in adult patients with acute myocarditis

Objective ST segment elevation in adult patients with acute myocarditis is rare in adult.The purpose of this study was to report the outcome of ST segment elevation in adult patients with acute myocarditis in our hospital,in order to provide reference for clinical diagnosis and treatment.Methods A retrospective analysis of Beijing Anzhen Hospital during January 2002 to December 2015 for the diagnosis of acute myocarditis were 105 cases,divided into two groups,one group of ST elevation (ST elevation group) total 57 cases,another group of non ST elevation (non ST elevation group) total 48 cases.The clinical features,laboratory examination,treatment and prognosis were compared between the two groups.The composite primary endpoint of major cardiovascular events (MACE) during hospitalization included:mortality,cardiac shock and ventricular fibrillation.Results The average onset age of patients with ST segment elevation group and non ST segment elevation group were similar [(27.7 ± 10.1) years vs.(28.7 ± 10.0) years,P =0.603].There was a significant difference between the two group in patients of heart rate [(89 ± 22) beats/min vs.(80 ± 23) beats/min,P =0.028],systolic blood pressure on admission [(105 ± 17) mmHg vs.(115 ± 17) mmHg,P =0.003],diastolic blood pressure on admission [(66 ± 11) mmHg vs.(74 ± 11) mmHg,P =0.000],and total number of acute fulminant myocarditis [20 (35.1%) vs.7 (14.6%),P =0.024)].ST segment elevation acute myocarditis patients were significantly more at risk for MACE than non ST segment elevation acute myocarditis patients during hospitalization (P =0.04).Conclusion ST segment elevation acute myocarditis has acute onset and rapid progression.The incidence of MACE during hospitalization was significantly high.Mechanical support can be ve,ry favourable.ST segment elevation acute myocarditis is associated with excellent short-term prognosis.
7.Effect of occlusion of the internal iliac artery during endovascular repair of abdominal aortic aneurysm on the occurrence of postoperative pelvic ischemic complications in patients

Objective:To explore the effect of occlusion of the internal iliac artery during endovascular aneurysm repair(EVAR)of abdominal aortic aneurysm (AAA) on postoperative pelvic ischemic complications in patients.Methods:A retrospective case-control study was conducted to analyze the clinical data of 177 patients with subrenal AAA who received occlusion of the internal iliac artery during EVAR treatment at Inner Mongolia People′s Hospital from January 2018 to October 2023. They were divided into groups A, B, and C based on the occlusion of the internal iliac artery during surgery. Patients in group A ( n=20) involved intraoperative occlusion of bilateral internal iliac artery; patients in group B ( n=75) involved intraoperative occlusion of unilateral internal iliac artery; patients in group C ( n=82) retained bilateral internal iliac artery during surgery. The incidence of pelvic ischemic complications were recorded and compared in three groups of patients after surgery and during follow-up. The measurement data was expressed as mean ± standard deviation ( ± s), and analysis of variance was used for comparison between the three groups; Chi-square test and their continuous correction method were used for comparison between data groups. Results:There was no significant difference in age, gender, tumor diameter, smoking, drinking and complications (hypertension, diabetes, coronary heart disease, cerebral infarction, etc.) among the three groups ( P>0.05). Compared with group B and group C, the incidence of pelvic ischemic complications in group A was significantly increased, and the difference was statistically significant ( P=0.012, 0.007); compared with group C, there was no significant change in the incidence of pelvic ischemic complications in group B, and the difference was not statistically significant ( P>0.05). Conclusions:In the treatment of subrenal AAA with EVAR, the intraoperative bilateral internal iliac artery occlusion is more common than unilateral or bilateral internal iliac artery preservation, and the incidence of postoperative pelvic ischemic complications is significantly increased. It is recommended to preserve the internal iliac artery as much as possible during surgery.
8.Prediction of microvascular invasion in hepatocellular carcinoma patients by preoperative neutrophil-lymphocyte ratio and cancer-related factors

Objective To predict the presence of MVI,the general clinicopathological of HCC patients' data of the preoperative neutrophil-lymphocyte ratio (NLR),platelet-lymphocyte ratio (PLR),altplatelet ratio (APRI) were evaluated.Methods 143 cases of HCC patients who underwent radical resection and ≤ 5 cm of tumor diameter in Chifeng Clinical Medical School of Inner Mongolia Medical University from January 2011 to December 2014 were analysed retrospectively and followed up.The relationship between NLR,PLR,APRI and other clinical parameters was evaluated.Results According to ROC Curve,the NLR truncation value was 2.00,PLR to 115.00,APRI 1.6.The single factor analysis of x2 test showed that NLR (x2=6.419;P<0.05),APRI (x2=3.975;P<0.05),AFP (x2=33.37;P<0.05),Degree of differentiation (x2 =9.839;P<0.05) were significant differences between MVI positive (MVI+) and negative (MVI-) groups,and the difference was statistically significant;Logistic regression multifactor analysis showed that NLR (OR 2.678;95% CI 1.033~6.944;P<0.05) and AFP (OR 1.724;95%CI 1.023~2.905;P<0.05) are independent predictors of MVI.Conclusion Preoperative NLR and AFP are convenient,economical and reliable hematological indices for predicting the presence of MVI in HCC patients.
9.Analysis of risk factors for adverse cardiac events in adults with acute myocarditis during hospitalization

