1.Correlation between uric acid level and carotid atherosclerosis plaque formation in the elderly

Objective To investigate the correlation between uric acid level and increase of carotid intima-media thickness (IMT) in the elderly.Methods Clinical data of elderly and middleaged people who took health physical examination from Jan.2013 to Dec.2014 in a 3A-level hospital in Beijing were collected.The elderly people were divided into IMT normal group and plaque group.Logistic regression and linear regression were used to analyze the related factors for IMT thickening and uric acid level.Results The occurrence rate of hyperuricemia had a significant difference between the elderly group and the middle-aged group (P<0.01).There were significant differences in uric acid level [(350.6±89.6) μmol/L vs.(359.6±86.6) μmol/L, t=5.19, P<0.05].Body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), blood urea,cholesterol, high density lipoprotein levels between the IMT normal group and plaque group were [(25.3±3.6)kg/m2 vs.(25.8±3.5) kg/m2, (115.9±13.8) mmHg vs.(126.8±16.9) mmHg, (74.6±11.3) mmHg vs.(77.8±12.1) mmHg, (4.9±0.9) mmol/L vs.(5.0±1.0) mmol/L,(1.2±0.3) mmol/L vs.(1.2±0.3) mmol/L, all P<0.01].Multi factor correlation analysis showed that the IMT thickening was correlated with gender, SBP, low density lipoprotein, blood glucose,while had no correlation with uric acid level.Blood uric acid level was related with gender, smoking, BMI, triglycerides, blood glucose, C-reactive protein, high-density lipoprotein.Conclusions Blood uric acid level, BMI, blood pressure and lipids are increased in the plaque group as compared with the IMT normal group.However, blood uric acid level can not be used as an independent risk factor for IMT lesions.
2.Coded harmonic angio:assessment of the therapeutic response in hepatocellular carcinoma treated by transcatheter arterial embolization and radiofrequency ablation

Objective To investigate the value of contrast-enhanced coded harmonic angio (CHA) in evaluating the therapeutic response to combinational treatment of transcatheter arterial embolization (TAE) and radiofrequency ablation (RFA) in hepatocellular carcinoma (HCC). Methods Nineteen patients with 28 HCCs underwent percutaneous RFA in one week after TAE. The therapeutic response was assessed by contrast-enhanced CHA and three-phase dynamic CT in one week before and after the treatment. Results After TAE and RFA, no enhancement was demonstrated in 20 HCCs (71.4%) on contrast-enhanced CHA, which were considered to be completely treated. of the 20 HCCs, no blood signal was identified in 18 on dynamic CT and the conclusion cannot be drawn in the other 2 nodules due to the retention of iodized oil. On contrast-enhanced CHA, enhanced blood signals or tumor stains were identified in the remaining eight HCCs, however, four of the eight could not be evaluated on dynamic CT because of the retention of iodized oil. Conclusions Contrast-enhanced ultrasound can effectively evaluate the treatment response of HCC in combinational therapies of TAE and RFA. It is superior to three-phase dynamic CT for no influence by retention of iodized oil.

4.Prospective study of improved elasticity scores in the differential diagnosis of malignant and benign breast lesions

Objective To explore the value of improved elasticity scores in differentiating malignant and benign breast lesions. Methods A prospective study was done in 394 patients with 508 breast lesions confirmed by pathology and the elastographic images of the lesions were evaluated by using old and improved elasticity scores respectively. Results The elastographic images of the lesions could be evaluated and classified in all patients by improved elasticity scores but could not be evaluated and classified in 82 lesions by old elasticity scores. The accuracy of ultrasonic elastography for diagnosing benign and malignant breast lesions was 88.8% and 78.1%, respectively by old elasticity scores and improved elasticity scores. Conclusions The elastographic images of breast lesions were accurately evaluated by using improved elasticity scores, and the use of improved elasticity scores is helpful to reduce the biopsy which is not necessary.
5.The changes of plasma soluble CD40 and soluble CD40L in patients with chronic renal failure and their clinical significance

Objective To investigate the changes of plasma soluble CD40 (sCD40) and soluble CD40L (sCD40L) in patients with chronic renal failure( CRF) and their potiential mechanisms in renal injury and immunodeficiency. Method From September to December 2006, 30 CRF patients without hemodialysis (CRF group) in department of Nephrology and 30 uremic patients receiving maintenance hemodialysis for more than 3 months ( HD group) in dialysis center in Beijing TongRen Hospital of Capital Medical University, were enrolled in this study. The normal control group consisted of 20 healthy volunteers who matched with study subjects for age and gender. The levels of plasma sCD40 and sCD40L were measured by the method of enzyme linked immunosorbent assay (ELISA). The relationship between sCD40, sCD40L and related factors were analyzed, and the influence of hemodialysis on sCD40 and sCD40L were observed. ANOVA and Pearson correlation analysis were used in statistical analysis. Results (1) The sCD40 levels in CRF group and HD group were significantly higher than those in control group(P < 0.01). The sCD40 levels in HD group were also higher than those in CRF group(P < 0.01). (2) The sCD40L levels in CRF group and HD group were significantly higher than those in control group (P < 0.01). The sCD40L levels in HD group were slightly higher than those in CRF group, but without significant difference (P > 0.05). (3) In CRF group, the sCD40 levels correlated positively with serum creatinine (Scr) and C-reactive protein(CRP) (r = 0.637,P < 0.01,r = 0.551,P < 0.05). The correlations between sCD40L and Scr and CRT were also seen (r = 0.553, P < 0.05, r = 0.686,P < 0.01), whereas the correlation with blood pressure,hemoglobin (Hb), platelet(PLT), white blood cell(WBC), serum albumin(Alb), glucose (Glu),blood urea nitrogen(Bun)and serum lipid was no seen (P > 0.05). In HI) group, there were no correlations between sCD40,sCD40L and above parameters (P > 0.05), there were also no correlations between sCD40, sCD40L and duration of the treatment by dialysis and dialysis adequacy(Kt/V) (P > 0.05). (4)After one HD session, the post-HD levels of sCD40 were slightly higher than pre-HD levels, but without statistical difference ( P > 0.05). The post-HD levels of sCD40L were obviously lower than pre-HD levels, with significant difference (P < 0.01). Conclusions The levels of sCD40 and sCLMOL in CRF patients,particularly in HD patients, were significantly higher than those of controls, which may participate in pathogenesis of renal injury and immunodeficiency.

