2.Aphasia after Electric Shock and Cerebral Trauma: A Case Report

3.Research progress of needles with knife-edge for carotid cardiac syndrome.

According to topographic anatomy, pathogenesis and by retrieving, summarizing and analyzing literature regarding needle-knife and needles with knife-edge for carotid cardiac syndrome, it is found out that clinical misdiagnosis rate of carotid cardiac syndrome is considerably high. Needle-knife and needles with knife-edge could significantly improve the clinical symptoms of carotid cardiac syndrome, showing characteristic and advantage in treatment, but it is deficient in technique standard and efficacy criteria that should be united and authoritative. Researches regarding pathogenesis of carotid cardiac syndrome are not systematic. Clinical observation regarding long-term efficacy and relapse of needle-knife and needles with knife-edge treatment is rare. It is believed that the awareness on carotid cardiac syndrome should be increased to reduce misdiagnosis; scientific and standardized technique standard and efficacy criteria should be established; systematic and comprehensive researches regarding mechanism of needle-knife and needles with knife-edge for carotid cardiac syndrome should be launched; besides, clinical discussion regarding its long-term efficacy should start to provide a better clinical guideline.

4.The efficacy of combined therapeutic life style change with metformin therapy in nonalcoholic fatty liver disease

Objective Insulin resistance is implicated in the pathogenesis of nonalcoholic fatty liver disease(NAFLD).This study was to investigate the effect of metformin on liver enzyme abnormalities in patients with NAFLD.Methods One-hundred and thirty cases of NAFLD were divided into 2 groups,control(n=43)and metformin treatment group(n=98).The former group(TLC)was offered therapeutic life style change advice,and the later group(Met)was given metformin 1.5 g/day on the basis of TLC for 6 months.The changes of liver enzyme tests,waist circumference,blood pressure,lipid profiles,plasma glucose,insulin and the HOMA insulin resistance index were analyzed in 2 groups.Results ALT and AST declined in 2 groups after 3,6 months' treatment;enzyme abnormalities were normalized among 65% and 35% patients in Met and TLC group(P
5.Effect of puerarin on the expression of HIF-1αin placenta of rats with preeclampsia

Objective To investigate the effect of puerarin on preeclampsia-like rat placental tissue hypoxia inducible factor (hypoxia-inducible factor,HIF)-1αexpression and study the mechanism of protective effects in rats.Methods 60 SD rats were randomly divided into control group,drugs intervention groups(low-dose,middle-dose,high dose)and model group.The rats in drugs intervention groups and model group were adopted endotoxin for induction of preeclampsia model.On the basis of the model group,low,medium and high dose groups were given 20.0,40.0,60.0 mg/kg puerarin by intraperitoneal injection,respectively.The blood pressure of tail artery,urine protein concentration were observed at different points and the expression of HIF-1αwere detected by PT-RCT in each group.Results The systolic blood pressure ,urine protein and HIF-1αprotein in model group were higher than control group(P<0.05 );serum NO concentration was lower than control group (P<0.05 );after the puerarin intervention,systolic blood pressure and urine protein concentration in drugs intervention groups were lower than model group (P<0.05 ),plasma NO concentration was higher than model group (P<0.05 )and HIF-1αprotein was lower than model group (P<0.05 );the placenta weight,weight and body length of newborn rats in drug intervention group were higher than model group (P<0.05 ).Conclusion Puerarin can reduce systolic blood pressure and urine protein levels,promote the level of NO in plasma and reduce HIF-1αlevel in placenta of rats.Puerarin can offer protection to rats with preeclampsia by decreasing the expression of HIF-1αin placental.
6.Influence of Buyanghuanwu Decoction on Cerebral Infarct Volume and Pathological Changes after Cerebral Ischemia and Reperfusion

Objective To investigate the influence of Buyanghuanwu (BYHW) Decoction on cerebral infarct volume and pathological changes after cerebral ischemia and reperfusion. Methods The model of focal cerebral ischemia and reperfusion was made by thread embolism method. 7.Therapeutic effects of different doses of recombinant human tumor necrosis factor-receptor Ⅱ:IgG Fc fusion protein on rheumatoid arthritis