Objective To analyze the risk factors of adverse cardiac events in adults with acute myocarditis during hospitalization and provide reference for clinical diagnosis and treatment.Methods A restrospective study was conducted in 80 patients (54 males and 26 females) with acute myocarditis over 18 years old admitted to our hospital between January 2007 and December 2016.Major adverse cardiac events (MACE) were defined as death,cardiac arrest,cardiogenic shock and ventricular fibrillation.According to whether MACE occurred during hospitalization,patients were divided into two groups:the MACE group and the non-MACE group.The differences between the two groups were compared,and the risk factors were analyzed by logistic regression.Results There were 12 patients in the MACE group and 68 patients in the non-MACE group.The age of patients in the two groups was similar.Compared with the non-MACE group,the proportion of female patient in the MACE group was higher (66.7% vs 26.5%,P=0.015),and the systolic pressure (mmHg) was lower at admission (89.75±17.63 vs 112.49±16.35,P<0.01),and the heart rate (beats/min) was faster (106.42±24.39 vs 82.66±20.92,P=0.001);ALT and creatinine levels in the MACE group were higher (P<0.01),while the levels of TnI,CK-MB,CK and LDH were significantly higher (P<0.05).The LVEF value of the MACE group was significantly lower at admission (45% vs 60%,P=0.022),and the proportion of LVEF < 50% was also higher (58.3% vs 19.1%,P=0.008) The proportion of prolonged QRS wave (>120 ms) was significantly higher in the MACE group (75% vs 17.6%,P<0.01).In the MACE group,the proportion of diuretics and vasoactive drugs (dopamine,norepinephrine,and adrenaline) was higher (66.7% vs 25%,91.7% vs 4.4%,66.7% vs 0,75% vs 0%,all P<0.01);the proportion of glucocorticoids and immunoglobulin was higher (33.3% vs 8.8%,P=0.038;33.3% vs 4.4%,P=0.008),and the proportion of ventilator,CRRT,ECMO and IABP were also higher (50%vs 1.5%,33.3% vs 0,25% vs 0%,25% vs 0%,all P<0.01).Logistic regression analysis showed that the OR value of MACE in female patients during hospitalization was 5.56 (95%CI:1.49-20.71,P=0.011).The OR value of MACE in patients with reduced LVEF at admission was 5.92 (95%CI:1.62-21.67,P=0.007).The OR value of MACE in patients with prolonged QRS wave was 14.00 (95%CI:3.29-59.55,P<0.01).Conclusions Female patients,LVEF<50% at admission,and prolonged QRS wave (QRS>120 ms) are independent risk factors for MACE in adult patients with acute myocarditis during hospitalization.
10.Short-term effects of hemogram in healthy donors after peripheral blood stem cell collection.

OBJECTIVETo observe the short- term effects of hemogram in donors after peripheral blood stem cell(PBSC)collection and donors' tolerance.
METHODSA total of 166 related allogeneic donors were selected from The First Affiliated Hospital of Medical School of Zhejiang University between January 2013 and December 2014, including 86 male and 80 female. All donors accepted granulocytecolony- stimulating factor(G-CSF)5-10 μg·kg⁻¹·d⁻¹ until collection finished and were measured by blood cells count before and after PBSC collection.
RESULTSAfter PBSC collection, the hemoglobin level decreased from 145(94-181)g/L to 138(93-167)g/L, and the platelet counts decreased in all donors from 231(105- 490)× 10⁹/L to 95(39- 210)× 10⁹/L. The amount of hemoglobin contamination in collection products was weak correlated with the decreased hemoglobin in peripheral blood(r=0.297, P=0.017), and the platelet contamination was high correlated with that decreased in peripheral blood(r=0.719, P<0.001). The decline of hemoglobin level after twice PBSC collection was of no significant difference between four groups in different ages(P≥0.05). The decline of platelet counts was out of a significant difference(P> 0.05). In addition, the decline of hemoglobin level after once and twice PBSC collection was of a significant difference between four groups in different body mass index(BMI)(P=0.003 and P<0.001), especially in thinner group with obvious decrease. But the decline of platelet counts was out of a significant difference (P>0.05).
CONCLUSIONThe hemoglobin level decreased mildly in healthy allogeneic hematopoietic stem cell donors after PBSC collection and it is better to adjust parameters every time to ensure their safety for thinner donors. However, it will increase the risk of platelet decline, which is unrelated with ages and BMI and can be tolerated.