Objective To study the relationship between apoptosis regulation and epidermal proliferation in psoriasis. Methods The expression of apoptosis molecule PDCD5 (programmed cell death 5) on psoriatic lesions was observed by direct immunofluorescence. The positive rates and the average of fluorescence intensity of PDCD5 were analyzed quantitatively with FACS and computer CELL Quest software. The expression of PDCD5 mRNA in psoriatic lesions was detected by RT-PCR. Results The expression of PDCD5 protein was obviously lower in psoriatic epidermal cells than that of the normal skin. 7.Relation of BMI, WC and WHR to cardiovascular diseases in middle and elderly patients

Objective:To investigate the effect of body mass index(BMI), waist circumference(WC) and waist hip ratio (WHR) on blood pressure, serum lipids, renal function, protein and lymphocyte. Methods:175 middle and elderly patients were measured. Results:Compared to normal weight cases, SBP and uric-acid were significantly higher in over-weight cases. Triglyceride, SBP and DBP were all significantly higher in obesity cases. When waist circumference was beyond 85 cm in women, triglyceride, lymphocyte and DBP were all significantly higher than those in normal weight women. When waist circumference was beyond 90 cm in men, triglyceride was significantly higher than that in normal weight men. Conclusion:When BMI increases, triglyceride and blood pressure will increase accordingly. When WC is higher than normal, triglyceride in male and female will be higher. Control of obesity, is helpful to prevent cardiovascular diseases.
8.Diagnostic value of strain ratio for differentiation breast benign and malignant solid lesions with ultrasonic elastography

Objective To investigate the diagnostic value of strain ratio for differentiation breast benign and malignant solid lesions. Methods Two hundred and sixty-nine patients with 377 lesions (278 benign, 99 malignant) were included in the study. Ultrasonography found the lesions, then ultrasonic elastography got the strain images. By the strain ratio measure method equipped with the ultrasonic machine, strain ratio of the lesion was calculated. Compared with the pathologic diagnosis, the cut-off point of strain ratio was determined with receiver operating characteristic curve. Results There was significant difference between the strain ratio of benign and malignant lesions( P <0. 05). The cut off point of strain ratio was determined as 3. 08, Youden's index was 0. 88,sensitivity was 97. 38% ,and specificity was 91. 33%. Conclusions Strain ratio could be used to differentiate benign and malignant lesions in breast and would be a new diagnostic standard in future.
9.Value of ultrasonic elastography in diagnosing breast small masses

Objective To investigate the value of ultrasonic elastography(UE) in diagnosing breast small masses (diameter≤10 mm). Methods The static sonograms of conventional ultrasound(US) and UE of the 274 breast small lesions which pathologically proved were reviewed. Images were assigned according to the B1-RADS criteria for US. The elastographic images were evaluated using improved 5-scoring system.Pathology was followed up as diagnostic criteria. The diagnostic performance of the 2 methods was evaluated with receiver operating characteristic curve(ROC). Results The areas under the ROC curve of the two modalities were 0. 891 (UE) and 0.742 (BI-RADS), respectively. The difference between them was significant. Conclusions UE is helpful in differentiating between malignant and benign breast small lesions.
10.Evaluation on ovarian function after hysterectomy by detection of serum anti-Mullerian hormone levels

Objective To evaluate the value of the new ovarian function marker serum anti-Mullerian hormone (AMH)and to clarify the effect of hysterectomy on the ovarian function of the younger women.Methods The serum AMH,follicle stimulating hormone (FSH)and luteinizing hormone (LH)levels in 35 women suffered uterus benign lesion aged 36-45 years and 35 women suffered hysteromyoma without operation were measured at different time.And the ovarian arterial blood flow resistance index (RI ) was measured by Doppler ultrasound. Results Compred with before operation, the serum AMH levels of the patients in hysterectomy group 2 d and 3 months after operation were significantly decreased (P<0.05 or P<0.01).Compared with control group,the serum AMH levels of the patients in hysterectomy group 2 d and 3 months after operation were also decreased (P<0.05).The ovarian arterial blood flow RI of the patients in hysterectomy group 1 and 3 months after operation were increased compared with control group (P<0.05).There were no significant differences in the serum FSH and LH levels in each group between different time points (P>0.05).Conclusion Hysterectomy can affect the ovarian function,and the serum AMH level 3 months after operation is decreased and the ovarian arterial blood flow RI is increased.AMH is superior to FSH or LH in evaluating the changes of ovarian function.