Objective To observe the therapeutic effects of different doses of recombinant human tumor necrosis factor-receptor Ⅱ:IgG Fc fusion protein (rhTNFR:Fc) on rheumatoid arthritis (RA). Methods Seventy-six patients with RA were randomly divided into rhTNFR:Fc treatment group A (n=22; rhTNFR:Fc 25 mg, twice/week and methotrexate 10 mg/week), rhTNFR:Fc treatment group B (n=24; rhTNFR:Fc 12.5mg, twice/week and methotrexate 10 mg/week) and control group (n=30; methotrexate 10 mg/week). Joint function related parameters including number of swollen joints, number of tenderness, pain degree, time of morning stiffness, Health Assessment Questionnaire (HAQ) score, erythrocyte sedimentation rate, C-reactive protein and rheumatoid factor were detected before and 2, 4, 6, 8, 12, 16 and 24 weeks after treatment in all the patients. After being treated for 3 months, the changes of joint function related paramters were evaluated and compared among groups. ACR20, ACR50 and ACR70 criteria were employed to evaluate the therapeutic effects. Results There was no significant difference in joint function related parameters among three groups before treatment (P>0.05), while those improved significantly after treatment for 3 months (P<0.05), with more favorable results in rhTNFR:Fc treatment group A and B than in control group (P<0.05). The percent of patients with ACR50 in rhTNFR:Fc treatment group A was significantly higher than that in rhTNFR:Fc treatment group B 2 and 4 weeks after treatment (P<0.05), while there was no significant difference in total effect between these two groups 8, 12, 16 and 24 weeks after treatment(P>0.05). Conclusion Small dose of rhTNFR:Fc in combination with methotrexate may yield effective and economical results for treatment of RA.
8.Efficacy of etanercept on ankylosing spondylitis

Objective To evaluate the efficacy and safety of etanercept in treatment of ankylosing spondylitis. MethodsEighty-six patients were randomized into experiment group and control group (n=43). Patients in experiment group were treated with non-steroidal antiinflammatory drugs (NSAIDs) and etanercept (25 mg, twice/week, subcutaneous injection in the upper arm), and those in control group were managed with NSAIDs and sulfasalazine. Reexaminations were performed after treatment for 3 months, and the prevalence of low back pain, time of morning stiffness, Bath ankylosing spondylitis disease activity index(BASDAI), Bath ankylosing spondylitis functional index(BASFI), erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were analysed, and the prevalence of adverse effects was observed. ResultsCompared with those at administration, the prevalences of low back pain significantly reduced (P<0.05 or P<0.01), the time of morning stiffness significantly decreased (P<0.05), BASDAI and BASFI significantly improved, and ESR and CRP significantly decreased 3 months after treatment in both groups (P<0.05). After treatment for three months, the clinical parameters in experiment group were more favourable than those in control group (P<0.05). There was no significant difference in the prevalence of adverse effects between two groups (P>0.05). Conclusion Etanercept has favourable short-term therapeutic effects on ankylosing spondylitis with minor adverse effects.
9.Effects of subcutaneous injection duration of heparin on side effects: A Meta-analysis

Objective To evaluate the effect of injection duration on bruising and pain after the subcutaneous injection of heparin.Methods The relevant randomized controlled trials,quasi-randomized controlled trials about subcutaneous heparin injection duration of ≥30 s and ≤ 10 s were searched in the databases of CNKI,CBM,VIP Database for Chinese Technical Periodicals,PubMed,Highwire,ScienceDirect,Chinese Cochrane Library,Wanfang database and Google.The quality of RCTs was assessed and Metaanalysis was conducted by RevMan 5.0 Software.Results Twelve RCTs involving 1 986 injections were included.The results of Meta-analysis showed that:There was significant difference between the two groups in the occurrence of bruising.The injection duration of 30 s was much better to prevent bruising.Statistical differences existed in pain intensity between the two groups.Conclusions Prolonging the injection duration to 30 s may reduce the occurrence of subcutaneous bruising.There is no enough evidence to draw a conclusion on pain intensity.Adequately,the current study is limited to a small sample,powered and high quality randomized controlled trials are still needed to be conducted.
10.Assessment of Tricuspid Insufficiency and the Function of Right Ventricle Using Cardiac Magnetic Resonance Imaging Combined with Echocardiography.

Right-sided cardiac valvular diseases have traditionally been considered less important than disease of mitral or aortic valve. However, severe tricuspid regurgitation could lead to right ventricle dysfunction and reduce patients' survival rate. In clinic setting, tricuspid valve disease should be paid more attention for patients with secondary tricuspid regurgitation caused by left-sided valvular surgery combined with irreversible annular dilatation increasing the risk of reoperation. In this review, we summarize the epidemiology, anatomy, pathology, diagnosis, ultrasound and cardiac magnetic resonance imaging findings in patients with tricuspid regurgitation.
